1.Effect of Nimodipine on the Cognitive Ability of Alzheimer Disease Patients

Objective:To investigate the effect of nimodipine on the cognitive ability of the Alzheimer disease (AD) patients and to presume its mechanism.Method:60 patients with AD received their examinations and treatments in our hospital from January to December,2006.The data included the patients' disease histories,physical examinations and radiological checks.All the patients were randomly divided into two groups,Nimodipine group ( N group) and controlled group (C group).N group was given nimotop 30mg tid orally,while C group was treated with neuroxin 200mg tid.The pa- tients' cognitive and daily life abilities were accessed by MMSE and ADL in the first day and the twelfth week from the be- ginning of the treatment.All the data were analyzed by SPSS 10.0.Result:Compared with those of the controlled group, the cognitive and daily living abilities of N group patients' were improved significantly in 12 weeks after the treatment by ni- modipine (P
2.Investigation of relationship between the phlegm and adhesion molecule of gastric cancer

Objective To observe relationship between the phlegm and adhesion molecule and further explore the functions of the phlegm in the metastatic potential of the tumors. Methods Examin the expression difference of ICAM-1,E-cad,MMP-9 of gastric cancer by enzyme linked immunosorbent assay between the phlegm-stamp type and non-phlegm-stamp type,then reducing phlegm by chinese medicine treatment and comparing theirs expression variation.Results The expressions of ICAM-1,E-cad, MMP-9 were obviouslv different between the phlegm group and non-phlegm group[(403.6±99.7)μg/L,(9.08±1.69)mg/L,(465.0±96.64)μg/L &(319.9±81.4)μg/L,(7.56±1.15)mg/L,(228.1±43.79)μg/L].After using xiao-tan-san-jie recipes,the expressions of ICAM-1 and E-cad of the phlegm group were obviously lower than before(P<0.05);the expression of MMP-9 was also falling down,but the difference was not significative.Conclusion There is a correlation between the phlegm and adhesion molecule, which could take action in tumor metastams by affecting adhesion molecule expression.
3.Covered expandable metallic stent placement combined with interventional chemotherapy for ;malignant esophago-tracheal fistulae:a randomized comparison study

Objective To evaluate the safety and therapeutic effect of covered expandable metallic stent placement combined with interventional chemotherapy in treating malignant esophago-tracheal fistulae. Methods A total of 60 patients with esophago-tracheal fistula were enrolled in this study. The patients were randomly divided into control group (n=28) and study group (n=32). Patients of the control group were treated with covered stent implantation only, while patients of the study group were treated with combination use of covered stent implantation and interventional chemotherapy. All the patients were followed up regularly, the short-term and long-term results were determined, and the effect rate, median survival time and complications were statistically analyzed. Results Among the 60 patients, successful obstruction of the fistula was obtained in 58 with a success rate of 96.7%. The median survival time of the 58 patients was 5.8 months. The median survival time of the control group and the study group was 3.6 months and 8.7 months respectively, the difference between the two groups was statistically significant (P<0.05). The one-month, 3-month and 6-month effect rates were 78.1%, 68.8% and 62.5% respectively. The incidence of esophageal stent re-stenosis in the study group was much lower than that in the control group. Conclusion The combination use of covered stent implantation and interventional chemotherapy can effectively improve the median survival time of patients with malignant esophago-tracheal fistula, and reduce the occurrence of stent-related complications. Therefore, covered expandable metallic stent placement combined with interventional chemotherapy is an effective and safe treatment for malignant esophago-tracheal fistulae.
4.Clinical effect of 125I seeds implantation combined with arterial infusion chemotherapy for patients with advanced pancreatic cancer

Objective To evaluate the clinical efficacy of 125I seeds implantation combined with transcatheter arterial infusion(TAI)in treating advanced pancreatic carcinoma(PC).Methods By using envelope sealing method,76 patients who met enrollment criteria were divided into observation group and control group.Finally,a total of 67 patients were enrolled in this study.The observation group had 31 patients and the patients received 125I seeds implantation combined with TAI therapy,and the control group had 36 patients and the patients received TAI therapy.After 3 courses of treatment,the clinical efficacy,pain score,incidence of obstructive jaundice(OJ),and survival time were compared between the two groups.Results In the observation group and the control group,the objective response rate(ORR)was 48.4%and 25.0%respectively,and the difference was statistically significant(P<0.05);the disease control rate(DCR)was 83.9%and 66.7%respectively,and the difference was not statistically significant(P>0.05).In both groups,the postoperative one-month and 3-month pain scores were decreased obviously when compared with the preoperative values(P<0.05),but the reduction of pain score in the observation group was more obvious than that in the control group(P<0.05).In the observation group and the control group,the pain relief rates were 23.06%and 13.4%respectively,and the difference was statistically significant(P=0.0252);the incidences of OJ were 19.4%and 50.0%respectively,and the difference was statistically significant(P<0.05);the median survival time was 7.9 months and 5.1 months respectively,and the difference was statistically significant(P<0.05).In both groups,the postoperative one-month and 3-month CA199 levels were decreased,and the difference was statistically significant(P<0.05).No statistically significant difference in the incidences of adverse reactions such as nausea,vomiting,diarrhea and leukopenia existed between the two groups(P>0.05).Conclusion 125I seeds implantation combined with TAI therapy can effectively relieve cancer pain,prolong survival time in patients with advanced PC,and it is clinically safe.Therefore,this therapy is worthy of clinical promotion.
5.Mechanism of action of bile-gut axis in the development and progression of intrahepatic cholangiocarcinoma

Intrahepatic cholangiocarcinoma is a malignant tumor with an extremely poor prognosis, and its pathogenesis is complex and remains unclear. In recent years, more and more studies have focused on the role of bile-gut axis in the development and progression of intrahepatic cholangiocarcinoma. Bile-gut axis refers to the complex interaction between bile and gut microbiota, including bile salt metabolism, dynamic changes of microbiota, inflammatory response, and immune system regulation. This article elaborates on the potential mechanisms of bile-gut axis in intrahepatic cholangiocarcinoma, especially gut microbiota dysbiosis, abnormal bile salt metabolism, chronic inflammatory response, and immune system interaction, this article aims to provide new perspectives and possible therapeutic targets for future research and promote the early diagnosis and effective treatment of intrahepatic cholangiocarcinoma.
