2.Visual prognosis factors analysis of scleral buckling for primary macula-off rhegmatogenous retinal detachment

Background Many surgical techniques are used to improve the postoperative visual acuity for rhegmatogenous retinal detachment(RRD)during recent decade,and scleral buckling surgery is one of these operations.Whether the visual function after operation can be rescued is an important issue. Objective The aim of this study was to analyze the risk factors of influencing the postoperative vision following scleral buckling surgery for primary macula-off RRD. Methods The clinical and follow-up data from consecutive series of 116 eyes of 116 patients received scleral buckling surgery for primary maeula-off RRD were retrospectively reviewed.The relationship of multiple factors,including age,preoperative best corrected visual acuity(BCVA),duration of disease,refractive error,location of retinal hole,number of retinal hole,area and height of retina detachment,management of subretinal fluid and intravitreal gas injection,with visual acuity were analyzed respectively by χ2 test.The correlations among statistically significant factors with postoperative vision were analyzed by multivariate Logistic regression analysis.Written informed consent was obtained from any patient before surgery. Results The postoperative vision outcome was found with significantly difference among different preoperative vision groups(P=0.002)and different course (P=0.009).There were significant differences between the groups with different preoperative BCVA(P=0.002)and duration of disease(P=0.009).Multivariate Logistic regression analysis showed that the preoperative BCVA was the only variable affecting postoperative visual result(r=0.400,P=0.009).Considerable linear correlation wag seen between preoperative vision and postoperative vision(r= 0.400,P=0.000).The probability with postoperative vision of t>0.4 in the eyes with preoperative≥0.05 was 3 folds more than that of preoperative<0.05(OR=2.992).The better visual outcome after scleral buckling surgery was seen in the eyes with the course≤7 days. Conclusion Preoperative BCVA and duration of disease are the key factors associated with the postoperative BCVA.Scleral buckling surgery should be performed within the first week for primary macula-off RRD.

4.Effect on the treatment of congenital ptosis in two kinds of operation method

AlM:To compare the functional and cosmetic effects of two different surgical techniques for congenital ptosis.
METHODS: The patients were divided into four groups according to the operation method: Patients undertook bilateral fascial suspension surgery as Group A ( 42 eyes of 21 cases ); Patients undertook bilateral levator muscle shortening surgery as Group B ( 38 eyes of 19 cases );Patients undertook unilateral fascial suspension surgery as Group C ( 24 eyes of 24 cases ); Patients undertook unilateral levator muscle shortening surgery as Group D (29 eyes of 29 cases). Each group patients were followed for postoperative function and appearance effect.
RESULTS: 1 ) Early postoperative of two operation function success rate was up to 100%, the function of levator muscle shortening surgery was 97. 01% in the late postoperative, was higher than bilateral fascial suspension surgery (87. 88%), with statistical difference in both surgerys (P<0. 05). 2) Appearance effect of two kinds of operation method in early postoperative had no statistical difference (P >0. 05); ln the late postoperative, the mean grades for “Lid Contour” and “Lid Crease” of Group B were better than that of Group A (P<0. 01). While the mean grades for “Lid Contour”, “Symmetry of Lid Height” and “Lid Crease” of Group D were similarly better than that of Group C (P<0. 01).
CONCLUSlON: Two kinds of operation method have good effects on the treatment of congenital ptosis. ln terms of cosmetic effect, levator muscle shortening surgery is better.
5.The role of human papillomavirus in the pathogenesis of pterygium

Background The pathogenesis of pterygium has been in the study.Relative molecular biology study showed that pterygium is a tumor-like lesion,and based on overseas literatures,human papillomavirus (HPV) is positively expressed in 100% patients with pterygium in some region.However,if this result is suitable for Chinese patients is unclear.Objective This study was to identify the role of human HPV in the pathogenesis of pterygia in Wuxi area.Methods Forty-eight pterygium specimens including 7 recurrent pterygia and 41 primary pterygia were collected during the operation,and these patients were from Wuxi area.Two cervical carcinoma specimens and 2 conjunctiva specimens from normal donors were obtained as positive control and negative control respectively.The fluorescent quantitative polymerase chain reaction (FQ-PCR) was used to detect HPV DNA of specimens.Results The amplified curves of HPV 6,11 of pterygium specimens,cervical carcinoma specimens and normal specimens were all below the positive quality control curve,but the amplified curves of HPV16,18 were above the quality control curve in cervical carcinoma specimens; while those of pterygium specimens and normal conjunctival specimens were all below the quality control curve.HPV16/18 was identified in 2 cervical carcinoma specimens,but no HPV6/11 was detected in 2 cervical carcinoma specimens.However,HPV DNA expression in primary and recurrent pterygias were absent.Conclusions According to these results,HPV is not the primary cause for the pathogenesis of pterygium in Wuxi region.
6.Effect of the evidence-based nursing educational programme for preventing ventilator-associated pneumonia in critical care nurses

Objective To establish the evidence-based nursing educational programme for preventing ventilator-associated pneumonia (VAP) and evaluate the effects in critical care nurses. Methods The evidence-based nursing practice guidelines for preventing VAP from 2008 American Association of Critical Care Nurses was used as training materials,which guided the curriculum design. Seventeen critical care nurses were trained. The difference of the nurses' knowledge and practice was evaluated before and after training by an evaluation scale of nursing quality criteria for preventing VAP. Results After the training,nurses' knowledge and practice of preventing VAP were improved(Z=-3.624,P=0.000). Except the factor score of handwashing,there were significant differences on the factor scores of body position care,endotracheal tube care,enteral nutrition,maintaining the tube cuff pressure,oral care and ventilator equipment management (P<0.01). Conclusion The educational programme of evidence-based nursing for preventing VAP can improve the nurses' knowledge and practice of preventing VAP. Furthermore,it can promote the application of evidence-based nursing for preventing VAP,and thereby to improve the quality of nursing,and provide guidance for the training of critical care nurses and continuing education.
7.Germ cell membrane lipids in spermatogenesis.

Spermatogenesis is a complex developmental process in which a diploid progenitor germ cell transforms into highly specialized spermatozoa. During spermatogenesis, membrane remodeling takes place, and cell membrane permeability and liquidity undergo phase-specific changes, which are all associated with the alteration of membrane lipids. Lipids are important components of the germ cell membrane, whose volume and ratio fluctuate in different phases of spermatogenesis. Abnormal lipid metabolism can cause spermatogenic dysfunction and consequently male infertility. Germ cell membrane lipids are mainly composed of cholesterol, phospholipids and glycolipids, which play critical roles in cell adhesion and signal transduction during spermatogenesis. An insight into the correlation of membrane lipids with spermatogenesis helps us to better understand the mechanisms of spermatogenesis and provide new approaches to the diagnosis and treatment of male infertility.

9.Sulfogalactosylglycerolipid in spermatogenesis and fertilization.

Sulfogalactosylglycerolipid (SGG) is the main glycolipid in male mammalian germ cells, which is selectively and highly expressed in mammalian testes and helps form the lipid bilayer of cell membrane. In the process of spermatogenesis, SGG is involved in the meiosis of spermiocytes. Either deficiency or accumulation of SGG will lead to male infertility. SGG homeostasis in the testis is the premise of normal spermatogenesis. In the process of sperm-zona binding, SGG becomes a component of lipid raft and provides a platform for signal transduction. The SGG binding protein plays a role in sperm-egg recognition and membrane fusion. SGG has a great research value and application prospect in male reproduction.

10.Expression of B cell activating factor-receptor in the sera of patients with primary Sj(o)gren' s syndrome

Objective To determine the serum levels of B cell activating factor-receptor(BAFF-R)in patients with primary Sj(o)gren's syndrome(pSS)and investigate the role of BAFF-R in the pathogenesis of pSS.Methods The serum concentration of BAFF-R was measured by ELISA in 120 patients with pSS and 62 age-and sex-matched normal controls.Results The median level of serum BAFF-R was(692.7 ± 536.9)ng/L in pSS and (279.7 ± 186.9)ng/L in healthy controls.The serum level of BAFF-R in the pSS was significantly higher than normal controls(P <0.01).Higher level of BAFF-R was associated with higher level of IgG(r =0.429,P <0.01).Serum level of BAFF-R had no correlation with IgA,IgM,C3,C4,RF,ESR and CRP.BAFF-R level was markedly increased in patients with positive ANA,anti-SSA,anti-SSB antibody compared with those with negative antibody.Conclusion The serum BAFF-R level was significantly elevated and correlated with the level of immunoglobulin and autoantibodies.BAFF-R play certain role in the pathogenesis of pSS.
