OBJECTIVE To analyze the cleaning and disinfecting effect of digestive endoscope by the Mall disinfection center for endoscopes.METHODS Fifty-six pieces of endoscopes were cleaned and disinfected by the Mall disinfection center,and checked randomly. 4.Standardized Management for Cleaning and Disinfection of Endoscope

OBJECTIVE To enhance the standardized management for cleaning and disinfection of endoscope,and prevent hospital infection due to endoscope examination and treatment.METHODS The main measured were established and consummated the system,increased the investment of facilities,strengthened personal training and standardized the cleaning and disinfection work procedures.RESULTS The endoscopes after disinfection were all reached the standards.CONCLUSIONS The standardized management for cleaning and disinfection of endoscope is the assurance for the endoscope sterilization reaching the standards.
5.Analysis of indicators in 2 521 patients before blood transfusion

Objective To investigate the infection status of infectious diseases for 2 521 patients before blood transfusion in a Hospital in Changsha.Methods A total of 2 521 patients who would be transfused were selected,and six kind of serum hepatitis B virus indicators,hepatitis C virus antibody (antiHCV),human immunodeficiency virus antibody (anti-HIV/1 + 2),and treponema pallidum antibody (antiTP) of nine common infectious disease targets were detected with enzyme-linked immunosorbent assay (ELISA).Results Among 2 521 patients,HBsAg-positive cases were 8.33%,anti-HCV positive were 0.59%,anti-HIV positive [confirmed by the Provincial Center for Disease Control (CDC)] was 6 cases,and TP-positive were 2.301%.A total of 289 patients were tested positively,with a total positive rate of 11.46%.Conclusions Detection before transfusion may reduce infection risk and decrease the risk of occupational exposure,strengthen medical staff self-protection,and reduce medical malpractice caused by blood transfusion.
6.Sphincter Oddi manometry and endoscopic sphincterotomy in diagnosis and treatment of patients with sphincter of Oddi dysfunction

Abstact: Objective To investigate the diagnostic value of endoscopic sphineter Oddi manometry (SOM) in patients with recurring abdominal pain and observe the value of endoscopic sphincterotomy (EST) in treatment of patients with sphincter of Oddi dysfunction (SOD).MethodsClinical data of 30 patients with chronic abdominal pain after cholecystectomy who were suspected SOD from 2012 October to 2014 September were collected and retrospectively analyzed. These patients received SOM in ERCP examination and the observation of Oddi sphincter basal pressure, contraction amplitude, frequency and mode of transmission were carried out. The EST were carried out in patients with basal pressure of Oddi sphincter higher than 40.0 mmHg or higher than 30.0 mmHg, and the presence of serum amylase, lipase, ALT, AST, AKP increased more than 2 times of the normal value and (or) of common bile duct, pancreatic duct widening.ResultThe SOM of all the 30 patients were all abnormal. The basal pressure of Oddi sphincter, the contraction amplitude, the contraction frequency and the reverse shrinkage were (36.6 ± 21.1) mmHg, (210.6 ± 25.7) mmHg, (10.1 ± 3.1) times/min and (55.0 ± 8.0)%. All the patients were treated with EST, of which 27 cases (90.0%) received good results.Conclusion SOM is helpful in evaluation of Oddisphincter function, it is of great value in diagnosis of SOD. EST treatment obtained satisfactory effect in patients with elevated basal pressure of Oddi sphincter.
7.The therapeutic value of endoscopic nasobiliary drainage for the infection of biliary tract associated with endoscopic retrograde cholangiography

Objective To explore major risk factors for postoperative biliary tract infection associated with endoscopic retrograde cholangiography (ERC), and to evaluate endoscopic nasobiliary drainage (ENBD) for intervening the infection.Methods A total of 512 patients who underwent ERC at the First People's Hospital of Yunnan Province from January 2010 to June 2016 were enrolled and divided into group A and B randomly.Group A underwent ENBD after ERC while group B without.The incidence rates of biliary tract infection in different causes and lesions were compared between the two groups.Results Among the 512 patients, there were 276 cases in group A and 236 cases in group B.The overall postoperative biliary infection rate was 4.30%(22/512).Patients in group A showed a smaller chance of developing postoperative biliary tract infection than that in group B [1.09%(3/276) VS 8.05%(19/236), χ2=15.00, P=0.000].Malignant biliary obstruction was the most common cause (13.46%, 14/104) and the most common site was hepatic portal (13.43%, 9/67).Conclusion ENBD can ensure smooth drainage of bile duct therefore effectively prevent biliary tract infection after ERC, especially for patients with malignant biliary obstruction and hepatic portal lesion.
8.Expression of Tspan-1 in benign and malignant lesions of the colon

Objective To study the protein expression of Tspan-1 gene in colorectal carcinoma,and investigate the relationship between the expression and biologic behavior of colorectal carcinoma. Methods Immunohistochemical ELIVISION two-step method was used to detect the protein expression of Tspan-1 gene in 80 cases of colorectal carcinoma,13 cases of colorectal adenoma and 27 cases of normal colorectal tissues. Results The positive expression rate of Tspan-1 in colorectal carcinoma was significantly higher than that in colorectal adenoma and normal colorectal tissues,there was significant difference in the expression among the three groups.The positive expression rate of Tspan-1 was 90% in colorectal carcinoma,23% in colorectal adenoma and 7% in normal colorectal tissues (P < 0.01).The positive expression rate of Tspan-1 in poor and moderately differentiated carcinoma was significantly higher than that in well-differentiated carcinoma (P < 0.01).The expression of Tspan-1 was significantly associated with lymph node metastasis (P<0.01),and with the 5-year survival rate of colorectal carcinoma patients,with also TNM stage and the depth of tumor invasion (P <0.01),and while it was not associated with gender,age,location,tumor size,and type (P > 0.05). Conclusion The expression of Tspan-1 gene could be as a marker predicting the prodnosis of colorectal adenocarcinoma patients,also it may play an important role in the pathogenesis of colorectal adenoma.
