1.Study on several immune molecules expressed on peripheral blood mononuclear cells which may play an important role in the aggravation of chronic hepatitis B

Objective To screen immune molecules expressed on peripheral blood mononuclear cells (PBMCs) which arc related to the aggravation of chronc hepatitis B (CHB). Methods Real-time polymerase chain reaction (PCR) was employed to detect the relative expression of 39 immune molecules at mRNA level and then compare the differences between groups (control group, mild CHB group, moderate CHB group and severe CHB group). The investigated immune molecules included leukocyte differentiation antigens, chemokine receptors, apoptosis-related ligands and receptors,adhesion molecules and Toll like receptors. Scheffe model was used to compare the differences among all groups and Spearman rank correlation was used for correlation analysis. Results Among the 39 immune molecules, the expressions of tumor necrosis factor related apoptosis inducing ligand (TRAIL), TRAIL receptor (TRAIL-R) 2, CD64, CD30, CD27, CD28, L-selectin (CD62L),intercellular adhesion molecule-1 (ICAM-1), chemokine (C-X-C motif) receptor (CXCR) 2 were significantly increased in severe CHB group compared with those in control group ( 1.96, 2.13, 1.33,1.16, 1.57, 2.14, 2.03, 2. 10 and 2.09, respectively; all P<0.05). The expressions of TRAIL,TRAIL-R2, CD64, CD30, CD27 were highly correlated with the expression of interferon gamma (IFN-γ) (r=0. 816, 0. 572, 0. 462, 0. 697 and 0. 793, respectively; all P<0.01). The expressions of TRAIL-R2, CD64, CD30, CD62L, ICAM-1 were highly correlated with the expression of tumor necrosis factor alpha (TNF-α) (r=0. 494, 0. 588, 0. 568, 0. 968 and 0. 976, respectively; all P<0.01). Conclusion The abnormal expression of TRAIL,TRAIL R2,CD64,CD30,CD27,CD28,CD62L,ICAM-1 and CXCR2 may play an important role in the pathogenesis and aggravatoin of hepatitis B.
2.Transfer of siRNA against CD73 suppresses human breast cancer cell line MB-MDA-231 adhesion to extracellular matrix

AIM:To study the effect of CD73 on breast cancer cell line MB-MDA-231 adhesion to extracellular matrix.METHODS: ① CD73 siRNA plasmid was constructed and transfected into MB-MDA-231 cells by lipofectamine 2000.② The transfection efficiency was analyzed by flow cytometry using eGFP as a marker gene.Stable transfected MB-MDA-231 cells were selected using G418.③ RT-PCR and Western blotting analysis of CD73 expression in MB-MDA-231 cells was performed.④ The effects of CD73 on MB-MDA-231 cells adhesion to extracellular matrix were assessed by cell adhesion assay.RESULTS: ① Transfection of MB-MDA-231 cells with CD73 siRNA plasmid significantly inhibited CD73 expression at mRNA level.The efficiency was up to 91%.② Transfection of MB-MDA-231 cells with CD73 siRNA plasmid significantly inhibited CD73 expression at a protein level.The efficiency was up to 79.3%.③ Treatment of MB-MDA-231 cells with CD73 siRNA resulted in diminished adhesion to extracellular matrix.CONCLUSION: This study demonstrates that CD73 siRNA effectively inhibits CD73 gene expression in MB-MDA-231 cells leading to adhesion to extracellular matrix suppression.
3.Study on the Th17 ,Th9 cells and the expression of the correlated cytokines in peripheral blood of patients with pediatric bronchial asthma and the clinical significance

Objective To detect the levels of helper T cells (Th) 17 and Th9 cells in the peripheral blood of patients with pedi-atric bronchial asthma and the expression of interleukin 17 (IL-17) ,IL-9 and total IgE in serum ,and to discuss their effect and clini-cal significance in pathogenesis of pediatric asthma .Methods A total of 46 children with bronchial asthma ,including asthma attack group (n=26) and asthma remission group (n=20) were selected .Healthy children were selected as healthy control group (n=20) .Flow cytometry was used to detect the percentages of Th17 and Th9 cells in the peripheral blood .The levels of IL-17 and IL-9 in serum were measured by enzyme-linked immunosorbent assay (ELISA) .Serum total IgE levels were measured by the specific protein analyzer .Results The percentages of Th17 and Th9 cells in the peripheral blood were significantly higher in asthmatic group compared with remission group and healthy control group (P<0 .05) .The percentages of Th17 and Th9 cells in the periph-eral blood was significantly higher in remission group compared with healthy control group (P<0 .05) .The levels of IL-17 ,IL-9 and total IgE in serum were significantly higher in asthmatic group compared with remission group and healthy control group (P<0 .05) .The levels of IL-17 ,IL-9 and total IgE in serum were significantly higher in remission group compared with healthy control group (P<0 .05) .The levels of IL-17 and IL-9 in serum of asthmatic children were positively correlated with total IgE levels (P<0 .05) .The correlation analysis results showed that the levels of IL-17 ,IL-9 and total IgE in serum of patients with pediatric bron-chial asthma have a positive correlation .(r=0 .717 ,0 .491 ,0 .786 ,P<0 .05) .Conclusion Th17 ,Th9 cells and their cytokines in pe-ripheral blood of patients with pediatric bronchial asthma play important roles in the pathogenesis of asthma and are positively cor-related with the severity of asthma .
4.Characterization and genomic analysis of bacteriophage SH-Kp152234 infecting extensively drug-resistant Klebsiella pneumonia

Objective · To identify broad-spectrum bacteriophages against extensively drug-resistant Klebsiella pneumonia and analyze their characteristics by biological and genomic methods. Methods · Multi-drug resistant Klebsiella pneumonia strains collected from a hospital were used as host bacteria to isolate and purify broad-spectrum phages in the wastewater at the same hospital area. The size and shape of phages were observed by transmission electron microscope. Titer, host range, pH stability and thermal stability were measured. Moreover, the DNA extracted from the phage SH-Kp152234 was sequenced and analyzed. Results · One strain of bacteriophage against Klebsiella pneumonia was isolated and named as SH-Kp152234. The electron microscope revealed it belongs to Podoviridae family. Moreover, genome of SH-Kp152234 showed to be a linear double-stranded DNA of 40578 bp with the GC content of 52.85%. It was predicted to have 49 open reading frames with related known functions.Conclusion · SH-Kp152234, with a broad host range and a short latent period, which could exert its activity in a wide range of temperature and pH, is a promising candidate to be exploited in the treatment of multiple drug-resistant Klebsiella pneumonia.
5.Nine foveal hypoplasia patients in a family

Objective:To observe the clinical features of nine foveal hypoplasia (FVH) patients in a family.Methods:A retrospective clinical study. In August 2018, nine patients with FVH from a family diagnosed in Qilu Hospital of Shandong University (Qingdao) were included in this study. Detailed medical history of the proband was collected. Best corrected visual acuity (BCVA), slit-lamp, cycloplegic refraction, fundus color photography, optical coherence tomography (OCT) and OCT angiography (OCTA) were performed on the proband. The peripheral venous blood of V7 (family member), the proband and the proband's parents were collected for DNA extraction, and gene detection was performed.Results:The proband, a four-year-old girl, had poor vision with BCVA of 0.4 in both eyes. OCT showed absence of foveal pit, absence of outer segment lengthening, absence of outer nuclear layer widening and incursion of inner retinal layers. The proband's mother was 32 years old, and macular foveal reflection was not observed in her eyes. OCT and OCTA examination showed no foveal pit and foveal avascular zone in both eyes. Both eyes of the proband and her mother were diagnosed with Thomas grade 4 FVH. The other seven patients also had no foveal pit, and could be categorized into Thomas grade 3 or 4. No significant pathogenic genes and mutation sites were detected in the proband through whole genome sequencing, and no copy number variation or chromosomal abnormality associated with the phenotype of the proband was detected. After seven months of amblyopia treatment, the proband's BCVA had improved to 0.5 in the right eye and 0.6 in the left eye, while the BCVA did not change after 2 years of follow-up.Conclusion:Nine FVH patients in this family had no foveal pit with similar OCT images, and their visual acuity was affected from lightly to severely. Early amblyopia training is helpful to improve the visual acuity of child patients.
6.Study on influencing factors of length of stay for type 2 diabetes mellitus with urinary tract infection

Objective:To investigate the related factors of length of stay in type 2 diabetes mellitus (T2DM) with urinary tract infection, and provide reference for clinical nursing.Methods:A retrospective analysis was used to collect patients ′ general demographic data, disease history, HbA1c, and other laboratory indicators, and etc. A total of 228 hospitalized patients with confirmed T2DM with urinary tract infection in Shanxi People ′s Hospital from January 2019 to September 2020 were selected as the study subjects. The patients were divided into observation group ( n=79) and control group ( n = 149) according to their length of stay. The electronic medical history and related laboratory examination indexes of the two groups during hospitalization in the Department of Endocrinology were analyzed retrospectively. The general demographic data, disease history, glycosylated hemoglobin and other laboratory indexes of the two groups were statistically analyzed. Results:The results of single factor analysis on the influence of length of stay showed that there were statistically significant differences in age, gender, medicare payment method, Charlson Complication Index (CCI), treatment, glycosylated hemoglobin, procalcitonin (PCT) ( χ2 values were from 5.107 to 20.134, t value was 3.946, Z value was -2.914, P<0.05). Logistic regression analysis showed that female ( OR=0.405, 95% CI 0.172-0.951), age≥70 years old( OR=0.528, 95% CI 0.263-0.922), insulin treatment ( OR=0.479, 95% CI 0.269-0.854), CCI≥3 points ( OR=2.555, 95% CI 1.346-4.851), payment method of new rural cooperative medical insurance ( OR=0.273, 95% CI 0.120-0.620), resident medical insurance ( OR=0.184, 95% CI 0.083-0.405), others ( OR=0.367, 95% CI 0.157-0.862), PCT( OR= 0.252, 95% CI 0.290-0.877), and HbA1c ( OR= 0.81, 95% CI 0.705-0.930) were the influencing factors for the length of stay in patients of T2DM with urinary tract infection. Conclusion:Patients have a longer hospital stay in women, age ≥70 years old, and insulin treatment, CCI≥3 points, HbA1c and PCT. Compared with the employee medical insurance, patients have a shorter hospital stay with new rural cooperative medical insurance, resident medical insurance and other payment methods. Therefore, clinical testing ability should be further improved to effectively reduce the pain and economic burden of patients.
7.Correlation between children's dental decay and the contents of saliva CCL28 and secretory immunoglobulin A.

OBJECTIVE:
To explore the association of the dental decay of children with the contents of chemokine CCL28 and secretory immunoglobulin A (sIgA) in saliva.
METHODS:
A total of 108 children in 2 kindergartens of Changsha, with age from 3 to 5 years old, were enrolled for this study. The saliva was collected from these children when they were in the examination of mouth. Th e children were divided into 3 groups: A non-caries group [dynamical mean-field theory (DMFT)=0], a low caries group (DMFT=1-4) and a high caries group (DMFT ≥ 5). Th e contents of CCL28 and sIgA were measured by ELISA.
RESULTS:
The contents of CCL28 and sIgA in saliva were (121.22 ± 32.63) pg/mL and (16.49 ± 8.02) μg/mL, respectively. A positive linear correlation was found between the CCL28 content and sIgA content in saliva (r=0.734). Th e CCL28 and sIgA contents in saliva were positively correlated with the degree of dental caries in children (P<0.05).
CONCLUSION
The dental decay of children leads to the secretion of chemokine CCL28, which promotes the secretion of sIgA in saliva.

8. One case of severe subcutaneous soft tissue infection caused by nonstandard insulin injection

On January 1st 2018, a male 44 years old diabetic patient with subcutaneous soft tissue infection in right thigh was admitted to our hospital. The patient repeatedly used the same needle to inject insulin subcutaneously in the unsterilized right thigh, and his blood glucose was badly controlled in the long term. Severe subcutaneous soft tissue infection of the right thigh occurred after his fatigue, accompanied with ketoacidosis. Then he received conservative treatment in the local hospital for one month, but the infection persisted. After being transferred to our hospital, we highly suspected the diagnosis of necrotizing fasciitis according to previous test indicators and local B-ultrasound results, but suggestion of aggressive surgery was refused. So we treated him with conservative therapies using sensitive antibiotics and supportive remedies. The patient was basically healed after treatment of 1 month and he was recovered well during the follow-up 2 months after discharged from our hospital. This case emphasizes the importance of standard injection of insulin and early diagnosis of severe subcutaneous soft tissue infection.
9.Current situation and influence factors of innovation ability of nursing graduate students

Objective To investigate the current situation and influence factors of innovation ability of nursing graduate students. Methods A total of 270 nursing graduate students from 42 universities were surveyed using the Innovation & Creativity Effectiveness Assessment (ICEA) by convenience sampling between March 2016 and November 2016. Results There were 88.1% of nursing graduate students with a total innovation ability score of lower than 4 points, which was at a lower level. Ages, international conferences, other academic activities and the time of supervisors on supervising students significantly affected scores of innovation ability on nursing students(P<0.05). Conclusions The current cultivation situation of innovation ability of nursing graduate students was not optimistic. The cultivation of the innovation ability is a systematic and complicated program which acquires multiply strategies to improve students' innovation ability.
10.Application of molecular simulation in teaching of Medical Immunology

Objective:To change the current situation that immune molecules presented only by schematic diagram or static model in the textbook of Medical Immunology,and to present the specific and vivid molecular morphology and functions to students.And set up corresponding elective course to let students personally explore the structure and function of immune molecules.Methods:On one hand,the teaching team applied the molecular simulation technology of scientific research to the production of teaching materi-als for the specific morphology,structural characteristics and functions of immune molecules,which were used in the teaching of medi-cal immunology for medical undergraduates majoring in clinical,oral and preventive medicine.On the other hand,a related elective course research on the love of molecule was opened to give students the opportunity to operate relevant software by themselves and learn the structure and function of immune molecules.Results:The application of molecular simulation in teaching improved teachers'teaching ability,strengthened teachers'immunology knowledge,enhanced the interaction between teachers and students,and achieved good teaching results.Conclusion:It is feasible to apply molecular simulation technology to medical immunology teaching for undergraduates,which can improve the teaching quality.
