1.Analysis on the associated factors of hyperuricemia: based on classification tree model

Objective To explore the associations between physiological factors, psychosocial factors, dietary habits, lifestyles and hyperuricemia (HUA) and provide the evidence for hyperuricemia intervention. Methods From 2016 to 2017, adults during their checkup in the health management center of the Second Affiliated Hospital of Dalian Medical University were interviewed with self-designed questionnaire and received the health examinations. To determine the influence of socio-economic data, life style habits and psychological factors on the risk of HUA, the classification tree model was adopted. Results A total of 4 118 subjects were enrolled in the study, with an average age of (52.8±7.9) years. The results showed that the prevalence of HUA was higher among people under 30 years old and 40-50 years old who are smoking, drinking and doing the sedentary work, who unsatisfied with their work and under a great mental stress. Factor analysis model identified three dietary patterns, traditional model,fried and smoked food and dessert model and high quality protein model, the cumulative variance contribution rate was 53.886%. In multivariate model, it was found that high-quality protein pattern, physical exercise and fasting plasma glucose (FPG) were negatively associated with HUA. Male, smoking, drinking, having dyslipidemia, higher body mass index (BMI) were risk factors for HUA. Subgroup analysis showed that in both male and female, the high-quality protein patterns were negatively associated with HUA. The pattern of fried and smoked food and dessert patterns was the independent risk factors for HUA in female. Classification tree model showed that male, dyslipidemia, higher BMI grade, and the fried and smoked food and dessert pattern were the risk factors for HUA. Conclusions The influencing factors of HUA in different gender were different, which indicate the individualized health management should be adopted. Male who is overweight or obese should quit smoking and drinking. Women should pay more attention to avoid of the excess intake of fried and smoked food and dessert. Meanwhile, a high-quality protein diet and more physical exercise should be encouraged.
2.Long-term surgical results for congenital aural atresia and hearing reconstruction.

OBJECTIVETo evaluate the stability of hearing results and complications in long-term following-up who underwent reconstruction surgery.
METHODSSix hundreds and seventy five cases (700 ears) of congenital aural atresia were reviewed from January 1984 to January 2001 at the Department of Otorhinolaryngology Head and Neck Surgery, Tongren hospital. Except 40 ears undone hearing reconstruction, 635 cases (660 ears) underwent long-term following-up for 3 to 19 years, with an average of 7.9 years.
RESULTSStenosis and recurrent infection of the external auditory canal (EAC) were the most frequent complications. Stenosis was seen in 120 ears, and 2 ears re-atresia, with an incidence of 18.48% (122/660). Recurrent infection of the cavity and canal skin happened in 6 ears. Closure of the air-bone gap (ABG) post-operation were gained in all cases, and ABG gains 20 dB or more occurred in 512 ears (77.57%), but 30 dB or more in 231 ears (35%). Following-up results: Stable hearing results gained in 450 ears over the length of following-up; the hearing worsened than that of 3 weeks postoperatively occurred in 160 ears, including 2 ears with sensorineural hearing loss. Hearing deteriorated more than 20 dB happened in 35 ears, and 10-15 dB in others cases but still be improved compared with that of preoperation.
CONCLUSIONSAtresiaplasty surgery in individuals with congenital aural atresia can yield reliable, lasting hearing results in 68.2% (450/660), with a low incidence of complications; the initial improved hearing deteriorated gradually over the first 6 months post-operation, which are related with the stenosis and infection of canal. Cavity adhesion, bony EAC re-growth, ossicular chain re-fixation or displace may affect the hearing results in some cases. Even unilateral aural atresia may benefit from the reconstruction surgery and achieve serviceable hearing results.

Background:Transcranial alternating current stimulation (tACS) offers a new approach for adult patients with major depressive disorder (MDD).The study is to evaluate the efficacy and safety of tACS treating MDD.Methods:This is an 8-week,double-blind,randomized,placebo-controlled study.Ninety-two drug-naive patients with MDD aged 18 to 65 years will receive 20 daily 40-min,77.5-Hz,15-mA sessions of active or sham tACS targeting the forehead and both mastoid areas on weekdays for 4 consecutive weeks (week 4),following a 4-week observation period (week 8).The primary outcome is the remission rate defined as the 17-item Hamilton depression rating scale (HDRS-17) score ≤7 at week 8.Secondary outcomes are the rates of response at weeks 4 and 8 and rate of remission at week 4 based on HDRS-17,the proportion of participants having improvement in the clinical global impression-improvement,the change in HDRS-17 score (range,0-52,with higher scores indicating more depression) over the study,and variations of brain imaging and neurocognition from baseline to week 4.Safety will be assessed by vital signs at weeks 4 and 8,and adverse events will be collected during the entire study.Discussion:The tACS applied in this trial may have treatment effects on MDD with minimal side effects.
5.Effect of electroacupuncture at "Zusanli" (ST 36) on duodenal mast cells, NGF and NTRK1 in rats with functional dyspepsia.

OBJECTIVE:
To observe the effect of electroacupuncture (EA) at "Zusanli" (ST 36) on duodenal mast cells, nerve growth factor (NGF) and neurotrophic tyrosine kinase receptor type 1 (NTRK1), and to explore the mechanism of electroacupuncture at Zusanli (ST 36) on functional dyspepsia (FD).
METHODS:
Sixty SPF-grade 10-day-old SD rats were randomly divided into a normal group, a model group, a ketotifen group and an EA group, 15 rats in each group. The FD model was prepared by iodoacetamide combined with rat tail clamping method in the model group, the ketotifen group and the EA group. The rats in the ketotifen group were injected intraperitoneally with ketotifen (1 mg•kg-1•d-1) for 7 days; the rats in the EA group were treated with EA at bilateral "Zusanli" (ST 36), with disperse-dense wave, frequency of 2 Hz/50 Hz and intensity of 0.5 mA, 20 min each time, once a day for 14 days. The gastric emptying rate and small intestinal propulsion rate in each group were observed; the morphology of duodenal mucosa was observed by HE staining; the toluidine blue staining was used to observe the number and degranulation of mast cells in duodenal mucosa; the protein and mRNA expressions of NGF, NTRK1 in duodenum were detected by Western blot and real-time PCR; the level of interleukin-1β (IL-1β) in duodenum was measured by ELISA.
RESULTS:
Compared with the normal group, the gastric emptying rate and small intestinal propulsion rate in the model group were decreased (P<0.01); compared with the model group, the gastric emptying rate and small intestinal propulsion rate in the ketotifen group and the EA group were increased (P<0.01); the small intestinal propulsion rate in the EA group was higher than that in the ketotifen group (P<0.01). In the model group, local defects in duodenal mucosa were observed with a small amount of inflammatory cell infiltration; no obvious abnormality was found in duodenal mucosa of the other groups. Compared with the normal group, the mast cells of duodenal mucosa in the model group were increased significantly with significant degranulation; compared with the model group, the mast cells of duodenal mucosa in the ketotifen group and the EA group were decreased significantly, and the degranulation was not obvious. Compared with the normal group, the protein and mRNA expressions of NGF, NTRK1 as well as the level of IL-1β in duodenum in the model group were increased (P<0.01); compared with the model group, the protein and mRNA expressions of NGF, NTRK1 as well as the levels of IL-1β in duodenum in the ketotifen group and the EA group were decreased (P<0.01, P<0.05); compared with the ketotifen group, the mRNA expression of NGF, as well as the protein and mRNA expressions of NTRK1 in duodenum in the EA group were decreased (P<0.05, P<0.01).
CONCLUSION
EA at "Zusanli" (ST 36) could inhibit the activation of duodenal mast cells and regulate the expressions of NGF and its receptor to improve the low-grade inflammatory response of duodenum, resulting in treatment effect on FD.

6.An analysis on senior pupil's tobacco use and exposure in Shandong province in 2012 and 2019.

Objective: To analyze the changes in tobacco use and exposure in primary school students in Shandong province in 2012 and 2019. Methods: A multi-stage stratified cluster random sampling method was used in the survey. In 2012 and 2019, 5 861 and 4 021 students from 3 different cities of Shandong province were selected as the study population. The questionnaire was filled anonymously by the subjects. χ2 test was conducted to compare the difference of groups. Results: In 2012 and 2019, the rate of attempting smoking among pupils under this study in Shandong province were 6.0%and 6.3%, respectively, while the current smoking rate were 1.2%and 2.3%, respectively. The sex ratio of male and female students attempting to smoke was 2.56∶1 in 2012 and 1.31∶1 in 2019. The sex ratio of current smoking rate was 2.43∶1 and 2.00∶1, respectively in 2012 and in 2019. The rate of tobacco exposure in the public places was 50.5%and 41.4%, respectively. The rate of tobacco exposure in family was 49.7% and 46.4%, respectively. Two rates of tobacco exposure decreased, but the reduction in family (3.3%) was far less than that in public places (9.1%). In 2019, the rate of tobacco exposure in family was higher than that in public places. Conclusions: The tobacco exposure rate declined in senior pupils in Shandong province. However, the situation is still grim for the current smoking rate, growth trend of girls tobacco use, and tobacco exposure in family.

7.Timeliness of health science popularization in local media of Shanghai during the outbreak of coronavirus disease 2019

ObjectiveTo analyze the timeliness of health science popularization during the outbreak of coronavirus disease 2019 (COVID-19) and its correlation with the epidemic situation and policies. MethodsThe original reports of health science popularization by 26 major media in Shanghai during the COVID19 outbreak between January 19 and March 25 of 2020 were retrieved, and the timeliness of the number of reports, media sources and categories, and contents were analyzed. ResultsDuring the epidemic of COVID-19, public media reported timely. Online media accounted for 63.35%, and text-based reports accounted for 85.90%. There was a correlation between the trend of the number of reports and the development of COVID-19 and the prevention and control policy issued by the government. After the change in the epidemic or the release of policy, the number of reports increased. The top four topics were personal protection, disease treatment, healthy lifestyle and psychological health, accounting for 18.62%, 18.54%, 12.96% and 11.74%, respectively. Reports focused on different aspects at different stages of COVID-19 epidemic, and the number of reports tended to increase one week after the occurrence of major events. ConclusionDuring the COVID-19 epidemic, the coverage of health science on media is timely and targeted. For future similar public health emergencies, it is suggested to respond quickly to major events according to the trend, to carry out publicity timely, to innovate the report forms, and thus make it easy for the public to accept and implement.
