1.An investigation in the draw-down area of Three-Gorge reservior: Food consumption and dietary nutrition in the urban and rural population and the nutritional status in children

BACKGROUND: At present, there are fewer studies on the monitoring of nutritional status of the residents living in the draw-down area of ThreeGorge reservoir.OBJECTIVE: To investigate the nutritional status and dietary pattern of the residents before the formation of the draw-down area of Three-Gorge reservoir, so as to provide basic data for the continuous dynamic surveillance of food and nutrition.DESIGN: A cross-sectional study.SETTING: Department of Nutrition and Food Hygiene, School of Public Health, Chongqing University of Medical Sciences.PARTICIPANTS: The investigation was carried out between February and March 2004 in the draw-down area of Three-Gorge reservoir with denser population, larger area and greater influence, such as Kaixian and Yunyang counties, etc. The first surveillance of food nutritional status of the residents in the draw-down area was performed in 52 urban families and 50 rural families of Kaixian county, one family member was voluntarily selected from each family, whose main duty was to buy vegetables and food. Totally 102 subjects (42 males and 60 females) were investigated in this study. At the same time, the data of physical examination and detection of blood Hb in children from Kaixian county between May 1999 and November 2003 were analyzed.METHODS: The subjects were investigated in their house with questionnaires. Their general condition of family, food consumption and dietary nutrition were surveyed and analyzed. Nutrients intake was assessed by 24-hour recall, combined with food consuming frequency questionnaire, the Engel index, which was an important index for the level of family consumption, was detected in both rural and urban residents. Engel index=cost in food/total family income×100%.MAIN OUTCOME MEASURES: ① Amount of food consumption, intakes of energy and nutrients in residents of the draw-down area; ② Prevalence rate of low body mass and rate of growth retardation in children of the draw-down area.RESULTS: All the 102 residents of the draw-down area entered the analysis of results without deletion. ① Amount of food consumption in residents of the draw-down area: Among the urban residents of the draw-down area, the per capita food consumption was 243 g per day, and the consumptions of vegetable, fruit and fish were lower than the average national level, but that of grease food showed a higher level. Among rural residents of Kaixian county, the per capita food consumption was 268 g per day, and the consumption of foodstuff was obviously decreased, but those of meet and grease food showed an ascending trend. ② Energy intake in residents of the draw-down area: Among urban residents of Kaixian county, the per capita energy intake was 6 909 kJ per day, which was obviously lower than that in the urban residents of Chongqing city in 1998 and the average national urban level. The per capita energy intake in rural residents of Kaixian county was 7 577 kJ per day, which was 48.4% lower than that in the urban residents of Chongqing city in 1998 and 37% lower than the average national rural level. ③ Nutritional status in children of the draw-down area: The incidence rate of low body mass in children younger than 5 years in 1999 was close to the average national level (9.6%), it was obviously decreased from 2000, and then fluctuated between 1.0% and 4.8%. Since 1999, the incidence rate of growth retardation in children younger than 5 years of Kaixian county showed a gradually descending trend, but anemia and other common diseases still kept a higher incidence rate, the detection rate of anemia was 33.5% in 1999, and decreased to 20.8% in 2003.CONCLUSION: The current nutritional status of the Kaixian people living in draw-down area of Three-Gorge reservoir still needs further improvement, and dynamic food and nutrition surveillance in succession is necessary.
2.Association of nonalcoholic fatty liver disease with insulin resistance and dyslipidemia in patients with type 2 diabetes mellitus

ObjectiveTo investigate the association of nonalcoholic fatty liver disease (NAFLD) with insulin resistance and dyslipidemia in patients with type 2 diabetes mellitus,and to analyze the risk factors.Methods A total of 200 patients with type 2 diabetes mellitus including 99 with NAFLD and 101 without NAFLD were recruited.Height,weight,waist circumference,hip circumference,liver enzymes,blood lipids,fasting and postprandial blood glucose,insulin,C-peptide,and HbA1C levels were detected.Body mass index ( BMI),waist-hip ratio( WHR),and improved insulin and C-peptide index(HOMA-C-peptide) were calculated and compared between two groups.ResultsCompared with non-NAFLD group,weight,BMI,waist circumference,hip circumference,alanine aminotransferase ( ALT),aspartate aminotransferase,triglyceride ( TG ),total cholesterol ( TC ),low density lipoprotein-cholesterol(LDL-C) were significantly higher in NAFLD group( all P<0.01 ),while age,duration,and high density lipoprotein-cholesterol were lower( all P<0.05 ).The incidence of combined hyperlipidemia,especially hypertriglyceridemia,was significantly higher in NAFLD group( P<0.01 ).Fasting and postprandial 1 h blood glucose [ ( 2.07 ±0.36 vs 1.83 ±0.43 ) mmol/L,( 14.04 ± 3.96 vs 12.59 ± 3.90 ) mmol/L ],fasting and postprandial 1 h Cpeptide [ (2.79± 1.15 vs 2.08±1.29 ) ng/ml,( 1.33 ±0.45 vs 1.12±0.54) ng/ml ],HbA1C [ (2.09±0.33 vs 1.96±0.28) % ],and HOMA-C-peptide index were significantly increased in NAFLD group ( P < 0.05 or P < 0.01 ).Logistic analysis showed that TG,BMI,and ALT were the major risk factors of NAFLD in type 2 diabetes mellitus( P<0.05 or P<0.01 ).ConclusionTriglyceridemia,obesity,and raised ALT level were significantly associated with an increased risk of NAFLD in patients with type 2 diabetes mellitus.
3.Clinical value of transorally inserted anvil in esophagojejunostomy after laparoscopic total gastrectomy for gastric cancer

ObjectiveTo evaluate the safety and feasibility of esophagojejunostomy using transorally inserted anvil ( OrVilTM ) after laparoscopic total gastrectomy for gastric cancer.MethodsThe clinical data of 8 patients with advanced gastric cancer who were admitted to the Nanfang Hospital of Southern Medical University from January 2011 to Febuary 2012 were retrospectively analyzed.Laparoscopic total gastrectomy + D2 lymph node dissection was first performed,and then esophagojejunostomy was completed using OrVilTM. Perioperative condition and prognosis of the patients were analyzed.ResultsAll the procedures were completed successfully,with no complications occurred.There was no extension of the incisiou during operatiou.The mean operation time,anvil inserting time and volume of operative blood loss were (203 + 38 ) minutes,( 10 -+ 4) minutes and ( 106 ± 18 )ml,respectively.Tumor-free proximal margins were confirmed by pathological examination in all the patients.The mean time to first flatus,time to liquid and semi-liquid diet and duration of hospital stay were (3.5 ± 1.3 )days,(5.5 ± 2.9 ) days,( 7.5 ± 3.2) days and ( 11.5 ± 3.5 ) days.The mean time of follow-up was 10 months ( rauge,1-14 months),no anatomosis-related complications were observed.Conclusion Esophagojejunostomy using OrVilTM after laparoscopic total gastrectomy for gastric cancer is safe and feasible.
4.Using somatosensory evoked potential to predict functional recovery in the acute phase of stroke

Objective To assess the predictive value of short-latency somatosensory evoked potential (SSEP) in the acute phase of stroke regarding functional recovery. Methods One hundred and fifty stroke patients were included. SSEPs were recorded on the first 7 days after stroke, and the patients were then stratified into groups with absent, abnormal and normal responses. Clinical state was determined according to the NIH stroke scale (NIHSS), the Fugl-Meyer assessment (FMA), the modified Barthel index (MBI) and the modified Rankin scale (MRS). SSEP was followed up at 1, 3, 6 and 12 months. Results The NIHSS and FMA scores were significantly different comparing any two groups or at different times in the same group. The median SSEP N20 latency and tibial nerve SSEP P40 latency during stroke were positively correlated with MRS scores 12 months later but negatively correlated with MBI results. The ADL results 12 months after stroke were used to evaluate functional recovery, and the prediction rate of the SSEP results alone was 40.8% , however the rate improved to 44.2% when SSEP results were combined with FMA scores and became 46.1% when SSEP, FMA and MRS results were all considered. Conclusion SSEP has independent predictive value regarding functional recovery after stroke. Combined assessment of initial FMA, MRS and SSEP substantially improves the predictive power for stroke prognosis.
5.Interventional effects of biscuits fortified with different doses of vitamin A on the vitamin A status in children of 3-6 years old

BACKGROUND: Vitamin A deficiency disorders (VADD) is a healthy problem of children in the world, especially in the west of China and remote areas, and the nutritional intervention is needed.OBJECTIVE: To compare the improved effects of biscuits fortified with three different doses of vitamin A on the vitamin A status in children aged 3-6 years and explore ap ideal dose of vitamin A supplement for preventing VADD.DESIGN: Randomized controlled observation.SETTING: Health Surveillance Institute, Chongqing Municipal Health Bureau; Staff Room of Nutrition and Food Hygiene, College of Public Health, Chongqing University of Medical Sciences; Center for Children Nutrition, Children's Hospital, Chongqing University of Medical Sciences.PARTICIPANTS: The investigation was done between March and December 2002. 753 children aged 3-6 years from 8 kindergartens in Banan district of Chongqing city were enrolled with the agreement of their guardians. They were divided randomly into 4 groups: 30% recommended intake group, 100% recommended intake group, 20 000 international unit (IU) and 200 000 IU groups.METHODS: ① The biscuits fortified with 30% recommended intake of vitamin A (500 IU) were once given to people of the 30% recommended intake group (177 cases) every day. ②The biscuits fortified with 100% recommended intake of vitamin A (1666 IU) were once given to people of the 100% recommended intake group (173 cases) every day. ③The biscuits fortified with 20 000 IU of vitamin A were once given to people of the 20 000 IU group (209 cases) every week. ④The soft gelatin capsule with 200 000 IU of vitamin A were once given to people of the 200 000 IU group (194 cases).Height, body mass, serum retinal, prealbumin, haematoglobin and retinal binding protein of all children were measured before intervention and after intervention for 3 months. Above indexes were rechecked after supplement for 9 months in 87 children of 30% recommended intake groupMAIN OUTCOME MEASURES: ①Prevalence rate of VADD before in-tervention and after intervention for 3 months in children of every group.② Serum retinal, serum prealbumin, serum retinal binding protein,haematoglobin, height and body mass of children before intervention and after interventional for 3 months in every group.RESULTS: Because of lose of samples and detective technology, only 580 children' examination results were got by rechecking. ①Comparison of the prevalence rate of VADD of children in every group: Three months supplementation later, the prevalence of VADD in every group all decreased sig nificantly [1.48%,1.42%,1.21%, 2.16% ;6.78%,6.54%,8.61%,8.25%(χ2=3.86-8.57, P < 0.05-0.01 )]. ②Comparison of serum retinal, serum prealbumin, serum retinal binding protein, haematoglobin, height and body mass of children of every group: After supplement for 3 months, except prealbumin and haematoglobin in the 30% recommended intake group ,other indexes in each group all increased significantly (t=2.52-37.44, P< 0.05-0.01 ). The increase of serum vitamin A in the 20 000 IU group was larger than that in the other groups (F=4.62,P< 0.01 ). The increases of haematoglobin, prealbumin and height in the 30% recommended intake group were less than those in the other groups (F=5.04-7.78 ,P < 0.01 ).After supplement for 9 months, the increases of haematoglobin and prealbumin in the 30% recommended intake group were larger than those in the other groups (F= 11.62,10.21 ,P < 0.01). The increase of serum retinal was still lower than that in the 20 000 IU group (F=4.21 ,P < 0.01 ).CONCLUSION: Supplement with biscuits fortified with 3 different doses of vitamin A and capsule with 200 000 IU of vitamin A can improve obviously the nourished status of vitamin A and the level of ferrohemoglobin, in which 30% recommended intake and 100% recommended intake have small risk, and everyday supplement can maintain stably the level of vitamin A. That may be suitable for the long-term supplement for children, and the effect of 30% recommended intake was better than that of 100% recommended intake.
7.Sequence analysis of a novel HLA-A?01 ∶130 allele and modeling of three-dimensional structure of HLA molecule

Objective:To confirm the novel allele HLA-A*01∶130 and analyzed the nucleotide sequence of the abnormal reaction pattern.Methods: The HLA typing of sample DNA was performed by PCR-SBT.The ambiguous novel HLA allele was confirmed with single stranded SBT method,then DNA sequencing was performed to identify the difference between the novel allele and HLA-A?01:66 allele.Finally, it was modeled by Swiss-Model to three-dimensional structure of HLA Molecule.Results: The novel allele was not the same with all known HLA-A allele sequence.After analysis,there was one nucleotide differed from the A?01:66 at position 368 where A→G( codon 99 TAT→TGT) resulting in a coding change,99 Tyr was changed to Cys.The amino acid substitution at residue 99 the HLA polypeptide was located in a beta-sheet of antigenic peptide-binding region.Conclusion: The allele is a novel allele that has now been officially named as HLA-A*01∶130 by the World Health Organization( WHO) HLA Nomenclature Committee.
8.Comparison of the effect of structured fat milk and non-structured fat milk on postoperative nutritional support in perioperative patients:a national questionnaire survey

Objective To investigate the difference between structured triglyceride(STG)and Non-structured triglyceride(NSTG)in nutritional support for perioperative patients.Methods A nationwide survey was conducted through online questionnaire.The questionnaire consisted of four modules:questionnaire information,patient information,prescription investigation and postoperative observation.SPSS 24.0 software was used for statistical analysis.Results Before and after parenteral nutrition(PN),the average levels of albumin(ALB)increased in both groups(P<0.01),while the levels of C-reactive protein(CRP)and alanine aminotransferase(ALT)decreased(P<0.01).The difference in serum triglyceride(TG)levels in the STG group was not statistically significant,while there was a decrease in the NSTG group(P<0.05).Generalized linear equation analysis of repeated measurements showed that compared to the NSTG group,the increase in ALB levels in the STG group patients was more significant(P<0.001),CRP and ALT decreases were more pronounced(P<0.001),but the impact on TG was smaller.Conclusions For perioperative patients with parenteral nutrition,STG supplementation can bring significant clinical benefits.STG should be used according to the organ function and economic situation of patients.

10.The regulation of ischemic postconditioning on metabolic disorders of type 2 diabetes mellitus combined with cerebral ischemia in tree shrews

Objective To establish tree shrew model of type 2 diabetes mellitus (T2DM) combined with cerebral ischemia (IS),and to explore the regulatory mechanism of ischemic postconditioning (PC) on gene differential expression in cerebral cortex under metabolic abnormalities and cerebral ischemia condition.Methods Seventy tree shrews were divided into control,T2DM,IS,T2DM+IS and T2DM +IS +PC groups (n =14 each group).The experimental diabetes model was established by the combined use of high fat diet breeding with streptozocin injection in tree shrew.The local cerebral thrombosis was induced by photochemical reaction in tree shrews,and ischemic PC was established at 4h after cerebral ischemia followed by clipped ipsilateral common carotid artery three times (5 min/time).The metabolic status of tree shrews was measured by serum biochemical markers.TTC staining,HE staining,and electron microscopy were used to observe the changes of the body's metabolic status at 24h after IS.RNA-seq was used to analyze differentially expressed genes.Results The ultrastructure of brain cells was abnormal and the cerebral infarction area was the largest in T2DM+IS tree shrews (P<0.01).Compared with control group,body weight of tree shrews in T2DM + IS group was significantly reduced (P< 0.01) while blood glucose,total cholesterol,low density lipoprotein-cholesterol,triglyceride,and C-reactive protein (CRP) levels were markedly increased(all P<0.01).The RNA-seq analysis showed that there were 1 629 differentially expressed genes (1 109 up-regulated genes and 520 down-regulated genes) in T2DM + IS group vs control group.However,ischemic PC deceased the cortical infarction area (P<0.01)and reduced blood glucose,lipid and CRP levels (P<0.05),with 520 differential expression genes (203 up-regulated genes and 317 down-regulated genes).Conclusion Ischemic PC improves the metabolic disturbance-aggravated ischemic brain injury in T2DM tree shrews,which may be related to its regulation on gene expression in cerebral cortex.
