1.Primary cutaneous CD30 positive lymphoproliferative disorders: a clinicopathologic analysis

Objective To analyze the clinicopathologic features of primary cutaneous CD30 positive lymphoproliferative disorders. Methods A clinical, pathological and immunohistochemical analysis was carried out in 4 cases of lymphomatoid papulosis and 5 cases of primary cutaneous anaplastic large cell lymphoma. Results Lymphomatoid papulosis was divided into 3 subtypes, A, B and C. The lymphomatoid papulosis of subtype A was pathologically characterized by pleomorphic anaplastic large cells or Steinberg-reed cells scattered or patchly distributed among many inflammatory cells; subtype B showed pathological changes characteristic of granuloma fungoides, and manifested as a broad infiltration zone of lymphocytes in dermis with scattered small- to middle-sized atypical gyrus-like lymphocytes; subtype C was characterized by a diffuse distribution of anaplastic large cells and could clinically subside spontaneously. Primary cutaneous anaplastic large cell lymphoma clinically manifested as subcutaneous nodules or papules, and was pathologically characterized by large, pleomorphic, round or ellipse cells with plentiful, eosinophilic or bicolor cytoplasm, large nuclei and obvious nucleoli. The neoplastic cells characteristically expressed CD30 antigen in both lymphomatoid papulosis and primary cutaneous anaplastic large cell lymphoma, and all the cases showed a favorable prognosis.Conclusions Primary cutaneous CD30 positive lymphoproliferative disorders are a spectrum of cutaneous T cell lymphoma with favorable prognosis, and a synthetic analysis of clinical, histopathological and immunohistochemical findings is beneficial to the diagnosis of these entities.
2.Quantitative and qualitative research in cognition of clinical nurse specialist

Objective To study the clinical nurses and nursing students' cognition of clinical nurse specialist (CNS) and provide the basis for promoting the development of CNS.Methods Quantitative and qualitative research were adopted in this study.One hundred and ninety clinical nurses and nursing students were investigated with a questionnaire about the cognition of CNS,and the qualitative research was conducted on 9 clinical nurses selected by purposive sampling method.Results Most participants realized the importance of CNS and 78.4% of them wished to be an excellent CNS.The cognition was different between the two groups,especially in terms of ability,working performance and status.Conclusions We should improve the clinical nurses and nursing students' cognition of CNS,and cultivate them with the core competence of CNS.Meanwhile,improving the status and confirming the role must be done to promote the development of CNS in China.
3.The comparison of MR intragate and electrocardiogram-respiration triggered fast low-angle shot in imaging the structure and function of the heart

ObjectiveA comparison between intragate and ECG-respiration triggered techniques was performed to determine their differences in measuring the structure and function of the heart at 7.0 T.MethodsTen normal ICR mice aged five to six weeks were examined on a 7.0 T MR scanner.A central slice with papillary muscle included at the short-axis view was scanned with a FLASH-cine bright blood sequence,FLASH-cine black blood sequence,IG-FLASH-sat-cine black blood sequence,and IG-FLASH-cine bright blood sequence.The area of the left ventricle of the end systole and end diastole ( including and excluding the myocardium) was measured with manually outlined ROIs.The increased area of the left ventricle and the myocardium from the end systolic to end diastolic phases was calculated.The signal intensity was measured from 8 ROIs which were evenly located at the myocardium of the end systole,and the mean and standard deviation were then determined.The coefficient of variation ( CV ) was derived by dividing the mean into the standard deviation.ResultsThere was no significant difference ( the increased area of the myocardium t =0,P =1,the increased area of the left ventricle t =2.12,P =0.06) in the function index between the ECG-triggered black blood sequences [the increased area of the myocardium (0.100 ±0.018) cm2,the increased area of the left ventricle (0.060 ± 0.024) cm2] and intragate black blood sequences[the increased area of the myocardium (0.090 ± 0.014) cm2,the increased area of the left ventricle (0.060 ±0.012) cm2].No significant difference(the increased area of the myocardium t =1.56,P =0.15,the increased area of the left ventricle t =2.08,P =0.07 ) in the function index was observed between the ECG-triggered bright blood sequences [the increased area of the myocardium ( 0.100 ±0.018) cm2,the increased area of the left ventricle(0.060 ±0.014) cm2]and intragate bright blood sequences [the increased area of the myocardium (0.090 ±0.019) cm2,the increased area of the left ventricle (0.050 ±0.015) cm2].Furthermore,there was no significant difference(t =1,P =0.34) in the CV of the myocardium signal intensity of bright blood sequences between the ECG-triggering ( 0.050 ± 0.013 ) and intragate (0.040 ± 0.015 ),but significant difference ( t =4.51,P =0.001 ) in the CV of the myocardium signal intensity of black blood sequences between the ECG-triggering ( 0.070 ± 0.033 ) and intragate ( 0.160 ± 0.046 ) was obtained.ConclusionsThe intragate sequences could take the place of the ECG gate sequences in functional analysis of the heart( including bright blood and black blood sequences).The bright blood intragate sequences also could replace the bright blood ECG-triggered sequences in analyzing the signal of the myocardium.

5.Establishment and functional analysis of CHO cells with stable expression of hGHR gene and its mutants

Objective To eonstruct three eukaryotic expression vectors containing wilde-type hGHR gene and its mutants(hGHR-E42K and hGHR-H56R) related to congenital growth hormone insensitivity, then check their expression in CHO cells. Methods With the available PUC-hGHR vector, two mutate hGHR genes (hGHR-E42K and hGHR-H56R) were obtained through mutagenesis. Then three recombinants were cloned into eukaryotic expression vectors pcDNA3.1/zeo(+) with restriction enzymatic reactions.Then with Lipofectamine2000, we trans-fected expression vectors to CHO cells and screened the stably expressed CHO cells by Zeocin. RT-PCR and/or Western blotting were used to examine hGHR and STAT5-P. Results After sequencing, two mutations were introduced to hGHR, three eukaryotic expression vectors were identified. The transfected CHO cells expressed vd-hGHR or its mutants. Compared with hGH-wt, two mutate cells (E42K and H56R) had decreased phosphorylated STAT5 levels. Conclusion Three CHO cells which stably expresses wide type hGHR and its mutants were successfully established, E42K and H56R partly interfered the phosphorylation of STAT5.
6.Pharmacodynamics of anti-diarrhea medicine of the compound catechu antidiarrheal ointment

Objective To observe the gastrointestinal function influence in the compound catechu antidiarrheal ointment when acted on normal and diarrheic mice .Methods The antidiarrheal function of the compound catechu antidiarrheal ointment was ob-served through the mice small intestinal propulsion experiment ,gastric emptying experiment and mice diarrhea situation influence test of senna .Results This ointment could reduce the mice small intestine propulsion rate which significantly restrained the mice small intestine propulsion ;Also it could improve the residual rate of mice stomach contents ,significantly restrained their gastric emptying velocity .Conclusion The compound catechu antidiarrheal ointment has strong and lasting effect on stopping diarrhea .
7.Effects of low-dose Dexmedetomidine on gastrointestinal reactions after minimally invasive surgery for cervical cancer

Objective To investigate the effect of low dose Dexmedetomidine on gastrointestinal reaction after minimally invasive surgery for cervical cancer. Methods 94 patients with cervical cancer treated with minimally invasive surgery from February 2014 to September 2016 were selected. According to the random number table method, the patients were divided into observation group (low-dose Dexmedetomidine) and control group (saline). The incidence of nausea and vomiting, the motilin level were compared between the two groups. Results The observation group was 47 cases, grade Ⅱ nausea in 7 cases, grade Ⅲ nausea in 2 cases, the total incidence was 19.15%; 47 cases in the control group, 11 cases of grade Ⅱ nausea, grade Ⅳ nausea in 4 cases, grade 2 nausea in 2 cases, the rate was 36.17%, the difference was statistically significant (P < 0.05). 47 cases of grade Ⅱ vomiting in 4 cases, grade Ⅲ vomiting in 5 cases, grade Ⅳ vomiting in 1 case, the total incidence rate was 14.89%, grade Ⅱin 4 case, grade Ⅲ vomiting in 5 cases, grade Ⅳ vomiting in 1 case, the total incidence was 21.28%, the difference was statistically significant (P < 0.05). There was no significant difference in motilin level between the two groups (P > 0.05). At 6 h, 12 h and 24 h after operation, the levels of motilin in the observation group were significantly lower than those in the control group, the difference was statistically significant (P < 0.05). Conclusion Low dose Dexmedetomidine can effectively prevent and alleviate the gastrointestinal reaction after minimally invasive surgery for cervical cancer, and avoid the excessive release of motilin, worthy of clinical promoting.

9.Comparasion of contrast-enhanced ultrasound in diagnosis of cholangiocarcinoma and different differentiated hepatocellular carcinoma

Objective To compare the characteristics of intrahepatic cholangiocarcinoma (ICC) and different differentiated hepatocellular carcinoma (HCC) by CEUS and evaluate the diagnostic value of CEUS.Methods The cases who underwent CEUS and were pathologically demonstrated as ICC (n=34) and HCC (n=136) were observed.The characteristics of ICC and different differentiated HCC in conventional ultrasound and CEUS were analyzed,and the diagnostic efficiency of wash out time were calculated.Results ICC had higher percentage (24/34,70.59%) of washout emerging in early portal phase than those of HCC.And ICC had lower percentage (0;4/34,11.76%) of washout emerging in middle and late portal phase than poorly and moderate differentiated HCC.Poorly differentiated HCCs had higher percentage (16/41,39.02%) of washout emerging in middle portal phase than well differentiated HCC.The sensitivity,specificity,positive predictive value,negative predictive value,accuracy,positive likelihood ratio and negative likelihood ratio of washout time in diagnosis of ICC were 82.35 % (28/34),91.18% (124/136),70.00% (28/40),95.38% (124/130),89.41% (152/170),9.4,0.2,respectively.The rise time of ICC and well,moderate and poorly differentiated HCC were (13.03 ± 3.49) s,(13.80 ± 3.04)s,(14.89±4.12)s,(16.00±3.38)s,respectively,and the difference was significant (F=4.369,P<0.05).The rise time of ICC was significantly higher than that of well differentiated HCC (P<0.05).Conclusion The CEUS performances are different significantly among ICCs and different differentiated HCCs,which has value for the differential diagnosis.
10.Preparation and in vitro Release Study of Diphenidol Hydrochloride Double-layer Osmotic Pump Tablets

OBJECTIVE:To prepare Diphenidol hydrochloride double-layer osmotic pump tablets,and study its in vitro release characteristics. METHODS:Double-layer compressing technique and film coating technology were conducted to prepare Diphenidol hydrochloride double-layer osmotic pump tablets. The in vitro releases of it,Difenidol hydrochloride tablets in market,self-made Difenidol hydrochloride single-layer osmotic pump tablets were compared. RESULTS:The formulation was as follow as diphenidol hydrochloride 75 mg,sodium chloride 10 mg,low-molecular-weight polyoxyethylene 15 mg and right amounts of 5% PVP K30 ethanol solution. Booster layer was high-molecular-weight polyoxyethylene 60 mg,sodium chloride 20 mg,PVP K306 mg,right amounts of magnesium stearate. 12 h cumulative release(Q)of prepared double-layer osmotic pump tablets reached 80%,and the release was in line with zero-order kinetic equation. Q15 min of Difenidol hydrochloride tablets had reached 90%;Q12 h of Difenidol hy-drochloride single-layer osmotic pump tablets was only 51.14%. CONCLUSIONS:The prepared Difenidol hydrochloride dou-ble-layer osmotic pump tablets have sustained release effect,with more complete drug release within 12 h than single-layer one.
