1.High resolution esophageal manometry of patients with lower esophageal leiomyoma

Objective To explore the effects of lower esophageal leiomyoma on esophageal dynamics.Methods A total of 25 patients with lower esophageal leiomyoma,31 patients with gastroesophageal reflux disease (GERD) and 16 healthy controls were selected.The differences of high resolution esophageal manometry results were compared among the groups.The t-test or one way analysis of variance was performed for normally distributed measurement data comparison.Rank sum test was used for non-normally distributed measurement data comparison.Results Among 25 patients with lower esophageal leiomyoma,14 patients accompanied with acid reflux,heartburn,chest pain or other GERD symptoms.In the group of patients with lower esophageal leiomyoma,the lower esophageal sphincter pressure (LESP),the lower esophageal sphincter relaxation rate (LESRR),low esophageal body pressure and peristaltic contraction percentage were (9.00 ± 6.30) mmHg (1 mmHg =0.133 kPa),53.0 % (0,334.0%),(34.66±18.33) mmHg and 55.6% (0,100.0%),respectively,which were lower than those of the healthy control group ((16.25 ± 3.71) mmHg,86.5% (49.0%,103.0%),(57.75 ± 22.49) mmHg,100.0% (80.0%,100.0%)),and the differences were statistically significant (t=-4.150,Z=-2.353,t=-3.601,Z=-3.798; all P<0.05).However there were no significant differences in esophageal dynamics indexes between patients with lower esophageal leiomyoma accompanied GERD symptoms and without GERD symptoms (all P>0.05).There were no significant differences in esophageal dynamics indexes between the GERD patients and patients with lower esophageal leiomyoma accompanied GERD symptoms (all P>0.05).LESRR level of patients with lower esophageal leiomyoma (71.4%(45.5%,150.0%)) increased after endoscopic submcosal dissection (Z=-2.194,P=0.028).Conclusions Lower esophageal leiomyoma may affect esophageal motor function,which contributed to anti-reflux esophageal function decline.Lower esophageal leiomyoma combined with gastroesophageal reflux symptoms are not uncommon.
2.Effects of hypoxia and vibration training on bone metabolism and insulin sensitivity in type 2 diabetic osteoporosis rats

BACKGROUND: Hypoxia program to alleviate type 2 diabetes insulin resistance has been recommended, but this program is still questioned because of the risk of osteoporosis caused by hypoxia in patients with diabetes. OBJECTIVE: To investigate the effect of vibration training on bone mineral density, bone structural mechanics, bone metabolism and insulin sensitivity in type 2 diabetic osteoporosis rats under hypoxia environment. METHODS: Ninety clean Sprague-Dawley rats were randomly divided into two groups, and subjected to high-fat diet (n=60), or normal diet (n=30), for 8 weeks. High-fat rats were given the injection of streptozotocin to establish the rat model of type 2 diabetic osteoporosis. The control rats were subdivided into normoxia control and hypoxia control groups; the model rats were subdivided into hypoxia modeling group, hypoxia modeling vibration group, normoxia modeling group, normoxia modeling vibration group. Hypoxia and vibration program was performed by hypoxia tank and vibration platform (PowerPlate?) for 12 weeks. Glucose metabolism, insulin sensitivity, bone metabolism and bone mineral density and modeling were detected at 4 weeks after modeling and 12 weeks after vibration training. RESULTS AND CONCLUSION: At 12 weeks after intervention, the fast insulin level, fast blood glucose, and homeostasis model assessment of insulin resistance in the hypoxia modeling vibration group were significantly superior to those in the hypoxia modeling, normoxia modeling, and normoxia modeling vibration groups (P < 0.05). The bone mineral density, maximum stress, maximum load, breaking load and elastic modulus in the normoxia modeling vibration and hypoxia modeling vibration groups were significantly lower than those in the normoxia control and hypoxia control groups (P < 0.05). After vibration training, all indexes were significantly increased (P < 0.05). These results suggest that hypoxic environment can promote the insulin sensitivity, improve glucose and lipid metabolism in type 2 diabetic rats, but can lead to a decrease in bone mineral density and increase bone resorption. Vibration training not only can significantly enhance the insulin sensitivity, but also can avoid the decreased bone mineral density, bone metabolism disorder, and biomechanical properties induced by hypoxia.
3.Expression of integrin?_3 and integrin ?_1 in breast cancer and its clinical significance

Objective To investigate the expression of inregrin?_3 and integrin?_1 in breast cancer and its bio- logical significance.Methods Immunohistochemical assay was used to determine the expression of integrin?_3 and integrin?_1 in the breast cancer(32 cases).Results In normal breast tissue,the positive expression rates of integrin?_3 and integrin?_1 were 0 % and 25 %.In the breast cancer tissue,the positive expression rates of integrin?_3 and inte- grin?_1 were 36 % and 81%.Conclusion The integrin?_3 and integrin?_1 are close associated with the biological sig- nificance of breast cancer.To examine its expression is useful to evaluate the aggressive degree,metastatic potential and prognosis in patients with breast cancer.
4.Flavonoids from whole plants of Lagopsis supina.

The flavonoids were investigated from the whole plants of Lagopsis supina. The compounds were isolated and purified by various column chromatography, and their structures were identified by physiochemical properties and spectroscopic data. Two flavones were isolated from the CH2Cl2 layer of Lagopsis supina extract and identified as genkwanin (1) and 5-hydroxy-7,4'-dimethoxyflavone (2), respectively. Ten flavonoid glycosides were isolated from the water layer of Lagopsis supina and elucidated as kaempferol-3-O-6" (3-hydroxy-3-methylglutaryl) -β-D-glucoside (3), quercetin-3-O-6"-(3-hydroxy-3-methylglutaryl) -β-D-glucoside (4), quercetin-3-O-β-D-glucoside(5), kaempferol-3-Of3-D-glucoside ( 6), isorhamnetin-3-O-/-D-glycopyranoside (7), apigenin-7-O-6-D-glucoside (8), luteolin-7-O-β-D-glucoside (9), chrysoeriol-7-O-β-D-glucoside (10), rutin (11 ), and kaempferol-3-β-(6"-p-coumaroyl) -β-D-glucoside (tiliroside, 12). Compounds 3 and 4 were firstly isolated from Lamiaceae family, and compounds 1-12 were isolated from the plants of Lagopsis genus for the first time.

5.Inhibitory action of Potentilla anserine polysaccharide fraction on H2O2-induced apoptosis of murine splenic lymphocytes.

A water-soluble polysaccharide fraction from root of Potentilla anserine was obtained. Gas chromatogram, FT-IR, physical and chemical characteristics of the Potentilla anserine polysaccharide fraction (PAPF) were analyzed. The protective effects of PAPF against the H2O2 induced process of apoptosis of murine splenic lymphocytes were investigated in vitro. Morphological assessment of apoptosis was performed with light microscope and laser scanning confocal microscope. DNA fragmentation was visualized by agarose gel electrophoresis. The amount of apoptotic cells was measured by flow cytometry. The results showed that PAPF is composed of rhamnose, arabinose glucose and galactose. H2O2 (200 micromol x L(-1)) induced apoptosis of murine splenic lymphocytes with the cell volume reduced, cytoplasm and nuclear shrunk and DNA stained non-uniformly. Condensed chromatin and formation of apoptotic body were observed in the apoptotic cells. Apoptotic bodies in the cells treated with PAPF and H2O2 were less than those in H2O2 treatment alone. DNA fragmentation assay showed that PAPF (50, 100, 200, and 400 microg x mL(-1)) obviously reduced H2O2-induced ladder bands. Flow cytometry analysis showed that H2O2 increased the populations of apoptotic sub-G1 cells from 5.60% (control) to 45.40%, and PAPF decreased H2O2-induced apoptosis to 37.80%, 22.70%, 17.70%, and 8.50%, respectively. In conclusion, PAPF reduced H2O2-induced oxidative damage in a dose dependent manner.

6.Direct Versus Pre-dilated Rotational Atherectomy for Treating Calcified Coronary Lesions

Objective: To compare the efifcacy of direct and pre-dilated atherectomy (RA) for treating the patients with calciifed coronary lesions. Methods: A total of 137 coronary artery disease (CAD) patients received RA treatment in our hospital from 2010-04 to 2014-09 were retrospectively studied. The ischemic related lesions were all deifned as calciifed coronary lesions. The patients were divided into 2 groups: Direct RA group,n=81 and Pre-dilated RA group, the patients received balloon dilatation followed by RA,n=56. The procedural features, complications, in-hospital and 1 year occurrence rates of major cardiaccerebral vascular events (MACCE) were compared between 2 groups. Results: Compared with Pre-dilated RA group, Direct RA group had the less pre-stent balloon application,P=0.000 and the higher maximum post-dilatationpressure,P=0.004; lower rate of in-operative complication (14.8% vs 32.1%),P=0.016; higher rate of acute lumen gain (128.52±75.77) % vs (77.12±27.01) %,P=0.004; lower MACCE occurrencerate(7.3% vs 23.6%) at 1 year period,P=0.006.Cox regression analysis presented that the following indexes were related to MACCE occurrence within 1 year of RA treatment: balloon dilatation before RA (HR=8.166, 95% CI 1.872-35.614,P=0.005), left main disease (HR=13.649, 95% CI 2.983-62.440,P=0.001), minimum post-operative lumen area (HR=0.583, 95% CI 0.378-0.879,P=0.010), post-dilatation (HR=0.066, 95% CI 0.013-0.332,P=0.001) and EF>40% (HR=0.019, 95% CI 0.002-0.158, P=0.000). Conclusion: Direct RA had the lower MACCE occurrence rate at 1 year period, this might be related to less operative complication and the optimal lumen gain.
8.Simultaneous determination of 3 phenolic acids in Usnea by HPLC-ESI-MS/MS.

A quick HPLC-ESI-MS/MS method was established for simultaneous determination of three chemical compositions in Usnea, including usnic acid, diffractaic acid, and ramalic acid. The separation was performed on a chromatographic column of Agilent ZORBAX SB-C, (4.6 mm x 250 mm, 5 µm), and the mobile phase was methanol (0.05% formic acid)-0.05% formic acid solution (4 mmol ammonium acetate), with an isocratic elution at a flow rate of 0.8 ml · min⁻¹. Multiple reaction monitoring scanning mode (MRM) was performed combined with the ion switching technology in positive and negative ion switching mode to apply for the quantitative determination. The calibration curves for the above three compounds were linear in corresponding injection amount. Their average recoveries were 95.0%-105.1%, with RSDs of 1.1%-5.2%. The method was simple, rapid, accurate with high repeatability, which could provide a reference for overcalling evaluation the quality of Usnea.

9.Advances in BET bromodomain protein inhibitors

Recognizing acetyl-lysine of histone is a key process of epigenetic regulation that is mediated by a protein module called bromodomain(BRD). The bromodomain inhibitors of the bromodomains and extra-terminal(BET)family have shown great potential in anti-inflammatory and antiproliferative effects. Through a review of diseases and structures about BET bromodomain, different kinds of inhibitors were analyzed and their structure-activity relationships were summarized. Herenin, the recent advances reported are reviewed for discovering more excellent small molecule inhibitors.
10.Tobacco epidemic and related factors among junior high school students in Beijing in 2019 and 2023

Objective:
To understand the prevalence and related factors of tobacco use among junior high school students in Beijing, so as to provide evidence to effectively conduct tobacco control intervention strategy.
Methods:
From April to June in 2019 and 2023, 6 489 and 6 898 junior high school students were selected by the probability proportion to size(PPS) method, and a total of 13 387 questionnaires were completed. Questionnaire on tobacco monitoring among junior high school students in Beijing was completed by selffilling. The monitoring content included demographic information, secondhand smoke exposure, tobacco product use, tobacco awareness, etc. Chisquare test was used to compare the difference of various indicators in different groups, and multivariate Logistic regression analysis was adopted to analysis the influencing factors related to the current smoking among junior high school students.
Results:
In 2019,the current traditional cigarettes and ecigarette smoking rates among junior high school students in Beijing were 1.34%, 1.88%, respectively, and decreased to 0.81%, 1.06% in 2023 (χ2=8.36, 15.17, P<0.01). The attempted traditional cigarettes and ecigarette smoking rates among junior high school students in Beijing decreased from 6.67%, 6.47% in 2019 to 3.93%, 4.16% in 2023 (χ2=49.99, 35.26, P<0.01). In 2019, the secondhand smoke exposure rates of junior high school students at homes, indoor public places, and outdoor public places were 31.04%, 44.94%, and 49.88% respectively which decreased to 22.59%, 30.23%, and 36.14% in 2023 (χ2=121.63, 308.60, 257.41, P<0.01). In 2023, male students (OR=2.88), senior students (grade 2 and 3) (OR=4.37, 4.92), disposable pocket money>20 yuan/week (OR=2.01), secondhand smoke exposure at home (OR=2.74), indoor public places (OR=2.64), perception that smoking makes young people more attractive (OR=6.29), and perception that ecigarettes help quit smoking (OR=4.31) were associated with higher tobacco use (P<0.05).
Conclusions
Tobacco use and secondhand smoke among junior high school students in Beijing decrease significantly. Tobacco control interventions should be provided for junior high school students continuously with a focus on ecigarettes use to promote physical and mental health development among students.
