1.A retrospective study of the factors of death by sudden abdominal condition change in terminally ill cancer patients

Purpose: To investigate the characteristics of sudden abdominal condition change that occur in a palliative care unit, we evaluated the association between these conditions and family acceptance at time of death. Methods: We retrospectively investigated the medical records of 30 terminally ill cancer patients who died of sudden abdominal condition change in our unit between January 2010 and March 2013. We focused particularly on the course of sudden change, symptom relief, explanation of the condition, and family acceptance at time of death. Results: The average time from onset of sudden change in abdominal condition until death was 20.6 hours and the median was 13 hours. Of the 30 total cases, 23 occurred in less than 24 hours. Frequent symptoms during the sudden change were abdominal pain in 28 patients, decreased blood pressure in 21, lowering of consciousness in 12 and disturbed behavior/excitement in 10. Impending symptoms were observed in 18 patients. For patients with good acceptance by family at time of death (21 patients), symptom relief was significantly better, frequency of disturbed behavior/excitement was significantly less, and prior specific explanation was significantly frequent, compared to the poor acceptance group (9 patients). The good acceptance group tended to be have a longer hospital stay, as well as longer time between sudden change and death. Conclusions: Death by sudden abdominal condition change came within 24 hours in a majority of cases. We consider that prior detailed explanation and improving symptom relief helps improve family acceptance.
2.Association between the G252A Tumor Necrosis Factor-beta Gene Polymorphism and Medication-Overuse Headache.

BACKGROUND AND PURPOSE: Migraine patients are particularly prone to the complication of medication-overuse headache (MOH). Although it has been shown that A allele carriers for the tumor necrosis factor (TNF)-beta gene G252A polymorphism are at high risk of the development of migraine without aura, the relationship between the TNF-beta gene G252A polymorphism and MOH is unknown. We investigated whether the TNF-beta gene G252A polymorphism is involved in the aggravation of migraine by overuse of medications. METHODS: Forty-seven migraine patients (6 males and 41 females; age 36.4+/-10.3 years, mean+/-SD) and 22 MOH patients (1 male and 21 females; age 39.6+/-9.9 years) who had migraine were included in this study. The genotype for the TNF-beta gene G252A polymorphism was determined by polymerase-chain-reaction restriction-fragment-length polymorphism analysis. RESULTS: The distribution of TNF-beta gene G252A genotype frequency differed significantly between migraine and MOH patients (p=0.013). The G/G genotype was carried by 23% of the migraine patients but it was absent in MOH patients. CONCLUSIONS: G/G genotype carriers appear to be less susceptible to the aggravation of migraine by overuse of medications. The G252A TNF-beta gene polymorphism may be one of the factors contributing to the complications of MOH in patients with migraine.

3.Kampo Medicine for the Treatment of Pain Due to Orthopedic Disorders

In our clinic, patients with relatively positive diseases, which form the orthopedic conditions with inflammation, were successfully treated with eppikajutsuto and daiobotampito after reisenjotsuin. Patients with pain due to orthopedic disorders where NSAIDs (non-steroidal anti-inflammatory drugs) were difficult to be used or ineffective were successfully treated with eppikajutsuto and daiobotampito. We present the representative cases and bibliographically discuss them.
4.An Acute Attack of Pseudogout Treated with Bofutsushosan and Orengedokuto —A Case Report

Pseudogout is a crystal-induced arthritis that often occurs in senile individuals. We report a case of an acute attack of pseudogout that was successfully treated with bofutsushosan and orengedokuto. The patient was an 87-year-old man who visited our clinic and complained of left wrist joint pain 2 days after onset. His physical examination revealed tenderness in his left wrist joint. Plain roentgenogram revealed slight calcification of the left wrist joint, and an inflammatory reaction was found on blood examination. Thus, he was diagnosed with an acute attack of pseudogout in his wrist joint. We prescribed bofutsushosan and orengedokuto. After internal use, his left wrist joint symptoms gradually improved. At the follow-up visit 3 days after his first visit, his left wrist joint pain was resolved completely, and after 9 days, his laboratory data were normalized. Thus, bofutsushosan and orengedokuto were effective in the treatment of an acute attack of pseudogout.
5.Relationships of habitual daily alcohol consumption with all-day and time-specific average glucose levels among non-diabetic population samples.

BACKGROUND:
Alcohol consumption is a prevalent behavior that is bi-directionally related to the risk of type 2 diabetes. However, the effect of daily alcohol consumption on glucose levels in real-world situations in the general population has not been well elucidated. This study aimed to clarify the relationship between alcohol consumption and all-day and time-specific glucose levels among non-diabetic individuals.
METHODS:
We investigated 913 non-diabetic males and females, aged 40-69 years, during 2018-2020 from four communities across Japan. The daily alcohol consumption was assessed using a self-report questionnaire. All-day and time-specific average glucose levels were estimated from the interstitial glucose concentrations measured using the Flash glucose monitoring system for a median duration of 13 days. Furthermore, we investigated the association between all-day and time-specific average glucose levels and habitual daily alcohol consumption levels, using never drinkers as the reference, and performed multiple linear regression analyses after adjusting for age, community, and other diabetes risk factors for males and females separately.
RESULTS:
All-day average glucose levels did not vary according to alcohol consumption categories in both males and females. However, for males, the average glucose levels between 5:00 and 11:00 h and between 11:00 and 17:00 h were higher in moderate and heavy drinkers than in never drinkers, with the difference values of 4.6 and 4.7 mg/dL for moderate drinkers, and 5.7 and 6.8 mg/dL for heavy drinkers. Conversely, the average glucose levels between 17:00 and 24:00 h were lower in male moderate and heavy drinkers and female current drinkers than in never drinkers; the difference values of mean glucose levels were -5.8 for moderate drinkers, and -6.1 mg/dL for heavy drinkers in males and -2.7 mg/dL for female current drinkers.
CONCLUSIONS
Alcohol consumption was associated with glucose levels in a time-dependent biphasic pattern.
