1.Correlation studies between oxygen uptaking efficiency and exercise endurance of patients with stable chronic obstructive pulmonary disease

Objective To study correlation between oxygen uptaking efficiency and exercise endurance of patients with stable chronic obstructive pulmonary disease (COPD).Methods From January 2013 to December 2014,50 patients with COPD plateau were selected as the research subjects,and according to the severity,28 cases of gradeⅠ -Ⅱ,22 cases of grade Ⅲ -Ⅳ.50 COPD patients with acute phase of disease were selected as disease control group.50 healthy people were selected as healthy control group.The forced vital capacity (FVC),forced expiratory volume in first second (FEV1 ),FEV1 /FVC,peak oxygen uptake (peak VO2 ),peak oxygen uptake of expected value (peak VO2% Mr Pred),oxygen uptake efficiency slope (OUES),oxygen uptake efficiency platform (OUEP),when the anaerobic threshold (OUE @ AT)such as index,and the oxygen uptake efficiency and movement endurance were detected and analyzed.Results FVC,FEV1 ,FEV1 /FVC index values of COPD group were less than those of the control group,the differences were statistically significant (all P <0.05),and COPD patients with acute phase of FVC(2.86 ±0.36)L,FEV1 (1.25 ±0.36)L,FEV1 /FVC(50.16 ±5.37)% index values were less than COPD stabilization [(3.12 ±0.46)L,(1.51 ±0.38)L,(60.16 ±6.16)%)],the differences were statistically significant (all P <0.05).The peak VO2 ,peak VO2% Mr Pred,OUES,OUEP,OUE @ AT index values of COPD group were less than those of the control group,the differences were statistically significant (all P <0.05),and COPD patients with acute phase of peak VO2 (1 072.35 ±271.21)mL/min,peak VO2%pred(62.79 ±6.31),OUES(1.59 ±0.29),OUEP (25.72 ±3.42),OUE@AT(26.75 ±3.12)index values were less than patients with COPD stabilization[(1190.67 ± 286.21)mL/min,(70.76 ±6.27),(1.89 ±0.31 ),(31.46 ±4.05 ),(31.62 ±3.16)],the differences were statistically significant (all P <0.05).As the stabilization of disease severity in patients with COPD was aggravated, peak,VO2 peak VO2% Mr Pred,OUES,OUEP,OUE @ AT declined (1 089.62 ±221.68)mL/min vs.(1236.75 ± 235.37)mL/min,(67.28 ±5.03)vs.(75.81 ±5.39),(1.62 ±0.18)vs.(2.04 ±0.25),(30.67 ±6.47)vs. (34.17 ±4.40),(30.31 ±3.42)vs.(36.59 ±3.63)(F =16.92,12.71,4.27,8.75,7.59,P <0.05).Patients with COPD stabilization OUES,OUEP,OUE @ AT peak oxygen uptake efficiency indexes and movement endurance targets were positively correlated with VO2% Mr Pred (r =0.451,0.462,0.451,P =0.019,0.012,0.26),there was a positive correlation with peak VO2 (r =0.446,0.452,0.446,P =0.023,0.011,0.29).Conclusion COPD patients oxygen uptake efficiency and movement endurance than healthy people,patients with COPD oxygen uptake efficiency and movement endurance is associated with illness and severity of COPD,and COPD patients exercise endurance on the oxygen uptake efficiency also has a certain degree of influence.
2.Aprotinin effect on patients' immunity system and complement during open heart operation

Objective:Study of aprotinin effect on patients immunity system and complement during open heart operation.Methods:Ig and complement in patients' blood were measured respectively before operation and on the operative day and the first,second,third postoperative day.Results:In both aprotinin and comparative groups,the plasma levels of IgG,IgA,IgM,IgE and complement C3,C4,CH50 dropped remarkably and began to rise from the first postoperative day.On the third postoperative day, the plasma levels of IgG,IgA and complement C3,CH50 basically restored to the levels before operation.While IgM,IgE and complement C4 in aprotinin group and all Ig with complement in comparative group did not.Conclusion:Aprotinin has not a protective function to patients' immnunity system and complement during open heart operation but it can improve their restoration postoperatively.
3.Factors influencing health insurance patients hospitalization expenses for three disease entities

Objective To identify the chief factors influencing health insurance patients' hospitali-zation expenses so as to provide basis for the control of irrational increase in medical expenses and reform in the mode of medical insurance payment. Methods All the medical insurance cases involving the three disease entities in two hospitals of a certain city for the past three years, which totaled 647, were collected and a statistical analysis was made of the possible influencing factors using multi-stepwise regression analysis. Results The factors influencing the health insurance patients' hospitalization expenses included choice of hospitals, length of stay, performance of operations, proportion of drug fees, and province- wide readjustment of medical service prices. Reducing length of stay and lowering proportion of drug fees were the key to controlling expenses. Conclusion It is imperative to formulate scientific diagnostic and treatment routines and expense standards for single disease entities, speed up and deepen reform in the medical insurance system, and, on the basis of trying out payment based on certain single disease entities, implement payment based on disease entities.
4. Antitumor effect of umbilical cord mesenchymal stem cells:research advances

Umbilical cord mesenchymal stem cells(UC-MSC)are a kind of stem cell group with high self-renewal ability and multi-directional differentiation potential. In recent years,studies have shown that UC-MSC is far more advan- tageous than the stem cells from other sources in terms of acquisition,storage and transplantation. UC-MSC also possesses the characteristics of secreting specific cytokines,playing an immunomodulatory role,or delivering”drugs”to the special tumor sites. Therefore,UC-MSC has become a promising tumor-targeting tool. This article reviews recent advances in the research on the antitumor mechanism and safety of UC-MSC.
5.Correlation between HLA-DQA1 allele and involved joints,gastrointestine and kidney in anaphylactoid purpura Han juvenile inhabited in lnner Mongolia

Objective To analyse the genetic susceptibility of HLA DQA1 allele to anaphylactoid purpura with involved joints (APIJ),gastrointestestine (APIG) and kidney (APIK) in juvenile Han inhabited in Inner Mongolia.Method There were 45 children with APIJ,39 with APIG and 32 with APIK and 90 normal children controls,all of which were proved to be no sibship and no rheumatic diseases and family history.Of them,HLA DQA1 allelic types were analysed by polymerase chain reaction sequence specific primer (PCR SSP) technique.Results Gene frequencies of HLA DQA10301 in APIJ,APIG and APIK groups (28 5%,24 9% and 25%,respectively) were significantly higher than those of control group (10 6%),and the difference had statistical significance( ? 2=12 008,7 639 and 6 882; P =0 001,0 006 and 0 009;RR=3 83,3 09 and 3 11;EF=0 36,0 295 and 0 297,respectively).In contrast,HLA DQA10302 allelic frequencies in APIJ,APIG and APIK groups (5 7%?3 9% and 6 5%,respectively) were significantly lower than those in control grorp (19%) respectively,and the difference had statistical significance ( ? 2=8 352,10 633 and 5 557; P =0 004,0 002 and 0 018;RR=0 24?0 159 and 0 272;PF=1 09?1 80 and 0 92,respectively).Conclusion The allele of HLA DQA10301 may be a susceptible gene for the Han children with APIJ,APIG and APIK in Inner Mongolia,while HLA DQA10302 gene may be the protective one.
6.Clinical observation of GEM+NVB regimen in retreatment of 24 Advanced NSCLC treated by TAX+DDP regimen

Objective To appraise the efficacy,survival rate and toxicity of GEM+NVB regimen in retreatment of 24 Advanced NSCLC treated by TAX+DDP regimen.Methods 24 patients with Advanced NSCLC received chemotherapy of GEM+NVB regimen.We evaluate the efficacy,survival rate and toxicity after two periods.Results Among these 24 patients,there was no complete response.Rates of partial response is 29.2 %,the average survival period is 28 weeks.One year survival rate is 34.3 %.Conclusions GEM+NVB regimen is effective as second-line chemotherapy for the Advanced NSCLC patients that have ever been treaded by TAX+DDP regimen.
7.Effects of Compatibility on Clinical Efficacy of the Prescription Containing Curcuma Zedoaria

OBJECTIVE:To investigate the role of compatibility on clinical efficacy action of the prescription containing Curcu-ma zedoaria,and to improve the pertinence of drug use by medical staff and clinical efficacy of C. zedoaria. METHODS:By re-viewing relevant literatures,the prescriptions containing C. zedoaria were selected to summarize the effects of compatibility on clini-cal efficacy of C. zedoaria. RESULTS:345 prescriptions containing C. zedoaria were collected,mainly including the compatibility of C. zedoaria with traditional Chinese medicine which promote the circulation of qi,activate blood and remove blood stasis,toni-fy deficiency,warm interior and clear heat. The prescriptions containing C. zedoaria with sparganii often included Rhizoma Sparga-nii and C. zedoaria,involving 219 prescriptions(13.94%). CONCLUSIONS:C. zedoaria have the effects of activating blood stag-nation,promoting the circulation of qi,removing food retention and relieving pain;different compatibility greatly influence the clinical efficacy of C. zedoaria;common couplet medicines are R. Sparganii and C. zedoaria,and it can play the role of activating blood stagnation,promoting the circulation of qi,removing food retention and relieving pain;Common Vladimiria Root combined with Rheum palmatum can promote the circulation of qi and relieve pain. Medical staff should prescribe the prescriptions and use drugs for the compatibility of C. zedoaria based on clinical demands.
8.Prevention of Amlodipine with ALT-711 on target organs remodeling in Spontaneously Hypertensive Rats

Objective To investigate the intervention of Amlodipine combined with alanine aminotransferase(ALT)-711 on blood pressure and preventing of target organ remodels of Spontaneously Hypertensive Rats(SHR),and to explore an optimized treatment design.Methods A total of 24 SHRs at 12-Week-old male were randomly divided into 4 groups,with 6 SHRs in each group.In the control group(Group A)the SHR were given normal saline 1 ml/(kg · d)via intragastric administration; the SHRs in Group B were treated with Amlodipine 1 mg/(kg · d) via ntragastric administration; the SHRs in Group C were treated with ALT-711 10 mg/(kg · d)via ntragastric administration;the SHRs were treated with Amlodipine 1 mg/(kg · d)combined with ALT-711 10 mg/(kg · d)via ntragastric administration in Group D.All groups were treated for 8 weeks,and the blood pressure in mouse tail were monitored.After 8 weeks,all SHRs were executed in the condition of anaesthedia.Right cervical artery,heart and kidney were measured through immunohistochemistry,histological stain and imagine analysis.The remodeling of vascular,heart,kidney were compared,and collagen volume fraction,advanced glycation end products (AGEs) and fibronectin (FN) expression were measured.Results (1) After treatment for 8 weeks,blood pressure of SHRs in the 4 groups were (184.72 + 15.14),(108.00 ±10.19),(155.64±10.03)and (99.83 ± 12.27) nun Hg in Group A,B,C and D,respectively.The blood pressure in Group D was the lowest in the 4 groups (F =14.6702,P < 0.05),whereas it is not significantly different from Group B (P > 0.05).(2) The mean thickness of carotid arteries were (0.450 ± 0.023),(0.380 ±0.021),(0.420 ±0.019) and (0.320 ±0.020) mm in Group A,B,C and D,respectively.The mean thickness of carotid arteries in Group D was the lowest in the 4 groups(F =4.1463,P <0.05).(3).The mean intimal area of carotid arteries were (19.0 + 1.3) × 105,(25.0 ± 1.4) × 105,(29.8 + 1.5) × 105 and (21.3 ±1.3) × l05 μm2 in Group A,B,C and D,respectively.The mean intimal area of carotid arteries in Group D was significantly smallest than the other 3 groups(F =4.4305,P <0.05).(4) The collagen content in different organs in the 4 groups after treatment 8 weeks:in carotid arteries,they were (6.25 ± 0.19)%,(3.56 ± 0.03) %,(4.25 ± 0.12) % and (1.32 ± 0.05) % in Group A,B,C and D,respectively.The collagen content in Group D were significantly lower than the other 3 groups (F =4.9316,P < 0.01).In myocardia:collagen content in Group A,B,C and D were (6.02 ± 0.21)%,(4.08 ± 0.14)%,(4.44 ± 0.11)% and (1.13 ± 0.08) % respectively,and it was the lowest in Group D (F =4.2469,P < 0.01).In kidney,the collagen content in Group A,B,C and D were (13.23 ±0.51)%,(7.28 ±0.35)%,(8.33 ±0.22)% and (5.66 ±0.13)% respectively,and it was the lowest in Group D(F=8.1038,P <0.01).(5).Express rate of AGEs in carotid arteries in Group A,B,C and D were (31.48 ±7.30)%,(20.55 ±5.91)%,(10.48 ±0.98)% and (7.56 ± 0.68) % respectively,with the lowest expression in Group D (F =11.4537,P < 0.01).Express rate of FN in carotid arteries in Group A,B,C and D group were (28.18 ±5.66)%,(17.26 ±5.83)%,(18.20 ±1.44) % and (15.12 ± 1.03) % respectively,with the lowest expression in Group D (F =9.0036,P < 0.01).Conclusion Amlodipine combined with ALT-711 decreased blood pressure more effectively and prevented the remodel of target organ in SHRs than Amlodipine or ALT-711 alone.
9.Epigenetic inactivation of SPOCK2 in the malignant transformation of ovarian endometriosis

Objective To investigate epigenetic inactivation of SPOCK2 gene in the malignant transformation of ovarian endometriosis ( EM ) by comparing the methylation status and protein expression of SPOCK2 gene in the malignant tissues,ectopic endometria and the eutopic endometria of endometriosis.Methods From Jan.2005 to Jan.2011,22 paraffin-embedded specimens diagnosed as malignant transformation of ovarian endometriosis ( EAOC ) including 11 cases with ovarian endometrioid carcinoma,8 cases with clear cell carcinoma,2 cases with serous cystadenocarcinoma and 1 case with mucous cystoadenocarcinoma matched with 22 cases with ovarian endometriosis and 16 cases with normal endometrium form cervical intraepithelial neoplasia(CIN) patients as controls in Department of Obstetrics and Gynecology of Shengjing Hospital.Twenty-two malignant tissues,15 ectopic endometria and 10 eutopic endometria were captured by microdissection in EAOC group; 22 eetopic endometria and 17 eutopic endometria were captured in EM group; 22 endometrium were captured in the NE group.The methylation statue of SPOCK2 was determined by combined bisulfite restriction analysis,and the protein expression of SPOCK2 was evaluated by immunohistochemistry.Results ( 1 ) Methylation of SPOCK2:in the EAOC group,the frequency of SPOCK2 hypermethylation in malignant tissue was 45% (10/22),which was significantly higher than 1/15 in the ectopic endometrium (P<0.05).There was no statistical difference of the frequency of SPOCK2 hypermethylation in ectopic endometrium in the EAOC group(1/15) and EM group (5%,1/22) (P>0.05).(2) SPOCK2 protein:the loss rate of SPOCK2 was 44% (11/22) in malignant tissue in EAOC group,which were significantly higher than 2/15 of in ectopic endometrium of EAOC (P < 0.05).However,there was no remarkable difference in loss rate of SPOCK2 protein between ectopic endeometrium of EAOC and endometrium of EM [ 2/15 vs.5% ( 1/22 ),P > 0.05 ].No significantly difference in loss rate of SPOCK2 in eutopic endometrium was observed among three groups ( P > 0.05 ).(3) The abnormal methylation of SPOCK2 could lead to loss expression of protein ( P < 0.05 ).Conclusion Epigenetic inactivation of SPOCK2 gene is involved in the malignant transformation of ovarian endometriosis.
