Treatment of male urinary incontinence is one of the most distressing problem in the urologic practice. Recent advances in implantable devices have improved the outlook for patients with incontinence, but the prosthetic surgery has mechanical problems to be solved, and does not maintain physiologic normality. Recently, we treated a case of postprostatectomy incontinence, and obtained a good result with surgical intercavernous embedding of the bulboperineal urethra.

3.Transurethral Dormia Dislodging in Patients with Lower Ureteral Stone.

Management of the small lower ureteral stone has been attempted by either transurethral manipulation or expectant therapy. To date, however, the choice of proper method has depended mostly on clinician's preference, and the effectiveness of the transurethral stone manipulation has remained controversial. Herein we evaluated the effectiveness of the transurethral stone manipulation, using with the Dormia dislodger, in 37 patients with lower ureteral stone less than 10mm in greatest diameter. The results are summarized as follows. 1. The overall success rate was 68% (25 of 37 patients); Immediate delivery of the stone was observed in 16 of 37 patients. In another 5 patients the stone was removed at the time when the indwelled ureteral catheter was taken out. In the remaining 4 patients the stone passed spontaneously within a week following the procedure. 2. Higher success rate was observed in smaller stone; 79% in less than 4 mm, 58% in 46 mm, and 33% in 610 mm in it's greatest diameter, respectively. 3. The morbidity rate was 8.1% (3 of 37 patients); In one patient a broken basket was retained in the ureter, which was removed at the time of ureterolithotomy. In another patient a ureteral stricture requiring periodic dilation was complicated. And in the other patient persistent hematuria was observed, which was relieved by conservative management. These results suggest that transurethral manipulation might have to be initially applied to unremittingly painful patients with small lower ureteral stone.

4.A Case of Retrocaval Ureter with Left Renal Agenesis.

Retrocaval ureter is a rare congenital anomaly of the urogenital organ originating from the venous maldevelopment. To our knowledge, retrocaval ureter with contralateral renal agenesis has not been reported in Korea. Recently, we experienced a case of retrocaval ureter with contralateral renal agenesis, and obtained a good result following ureteral resection with end-to-end anastomosis of the ureter.

6.Clinical Experiences of the Ureteroscopic Management for the Lower Ureteral Stone.

Ureteroscopic removal of the stone is now popular for the management of the lower ureteral stone. A clinical study was performed on our 75 patients with lower ureteral stone treated with ureteroscopy. Of the 75 stone manipulations 62 (83%) were immediately successful and the final success rate including spontaneous delivery of stone or fragment after the procedure was 87 percents (65 cases). Of 57 smaller calculi than 1 cm (radiographic largest diameter) 50 (88%) were removed successfully. Mean duration of postoperative hospitalization was 5.6 days. There were no interrelations between the success rate and anesthetic methods. Significant complications during or after procedure were not identified. We conclude that ureteroscopic removal of stones under direct vision can be done safely and be the first choice of procedure for the lower ureteral stones.

7.Complications of transurethral prostatectomy.

A clinical observation was made on the 22l consecutive patients who had underwent transurethral resection of the prostate(TURP) between Jan. 1984. and Dec. 1989 in the Department of Urology, Yeungnam University Hospital focussing on immediate and delayed complications. The operation was done by 3 urologists(P. K. S). The mean resection time of all patients was 98 minutes between 1984 and 1986(n=83) and 74 minutes between 1987 and 1989(n=138). The mean weight of resected prostatic tissue was 15.0 gm between 1984 and 1986 and 17.9 gm between l987 and 1989.The most common immediate complication was voiding difficulty (7.2%), the remainders were in order of nonspecific sepsis (3.2%), delayed bleeding (3.2%), bleeding requiring T-U fulguration tl.8%), post-TUR syndrome (1.4%) and epididymitis (1.4%). The immediate postoperative morbidity was 18.2% and risk factors for immediate morbidity after TURP were age than TO years. associated neurogenic bladder and prostatic cancer on biopsy. The most common delayed complication was obstruction requiring TURP(2.3%), the remainders were in order of urethral stricture(1.8%). urinary incontinence(0.9%), pulmonary embolism(0.9%), deep vein thrombosis(0.9%). The delayed morbidity was 6.8% and the only risk factor for delayed morbidity after TURP was resection time longer than 90 minutes. There were no deaths within 3 months of surgery, but operative morbidity rate still remains significantly high. Careful attention to surgical details and indications is needed to reduce the amount and significance of the postoperative morbidity.

8.Multiple Calcium-Uric acid Urolithiasis in a Child: Report of a Case.

Uric acid, pure or admixed, calculous disease during childhood is uncommon. We recently encountered a case of multiple calcium-uric acid urolithiasis in a l7-month-old male patient presented with anuria owing to bilateral complete obstruction of the ureteropelvic junction in which acute renal failure developed following emergency removal of the stones.

9.Value of Scrotal Ultrasonography in Diagnosis of Acute Scrotum.

Differential diagnosis of acute scrotum is very important. As the chances of salvaging testicular function are inversely proportional to the elapsed time between onset of torsion/rupture and surgical repair of testes, prompt and accurate diagnosis is imperative. 17 patients with acute scrotum who had visited to the Department of Urology, Yeungnam University Hospital from June l985 to May l986 underwent scrotal ultrasonography to evaluate the value of scrotal ultrasonography for the differentiation of the diseases that cause acute scrotum. Scrotal ultrasonographic findings in 8 patients with acute epididymitis were enlargements of the epididymes with decreased echogenicity in 7 and enlargement of the epididymis and testis with mixed echogenicity in 1. In 3 patients with testicular torsion ultrasonogram revealed enlarged epididymis and testis with homogeneous and slightly decreased echogenicity in 1 patient of acute stage (within 24 hours) and enlarged epididymis and testes with diffuse mixed echogenicity in 2 patients. In 6 patients with scrotal trauma ultrasonogram revealed extratesticular fluid collection with well-defined intact in 1 patient of intrascrotal hematoma. 2 patients with testicular hematoma showed well-defined testes with intratesticular focal echolucent region on ultrasonogram. 3 patients with testicular rupture revealed mixed echogenicity in an injured hemiscrotum with well-defined testes in 2 out of 3.
