1.Therapeutic Effects of Bailing Capsules Combined with Dermatophagoides Farinae Drop in the Treatment of Allergic Rhinitis

Objective:To investigate the clinical effects of dermatophagoides farinae drop combined with Bailing capsules in the treatment of allergic rhinitis. Methods:Totally 96 patients with allergic rhinitis in our hospital were selected and randomly divided into two groups with 48 cases in each by a randomization table. The patients in the treatment group received dermatophagoides farinae drop and Bailing capsules,and those in the control group only received dermatophagoides farinae drop. The scores of symptoms and physical signs and adverse reactions were compared between the two groups. Results:After the treatment,the scores of symptoms and physical signs in the two groups were significantly decreased(P < 0. 05),and the decrease in the treatment group was more notable(P < 0. 05). The effective rate of the treatment group(93. 75% )was higher than that of the control group(62. 50% ,P <0. 05),while the incidence of adverse reactions of the two groups was without significant difference(P > 0. 05). Conclusion:Bailing capsules combined with dermatophagoides farinae drop can significantly relieve the clinical symptoms and signs,and improve the quality of life of the patients with allergic rhinitis,which is worthy of promoted application in clinics.
2.Fragmentation pathways of five estrogens using electrospray ionization quadrupole time-of-flight mass spectrometry.

The fragmentation pathways of five estrogens (estradiol, estrone, equilin sulfate, 17 a-dihydroequilin sulfate and equilenin sulfate) have been studied with high resolution and high mass accuracy using electrospray ionization quadrupole time-of-flight mass spectrometry (ESI-Q-TOF/MS) in the negative ion mode. Molecular weights were obtained from [M-H](-) ions in the product ion spectra. The results indicate that the five structurally similar estrogens have similar fragmentation pathways. Using their stable isotope forms as internal reference compounds, the accurate mass and composition of the fragment ions were determined. During collision-induced dissociation (CID), cleavage is initiated by loss of oxygen atoms from carbon-17, after which D and C rings cleave sequentially and rearrange to finally form stable conjugate structures with highly abundant characteristic fragment ions at m/z 183 (accompanied by m/z 181), m/z 169 and m/z 145 (accompanied by m/z 143). Understanding these characteristic fragmentation pathways of estrogens will be helpful in identifying the structures of steroid hormones in general.
3.Blinding Designs and Critical Issues in Acupuncture Clinical Trials

Blinding is an important method to control the measure bias in clinical trials.As a complex and invasive intervention,acupuncture has more difficulty in blinding implementation compared to drugs and has a higher risk of unblinding breakage.This article provides an overview of the blinding in acupuncture clinical study and summarized the key aspects,including:there is no standard for the application and reporting of sham acupuncture design and blinding measures,appropriate sham acupuncture devices still need to be developed,currently there are no effective methods of operator blinding,blinding assessment has not been given due attention,sham acupuncture design should be standardized and reported.Researchers should conduct further studies to address critical questions and challenges to provide methodological support to improve and promote the quality of acupuncture clinical research.
4.Construction of tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning

Objective:To construct the tunnel-type peripherally inserted central catheter (PICC) catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning, so as to provide guidance for clinical practice.Methods:From May to July 2021, the first draft of the tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning was formed through literature review and expert meeting. The convenience sampling method was used to select 18 experts from 8 provinces/municipalities across the country to conduct Delphi expert consultation, solicit expert opinions, and then discuss in groups to determine the final draft of the tunnel-type PICC catheterization procedure. The degree of enthusiasm, authority, and coordination of opinions of experts were expressed by the effective recovery rate of the questionnaire, the ratio of experts who put forward opinions, the expert authority coefficient, Kendall coordination coefficient, and the coefficient of variation.Results:A total of two rounds of expert consultations were carried out. The effective recovery rates of the questionnaires were 90.00% and 100.00%, respectively. The ratio of experts who proposed revisions were 83.33% and 5.56%, respectively, and the expert authority coefficients were both 0.90, Kendall coordination coefficients were from 0.112 to 0.170 ( P<0.05) , and the coefficient of variation was 0 to 0.122. In the second round, the mean score of importance of each level of operation steps was 4.64 to 5.00. The final constructed tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning includes 4 first-level operation steps, 19 second-level operation steps and 39 third-level operation steps. Conclusions:The tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning is reliable and of high quality, which has important guiding significance for clinical catheterization practice.
5.Investigation of Mechanism of Aurantii Fructus Immaturus and Naringin in Preventing Postoperative Intestinal Adhesion

ObjectiveTo investigate the preventive effect and mechanism of Aurantii Fructus Immaturus and naringin on postoperative intestinal adhesion in rats. MethodThe preventive effect of Aurantii Fructus Immaturus and naringin on intestinal adhesion was studied by cecal scraping model of rats, the model rats were randomly divided into model group, dexamethasone sodium phosphate group and Aurantii Fructus Immaturus low, medium and high dose groups (1.58, 3.15, 6.30 g·kg-1·d-1), tsham-operated group was treated with an incision in the abdomen. Adhesion was assessed by Nair method after 7 d of administration. Hematoxylin-eosin (HE) staining was used to observe the pathological morphology and inflammatory cell infiltration of cecum, the expression of matrix metalloproteinase-9 (MMP-9) in cecal adhesion was detected by immunohistochemistry. Meanwhile, intestinal adhesion model rats were randomly divided into model group, dexamethasone sodium phosphate group, naringin low, medium, high dose groups (50, 100, 200 mg·kg-1·d-1), and the sham-operated group was treated with an incision in the abdomen. Adhesion was assessed after 7 d of administration, HE staining was used to observe the pathological morphology and inflammatory cell infiltration in the cecum, and the expression of MMP-9 in the cecal adhesion tissue was detected by immunohistochemistry. Expressions of transforming growth factor-β1 (TGF-β1) and α-smooth muscle actin (α-SMA) in cecum were analyzed by western blot. ResultAurantii Fructus Immaturus could inhibit the formation of postoperative intestinal adhesions in rats, reduce the inflammatory response of damaged cecum tissue, and up-regulate the expression of MMP-9 in the adherent tissue in a dose-dependent manner. All dose groups of naringin could significantly inhibit the formation of postoperative intestinal adhesions in rats, reduce inflammatory cell infiltration and fibrous proliferation in tissue, up-regulate the expression of MMP-9 in the adherent tissue in a dose-dependent manner, and inhibit the expression of TGF-β1 and α-SMA in cecal tissue. ConclusionAurantii Fructus Immaturus and naringin can reduce postoperative intestinal adhesion formation in rat model, and their effects may be related to reducing tissue fibrosis and accelerating extracellular matrix degradation.
6.Study on Toxic Effect of the Extract from Tripterygium hypoglaucum on Embryo-fetal Development of New Zealand Rabbits

OBJECTIVE:To study the toxic effect of the extract from Tripterygium hypoglaucum on pregnant animal and embryo-fetal development . METHODS :Successfully mated New Zealand female rabbits were randomly divided into solvent control group,positive control group (cyclophosphamide,20 mg/kg)and T. hypoglaucum extract high-dose ,medium-dose and low- dose groups(15.0,7.50,3.75 g/kg,by the amount of crude drug )according to their body weight ,with 18 rabbits in each group at least. The female rabbits in solvent control group and T. hypoglaucum extract groups were given water or the corresponding T. hypoglaucum extract solution from 6th to 18th day of pregnancy ,5 mL/kg,once a day. Positive control group was given cyclophosphamide subcutaneously into the neck from 10th to 13th day of pregnancy ,1 mL/kg,once a day. According to the related requirements of Technical Guidelines for the Study of Drug Reproductive Toxicity ,the general situation ,body weight ,body weight increase and food intake of female rabbits were observed and recorded during the experiment ,and euthanasia was carried out on 28th day of pregnancy ;the relative indexes of main organs ,fetal uterus ,placenta uterus ,placenta,ovary,implantation gland , absorption fetus ,stillbirth,live fetus and corpus luteum were observed and recorded after anatomy ;the relative indexes of body weight,appearance,visceral and skeletal alterations of the fetus were detected . RESULTS :Compared with solvent control group , the body weight ,body weight increase ,food intake ,main organs ,pregnancy of pregnant rabbits ,as well as reproductive function,embryo formation ,fetal growth and development ,appearance,visceral and skeletal development indexes in T. hypoglaucum extract groups had no significant abnormal changes (P>0.05);above indexes in the positive control group had significant changes (P<0.05),and significant maternal toxicity and embryotoxicity were found. CONCLUSIONS :T. hypoglaucum extract 15.00-3.75 g/kg(by the amount of crude drug )have no significant maternal toxicity ,embryotoxicity or fetal development toxicity to New Zealand rabbits.
