5.Descending necrotizing mediastinitis: report of two cases and literature review
Chinese Journal of General Practitioners 2009;8(4):262-264
We reported 2 cases of descending neerotizing mediastinitis (DNM) and reviewed the related literatures. The etiology, clinical manifestations, imaging manifestations, diagnostic criteria and therapeutic strategies of DNM were discussed. It is concluded that descending necrotizing mediastinitis is a life-threatening disease with high mortality and is prone to be misdiagnosed. Early diagnosis, prompt application of sensitive broad-spectrum antibiotics and abscess drainage with proper surgical medality may improve the prognosis of DNM.
6.Therapeutic effect of rehabilitation intervention on patients with diabetes mellitus complicated tubercu-losis
Chinese Journal of cardiovascular Rehabilitation Medicine 2015;24(4):359-362
Objective:To explore therapeutic effect of rehabilitation intervention on patients with diabetes mellitus (DM)complicated tuberculosis (TB).Methods:A total of 108 DM + TB patients treated in our hospital from Jan 2008 to Aug 2013 were enrolled.According to random number method, they were randomly and equally divided into rehabilitation intervention group and routine treatment group. The intervention course was eight weeks, and then rehabilitation effect was compared between two groups. Results:Compared with before treatment, after treatment, there were significant reductions in fasting blood glucose (FBG)and 2h postprandial blood glucose (2hPBG)in both groups,P <0.01 all;compared with routine treatment group, after treatment, there were significant reductions in levels of FBG [(6.77±1.82)mmol/L vs.(5.54±1.46)mmol/L]and 2hPBG [(7.94±1.83)mmol/L vs.(6.87 ±1.42)mmol/L],P <0.01 both, and significant rise in percentages of significant reduced or disappeared pleural effusion (51.85% vs.83.33%)and significant reduced or closed TB focus (44.44% vs.72.22%)in rehabilitation intervention group,P <0.01 both. Conclusion:Rehabilitation intervention can make patients get better control of blood glucose, promote absorption of pleural effusion and pulmonary tuberculosis focus.
7.Visual prognosis factors analysis of scleral buckling for primary macula-off rhegmatogenous retinal detachment
Chinese Journal of Experimental Ophthalmology 2011;29(3):261-264
Background Many surgical techniques are used to improve the postoperative visual acuity for rhegmatogenous retinal detachment(RRD)during recent decade, and scleral buckling surgery is one of these operations.Whether the visual function after operation can be rescued is an important issue. Objective The aim of this study was to analyze the risk factors of influencing the postoperative vision following scleral buckling surgery for primary macula-off RRD. Methods The clinical and follow-up data from consecutive series of 116 eyes of 116 patients received scleral buckling surgery for primary maeula-off RRD were retrospectively reviewed. The relationship of multiple factors, including age, preoperative best corrected visual acuity(BCVA),duration of disease, refractive error, location of retinal hole, number of retinal hole, area and height of retina detachment, management of subretinal fluid and intravitreal gas injection, with visual acuity were analyzed respectively by χ2 test. The correlations among statistically significant factors with postoperative vision were analyzed by multivariate Logistic regression analysis.Written informed consent was obtained from any patient before surgery. Results The postoperative vision outcome was found with significantly difference among different preoperative vision groups(P=0.002)and different course (P=0.009).There were significant differences between the groups with different preoperative BCVA(P=0.002)and duration of disease(P=0.009).Multivariate Logistic regression analysis showed that the preoperative BCVA was the only variable affecting postoperative visual result(r=0.400,P=0.009).Considerable linear correlation wag seen between preoperative vision and postoperative vision(r= 0.400,P=0.000).The probability with postoperative vision of t>0.4 in the eyes with preoperative≥0.05 was 3 folds more than that of preoperative<0.05(OR=2.992).The better visual outcome after scleral buckling surgery was seen in the eyes with the course≤7 days. Conclusion Preoperative BCVA and duration of disease are the key factors associated with the postoperative BCVA.Scleral buckling surgery should be performed within the first week for primary macula-off RRD.
8.Improving effect of testicular tissue graft on impaired learning and memory in endocrine recession mice
Chinese Journal of Organ Transplantation 2015;36(4):235-240
Objective To study the improving effect of testicular tissue graft on impaired learning and memory in endocrine recession mice, and to explore the possible mechanisms.Method Forty Kunming male mice were randomly divided into four groups by random number table:sham operation group, castration group, androgen (30 mg/kg) control group and testicular tissue graft group. The bilateral testes of mice were cut off to establish castrated mouse model. After modeling, the back subcutaneous muscle layers of castrated mice were implanted on the allogeneic testicular tissue blocks in graft group.Androgen group mice were intragastrically administrated with androgen for three months, and other two groups of mice were given an equal volume of normal saline At the end of treatment, the levels of serum testosterone in each group of mice were measured by ELISA.Learning and memory abilities of the mice were assessed by Morris water maze behavioral test.Morphological changes in the hippocampal CA1 region were observed through hematoxylin-eosin staining (HE).The expression levels of AR and apoptosis-related proteins (Bcl-2,Bax,caspase-3 and caspase-9) in the brain were detected by Westem blotting.Result As compared with castration group, the levels of serum testosterone in graft group were increased significantly (P<0.01),learning and memory abilities were greatly strengthened (P < 0.01),the morphological structures in hippocampal CA1 neurons were improved, and the expression levels of Bax,caspase-3 and caspase-9 were significantly reduced (P<0.01),while the expression levels of AR and Bcl-2 had obvious enhancements (P <0.01).Conelusion Testicular tissue graft can improve learning and memory ability probably by secreting endogenous testosterone which will bind with androgen receptor in the brain, then regulating the expression of neuronal apoptosis related genes, increasing Bcl-2 protein level and reducing the expression of Bax,caspase-3 and caspase-9 protein, and finally inhibiting neuronal apoptosis.
9.Pay attention to the research hotspots and evidence quality for the diagnosis and treatment of non-arteritic anterior ischemic optic neuropathy
Chinese Journal of Ocular Fundus Diseases 2015;31(6):521-523
Non-arteritic anterior ischemic optic neuropathy (NAION) is one of the most common acute optic neuropathy in adult characterized with impaired visual acuity and visual fields. The pathogenesis of NAION mostly result from the interactions between the systemic risk factors (such as diabetes mellitus, night hypotension, hereditary) and the local ocular risk factors (such as small optic disc and vitreo-papillary traction).A fully promoted diagnosis and treatment of NAION are based on the higher levels of clinical cvidence, as well as the comprehensive assessment of relationship between the systemic and ocular risk factors in the pathogenesis of NAION.Secondary optic neuropathy of NAION and the early diagnosis with effective treatment of the fellow eye would be highly emphasized.
10.Diagnostic significance of the uric acid in preeclampsia
Journal of Chinese Physician 2017;19(1):75-78
Objective To explore the diagnostic significance of the uric acid in preeclampsia. Methods The samples were chosen between January 2008 and August 2015 in our hospital maternity clinic,at 24 to 28 weeks of gestation,singletons,1 357 cases of pregnant women,136 cases for the development of preeclampsia, and 1 221 cases of normal pregnancy.the clinical data of 1 357 cases were retrospectively analyzed. Results (1) The maternal age, birth gestational age, body mass index, systolic blood pressure, diastolic blood pressure, urine protein, cholesterol, creatinine, uric acid, and uric acid increased value of the control group of preeclampsia were significandy higher than the normal group (P < 0.05).The incidence of preeclampsia, cesarean delivery rate, fetal growth restriction, small gestational age,and newborn asphyxia rate of high uric acid hematic disease group were significandy higher than the normal group (P < 0.05).(2) Logistic regression analysis showed that systolic blood pressure, urinary protein and uric acid were the predictors, and uric acid was the best predictor.(3) By using receiver operating characteristic (ROC) curve, the growth of uric acid was 112 μmol/L,and the sensitivity was 85.2%,the specificity was 90.1%.Conclusions Uric acid level can be used as an important indicator to monitor the pathophysiology of preeclampsia, and uric acid value can be accurately predicted.
