1."Effect of the Implementation of National Essential Medicine System on the Operation of Primary Health Care Institutions in""Bull's-eye""City-Taking Chengdu as An Example"

OBJECTIVE:To provide reference for the further improvement of national essential medicines system. METHODS:Taking theBull's-eyecity Chengdu as an example,based on the trading areas,the existing problems in implementing essential medicine system were analyzed by qualitative and quantitative research methods,and countermeasures were put forward. RESULTS:From the point of view of quantitative data,the first level trading areas of both proportions of medical personnel per thousand ur-ban population and numbers of primary medical and health institutions per 100 thousands population in Chengdu were the lowest. From 2010 to 2012,the outpatient and inpatients increased 1.09 million people and 0.16 million beds per day in Chengdu primary health care institutions,the largest increase in the first level trading areas,and increase and decrease in the second and third level trading areas;outpatient and inpatient drug costs fell respectively 4 yuan and 109 yuan,medicine proportion declined 6.7%,in-crease in the first level trading areas,and decreased to varying degrees in the second and third level trading areas. From the point of view of qualitative interviews,the service ability got enhancement,but still could not meet the services demand,especially the pressure in the first level trading areas near center district remains heavy;drug cost was under controlled,but the subsidies needs to be increased,the per capita drug costs in the first level trading areas remains high;the drug income and medicine proportion were inhibited,especially the significant fell in the second and third level area,but the decrease of income may restrict develop-ment. CONCLUSIONS:It is suggested that more targeted drugs are needed for meeting the services demand,more sophisticated policy are needed for improving the subsidies,and more comprehensive medical conditions are needed for ensuring institutions'de-velopment.
2.Clinical study of small incision operation and traditional operation in treatment of infantile hernia

Objective To analyze the clinical effect of small incision operation and traditional operation in treatment of pediatric hernia,and to provide a theoretical basis for the clinical treatment of pediatric hernia.Methods 68 children with pediatric hernia were treated as the research subjects,they were randomly divided into control group and observation group,34 children in each group.The control group adopted the traditional operation treatment,while the observation group was taken small incision.The clinical efficacy and complications of both two groups were compared. Results The operation time,bleeding volume,hospitalization time and incision length of the observation group were (10.45 ±1.35)min,(5.21 ±0.78)mL,(1.98 ±0.76)d,and (1.04 ±0.23)cm,which were better than those of the control group [(15.67 ±3.24)min,(8.56 ±1.87)mL,(3.78 ±1.12)d,and (2.54 ±0.65)cm,respectively],the differences were significant (t =5.760 0,5.747 7,4.171 7,7.986 8,all P <0.05).The incidence rate of complica-tions of the observation group was 2.94%,which was significantly lower than that of the control group (11.76%),the difference was significant (χ2 =5.7118,P <0.05).Conclusion The clinical curative effect of using small incision in the treatment of pediatric hernia is significant,this method has the advantages of small operation wound,less pain, faster postoperative recovery,less complications and other advantages,it is worthy of clinical application.
4.Advance in Metabolism of n-Alkane and Metabolic Engineering of Candida tropicalis

Metabolism of n-alkane mainly includes ?-oxidation pathway and ?-oxidation pathway in Candida tropicalis,which are both catalyzed by a series of enzymes. The transformation from n-alkane to fatty acid is accomplished via ?-oxidation pathway,and the degradation of fatty acid is involved in ?-oxidation pathway. The mechanism on metabolism of n-alkane and studies on related enzymes are summarized in the review. Furthermore,some molecular genetics and metabolic engineering techniques related to Candida tropicalis developed in recent years are introduced.

Microenviroment hypoxia is one of the common phenomenon in cancer,studies have indiacat-ed that hypoxia induces genetic instability via activating many DNA danlage-repair signal pathways,which asso-ciates with tumor invasion and resistance and chemi-radiotherapy resistivity.Here,we overview three main DNA damage-repair pathways:mismatch repair,homologous recombination and non-homologous end joining,and how hypoxia influences their mechanisms.
6.Effects of light on the expression patterns of circadian clock-related genes in the peripheral lymphocytes

Objective To explore the effects of light on the expression patterns of clock and clock-related genes in peripheral lymphocytes.To develop basic knowledge needed for clinical application of the circadian clock.Methods One hundred Sprague-Dawley rats were housed under constant dark(DD)or normal light-dark(LD 12:12)conditions for six weeks.Peripheral iymphoeytes were collected at different time points.The expression level of the clock gene and melatonin receptor genes mt1 and mt2 were detected using semi-quantitative RT-PCR.The data were analyzed with cosine software.Results Circadian expression of the genes was observed in both groups,but the peak phase,amplitude and strength of expression of each gene differed with the light conditions.Conclusion Light influences the expression of clock and clock-related genes in rats' peripheral lymphocytes.The clock gene might play an important role in regulating the expression of mt1 and mt2.
9.A comparison of three fixations for intertrochanteric femoral fractures in the elderly

ObjectiveTo compare the efficacy of 3 internal fixations, locking proximal femoral plate(LPFP), ASIAN IMHS and InterTAN, for intertrochanteric femoral fractures in elderly patients.MethodsA retrospective study was done to analyse 185 elderly patients who had been treated for intertrochanteric femoral fractures from February 2008 to December 2009 in our hospital and fully followed up.They were treated with LPFP (group A, 95 cases), ASIAN IMHS (group B, 52 cases) and InterTAN (group C, 38 cases) . Statistical analyses were conducted to compare the 3 groups in operative time, blood loss,fluoroscopy exposure, hospital charges, intrraoperative complications and functional outcome(Harris hip score).Results The average follow-up was 14. 5 months(from 6 to 28 months). Compared with group A, groups B and C showed significant advantages in operative time, blood 1oss and functional outcome but significant disadvantages in fluoroscopy exposure and hospital charges ( P < 0. 05 ). Compared with group A,groups B and C had significant fewer implant failures ( P < 0. 05).There were no significant differences in all the indexes between groups B and C( P > 0. 05) .Conclusions The ASIAN IMHS and InterTAN appear to be more reliable than LPFP for the treatment of intertrochanteric femoral fractures in the elderly.There appear to be no significant differences between ASIAN IMHS and InterTAN.
10.Content Determination of Isoniazid in Isoniazid Tablets by Microfluidic Chip with Contactless Conductivi-ty Method

OBJECTIVE:To establish a method for the content determination of isoniazid in Isoniazid tablet. METHODS:Mi-crofluidic chip with contactless conductivity detection was adopted. The effects of electrophoresis parameters,such as the variety, concentration,pH,additive types and dosage,injection time and separation voltage on the separation detection were explored. RE-SULTS:The optimized buffer system was 1.86 mmol/L citrate-0.14 mmol/L sodium citrate buffer solution (pH3.0),no additive , the injection time was 20 s and the separation voltage was 2.5 kV. In the optimal conditions,the linear range of isoniazid was 10-200 μg/mL(r=0.9993);the limits of quantification and detection were 8.0 μg/mL and 2.4 μg/mL;RSDs of precision,stability and reproducibility tests were lower than 2.0%;recovery was 97.8%-102.9%(RSD=1.9%,n=9). CONCLUSIONS:The method is rapid and simple,and suitable for the content determination of isoniazid in Isoniazid tablet.
