1.A clinical study of perforating artery occlusion occurring after stent implantation of intracranial branch of vertebral artery

Objective To explore the methods to reduce the occlusion of perforating arteries after intracranial stenting of the vertebral artery. Methods Clinical data of 32 cases of Gateway-Wingspan stent implantation for intracranial branch of vertebral artery were retrospectively analyzed. The postoperative stricture and perfusion improvement situation were evaluated, the reason of perforating artery occlusion was analyzed. Results Thirty-two patients were implanted with 33 pieces of Wingspan stent and 1 piece of Apollo bracket. The operation success rate were 100%, and the stenosis rate reduced from (76.6±6.1)%to (27.9±5.2)%. After three months, the transcranial doppler sonography (TCD) and CT angiography were checked, showing no in-stent restenosis in all patients. Two patients occurred the perforating artery occlusion within 24 hours after operation. The possible reason was the change of stability of atherosclerotic plaque at the stenosis and the plaque displacement caused by the mechanical action of the balloon or stent, which may lead to medulla oblongata artery block. After drug and rehabilitation treatment, the symptoms in patients were improved significantly. Conclusion The perforating artery occlusion after stent implantation in intracranial branch of vertebral artery can be prevented by strict evaluation and preoperative preparation, the right selection of intraoperative balloon and stent, which still needs larger sample data to prove.
2.Efficacy observation of percutaneous vertebroplasty combined with zoledronic acid in treatment of spinal metastatic carcinoma

Objective To observe the efficacy of percutaneous vertebroplasty (PVP) combined with zoledronic acid in treatment of spinal metastatic carcinoma.Methods Fifty cases of spinal metastatic carcinoma were divided into control group and treatment group by random digits table method with 25 cases each.The control group was given zoledronic acid 4 mg,intervals of 28 d intravenous drip 1 time.The treatment group was given PVP besides zoledronic acid.The visual analog scale score,activity ability score and analgesic usage score after treatment for 3 months were observed and compared.The quality of life was evaluated and the occurrence of relapse of fracture was also recorded.Results After treatment for 3 months,the visual analog scale score,activity ability score and analgesic usage score were (2.00 ± 0.17),(2.12 ± 1.38),(1.47 ± 0.10) scores in treatment group,and (7.84 ± 0.18),(3.21 ± 0.13),(3.30 ± 0.09) scores in control group,and there were significant differences between two groups (P < 0.01 or < 0.05).The occurrence of relapse of fracture was 2 cases (8%,2/25) in treatment group and 9 cases (36%,9/25) in control group,and there was significant difference between two groups (P < 0.05).Conclusions PVP combined with zoledronic acid in treatment of spinal metastatic carcinoma has many advantages such as small incision,short surgery time and rapid relief from pain.It can obviously improve the quality of life,and it is worthy of spreading in the clinic.

Staphylococcus epidermidis is the most important member of the coagulase-negative staphylococci and one of the most abundant colonizers of human skin.S.epidermidis is a bacterial species that is commonly described as relatively innoc-uous,which is in part due to the notion that it lacks secreted toxins with limited capability of infection.S.epidermidis pro-duces a series of molecules that provide protection from host defenses,including many proteins and exopolymers.Further-more,recent research has identified a family of cytolytic peptidesin S.epidermidis,the phenolsoluble modulins (PSMs),and the notion that S.epidermidis is virtually toxin-free had to be somewhat revised.
4.Sterol composition in field-grown and cultured mycelia of Inonotus obliquus

Sterols are one of the active classes of compounds in Inonotus obliquus for their effective therapy of many diseases. In field environment, this fungus accumulates large amount of sterols. In cultured mycelia, however, this class of compounds is less accumulated. For analyzing the factors responsible for differing sterol composition, the field-grown and cultured mycelia were extracted with 80% ethanol at room temperature and total sterols were prepared using silicon gel column chromatography followed by identification using either GC-MS or spectroscopic methods. For culturing Inonotus obliquus, the seed culture was grown either in basic medium consisting of glucose (2%), yeast extract (0.5%), KH2PO4 (0.01%), MgSO4·7H2O (0.05%) and distilled water at pH 6.5, or the basic medium supplemented with serial concentrations of AgNO3. The results indicated that field-grown mycelia contained lanosterol and inotodiol (comprised 45.47% and 25.36% of the total sterols, respectively) and other 10 sterols (comprising the remaining 30.17%) including ergosterol biosynthetic intermediates such as 24-methylene dihydrolanosterol, 4,4-dimethylfecosterol, 4-methyl fecosterol, fecosterol and episterol. Column chromatography also led to the isolation of lanosterol, Inotodiol, trametenolic acid, foscoparianol B and a new triterpenoid foscoparianol D in field-grown mycelia. In comparison, the cultured mycelia only contained three sterols with ergosterol as the predominant one (82.20%). Lanosterol only accounted for 3.68%. Supplementing Ag+ into the culture at 0.28 μmol·L-1 greatly enhanced content of lanosterol (accounting for 56.81%) and decreased the content of ergosterol (18.5%) together with the presence of intermediates for ergosterol biosynthesis. These results suggested that the sterol composition in mycelia of the fungus can be diversified by supplementing substances inhibiting enzymatic process towards the synthesis of ergosterol. Harsh growth conditions in field environment (I.e. temperature variation, UV irradiation etc.) can delay the synthesis of ergosterol and hereby diversify the sterol composition in the mycelia of Inonotus obliquus.
5.A clinical application research of 3D dose verification for esophageal carcinoma intensity-modulated radiation therapy with Compass (R)

Objective To study the clinical application of Compass (R) system,a novel 3D quality assurance system for the verification of esophageal carcinoma intensity-modulated radiotherapy (IMRT) plan.Methods 12 esophageal carcinoma IMRT plans were optimized with Eclipse 8.6 treatment planning system (TPS),and then Compass (R) reconstructed 3D dose distributions with the patient anatomy.Comparison was performed among the reconstructed and calculated with TPS,Dose-volume parameters (γ pass rate、average dose deviation) to the planning target volume (PTV) and critical structures were quantitative valuated.Furthermore two-dimensional dose verification were performed use MatriXX,γ pass rate were evaluated with 3%/3 mm criteria.Results The γ pass rate of actual gantry angle was found generally declined seemingly compared with 0 degree gantry angle in two dimensional verification,difference was statistically significant (P =0.018-0.001).In 3D dose verification,the γvolume of PTV were exceed 93%,the deviation of D95,D50,D2 were less than 3%;The γvolume of lungs and heart were exceed 95%,the average dose deviation were less than 3%;The γ pass rate of spinal cord and trachea were exceed 98%.The independent check verified more conformed with the TPS calculated.Dose deviations appeared in the radiation field edge area.Conclusions 3D dose verification can provide more information to comprehensively evaluate the plan which is benefit for evaluating the clinical value of verification.
6.Operative strategy of cerebral ischemia induced by extra-and intracranial artery stenosis using endovascular stent angioplasty

Objective To explore the therapeutic strategy for intracranial ischemia due to extraand intracranial artery stenosis using endovascular stent angioplasty.Methods Totally 109 patients with extra-and/or intracranial artery stenosis were treated by endovascular stent in our hospital from March 2008 to September 2011.There were 93 cases with single stenostic lesion,16 cases with multiple stenosis and 5 cases with distal unruptured aneurysm.All patients received endovascular stentings and 94 cases were followed up for 3 20 months.Results All of the patients underwent endovascular stenting placement successfully.The average stenosises were(79.2 ± 13.4)% before operation and(18.1 ± 9.20)% after operation.During follow-up for(10.8 ± 4.2)months,mild restenosis occurred in 9 stenosises(7.0%)among 129 lesions and no serious stroke event was found.Conclusions Endovascular stent is safe and effective in treating extra and intracranial stenosis.It is important to investigate adequately cerebral blood flow in the multiple stenosis or in complicating other kind of vascular diseases before proper treatments.
7.Study of immunophenotype of acute lymphoblastic leukemia and its correlations to cytogenetics and clinical features

objective To investigate the immunophenotype of Chinese patients with acute lymphoblastic leukemia(ALL)and its association to cytogenetics and clinical features.Methods In this study.a total of 113 Chinese patients with ALL were immunophenotyped by flow cytometry using a panel of monoclonal antibodies.50 000 cells were acquired and analyzed with Cell Quest and abnormal populations were recognized by CD_(45)/SSC gating.Among the 113 patients enrolled in this study,bone marrow cells of 74 patients were cultured for 72 hours to prepare for conventional chromosome detection.and karyotype was analyzed with R-banding technique.Results Of 113 ALL patients,14.2%(16/113)were identified as TALL,85.8%(97/113)as B-ALL Among the 106 out of 113 ALL cases,myeloid antigen(MyAg)expression was documented in 34.9%(37/106)and CD_(13) was the most commonly expressed MyAg in ALL patients(23.6%,25/106).MyAgs were more frequently associated with T-ALL(46.2%,6/13)than with B-ALL(33.3%,31/93)but there was no significant difieFence(X~2=0.825,P>0.05)between the two groups.MyAg positivity in adult ALL patients(47.6%,20/42)was higher than that in children(26.6%,17/64).There was a significant difference between the two groups(X~2=4.948,P<0.05).Abnormal karyotypes were detected in 39 out of 74(52.7%)patients.It showed that percentage of patients with high WBC count(>50×10~9/L)in MyAg~+(48.6%,18/37)was higher than that in MyAg~-ALL types (20.3%,14/69).There was a significant difference between the two groups(X~2=9.191,P<0.01).CD34 expression was found in 83.8%(31/37)of MyAg~+ and 53.6%(37/69)of MyAg~-cages.There was a significant difference between the two groups(X~2=9.527,P<0.05).In addition,the percentage of adult ALL patients in MyAg~+ group(54.1%,20/37)was higher than that in MyAg~-group(31.9%,22/69).There was a significant difference between the two groups(X~2=4.948,P<0.05).Conclusion lmmunophenotype analysis is useful for ALL diagnosis and classification and the immunophenotype has relevance to the abnormal cytogenetic changes and clinical features in ALL patients.
8.Improvement of Field Liquid Transfusion Control System
