1.Clinical efficacy of ranibizumab in the treatment of corneal neovascularization in elderly patients

Objective To evaluate the clinical efficacy and safety of ranibizumab in the treatment of corneal neovascularization(CNV)in elderly patients.Methods A total of 40 elderly patients(50 eyes)with conceal neovascularization were enrolled in this study at our hospital.Among them,20 patients(22 eyes)in observation group enrolled from January 2015 to December 2015,received subconjunctival injection of ranibizumab,and 20 patients(28 eyes)in control group enrolled from June 2014 to December 2014,receivedlaser photocoagulation.The visual acuity,the area of corneal neovascularization,intraocular pressure and therapeutic response rate were measured and compared between two groups before and 1 month,6 months and 12 months after treatment.All patients were followed up for 12 months to record the recurrence and complications.Results In the observation group,20 eyes(90.9%),19 eyes(86.3 %)and 19 eyes(86.3%)got an improved eyesight at 1 month,6 months and 12 months of ranibizumab treatment,respectively,which were significantly higher than those in the control group(16/57.1%,14/50.0%,12/42.9%,x2 =6.910,5.866,8.010;P=0.031,0.020,0.016,respectively).The area of corneal neovascularization was decreased in both groups after treatment in the comparison of pretreatment.What is more important:at 1 month,6 months and 12 months of treatment,the areas of CNV were all significantly smaller in the observation group than in the control group (t =6.109,5.291,8.330;P =0.019,0.033,0.009,respectively).There was no significant difference in intraocular pressure before versus after treatment in both groups(both P> 0.05).The therapeutic response rate was 90.9% (20/22)in observation group,and 75.0% (21/28)in control group(x2 =6.109,P =0.006).During the 12-month follow-up after operation,the recurrence rate of CNV was significantly higher in control group(7 eyes,25.0%)than in observation group(2 eyes,9.1%) (x2 =8.668,P =0.003).Conclusions Subconjunctival injection of ranibizumab is effective and safe with a fewer complications in the treatment of corneal neovascularization in elderly patients,which is worthy of generalizing in clinical treatments.
2.Short-term prognostic value of soluble Axl receptor tyrosine kinase for elderly patients with acute decompensated heart failure

Objective To investigate the clinical value of serum soluble Axl receptor tyrosine ki-nase(sAxlTK)in evaluating short-term prognosis in patients with acute decompensated heart failure(ADHF).Methods A total of 238 elderly ADHF patients admitted to Bozhou People's Hospital from January 1,2018 to October 1,2021 were recruited and divided into poor prognosis group(45 patients)and good prognosis group(193 patients)according to the occurrence of com-plex events within 90 d of follow-up.Based on the optimal cut-off value of serum sAxlTK level,they were also assigned into high level group(80 cases)and low level group(158 cases).Serum levels of sAxlTK,troponin Ⅰ(cTnⅠ)and N-terminal B-type natriuretic peptide precursor(NT-proBNP)were detected.Results Serum sAxlTK level was significantly higher in the poor prognosis group than the good prognosis group[43.89(33.95,51.44)μg/L vs 23.89(18.73,33.92)μg/L,P<0.01].Multivariate logistic regression analysis showed that serum cTnⅠ and sAxlTK levels were independent risk factors for short-term poor prognosis in ADHF patients(OR=1.922,95%CI:1.035-3.568,P=0.039;OR=1.021,95%CI:1.008-1.034,P=0.001).ROC curve analysis indicated that the AUC value of combined serum sAxlTK,cTnⅠ and NT-proBNP levels to predict short-term poor prognosis was 0.836(95%CI:0.778-0.895).The incidence of complex events within 90 d was significantly higher in the high level group than the low level group(45.0%vs 5.7%,P<0.05).Kaplan-Meier curve analysis revealed that the cumulative inci-dence of complex events was also higher in the high level group than the low level group(X2=66.991,Plog rank<0.01).The high level group had significantly lower overall survival rate and worse survival prognosis than the low level group(X2=16.899,Plog rank<0.01).Conclusion High serum sAxlTK level in elderly ADHF patients at admission is associated with a higher risk of 90-day short-term poor prognosis.Serum sAxlTK has the potential to become a useful tool for early prediction of short-term poor prognosis,and its combination with cTnⅠ and NT-proBNP can fur-ther improve the accuracy of prognosis prediction.
3.Analysis on the current monitoring and management of blood donation adverse reaction in Chongqing

【Objective】 To investigate the management of adverse reactions to blood donation(ARBD) in blood services, so as to promote the surveillance of ARBD and improve the quality of blood donation service in Chongqing. 【Methods】 A questionnaire, involving the staff and facilities in blood donation sites as well as the prevention and treatment, the record and report, the following up and data related to ARBD was developed by Chongqing Society of Blood Transfusion in February 2019, and was issued to 18 blood services(1 blood center and its sub-center, 6 central blood stations and 11 hospital blood banks) in the Chongqing via email. The questionnaire was filled in and submitted before March 31 by management personnel participating in the investigation, and the data was collected, collated, revised and analyzed by Excel 2011. 【Results】 A total 19 questionnaires were collected, with the valid rate at 100%(19/19). 78.95%(15/19) of the blood services met the requirements of medical personnel allocation(>6 medical staff) when the number of daily blood collection was more than 60, and 100%(19/19)met the requirements of medical personnel allocation(2 to 6 medical staff) when the number of daily blood collection was less than 60. 89.47%(17/19) of the blood services were equipped with epinephrine hydrochloride, and 84.21%(16/19) with dexamethasone(an anti-allergic drug). There were significant differences in the allocation of other types of drugs. 100.00%(19/19) of the blood services formulated prevention and treatment measures concerning ARBD. In 2019, the incidence of ARBD in Chongqing was reported to be 0.54%(1 958 / 359 871), with the highestas [1.35%(223/16 543)] in subcenters and the lowest [0.32%(179/56 299)] in central blood centers (P<0.05). There was statistical significances in the incidences of ARBD reported by different blood stations(P<0.05). 【Conclusion】 The monitoring and management of ARBD among blood services in Chongqing should be further standardized in terms of staffing allocation, emergency drugs allocation and reporting, so as to gradually realize regional homogenization and ensure blood safety.
4.Preparation and bioactivity evaluation of streptavidin-tagged human interferon- inducible T cell alpha chemoattractant bifunctional fusion protein.

OBJECTIVETo prepare streptavidin-tagged human interferon-inducible T cell alpha chemoattractant bifunctional fusion proteins (SA/hI-TAC) and evaluate its biological activity.
METHODSpET24a-SA-hI-TAC/pET21a-hI-TAC-SA plasmids were constructed and expressed in BL21. SA-hI-TAC and hI-TAC-SA fusion proteins were purified by Ni-NTA affinity chromatography, refolded by dialysis and identified by Western blotting. The bifunctionality of the fusion proteins (biotin-binding function and hI-TAC activity) was analyzed by flow cytometry and lymphocyte chemotaxis experiment, respectively.
RESULTSSA-hI-TAC/hI-TAC-SA fusion proteins were expressed at about 12% and 25% of the total bacterial protein, respectively. The two fusion proteins had a purity of about 85% and 90% after purification, and their purity reached 98% after purification with S-100 gel filtration chromatography. Both of the fusion proteins were efficiently immobilized on the surface of biotinylated mouse bladder cancer MB49 cells (91.3% for SA-hI-TAC and 98.8% for hI-TAC-SA). SA/hI-TAC induced lymphocyte chemotaxis in a dose-dependent manner, and hI-TAC-SA showed a stronger chemotactic effect than SA-hI-TAC.
CONCLUSIONSWe successfully obtained SA/hI-TAC bifunctional fusion proteins, which may potentially be used in local treatment of tumor and as a tumor vaccine.
