2.Study on hypocarbia in newborns during mechanical ventilation

Objective To explore the causes and adverse effects of hypocarbia in newborns during mechanical ventilation. Methods Two hundred and forty-six newborns received assisted ventilation from Jan. 1994 to Dec. 2003 were retrospectively reviewed. Results The morbidity of hypocarbia was 14. 2%. It is common in preterm infants and the onset time of which were (31. 6?26. 9) hours after mechanical ventilation. The common primary disease was respiratory distress syndrome (RDS) (22. 6%). The mortality (32. 9%) and incidence of intracranial hemorrhage (20%) in hypocarbia cases were higher than those of the control group (7. 1 % and 5. 7%) (P
4.Neural stem cell transplantation and cerebral infarction

BACKGROUND:Neural stem cells transplanted into the infarct region can promote the recovery of damaged nerve cells after cerebral infarction.
OBJECTIVE:To analyze the relevant influential factors of neural stem celltransplantation in the treatment of cerebral infarction.
METHODS:In this paper, we analyzed the experimental studies concerning neural stem celltransplantation for cerebral infarction published in recent years based on database search. There were main two aspects for discussing the progress in neural stem cells and cerebral infarction, the Chinese clinical trial registration and basic experimental studies.
RESULTS AND CONCLUSION:After cerebral infarction, neural stem cellproliferation and differentiation is closely related to the brain microenvironment. Large amount of nerve cellloss can be found in the infarct region. Cytokines can play a role in the neural stem celltransplantation to repair neurological injury after cerebral infarction, and also can induce neural stem cellproliferation, differentiation and migration, including epidermal growth factor, brain-derived neurotrophic factor, insulin-like growth factor 1, nerve growth factor, and basic fibroblast growth factor. Acupuncture and traditional Chinese medicine can promote the proliferation, migration and differentiation neural stem cells in the subependymal zone after cerebral infarction. Neural stem celltransplantation in the treatment of cerebral infarction has yielded progress, but there are stil many issues that need to be resolved in the future.
5.Potency of a novel multi-epitope vaccine against foot-and-mouth disease type Asia 1 in guinea pigs

The potency of an improved recombinant multi-epitope vaccine against FMDV type Asia1 was evaluated in this study .A multi-epitope gene based on FMDV type Asia1 was designed and a recombinant expression plasmid (pRE-oIgG) was constructed .The proteins ,RE-oIgG and 3D were expressed in E .coli cells and purified with Ni-NTA agarose resin by affinity chromatography .The proteins ,RE-oIgG ,3D and RE-oIgG plus 3D ,were emulsified in an oil adjuvant ISA 206 .Twenty-five female guinea pigs were randomly divided into five groups and intramuscularly vaccinated for with RE-oIgG ,3D ,RE-oIgG plus 3D ,an inactivated FMDV vaccine (type Asia1) ,and PBS .All animals were vaccinated for two times .Anti-FMDV specific an-tibodies ,neutralization antibodies ,protection potency ,and lymphoproliferation assay were detected by ELISA ,virus neutrali-zation assay ,challenge test ,and flow cytometry ,respectively .Results showed that RE-oIgG plus 3D elicited significant high-level anti-FMDV specific antibodies compared to RE-oIgG alone (P<0 .05) .All the vaccinated animals induced higher level lymphoproliferation responses in vitro except PBS .Both 3D alone and PBS produced the negligible neutralizing antibodies and anti-FMDV specific antibodies .RE-oIgG plus FMDV 3D not only elicited high levels of anti-FMDV neutralizing antibodies ,but also induced significant lymphoproliferation responses .More importantly ,RE-oIgG plus 3D conferred complete protection to guinea pigs against challenge with 1 000 GPID50 .Interestingly ,two of five vaccinated animals with 3D alone were full protected against challenge ,and other three animals significantly showed a delay of 2-3 days in the onset of clinical signs .Therefore ,we considered that RE-oIgG plus 3D induces strong humoral and cellular immune responses ,which may be used for control and prevention of FMD in the future .
6.Blastoid mantle cell lymphoma involving the skin: a case report

A 53-year-old man presented with prunosus nodules and plaques on the trunk and extremities for half a month and with periorbital swelling for 4 days.Hematological examination in a local hospital showed thrombocytopenia,and pulsed corticosteroid therapy did not work.On physical examination,there were splenomegaly and multiple enlarged superficial lymphnodes.Serum calcium and 1-lactate dehydrogenase (LDH)were increased to 3.12 mmol/L and 853 U/L,respectively.Serum immunofixation electrophoresis evidenced the presence of a monoclonal immunoglobulin IgM (κ chain).Positron emission tomography (PET-CT) showed abnormal uptake in multiple lymph nodes,back wall of the pharynx,and spleen.The biopsy of a nodule in the neck revealed a diffuse infiltration of numerous atypical lymphoid cells in the subcutaneous fat tissue,which were medium-sized with round nuclei,obvious nucleoli and karyokinesis, Immunophenotyping of the abnormal lymphocytes indicated positive reactions for L26,CD79a,Bcl-2,cyclin D1,multiple myeloma oncogene 1 (partly),Ki-67 (>80%),but negative for CD5,CD21,CD23,CD38,CD3,CD10,Bcl-6,CD45RO,terminal deoxynucleotidyl transferase (TdT),myeloperoxidase (MPO),CD30,anaplastic lymphoma kinase (ALK),CD117,or CD34.Fluorescence in situ hybridization (FISH) revealed the presence of a fusion gene (t(11:14) CCND1/IGH) in the abnormal lymphocytes.Based on the above findings,the diagnosis was made as blastoid mantle cell lymphoma with skin and periorbital involvement complicated by hypercalcemia.After treatment with rituximab injection,cyclophosphamide,vincristine,doxorubicin,dexamethasone,and intermittent treatment with intravenous high dose of methotrexate and cytarabine (R-Hyper-CVAD),serum calcium returned to a normal level three days later,and the patient made a quick and excellent recovery on the 6th day,with the regression of skin lesions and poriorbital swelling.Unfortunately,the patient eventually died of severe pulmonary infection one month later.
7.Prognostic effect of number of lymph nodes sampled in patients with stage Ⅰ non-small cell lung cancer

Objective To investigate the prognostic effect of quantity of lymph node(LN)resected in operations of patients with stage Ⅰ non-small cell lung cancer(NSCLC).Methods The clinical, pathological and follow-up data of 74 patients with stage Ⅰ NSCLC who were treated with surgery from January 1998 to December 2002 Beijing Friendship Hospital, Affiliated to Capital Medical University were reviewed retrospectively.Grouping the patients, according to the quantity of lymph node resected, the Kaplan-Meier method and Cox proportional hazards model was used for univariate analysis and multivariate analysis of factors with prognostic effect.Results The five year survival rate and disease-free survival(DFS)rate of these 74 patients were 64.9% and 47.3%.The univariate analysis showed that tumor size(P =0.016),T-staging (P =0.008)and extent of lymph node dissection(P =0.013)could influence the survival rate.The 5-year OS and DFS rates of patients with less than 6 LNs resected were less than the other group(more than 6 LNs)apparently.The multifactorial analysis indicated that other than staging, the quantity of lymph node resected was also an influence factor of prognosis.Conclusions The OS rate of patients has positive correlation with quantity of lymph node resected in operations.Six LNs must be resected leastways in operations of patients with stage Ⅰ NSCLC.
8.Protection of ischemic postconditioning on cochlear ischemia and reperfusion injury in rats

OBJECTIVE To investigate the effect of ischemic postcondioning on cochlea following cochlear ischemia-reperfusion injury in rats. METHODSForty two healthy male SD rats were randomly divided into 3 groups, namely, the sham operation control group, the ischemia reperfusion group and ischemic postcondinging group. There were 14 rats in each group. The content of molondialdehyde(MDA) as well as the activity of catalase(CAT)in cochlea was measured by histochemistry. The ultrastructural changes of stria vascularis capillaries of the cochlea in experimental rats were examined by transmission electron microscopy(TEM). RESULTS In the ischemia reperfusion group, the CAT activities were decreased and MDA concentrations were increased significantly compared with those in the control group. However, in the ischemic postcondinging group, the activities of CAT were increased and MDA concentrations were decreased compared with those in the ischemia reperfusion group. Moreover, lesions were detected in the stria vascularis capillaries in all the three groups. The capillaries of stria vascularis were injured severely in ischemia reperfusion group. In the ischemic postcondinging group, the damage of capillary of stria vascularis were reduced significantly compared with that in ischemia reperfusion group, the structures were near normal, and no obvious destruction was observed. COCLUSION Ischemic postconditioning may markedly reduce the excessive generation of oxygen free radical during the process of ischemia-reperfusion injury and might be a potential strategy for its therapy.

10.Correlation between Sit-to-Stand Velocity and Dynamic and Static Balance in Stroke Patients

ObjectiveTo discuss the relationship between sit-to-stand velocity and dynamic and static balance respectively in stroke patients.Methods22 stroke patients were measured their time for Five Times Sit to Stand Test (FTSTS scores) and the static balance performance as well as swaying of center of gravity in lateral directions was recorded with GS-10A. In addition, the distances of functional reach test (FRT) when sitting were measured.ResultsBy using Pearson's correlation coefficient, FTSTS scores showed the moderate correlation with hemiparetic side, ENV. Area and distance of sitting FRT (r=-0.512, P<0.05;r=0.632, P<0.05;r=-0.662, P<0.01). Linear regression model, in terms of balance ability, showed the main factors associated with the sit to stand velocity were the distance of sitting FRT and ENV.Area. ConclusionENV.Area and distance of sitting FRT are the main factors associated with sit to stand velocity and both static and dynamic balance can predict sit to stand velocity.
