1.Cinical observation on early fluid resuscitation in treatment of septic shock

ObjectiveTo evaluate the effect of early fluid resuscitation in treatment of septic shock.Methods56 patients with septic shock in ICU were divided into two groups by the method of the treatment. Early goal-directed therapy were given in the early fluid resuscitation group except anti-infective treatment, and the traditional therapy were given in the traditional group. The CVP,MAP,ScvO2 ,SaO2 of 6h and 12h of intra-treatment were observed in the two groups, and the incidence of MODS and the mortality rate were observed also. ResultsThe CVP,MAP, ScvO2, SaO2 of the early fluid resuscitation group were improved better than that in the traditional group. The incidence of MODS and the mortality rate in the early fluid resuscitation group were lower than that in the traditional group. ConclusionThe prognosis of septic shock was better for the early fluid resuscitation, and it could lower the incidence of MODS and the mortality rate.
2.Fingerprint analyses of fructus Forsythiae.

OBJECTIVETo provide application basis of the Forsythia suspensa by studying the difference of HPLC-FP of F. suspensa fructification (medicinal materials).
METHODComparative work was done on F. suspensa produced in different areas, on different parts of Forsythia suspensa, and on the pseudo preducts with methods of HPLC-FP.
RESULTDifferent FP characteristics were shown respectively by different samples, which were from different producing areas, from different parts, and the pseudo products including the fructification of Syringa reticulata var. and F. viridissimac.
CONCLUSIONThe FP can be used to distinguish the F. suspensa coming from different producing areas and different sources.

3.Role of emergency and staged hepatectomy in peritoneal metastasis associated with spontaneous rupture of resectable hepatocellular carcinoma patients

Objective To evaluate the role of emergency and staged hepatectomy in peritoneal metastasis associated with ruptured hemorrhage of resectable hepatocellular carcinoma (HCC) patients,and investigate the impact of surgery timing-selecting on peritoneal metastasis of postoperative HCC patients.Methods A retrospective analysis was conduct on the pooled data from 38 HCC patients with spontaneously ruptured hemorrhage treated in our hospital from August 2011 to January 2016.These patients were divided into emergency group who underwent hepatectomy within 24 hours at admission,and staged group who underwent the procedure one week after admission.Perioperative events,overall survival (OS) and disease-freesurvival (DFS) rates,incidence of recurrent and metastatic disease were compared between the two groups.Results The perioperative blood loss and transfusion were much more in emergency group than staged group (both P < 0.05).Nevertheless,the incidence of postoperative mortality was not significantly different (6.0% vs 0%,P > 0.05).The median survival was 22.5 months in emergency group versus 14.2 months in staged group.The 6-month,1-year,3-year OS rates in emergency group were 88.2%,82.4% and 30.3% respectively,and 6-month,1-year,3-year DFS rates were 81.3%,54.7% and 27.3%.The 1-year OS and 6-month DFS rates were higher than those of staged group (both P < 0.05).The incidence of peritoneal metastasis in staged group was higher than that in emergency group,but it was not significantly different (38.1% vs 29.4%,P > 0.05).Univariate and multivariate analysis indicated that tumor diameter ≥ 10 cm and AFP > 10 000 μg/L were the risk factors for peritoneal metastasis after hepatectomy for HCC patients with spontaneously ruptured hemorrhage.Conclusions Emergency hepatectomy would warrant a better short-term prognosis compared with staged hepatectomy for the HCC patients with spontaneously ruptured hemorrhage.Staged hepatectomy would not raise the possibility of postoperative peritoneal metastasis.The predictors of tumor diameter ≥ 10 cm and AFP > 10 000 μg/L were risk factors for peritoneal metastasis after hepatectomy for spontaneously ruptured HCC patients.

Hepatic encephalopathy is a common metabolic neuropsychiatric syndrome in the development of end-stage liver disease. Since the concept of intestinal-liver-brain axis was proposed, the relationship between the pathogenesis of hepatic encephalopathy and the gut microbiota has been a hot research topic. In recent years, studies have confirmed that gut microbiota is involved in and affects various pathological processes of hepatic encephalopathy. Objective:To study therapeutic effect of Qiliqiangxin capsule combined Xinhuosu on refractory heart fail-ure(RHF)and its influence on miR-132 and miR-155.Methods:A total of 100 RHF patients treated in our hos-pital were divided into Xinhuosu group(received Xinhuosu based on routine treatment)and combined treatment group(received Qiliqiangxin capsule based on Xinhuosu group).Both groups were treated for one month.Serum levels of N-terminal pro-brain natriuretic peptide(NT-proBNP),transforming growth factor(TGF)-β1,ma-trix metalloproteinase(MMP)-9,interleukin(IL)-6,tumor necrosis factor(TNF)-αand soluble intercellu-lar adhesion molecule-1(sICAM)-1,expression levels of miR-132 and miR-155 before and after treatment and total effective rate were compared between two groups.Results:Compared with Xinhuosu group,after treat-ment,there were significant reductions in serum levels of NT-proBNP[(2838.76±380.26)pg/ml vs.(2450.53±289.83)pg/ml],TGF-β1[(39.74±5.40)μg/ml vs.(35.34±4.72)μg/ml],MMP-9[(276.62±30.61)ng/L vs.(230.37±27.86)ng/L],IL-6[(7.09±1.54)ng/L vs.(5.10±1.25)ng/L],TNF-α[(90.11±16.38)ng/L vs.(63.82±12.25)ng/L]and sICAM-1[(239.30±45.30)ng/ml vs.(192.33±34.41)ng/ml],and ex-pression level of miR-155[(1.37±0.22)vs.(1.24±0.22)],and significant rise in expression level of miR-132[(1.47±0.24)vs.(1.62±0.33)]in combined treatment group(P<0.05 or<0.01).Total effective rate of combined treatment group was significantly higher than that of Xinhuosu group(94.00%vs.76.00%,P=0.012).Conclusion:Qiliqiangxin capsule combined Xinhuosu can significantly improve clinical therapeutic effect,relieve my-ocardial injury,improve myocardial fibrosis and ventricular remodeling,alleviate inflammatory response and regu-late expression of miR-132 and miR-155 in patients with refractory heart failure.

OBJECTIVETo explore roles of mRNA and protein expressions of organic anion transporting polypeptide (oatp2b1) of rats with high fat diet and overstrain induced Pi deficiency syndrome in the transporting of damp turbidity.
METHODSTotally 24 SD rats were randomly divided into three groups, i.e., the normal group, the overstrain group, and the high fat diet group, 8 in each group. After successful modeling, one piece of tissues such as spleen, kidney, liver, lung, stomach, small intestine, and large intestine was taken from each rat. Rats of the overstrain group were bonded by specially made bondage cylinder, 3 h each time on odd days, and forced to swim in cold water (10 +/- 1) degrees C for 7 min on even days alternatively for twelve weeks. Rats in the model group and the normal group were fed with standard routine granular forage for 12 weeks. Rats in the high fat diet group were fed with high fat forage for twelve weeks. All rats drank and ate freely. The mRNA and protein expressions of oatp2b1 were detected in the seven tissues using RT-PCR and Western blot.
RESULTSThe mRNA expression of oatp2b1 in liver and kidney tissues of rats in the high fat diet group was higher when compared with that of the normal group and the overstrain group (P < 0.01, P < 0.05). The oatp2b1 mRNA expression in the normal group was sequenced from high to low as liver > lung > spleen > larger intestine > small intestine > kidney > stomach. The oatp2b1 mRNA expression in the overstrain group was sequenced from high to low as liver > lung > larger intestine > spleen > kidney > stomach > small intestine. The oatp2b1 mRNA expression in the high fat diet group was sequenced from high to low as liver > lung > spleen > small intestine > kidney > larger intestine > stomach. The oatp2b1 protein expression in the lung tissue was sequenced from high to low as the overstrain group > the normal group > the high fat diet group (P > 0.05). The oatp2b1 protein expression in the spleen tissue was sequenced from high to low as the high fat diet group > the normal group > the overstrain group (P > 0.05). The oatp2b1 protein expression in the kidney tissue was sequenced from high to low as the normal group > the overstrain group > the high fat diet group (P > 0.05). The oatp2b1 protein expression in the liver tissue was sequenced from high to low as the normal group > the high fat diet group > the overstrain group (P > 0.05). Of them, the oatp2b1 protein expressed extremely less in the stomach, large intestine, and small intestine. The oatp2b1 protein expression in the normal group was sequenced from high to low as lung >spleen > liver, kidney > stomach, larger intestine, and small intestine. The oatp2b1 protein expression in the overstrain group was sequenced from high to low as lung > spleen > kidney > liver > stomach, larger intestine, and small intestine. The oatp2b1 protein expression in the high fat diet group was sequenced from high to low as spleen > lung > kidney > liver > stomach, larger intestine, and small intestine. However, there was no statistical significance among the three groups by pair-wise comparison (P > 0.05).
CONCLUSIONSKidney and liver might play important roles in the transportation and transformation of damp under the state of Pi deficiency syndrome. Oatp2b1 may be one of the material bases involved in the transportation and transformation of damp turbidity. Pi's function of governing transportation and transformation of damp might not only include the functions of the gastrointestinal tract, but also include partial liver and kidney functions.
