1.Glycemic control in diabetic patients during perioperative period

Today,more and more patients with diabetes are requiring surgical intervention.Not only the metabolic disturbance of carbohydrate will be aggravated by surgical stress,but also the risk of perioperative complications and mortality have been greatly increased in patients with diabetes.Therefore,a complete preoperative assessment and blood glucose management of preoperative,intraoperative,and postoperative periods are needed to get full assurance for diabetic patients to undergo surgery and to pass through the perioperative period smoothly.
2.Level of Blood 8-iso-Prostaglandin F_(2a) and the Intervention Effect of N-acetylcysteine on Neonatal Rats with Hypoxic-Ischemic Encephalopathy

Objective To explore the changes of blood 8-iso-Prostaglandin F_(2a)(8-iso-PGF_(2a))content in different time after building-model and study its significance,and study the intervention effect of N-acetylcysteine(NAC) on it.Methods Seven-day-old Sprague-Dawley (SD) rats were randomly divided into 3 groups:intervention,operation and false operation groups.rats in intervention group were administered with NAC 0.1 mg/(g?d),Rats in operation group were given an equal volume of normal saline,and rats in false group operation had no administration.The blood was obtained to determine serum of 8-iso-PGF_(2a) content in 30 minutes,on the first,third,seventh and twenty first day after the building-model,respectively.EIA assay method was used to determine serum 8-(iso-PGF)_(2a) content.Results Serum 8-iso-PGF_(2a) concentration in rats from operation group,intervention and false operation groups were(168.7?(24.2)),(120.9?23.4) and (50.0?8.8) ng/L within 30 minutes after the building-model, respectively.There were a remarkable difference between serum 8-iso-PGF_(2a) concentration in three groups(P0.05).Conclusions The concentration of serum 8-iso-PGF_(2a) of rats rises remarkably early after building-model, and the peak concentration appears at 30 minutes after the building-model. NAC can effectively decrease serum 8-iso-PGF_(2a) content, which suggest that there is large value of clinical administration.
3. Chemical constituents of Artemisia anomala

Objective: To study the chemical constituents of Artemisia anomala. Methods: Various chromatographic techniques were adopted to separate the constituents, and the spectroscopic analysis was used to identify their structures. Results: Seven compounds were isolated and identified as (4aS, 7S, 7aR)-methyl 7-hydroxy-7-methyl-1, 4a, 5, 6, 7, 7a-hexahydrocyclopenta [c] pyran-4- carboxylate (1), rehmaglutin D (2), (E)-6-hydroxy-2, 6-dimethylocta-2, 7-dienoic acid (3), chrysoeriol (4), luteolin (5), apigenin (6), and p-coumaric acid (7). Conclusion: Compound 1 is a new compound named artanoiridoid; Compound 2 is firstly reported in this genus; Compounds 3, 4, and 7 are separated from this plant for the first time.

7.Investigation and Analysis on Clinical Application of Proprietary Chinese Medicine by Clinicians in Integrat-ed Medical Institutions

OBJECTIVE:To understand the application of proprietary Chinese medicine by clinicians in integrated medical insti-tutions,and provide reference for its rational use. METHODS:The questionnaire was designed,including related issues of proprie-tary Chinese medicine clinical application(such as medication basis of proprietary Chinese medicine,combined medication,clini-cal efficacy and its influencing factors,medication for special groups and so on)and suggestions;the questionnaires were delivered to the clinicians who worked in some second and third grade of integrated medical institutions located in southwestern regions and Chengdu,and the questionnaires was in the form of an anonymous;relevant data of available questionnaire were collected statisti-cally;the application situation of proprietary Chinese medicines and the existence question were analyzed,and suggestions were put forward. RESULTS:Totally 300 questionnaires were sent out,264 were effectively received,with effective rate of 88.0%. The clinicians involved in investigation were mainly in undergraduate,mostly the resident physicians and attending physician;only 6.8%of the physicians received the training of basic theory of traditional Chinese medicine after work;36.4%physicians often pre-scribe proprietary Chinese medicines,and the medication basis was mainly in package insert (64.0%) and clinical experience (55.3%);51.1% thought unclear indications affected the application of proprietary Chinese medicines,64.0% thought package in-sert can not meet clinical needs. In terms of answers in brief questions,the surveyed clinicians generally considered the package in-sert was too simple,which needed further perfected. CONCLUSIONS:The problems in clinical application of proprietary Chinese medicines should not be ignored. The relevant departments should strengthen the supervision of clinical application of proprietary Chinese medicines,and strengthen the re-evaluation of proprietary Chinese medicines market,as to provide data support for the continuous improvement of package insert. Medical institutions should carry out training timely for the physicians who prescribe pro-prietary Chinese medicines,the pharmacists should strengthen checking of proprietary Chinese medicines prescription and special prescription comment,and ensure prescription checking and comment play a active role in promoting rational drug use based on“proprietary Chinese medicines clinical application guiding principle”“prescription management method”.

Bacterial biofilm is responsible for a wide variety of nosocomial infections.Many chronic infectious diseases are closely correlated with biofilm formation in the clinic.As the biofilm growth,bacterial clusters detachment and dispersal from mature mixed-species biofilms will result in repeated acute infections.Here reviews three distinct biofilm dispersal strategies: swarming dispersal,clumping dispersal and surface dispersal.Many factors,such as heredity and increased fluid shear in the environment,contribute to this complex and dynamic process.Fundamental investigation upon biofilm will benefit clinical therapy of biofilm-associated infections.

?-(1,3)-D-glucan is a core structure of many bioactive fungal polysaccharides. It has drawn intensive attentions in recent because of its multiple bioactivities, particularly the immunomodulatory and antitumorous actions. In this review, we introduced the studies of structure-activity correlation of these ?-(1,3)-D-glucans, particularly the contributions of main backbone, branch degree and residues, molecular weight and conformation to the activities. We also summarized the recent progress in the detective methods for both quantitation and qualification using limulus G factor assay,galactosyl ceramide ELISA and etc.
10.CpG-Oligodeoxynucleotide is the ideal adjuvant for development of a Helicobacter pylori vaccine

Objective To investigate whether a novel mucosal adjuvant (DNA containing six base pair motifs consisting of an unmethylated CpG dinucleotide flanked by two 5′ purines and two 3′ pyrimidines, CpG Oligodeoxynucleotide, CpG ODN),which has not been shown to have significant toxicity,could be an ideal mucosal adjuvant for the development of a H. pylori vaccine in mice model. Methods C57BL/6 mice were orally or intranasally immunized with H. pylori whole cell sonicate(WCS) / cholera toxin (CT) or WCS /CpG ODN, and the corresponding control groups were set. Mice were dosed once a week for four weeks. One week after the last immunization, all animals were challenged by live H. pylori (5?10 8) three times in a five day duration. Two and 8 weeks after the last challenge, all animals were sacrificed to examine infection of H. pylori. Sera, saliva, gastric juice were collected to measure the concentrations of IgG, IgG1, IgG2a and IgA by ELISA. Results The protecting rates against H. pylori infection were 75%(9/12), 70% (7/10) and 0 (0/10) in the group of WCS/CT orally, WCS/CpG ODN intranasally and WCS/CpG ODN orally, respectively. Significantly higher levels of serum IgG2a antibody was found in the group immunized with WCS plus CpG ODN than those found in the sham immunized controls ( P
