1.Tinnitus assessment by THI and VAS in patients with sudden sensorineural hearing loss.

OBJECTIVE:
To verify the relationship between THI and VAS scores in patients with sudden hearing loss and tinnitus in a prospective study.
METHOD:
Forty-four patients classified their tinnitus according to VAS and THI before and after treatment, and both scores were compared through the Pearson's correlation coefficient test and Nonparametric tests.
RESULT:
There was a correlation between VAS and THI scores in patients with sudden hearing loss and tinnitus. Tinnitus assessment scores were significantly reduced after treatment. Decreasing of THI and VAS scores were not depended on frequency and loudness of tinnitus and severity of deafness, but therapeutic effect of sudden sensorineural hearing loss.
CONCLUSION
THI and VAS scores are useful for assessment of tinnitus in patients with sudden hearing loss and tinnitus.

OBJECTIVE:
To investigate the characteristics of tinnitus in patients with auditory neuropathy spec- trum disorder (ANSD).
METHOD:
This study recruited 14 ANSD patients with tinnitus. All the ANSD patients un- derwent detailed history taking, audiological examinations and assessments of tinnitus. This study analyzed the correlation of tinnitus status and hearing loss, and discussed the effects of sex, age, and the course of disease on tinnitus in ANSD patients.
RESULT:
(1) In the ANSD patients, tinnitus often occurred in 3 years after the onset of hearing loss; (2) Tinnitus was highly prevalent in ANSD patients, and the severity of tinnitus was mostly from mild to moderate; (3) There was no obvious correlation between the subjective grading of tinnitus and hearing loss de- gree, and the impact of curve patterns of hearing loss on the level of tinnitus need much more evidence-based proof; (4) Along with the course extension, the impact of tinnitus on the quality of life was much more obvious; (5) Some risk factors such as noise exposure could be the reasons of aggravating the degree of tinnitus.
CONCLUSION
Tinnitus in ANSD patients has its unique clinical features. The study of Tinnitus in ANSD patients can provide clinical basis for further research in ANSD.

3.The study of clinical characteristics of sudden sensorineural hearing loss patients with tinnitus.

OBJECTIVE:
To analysis the characteristics of sudden sensorineural hearing loss (SSHL) patients with tinnitus, and explore the relationship of characteristics of tinnitus and audiology.
METHOD:
Patients diagnosed as SSHL with tinnitus were studied in the research. All patients' clinical features were analyzed, such as tinnitus frequency, pure tone audiometry, tinnitus, hearing loss degree, results of residual inhibition test.
RESULT:
Thirty cases were identified as mild degree hearing loss, 13 cases as moderate degree, 28 cases as severe degree and 34 cases as profound degree. And hearing impaired frequency of 13 cases was ascertained at low-frequency, 39 cases at middle-high-frequency, and 53 cases at full-range-frequency. The incidence of patients with low-frequency was about 41. 9% (44/105), and it was about 21. 9% (23/105) in those with middle-frequency. And it was 36. 2% (38/105) in cases of high-frequency tinnitus. The chi-square test show statistically significant differences between patients with the low-frequency, middle-frequency and high-frequency of the hearing loss (P<0. 05). In tinnitus residual inhibition test, positive rate of convergence type masking curve was about 72.0%, tinnitus separated type masking curve 20.0%, overlapping type was 57.9%, and the spacing type was 43.5%. There was a statistically significant difference among cases with different type masking curve (P<0. 05)with the spacing residual inhibition test positive rate.
CONCLUSION
There are individual differences of clinical characteristics among SSHL patients with tinnitus. Tinnitus frequency is consistent with the frequency of hearing loss. Patients had the more serious the degree of hearing loss, who had more serious tinnitus grading. Cases with the converged type curve will be fit for tinnitus masking. Therefore, combining the tinnitus detection with the audiological tests, we could obtain the clinical characteristics of SSHL patients with tinnitus.

4.The correlations between varying tinnitus severity and anxiety and depression in non-acute tinnitus patients.

OBJECTIVE:
To investigate the correlation betwen varying degrees of non-acute tinnitus and anxiety and depression.
METHOD:
Seventy-seven outpatients with non-acute tinnitus as their in chief complaint were submitted to Tinnitus Handicap Inventory(THI), Self-Rating Anxiety Scale(SAS), and Self-Rating Depression Scale (SDS).
RESULT:
THI and its three subscales were found to have significant correlations with SAS and SDS. The group (THI ≥ 38) had more anxiety and depression than the mild (THI < 38). Significant correlations were also observed between THI, SAS and SDS in the group with THI ≥ 38, in contrast with the group of THI < 38.
CONCLUSION
Patients with THI ≥ 38 suffered from severe anxiety and depression than the mild. Doctors should pay more attention to these patients, especially their psychological disorders.

5.The principal component analysis of clinical symptoms in patients with Meniere's disease during acute phase.

OBJECTIVE:
To explore the severity index of Meniere's disease during acute phase by a statistical method of principal component analysis (PCA).
METHOD:
Ninety-five patients with Meniere's disease in the acute phase who saw the doctor with 24 hours after onset were included in this study and their clinical data were analyzed retrospectively. The subjective symptoms of hearing loss (X1) and ear stuffy plug (X4) were evaluated by VAS scoring system, and the impact of vertigo (X2) and tinnitus (X3) on the patients were evaluated by DHI and THI score. The principal component analysis (PCA) method was used to analyze the quantified data and construct a synthetic evaluate function of subjective symptoms.
RESULT:
(1) The quantitative results of subjective symptoms in patients with Meniere's disease were as follow: VAS score (X1) of hearing loss was 0-91 points, with an average of 46.23 ± 18.80, DHI score (X2) of vertigo was 8-98 points, with an average of 49.66 ± 15.67, THI score (X3) of tinnitus was 10-100 points, with an average of 47.53 ± 17.44, and ear VAS score (X4) of stuffy plug feeling was 0-82 points, with an average of 21.55 ± 27.54. (2) The eigenvalue of principal components Z1, Z2 and Z3 were 1.876, 0.984 and 0.703 respectively, and the variance contribution were 46.898%, 24.592% and 17.574% respectively. (3) The constructed synthetic evaluate function of the disease was as follow: The Meniere's disease severity index ƒ = 0.213ZX1 + 0.398ZX2 + 0.370ZX3 + 0.455ZX4.
CONCLUSION
The method of PCA for the subjective evaluation of symptoms in Meniere's disease can be constructed as a model of comprehensive evaluation system, which may provide relatively comprehensive information of clinical original variables included in the four main symptoms, reflecting the severity of the disease.

6.The analysis of correlative factors on mental health of post-linguistic hearing loss adults.

OBJECTIVE:
To evaluate the mental health level of the post-linguistic deafen adult, to analyze factors affecting the mental health level.
METHOD:
Fifty-two adult patients with severe to extremely severe post-linguistic hearing loss were randomly selected to participate the study. A self-reporting questionnaire drew up by researchers was used to collect sociological data of the participants. The self-rating anxiety scale (SAS), self-rating depression scale (SDS) and personal report of communication apprehension (PRCA-24) were used to measure the mental health degree of 52 post-linguistic hearing loss adults. To analyze related factors affecting the mental health in post-linguistic deafen adult.
RESULT:
(1) Majority of post-linguistic deafen adult have mental symptoms, including 24 subjects (46.15%) with anxiety feeling, 18 subjects (34.61%) with depression feeling and 45 subjects (86.54%) with communication apprehension. (2) Gander, with/without tinnitus, duration of hearing loss, education background and family support had significant correlation with the mental health level of subjects. Higher incident rate of mental symptoms were found in male (OR = 2.246), with tinnitus (OR = 0.536), short duration (OR = 1.397, 1.530), high education background (OR = 0.323) and poor family support (OR = 1.724).
CONCLUSION
The mental health of the post-linguistic deafen adult was worse than that of the normal people, pathological feature and sociological factors were significantly correlated to the psychological evaluation. It is very important to pay attention on mental health and to give effective counseling of hearing loss people.

7.Clinical comparison of idiopathic sudden deafness in children and the elderly.

OBJECTIVE:
This retrospective study compared clinical manifestations of idiopathic sudden hearing loss between children and the elderly.
METHOD:
44 pediatric patients and 76 elderly patients diagnosed with idiopathic sudden deafness in our clinic from December 2009 to September 2014 were enrolled. Different clinical parameters were compared.
RESULT:
The incidence of initially profound hearing loss was highest and mild hearing loss was lowest in both groups (P < 0.05). There were no differences in the proportion with initially mild, moderate, severe, and profound hearing loss between both groups (P > 0.05). The number of patients was the most in initially profound type of audiogram pattern and the fewest in ascending type in both groups (P < 0.05). Hearing recovery rates in children (27/44, 61.4%) was higher than that in elderly patients (32/76, 42.1%) (P < 0.05). There were no differences in hearing recovery rates of the patients with initially level of hearing loss in both groups (P > 0.05). The highest recovery rate in children was in those with descending type and the lowest was in those with profound type (P < 0.05). There were no differences in hearing recovery rates in elderly patients with initially different types of audiogram pattern (P > 0.05). Hearing recovery rates of descending type in children were higher than that in elderly patients. Presence of tinnitus in pediatric patients was not relavent to the outcome (P > 0.05). Presence of tinnitus in elderly patients was associated with favorable outcomes. (P < 0.05). Presence of dizziness in pediatric patients was not relavent to the outcome (P > 0.05). Presence of dizziness in elderly patients was associated with poor outcomes (P < 0.05). Presence of initially different degrees of opposite side hearing loss in elderly patients was not relavent to the outcome (P > 0.05). Presence of chronic diseases in elderly patients was not associated with the outcome (P > 0.05).
CONCLUSION
The clinical manifestion of idiopathic sudden deafness is respective in Children and in elderly patients.

8.Reconstruction of the Craniectomy Site with the Bone Chips in Patients with Hemifacial Spasm.

OBJECTIVE: This study is designed to demonstrate a relationship between reconstruction methods of the craniectomy site and the development of postoperative complications including headache, tinnitus, and operation site pain after microvascular decompression(MVD) in patients with hemifacial spasm. METHODS: This study included 44 consecutive patients who underwent MVD due to hemifacial spasm from 1995 to 2001. Craniectomy site was reconstructed with only Gelfoam(group 1) in 20, the bone dust and Gelfoam(group 2) in 11, the bone chips and Gelfoam(group 3) in 13. Group 1, 2 were studied retrospectively and group 3 was studied prospectively on the postoperative complications. RESULTS: Postoperatively, wound infection occurred in 3(6.8%), headache in 14(31.8%), operation site pain in 17(38.6%), tinnitus in 15(34.1%), hearing loss in 1(2.2%). Wound infection was noticed in 5% of group 1, 18.2% of group 2, and none of group 3. Headache developed in 45% of group 1, 27.3% of group 2, and 15.4% of group 3. Operation site pain was noticed in 50% of group 1, 27.3% of group 2, 15.4% of group 3. Tinnitus revealed in 45% of group 1, 18.2% of group 2, 30% of group 3. CONCLUSION: Reconstruction of the craniectomy site with the bone chips in patients with hemifacial spasm might have effects on diminution of the postoperative headache, operation site pain and wound infection.

10.2 cases of pulsatile tinnitus caused by sigmoid sinus diverticulum and literature review.

OBJECTIVE:
To describe the anatomy,radiographic manifestation, and surgical treatment of pulsatile tinnitus due to sigmoid sinus diverticulum.
METHOD:
The retrospectively study of diagnosis,clinical and radiographic manifestations,and surgical treatment of 2 cases of pulsatile tinnitus caused by sigmoid sinus diverticulum was carried out.
RESULT:
Bony defect was found in the anterior sigmoid wall in both cases, and one of them was combined with invasion of soft tissues in mastoid cells. Both of them undertook a surgery to reconstruct sinus wall and received a satisfactory results.
CONCLUSION
Sigmoid sinus diverticulum is one of causes which lead to pulsatile tinnitus. It can be identified by imaging examination and can be cured by sinus wall reconstruction.
