1.Study of the alleviation mechanism of thalidomide on the immunological liver injury

Objective To study the impact of thalidomide on the immunological liver injury animal model in mice .Methods The immunological liver injury animal model was established in mice by injection of Bacillus Calmette-Guerin(BCG) and lipopolysaccha-ride (LPS) .These mice were randomly divided into five groups :normal control group ,model group ,positive medicine group (cyclo-phosphamide 100 mg/kg ,ip ,1 ,3 and 5 days after inoculation) ,Salle Lenalidomide (low ,medium and high dose group) ,and all of the mice were continuously administrated for 15 days ,the index of liver and spleen were detected ;Spectrophotometric was used to detect the levels of serum ALT ,AST and the content of MDA ,SOD ,GSH-Px in liver homogenates ;HE stain was used to exanimate the liver histopathology for the pathological grading and scoring of liver injury ;ELISA was used to measure the expression of TGF-β1 , IL-6 in liver homogenates ;RT-PCR was used to detect the mRNA expression of TNF-αin liver tissue .Results Thalidomide can re-duce the ALT ,AST ,MDA contents in these mice model ,increase the SOD ,GSH-Px activities and significantly reduce the index of liver and spleen of the mice with immunological liver injury (P<0 .05 or P<0 .01);also it can reduce the TGF-β1 ,IL 6 expression , inhibit TNF-αmRNA expression ,and reduce the damage degree of liver pathology .Conclusion Thalidomide can relieve the liver fi-brosis after immunological liver injury ,playing a protective role in the immunological liver injury mice model ,and its mechanism may be related to the inhibition of TGF-β1 and TNF-αexpression and the balance of cytokines .
2.Posterior laminectomy for thoracolumbar fracture and spinal cord compression:a follow-up on Cobb’s angle and vertebral height

BACKGROUND:Due to special physiological and anatomical location, stability of the spine is very complicated during thoracolumbar fractures. It is difficult to identify the stability of the spine. It should be based on their individual circumstances, to explore more effective internal fixation repair method.
OBJECTIVE:To explore the Cobb’s angle and vertebral height of patients with thoracolumbar fracture and spinal cord compression treated with posterior laminectomy and screw fixation, and compared with anterior laminectomy.
METHODS:One hundred patients with thoracolumbar fracture and spinal cord compression, who were treated in the Panyu District Central Hospital from January 2013 to November 2014, were enroled in this study. The patients were equaly and randomly divided into posterior laminectomy fixation group and anterior laminectomy fixation group. Tactile and sports of American Spinal Injury Association scores, Cobb’s angle and vertebral height were assessed before treatment and 1 month after treatment, and fixation effects were compared between the twogroups.
RESULTS AND CONCLUSION:(1) No significant difference in each index was detected between the two groups preoperatively (P> 0.05). (2) Tactile and sports of American Spinal Injury Association scores, Cobb’s angle and vertebral height were better in the posterior laminectomy fixation group than in the anterior laminectomy fixation group at 1 month postoperatively (P< 0.05). (3) These findings indicated that compared with the anterior laminectomy fixation, posterior laminectomy fixation for thoracolumbar fracture combined with spinal cord compression obtained better outcomes, and could obviously relieve spinal cord compression. Posterior laminectomy fixation isasafe and effective treatment method for thoracolumbar fracture and spinal cord compression.

4.An investigation and analysis for dietary nutrition in bedridden elderly patients

In total, 607 bedridden patients aged 60 years and over at Tianlin community hospital and four other hospitals or at home-care in Shanghai were investigated with questionnaire, physical examinations and related laboratory tests. Contents of investigation include general situation, medical conditions, 24-hour recall of diet, intake of nutritional supplement preparation, and so on. As compare to the reference for nutrient intake of the elderly set in the Guide to Diet for Chinese Residents (2007), total nutritional intake was obviously insufficient and three nutrients in diet were imbalanced for most bedridden elderly patients. Intake of most nutrients (including proteins, dietary fiber, calcium, iron, zinc, selenium,copper, magnesium, phosphor, retinol equivalent, vitamin B1 , vitamin B2, vitamin C) was obviously low in bedridden elderly patients, with a larger gap from the requirements of the Dietary Guide.
5.Expressions and Significance of CXC Chemokine Receptor Type 2 and Interleukin-8 in Patients with Inflammatory Bowel Disease

Background:CXC chemokine receptor type 2(CXCR2)is a member of G protein coupled receptor superfamily,and is mainly involved in the growth of tumor,angiogenesis and the pathogenesis of inflammatory diseases. Studies showed that CXCR2 was associated with the pathogenesis of inflammatory bowel disease(IBD),but the exact role has not yet been clarified. It was found that the interaction of interleukin-8(IL-8)with CXCR1 and CXCR2 played an important role in the pathogenesis of IBD. Aims:To investigate the expressions and significance of CXCR2 and IL-8 in patients with IBD. Methods:A total of 121 IBD patients in active stage from October 2013 to December 2014 at Zhongnan Hospital of Wuhan University were enrolled and assigned into Crohn’s disease(CD)group and ulcerative colitis(UC)group. Seventy healthy subjects were served as controls(HC). Expressions of IL-8 mRNA and CXCR2 mRNA in peripheral blood and intestinal mucosal tissue were determined by real-time PCR;expression of CXCR2 protein in intestinal mucosal tissue was determined by Western blotting. Results:In peripheral blood,expression of IL-8 mRNA in UC group was significantly higher than that in HC group(P = 0. 017),while expressions of CXCR2 mRNA in CD,UC and HC groups were not significantly different (P = 0. 285). In intestinal mucosal tissue,expressions of IL-8 mRNA in CD,UC and HC groups showed no significant difference(P = 0. 206),while expressions of CXCR2 mRNA in CD and UC groups were significantly higher than that in HC group(P = 0. 002;P < 0. 001),and expression of CXCR2 mRNA in UC group was significantly higher than that in CD group(P = 0. 005);expressions of CXCR2 protein in UC and CD groups were higher than that in HC group(P = 0. 049 P =0. 080). Conclusions:Expressions of CXCR2 mRNA and protein in intestinal mucosal tissue of patients with IBD, especially UC are significantly increased. Down regulation of CXCR2 expression in intestinal mucosa may provide a new target for treatment of UC. IL-8 is significantly highly expressed in peripheral blood of patients with UC,which suggests that IL-8 might be related mainly with UC.
6.The expression of integrinbeta1 and FAK in pituitary adenomas and their correlation with invasiveness.

The expression and possible role of integrin-focal adhesion kinase signal pathway in invasive pituitary adenomas were explored. Forty-nine human pituitary adenomas were detected for the expression of integrinbeta1 (INTbeta1) and focal adhesion kinase (FAK) by immunohistochemistry, and their correlation with the invasiveness of pituitary adenomas as well as between themselves was analyzed. The results showed that INTbeta1 was expressed in 46 cases (93.9%) and FAK in 36 cases (73.5%), respectively, and their expression levels were highly correlated with tumor invasiveness, but not with the tumor types. It was suggested that the integrin-focal adhesion kinase signal pathway plays a role in the invasiveness of pituitary adenomas.
7.Efficacy and Safety of Ramucirumab in the Treatment of Adenocarcinoma in Stomach or Gastroesophageal Junction:A Meta-analysis

OBJECTIVE:To systematically review the efficacy and safety of ramucirumab in the treatment of adenocarcinoma in stomach or gastroesophagealjunction,and provide evidence-based reference for clinic. METHODS:Retrieved from Wanfang database,CJFD,PubMed,EBSCO,Medline and Cochrane Library,randomized controlled trials(RCT) oframucirumab and (or) other medicines or other chemotherapy (test group) versus placebo or and Clinical Trials. gov other antibiotics(control goroup) were collected with time limit from establishment to Sept. 2015. Meta-analysis was performed by using Rev Man 5.3 software after data extraction and quality evaluation by Cochrane systematic review manual 5.1.0. RESULTS:Totally 3 RCTs involving 1 188 patients were enrolled,including 652 patients in the test group and 536 patients incontrol group. Results of Meta-analysis showed, the overall survival[HR=0.83,95%CI(0.72,0.95),P=0.006] and progression-freesurvival[HR=0.66,95%CI(0.47,0.92),P=0.01] in test group were significantly longer than control group,the difference was statistically significant;there was no significant difference in the objective response rate[RR=1.31,95%CI(0.81,2.11),P=0.28]. The incidence of severe adverse reactions(degree level:3-4 grade) in test group was significantly higher than control group,the difference was statistically significant [RR=1.14, 95%CI (1.00,1.29),P=0.04].CONCLUSIONS:Ramucirumab can extend overall survival and progression-free survival of adenocarcinoma in stomach or gastro-oesophageal junction,but the incidence of adverse reactions should be noticed.

9.Clinical Effects of Tacrolimus Combined with Okra Capsule in Treatment of Refractory Membranous Nephropathy

Objective:To investigate the effect and safety of tacrolimus combined with Okra capsule in treatment of the refractory membranous nephropathy.Methods:Selected 60 patients with refiactory membranous nephropathy who were treated in our hospital from March 2014 to October 2015,and they were divided into observation group and control group with 30 cases in each group according to the random number table method.The control group was treated with prednisone,and the observation group was treated with tacrolimus combined with Okra capsule on the basis of the control group,the courses of treatment were 6 months in two groups.The clinical effects were observed in two groups,the levels of urine protein (uPRO),serum Albumin (sALB),serum Creatinine (sCr),serum Alanine aminotransferase (sALT),tumor necrosis factor-α (TNF-α) and transforming growth factor-β 1 (TGF-β1) were detected and compared in two groups before and after treatment,and the adverse reactions were recorded in two groups.Results:The total effective rate of the ob-servation group was significantly higher than control group (P<0.05).Compared with before treatment,the levels ofuPRO,TNF-α,TGF-β1 in two groups were decreased after treatment,while sALB,sCr were increased (P<0.05),and uPRO,TNF-o,TGF-β1 in the observation group were lower than the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion:Tacrolimus combined with Huangkui capsule in the treatment ofrefractory membranous nephropathy has good curative effect,reduce renal damage,and low adverse reaction,worthy of promotion.
10.Clinical and genetic analysis of a pedigree with acne inversa

Objective To detect γ-secretase gene mutations in a large Chinese pedigree with acne inversa (AI).Methods Clinical evaluation was carried out in a large pedigree with AI through field investigation.Peripheral blood samples were obtained from 17 family members (11 affected and 6 unaffected) and 100 unrelated healthy human controls.DNA was extracted from the blood samples,and PCR was performed to amplify all the coding regions of PSEN 1,PSENEN and NCSTN genes followed by DNA sequencing analysis.Results There were 67 members over 5 generations in this family,of whom,25 (13 males and 12 females) were affected by AI.AI was inherited in an autosomal dominant manner in this family.Skin lesions were mainly distributed on the neck,back,chest and buttocks,and occasionally in subaxillary regions.DNA sequencing revealed a novel missense mutation,c.1258C> T (p.Q420XP),in the exon 11 of the NCSTN gene in 11 affected family members,which leads to a substitution of glutamine by a premature termination codon at amino acid 420 (p.Q420X).The mutation was undetected in either the unaffected members or the unrelated healthy controls,and had not been registered in the single nucleotide polymorphism (SNP) database in National Center for Biotechnology Information.Conclusions There is a novel heterozygous missense mutation,c.1258C > T in the exon 11 of the NCSTN gene,which may be the molecular basis of pathogenesis of AI in this family.
