Objective Clinical data of 19 Chinese patients with 21 hydroxylase deficiency (21OHD) were analyzed to improve the diagnosis and treatment level. Methods Clinical features and laboratory data were collected from 19 patients with 21OHD before and after treatment. Results In male patients, the average age of early appearance of secondary sexual character was (9.3?2.8)yrs, and excess androgen resulted in phallic enlargement. Primary amenorrhea was the most common complaint in female(87.5%), and the signs included a varying degree of labioscrotal fusion and clitoral enlargement. The average level of 17-hydroxy progesterone(17OHP) was (63.42?35.07) ?g/L, and adrenocorticotrophic hormone(ACTH), dehydroepiandrosterone(sodium) sulfate(DHEAS) and testosterone(T) were obviously elevated. 3.Balloon 32P internal radiotherapy with concurrem local chemotherapy for brain glioma: a clinical evaluation

Objective To evaluate the clinical efficacy of balloon 32P internal radiotherapy with concurrent local chemotherapy in the treatment of brain glioma. Methods A total of 228 patients with brain gliomas were enrolled in this study. Of these patients, 64 received conventional radiotherapy (group A), 93 received balloon 32P internal radiotherapy (group B), and 71 had internal radiotherapy with concurrent local chemotherapy (group C). The response rates, average survival period, and the 1, 3, and 5-year survival rates were compared between the 3 groups to evaluate the clinical efficacy of the treatments. The Kamofsky's Performance Scores (KPS) after the treatments were also compared to assess the adverse effects associated with the treatments Results Significant differences were found in the response rates, average survival period and 1, 3, and 5-year survival rates between the 3 groups. The KPS scores after the treatment differed significantly between groups B and A and between C and A, but not between B and C. Conclusion Balloon 32P internal radiotherapy produces significantly better effect than conventional radiotherapy, and concurrent local chemotherapy even further enhances the effect of balloon 32P internal radiotherapy. Conventional radiotherapy results in greater adverse effects than the other two procedures, which produce comparable adverse effects.
4.Application of urinary proteomics in early diagnosis of bladder urothelial carcinoma.

OBJECTIVETo investigate the difference in urinary proteome between patients with bladder urothelial carcinoma (BUC) and healthy volunteers and to provide a basis for the early diagnosis of BUC.
METHODSThe urine samples from BUC patients and healthy volunteers (controls) were treated by 25% ethanol precipitation and two-dimensional gel electrophoresis (2-DE), and the obtained urinary proteins were subjected to Coomassie brilliant blue staining and analysis by PDQuest 8.0 (2-DE image analysis software); the differentially expressed proteins were sequenced by matrix-assisted laser desorption/ionization time-of-flight/time-of-flight mass spectrometry and identified using the Swiss-Prot database; the differential expression of these proteins was verified by western blot.
RESULTSHigh-resolution and high-reproducibility 2-DE images were obtained from the urine samples of BUC patients and controls, with 789 ± 18 and 762 ± 14 protein spots, respectively. Compared with the control group, the BUC grouP had significantly decreased expression of 6 protein spots and significantly increased expression of 11 protein spots. The mass spectrometry revealed five proteins with increased expression in the BUC group, including fibrinogen, lactate dehydrogenase B, apolipoprotein A1, clusterin, and haptoglobin, and the results were confirmed by western blot.
CONCLUSIONThere is significant difference in urinary proteome between BUC patients and healthy volunteers; the identification of differentially expressed proteins in urine lays the foundation for identifying potential molecular markers in early diagnosis of BUC.

5.A cross-sectional study on injuries in residents from the countryside of Huanghe Delta rural areas.

OBJECTIVETo study the incidence of injuries among residents living in the countryside of Huanghe delta area.
METHODSA household questionnaire survey was conducted to 15 276 residents in 20 villages of Dongying municipality of Shandong province with a stratified-cluster sampling on their injuries from March 1, 2002 to February 28, 2003. Data were analyzed with Excel 2000 and SPSS 11.0 software.
RESULTSThe crude incidence of injuries was 5.90% in total, and the standardized incidence was 5.93%. It was higher in men (7.79%) than in women (4.03%). There were 19 deaths with 20 cripples. The standardized death rate was 122.56 per 100 000 with leading causes of injuries was blunt or by sharp articles (24.61%), traffic accident (24.17%), falls (22.62%) and animal bites (13.08%). Peak incidence of age group was high in 25 - 54 age group and 0 - 4 age group (> 6.0%). 267 cases (29.60%) inpatients had had about 15.89 days hospitalization for each case. Rest of each case with injury had 19.20 days of rest. Direct economic loss for treatment would cost 904.85 RMB Yuan and 10.15 days with care takers and 221.88 RMB for other cost. The potential years of life lost was 24 years, the working years of life lost was 19.6 year, the valued years of life lost was 8.7 year, and the standardized period expected years of life lost was 31.73 year.
CONCLUSIONInjury was common and frequently occurred among residents in the countryside of rural Huanghe delta areas, that seriously endangered the health care systems and burden on families.

6.Nerve growth factor upregulates sirtuin 1 expression in cholestasis: a potential therapeutic target

This study investigated the regulatory role of nerve growth factor (NGF) in sirtuin 1 (SIRT1) expression in cholestatic livers. We evaluated the expression of NGF and its cognate receptors in human livers with hepatolithiasis and the effects of NGF therapy on liver injury and hepatic SIRT1 expression in a bile duct ligation (BDL) mouse model. Histopathological and molecular analyses showed that the hepatocytes of human diseased livers expressed NGF, proNGF (a precursor of NGF), TrkA and p75NTR, whereas only p75NTR was upregulated in hepatolithiasis, compared with non-hepatolithiasis livers. In the BDL model without NGF therapy, p75NTR, but not TrkA antagonism, significantly deteriorated BDL-induced liver injury. By contrast, the hepatoprotective effect of NGF was abrogated only by TrkA and not by p75NTR antagonism in animals receiving NGF therapy. Intriguingly, a positive correlation between hepatic SIRT1 and NGF expression was found in human livers. In vitro studies demonstrated that NGF upregulated SIRT1 expression in mouse livers and human Huh-7 and rodent hepatocytes. Both NGF and proNGF induced protective effects against hydrogen peroxide-induced cytotoxicity in Huh-7 cells, whereas inhibition of TrkA and p75NTR activity prevented oxidative cell death. Mechanistically, NGF, but not proNGF, upregulated SIRT1 expression in human Huh-7 and rodent hepatocytes via nuclear factor (NF)-κB activity, whereas NGF-induced phosphoinositide-3 kinase/Akt, extracellular signal–regulated kinase and NF-κB signaling and SIRT1 activity were involved in its hepatoprotective effects against oxidative injury. These findings suggest that pharmacological manipulation of the NGF/SIRT1 axis might serve as a novel approach for the treatment of cholestatic disease.

OBJECTIVEIn mid-July 2005, five patients presented with septic shock to a hospital in Ziyang city in Sichuan, China, to identify the etiology of the unknown reason disease, an epidemiological, clinical, and laboratory study were conducted.
METHODSAn enhanced surveillance program were established in Sichuan, the following activities were introduced: active case finding in Sichuan of (a) laboratory diagnosed Streptococcus suis infection and (b) clinically diagnosed probable cases with exposure history; supplemented by (c) monitoring reports on meningococcal meningitis. Streptococcus suis serotype 2 infection was confirmed by culture and biochemical reactions, followed by sequencing for specific genes for serotype and virulence factors.
RESULTSFrom June 10 to August 21, 2005, 68 laboratory confirmed cases of human Streptococcus suis infections were reported. All were villagers who gave a history of direct exposure to deceased or sick pigs in their backyards where slaughtering was performed. Twenty six (38%) presented with toxic shock syndrome of which 15 (58%) died. Other presentations were septicaemia or meningitis. All isolates were tested positive for genes for tuf, species-specific 16S rRNA, cps2J, mrp, ef and sly. There were 136 clinically diagnosed probable cases with similar exposure history but incomplete laboratory investigations.
CONCLUSIONAn outbreak of human Streptococcus suis serotype 2 infections occurred in villagers after direct exposure to deceased or sick pigs in Sichuan. Prohibition of slaughtering in backyards brought the outbreak to a halt. A virulent strain of the bacteria is speculated to be in circulation, and is responsible for the unusual presentation of toxic shock syndrome with high case fatality.
