2.Role of postsynaptic density 95 in Parkinsonian rat models receiving chronic L-dopa treatment

Objective To explore the role ofpostsynaptic density 95 (PSD-95) in Parkinsonian rat models receiving chronic L-dopa treatment. Methods The hemi-parkinsonian rat models were established by stereotaxically injecting 6-hydroxydopamine (6-OHDA) into the right medial forebrain bundle of 60 rats. Thirty-two of them were successfully induced into models with PD and equally randomized into PD group, L-dopa plus physiological saline treatment group, L-dopa plus PSD-95antisense oligonucleotide treatment group and L-dopa plus TE treatment group (n=8). The rats in the PD group were treated intraperitoneally with 0.2% ascorbic acid physiological saline water. The rats in the for 22 d. On the following 3 days, the rats were performed intraperitoneal injection of physiological saline water, injection of PSD-95 antisense oligodeoxynucleotide and TE buffer in the striatum,respectively. Another 8 rats were chosen as normal control group, only given solvent. Rotational reaction time and the largest number of revolutions within 5 minutes of rotation were estimated on the 25th d of injection. After rats being sacrificed, Mrna and protein expressions of PSD-95 at the corpus striatum were observed by RT-PCR and Western blotting, repectively. Results After chronic treatment with L-dopa methylester, the rotational reaction time was prolonged and the the largest number of revolutions within 5 minutes of rotation of PD rats was decreased in the L-dopa plus PSD-95 antisense oligodeoxynucleotide group on the 25th d, as compared with those in the L-dopa plus physiological saline water group and L-dopa plus TE group (P<0.05). In the lesioned striatum of PD rats, the abundance of PSD-95 Mrna and protein expressions were statistically and evidently reduced as compared with those in the control group (P<0.05). After chronic treatment of PD rats with L-dopa, the abundance of PSD-95mRNA and protein expressions were statistically and evidently increased as compared with that in the PD group (P<0.05). Pretreatment with PSD-95 antisense oligodeoxynucleotide statistically and obviously reduced the abundance of PSD-95 protein expression as compared with pretreatment with chronic L-dopa (P<0.05). Conclusion PSD-95 may play a significant role in the pathogenesis of motor complications of rats with PD receiving chronic L-dopa treatment.
3.Mid- and long-term follow-up for the graft patency after coronary artery bypass graft surgery

Objective To analyze the mid- and long-term results after coronary artery bypass graft surgery (CABG), to evaluate the accuracy of multi-detector CT coronary angiography (MDCT) on the graft patency status and to compare the patency for different grafts. Methods One hundred and one cases underwent CABG from June 1992 to March 2008 were followed up by searching the database of MDCT (42 cases) and selective coronary angiography (SCA, 59 cases). The mean following up period was (66.79 ±44.27) months. Three hundreds and 10 grafts including 115 arterial and 195 venous were analyzed.A comparison of the patency rate between the arterial and venous grafts was also carried out. At the mean time, 2 groups were divided according to the paft lesions( patency of not). Uunivariate analysis and Logistic regression analysis were made for statistics. Results At the follow-up of (53.93 ± 36.80) months, the patent rate of LIMA, RA, SV and CV examined by MDCT was 94.7%, 92.0%, 85.9% and 60.0%, respectively. The patent rate for MDCT patients with angina was 83.5%, and 95.2% for angina-free group ( P = 0.046). At the follow-up of (75.95 ±47.09) months, the patent rate of LIMA, RA, SV and CV examined by SCA was 87.1%, 81.0%, 53.6% and 57. 1%, respectively. In SCA patients the patent rate was 62.0% for angina group and 100% for angina-free group ( P = 0.025 ). According to logistic regression analysis, only the postoperative period was statistically related to the graft lesion. Conclusion MDCT could find out the occlusions and obvious stenoses of the grafts after CABG. It might be served as a noninvasive method to evaluate the grafts patency postoperatively. The mid- and long-term patency of arterial grafts is better than venous grafts. Graft lesion is an important factor leading to postoperative recurrent angina pectoris. The severity of the graft lesion has a close relation to the lenth of postoperative period.
4.Factors influencing the operative results of displaced intra-articular calcaneal fracture

Objective To determine the factors influencing the results of open reduction and internal fixation for displaced intra-articular calcaneal fracture (DIACF).Methods From May 2009 to June 2013, 80 patients with DIACF involving in 91 feet were treated by open reduction and internal fixation.Mean age was 38.7 years (range, 18-72 years).Patients were grouped according to the possible factors related to the operative results, including the Sanders classification of fracture, quality of fracture reduction, postoperative B(o)hler angle, width of postoperative calcaneus, and time to surgery after injury.Clinical result was evaluated and compared within each group.Results Mean duration of follow-up was 18.3 months (range, 14-26 months).The clinical results were excellent for 67 feet (74%), good for 12 feet (13%), fair for 7 feet (8%) and poor for 5 feet (5%), which yielded 87% good to excellent results.Good to excellent results in Sanders Ⅱ , Ⅲ and Ⅳ groups reached 94%, 85% and 53% respectively, in quality of fracture reduction ≤ 2 mm and > 2 mm groups reached 93% and 30% respectively, in postoperative B(o)hler angle < 15° and ≥15° groups reached 33% and 90% respectively;in broadening of postoperative calcaneus < 1 cm and ≥ 1 cm groups reached 92% and 17% respectively, in time to surgery after injury ≤14 days and > 14 days groups reached 91% and 43% respectively, in age ≤60 years old and > 60 years old groups reached 85% and 88% respectively.Except for the age group, the clinical result differed significantly within group (P < 0.05).Conclusions Fracture type, reduction quality, postoperative B(o)hler angle, width of postoperative calcaneus, time to surgery after injury are the factors influencing on the operative results of patients with displaced intra-articular calcaneal fracture.Thereupon the unfavorable influences should be avoided to improve the operative results.

Objective To compare the influences of off-pump and on-pump coronary artery bypass grafting operation on plasma thyroid hormones.Methods Forty patients undergoing coronary surgery were randomly divided into off-pump group(n=20)and on- pump group(n=20).Plasma thyroid hormones(T_3,T_4,FT_3,FT_4,rT_3)and thyroid-stimulating hormone(TSH)were studied perio- peratively.Results Before operation,the values of aU the variables were within normal range in the two groups.In off-pump group, T_3 and FT_3 concentration dropped significantly after operation,reaching the lowest value at first day postoperatively[T_3:(39.7?6.2) ng/dl vs.(78.6?2.1)ng/dl,P0.05),which were within normal range all along,rT_3 concentration rose significantly after operation,reaching the highest value at third day postoperatively[(82.1?26.8)ng/dl vs.(42.3?15.6)ng/dl, P0.05),whereas baseline,operative and postoperative variables were similar.Con- clusion Off-pump operation has some influences on plasma thyroid hormones,showing"low T_3 syndrome"postoperatively,that is similar to what on-pump operation does.
6.Study on correlation between the pathological changes under arthroscopy and the cytokine levels in the knee osteoarthritis of the Blood Stasis type.

OBJECTIVETo explore the correlation between cytokine levels and the pathological changes under arthroscopy in knee osteoarthritis of Blood Stasis type.
METHODSFrom 2009.2 to 2010.3, 90 patients with knee osteoarthritis were reviewed. Among the patients, 17 patients were male and 73 patients were female, ranging in age from 40 to 70 years, averaged 57.2 years, the duration of the disease ranged from 1 month to 10 years, with a mean of 3.4 years. Thirty-one patients had osteoarthritis in left knee, and 59 patients in right knee. The patients had the syndrome of blood stasis. All the patients had pain and morning stiffness; most patients had joint interlocking; and all the patients didn't have joint swelling. The synovial fluid was collected before surgery, and ELISA was used to detect the contents of interleukin-1beta and transforming growth factor-beta1. At the same time, the pathological changes of the joint were observed under the arthroscopy. Based on the above datum analysis, the severity of knee osteoarthritis of blood stasis type was studied, and the correlation between different types of pathological changes under arthroscopy and cytokine levels was analyzed.
RESULTSThe contents of IL-1beta and TGF-beta1 in synovial fluid were (28.18 +/- 5.57) pg/ml and (51.69 +/- 6.56) pg/ml respectively. The level of IL-1beta of grade III-IV cartilage degeneration was (30.65 +/- 3.48) pg/ml, which was significantly higher than (20.55 +/- 3.50) pg/ml of grade I-II cartilage degeneration group; the level of TGF-beta1 of grade I-II cartilage degeneration was (58.18 +/- 3.98) pg/ml,which was significantly higher than (49.59 +/- 5.83) pg/ml of grade II-IV cartilage degeneration group. IL-1beta and cartilage degeneration was positively correlated, the correlation coefficient was 0.744; TGF-beta1 and cartilage degeneration was negatively correlated, the correlation coefficient was -0.563. The level of IL-1beta of grade II-III synovial hyperplasia was (33.48 +/- 2.95) pg/ml, which was significantly higher than (25.40 +/- 4.50) pg/ml of grade I synovial hyperplasia group; IL-beta was positively correlated with synovial hyperplasia, the cor- relation coefficient was 0.801. The levels of IL-1beta of grade I osteophyte formation was (34.18 +/- 2.69) pg/ml, which was significantly higher than (25.74 +/- 4.48) pg/ml of grade 0 osteophyte formation group; the level of TGF-beta 1 of grade 0 osteophyte formation was (53.11 +/- 6.78) pg/ml, which was higher than (48.21 +/- 4.47) pg/ml of grade I osteophyte formation group. IL-1beta was positively correlated with osteophyte formation, the correlation coefficient was 0.762; TGF-beta1 was negatively correlated with osteophyte formation, the correlation coefficient was - 0.340.
CONCLUSIONAll the patients with knee osteoarthritis identified as blood stasis syndrome have pathological changes such as articular cartilage degeneration and synovial hyperplasia. The level of IL-1beta has important reference value to estimate the severity of cartilage degeneration, synovial hyperplasia and osteophyte proliferation.

7.Study on multi-target optimization of prescription dose of Mahuang decoction.

OBJECTIVETo optimize the prescription dose of Mahuang decoction in a multi-target manner, in order to provide reference for the quantitative optimization of the prescription dose of the traditional Chinese medicine compound.
METHODThe number of diaphoretic spots in rats, the tracheal antispasmodic rate in guinea pigs and the writhing times by acetic acid in mice were taken as the indexes for evaluating the diaphoretic, antispasmodic and analgesic effects. According to the experimental results of the 16 orthogonal combination prescriptions, a mathematical dose-effect model was built by support vector regression (SVR) and quadratic response surface regression (RSR) respectively. The multi-target optimization was achieved by elitist non-dominated sorting genetic algorithm (NSGA-II) and entropy weight TOPSIS method.
RESULTThe optimal dose of Mahuang decoction after being optimized by SVR modeling contained 17.71 g of Ephedrae Herba, 9.57 g of Cinnamomi Ramulus, 11.75 g of Armeniacae Semen Amarum and 4.39 g of Glycyrrhizae Radix et Rhizoma Praeparata Cum Melle. The optimized result by RSR modeling contained 13.37 g of Ephedrae Herba, 11.61 g of Cinnamomi Ramulus, 11.98 g of Armeniacae Semen Amarum and 5.67 g of Glycyrrhizae Radix et Rhizoma Praeparate Cum Melle. SVR was superior to RSR in both of the forecast capacity and optimization results.
CONCLUSIONSVR-NSGA-II-TOPSIS method could be adopted for the multi-target optimization for the dose of Mahuang decoction and other traditional Chinese medicine compounds. It is proved to be the optimal prescription with the best efficacy, and could provide scientific quantitative basis for determining the dose of traditional Chinese medicine compound prescriptions and developing new traditional Chinese medicines.

8.Comparison of curative effect between two mesh suspensions in treating female stress urinary incontinence

ObjectiveTo evaluate the curative effect difference between Tong's anterior mesh suspension and modified tension-free vaginal tape-obturator (TVT-O). Methods75 SUI cases were random divided into two groups: Group A (35 cases) were accepted Tong's anterior mesh suspension and group B (40cases) were accepted modified tension-free vaginal tape- obturator. Curative effects, operation safety,and complications and so on, were recorded and compared. ResultsThere had no statistical differences between these two groups on operation time, bleeding lose, operation injury, immediate postoperative urinary retention andcure rates, but the operation expense of group A was cheaper than group B [ (980. 74 ±212.45)yuan vs (2879.06 ±467. 13)yuan , P <0.05). ConclusionThe curative effect between two methods were similar. It's a little complicated and fit to be popularized in large hospital to modified tensionfree vaginal tape-obturator. Tong's anterior mesh suspension was performed under completely direct vision,not through pelvic cavity, and it was an economic, convenient and easy way, which not only fit to be popularized in basic hospital, but also a surgical remedial measure to the failures who accepted these operations,such as TVT, SPARC, IVS, MONARC, TOT, TVT-O, etc.
9.Quantification of acupuncture manipulation based on video motion capture

Objective To accurately measure the motion angels of hand-related joints during manipulations of acupuncture needle thrusting-pulling and twirling, so as to provide quantitative references for acupuncture manipulation. Methods Six acupuncturists with over 3-year acupuncture experience and one volunteer were enrolled in this study. The angles of the forearm, wrist, metacarpophalangeal & interphalangeal joints of the thumb and index finger in each acupuncturist when performing thrusting-pulling and twirling manipulation were measured by the video motion capture (VMC) system, and the different ranges of above-mentioned angles among the 6 acupuncturists when performing 10 trails of thrusting-pulling and twirling manipulations on the volunteer’s thigh were compared. Results There was no significant difference in the ranges of relevant hand-related angles in the acupuncturist (P>0.05). The manipulation of thrusting-pulling was mainly managed by the wrist joint, with the range of (7.23±1.87)°, while the manipulation of twirling was mainly managed by the interphalangeal joints of the index finger, and the range of the first and second interphalangeal joints of the index finger was (28.33±2.18)°and (10.43±1.69)°, respectively. Conclusions The VMC can be a reliable method to quantify the parameters of acupuncture manipulation. Different acupuncture manipulation shows particular variation of the joint angles, which can be used as a reference to quantify the acupuncture manipulation.
10.Method for analyzing the signal of hip joint angles during human walking using Hilbert-Huang transform

Objective To propose a method for analyzing the hip joint signals during human walking based on Hilbert-Huang transform (HHT) method and verify its feasibility. Methods First, the hip joint angles of one healthy subject were measured by using the hip joint measuring platform composed of acceleration sensors and gyroscopes. Second, all intrinsic mode functions (IMFs) at different scales, which could be further analyzed and combined, were obtained by applying the ensemble empirical mode decomposition (EEMD) to original signals. Finally, the Hilbert spectrum of original signals were plotted and analyzed. Results The signals representing different motion modes as well as gait characteristics indicated by rotating track of the hip joint were obtained. The Hilbert spectrum could show the intra-wave frequency modulation in the main motion mode and the characteristics of walking frequencies. Conclusions This method can be used in rehabilitation and treatment of patients with gait diseases. By using this method, the characteristic signals of the hip joints at different frequency scales can be effectively decomposed, and the post-processing signals can be filtered and centrally corrected, so as to adaptively analyze gait signals of the patients.
