1.The electroretinogram photopic negative response of idiopathic macular hole (stage 2) by vitrectomy with or without internal limiting membrane peeling

Objective To observe the electroretinogram (ERG) photopic negative response (PhNR) of idiopathic macular hole (IMH) in stage 2 by vitrectomy with or without internal limiting membrane peeling (ILMP).Methods Twenty-three stage 2 IMH patients (23 eyes) were enrolled in this prospective study.All patients received the best corrected visual acuity (BCVA),optical coherence tomography and flash-ERG examinations.The patients were randomly divided into group A (11 eyes,vitrectomy) and B (12 eyes,vitrectomy with ILMP).There was no significant difference in BCVA (t=0.96,P=0.350),diameter of macular hole (MH) (t=3.21,P=0.580) and the PhNR amplitude (t=0.98,P=0.353) in group A and B.All patients underwent 25G vitrectomy,ILMP was carried out in group B.The follow-up time was 3 to 6 months,with the mean follow-up time of 4.3 months.BCVA,MH closure rate and PhNR amplitude in group A and B were analyzed before and after surgery.Results Three months after surgery,10 eyes (90.9%) gained MH closure but 1 eye (9.1%) failed in group A.In group B,12 eyes (100.0%) gained MH closure.There was no significant difference in MH closure rate between the two groups (P=0.462).The mean BCVA of group A and B was 0.69 ± 0.24 and 0.65 ± 0.22,there was no significant difference between the two groups (t=0.49,P=0.722).The amplitude of PhNR in group A was (36.647.4) μtV,which was lower than the pre-surgery PhNR,but the difference was not significant (t=0.73,P=0.472).The amplitude of PhNR in group B was (27.1 ± 12.4) μV,which was lower than that the presurgery PhNR,and the difference was significant (t =3.56,P =0.002).The difference of PhNR amplitude in group A and B was statistically significant (t=2.17,P=0.042).Conclusion Compared with non-ILMP,vitrectomy combined with ILMP will significantly reduce the PhNR amplitude ofIMH in stage 2.
2.Clinical Observation on Reduction of Thyroid Hormone and Improvement of Thyroid Function by Combination of Traditional Chinese and Western Medicine

Objective To observe the clinical effect of combination of traditional Chinese and western medicine to treat the hypothyreosis disease, and dosage reduction of thyroid hormone and improvement of thyroid function. Method The patient of hypothyreosis were divided into two groups, 32 cases in treatment group and 30 cases in control group. Both groups were treated with thyroid hormone. The treatment group reduced the dosage of thyroid hormone to high limit in the prerequisite of ensuring the curative effect, and was given the decoction of warming Yang and benefiting Qi, removing stasis and dampness. Then compare the improvement of main clinical symptoms of the two groups, as well as their function of thyroid and side effects of hormone. Result There was no significant difference in total effective rate between two groups, while there was notable diffenrence in excellent effective rate. Compare with the control group, the symptoms of the treatment group were improved obviously and rapidly, the falling extent of the increasing thyroid-stimulating hormone (TSH) was large and quick, so as the enhancing extent of the decreasing T3 and T4, the sid effect of hormone was eradicated. Conclusions To treat the hypothyreosis disease, combination of traditional Chinese and western medicine can improve the symptom quickly, promote the recovery of thyroid function, and get the satisfactory curative effect, even if reduce the dosage of hormone.
3.Molecular Characterization and Functional Prediction of a Novel Leaf SAG Encoding a CBS-domain-containing Protein from Coleus blumei

Leaf senescence is considered as one of important factors to decrease ornamental values of foliage plants. In the attempt to study and understand the molecular mechanism of leaf senescence, a senescent leaf cDNA library of Coleus blumei was constructed and a small EST library was obtained. According to the sequence of an EST fragment with a cystathionine beta synthase (CBS) domain, a novel leaf senescenceassociated gene (SAG) full-length cDNA encoding a CBS-domain-containing protein, denoted Cbcbs, was rapidly cloned using a strategy of RACE combined with cDNA library. The full length of the Cbcbs gene was 859 bp long (accession No. EF076754) and contained a 609 bp open reading frame (ORF) encoding a 202amino acid protein. One stop codon (TAA) was found in 5' UTR and one possible polyadenylation signal,AATAAA, and a pentanucleotide motif, ATTTA, were found in 3' UTR. The CbCBS contained a predicted mitochondrial targeting peptide in the N-terminal region, two conserved and intact CBS domains, four casein kinase Ⅱ (CK Ⅱ) phosphorylation sites, three protein kinase c (PKC) phosphorylation sites and one tyrosine sulfation (TS) site. Sequence comparisons and phylogenetic analysis showed that CbCBS was a novel senescence or stress-associated protein. The prediction analysis of secondary structure and three dimensional structure of CbCBS suggested that the chief function of the protein was decided by the CBS domain pair. The expression pattern of Cbcbs in leaves was analyzed by RT-PCR. It was demonstrated that Cbcbs gene was a senescence-associated gene (SAG) and expressed in all leaf stages, young stage (Y) being the lowest and terminal senescence stage (S3) being the highest, and was upregulated along with the leaf senescence.Function analysis showed that the mature CbCBS maybe acts as a sensor of cellular energy status and directly or indirectly regulates cellular energy levels to increase ATP content in mitochondria during periods of metabolic stress of senescent leaves.
4.Study on the trend of nutritional biochemical indexes in patients with paraquat poisoning

Objective To observe the nutritional biochemical indicators of paraquat poisoning patients, analyze and compare the nutritional status of patients and understand the changing trend of each indicator. Methods A total of 104 patients with acute paraquat poisoning who were admitted to the emergency department of the Second Hospital of Hebei Medical University from December 2015 to December 2017 were enrolled, and divided into the cure group (patients who survived >30 days) and the death group. Nutritional biochemical indicators including serum protein (ALB, PA, TP) and serum lipids (TCh, TG, LDL) were selected for dynamic observation. The observation time points were set as follows: immediate treatment of poisoned patients (day 1 on admission), on day 4, 7, 10, 13 and 16 after admission, and on day 30 after follow-up. The nutritional biochemical indicators of the two groups on day 1 and 4 were statistically analyzed and compared by t test. The nutritional status of the patients in the cure group was analyzed, and the Repeated Measures Anova was performed to understand the trend of each indicator over time. Results In the cure group, the TP level decreased from (73.34±5.75)g/L on day 1 to (51.95±6.05)g/L on day 4, t=20.34, P<0.01; and the TCh level decreased from (4.37±0.98) mmol/L on day 1 to (3.03±1.01)mmol/L on day 4, t=7.56, P<0.01. In the death group, the TP level decreased from (72.25±8.80)g/L on day 1 to (49.07±5.48)g/L on day 4, t=12.38, P<0.01, and the TCh level decreased from (4.38±0.88)mmol/L on day 1 to (2.51±1.07) mmol/L on day 4, t=7.94, P<0.01. Compared with the cure group, serum levels of ALB, TP and TCh in the death group decreased greater from day 1 to day 4 (all P<0.05). In addition, dynamic observation of the indicators in the cure group within 16 days after admission showed that, after treatment, the levels of ALB and TP recovered slowly and were still lower than normal . While the levels of PA and lipid rose rapidly after 10 days of admission. Conclusions Paraquat poisoning seriously affects the nutritional status of patients, and the serum protein levels decline significantly and can not be recovered easily. Therefore, sufficient attention should be paid to the treatment, and timely and appropriate nutritional support should be provided.
5.Cloning and molecular characteristics analysis of a MYB gene from Fagopyrum dibotrys.

Using RACE with a Fagopyrum dibotrys callus cDNA library, one clone, named FdMYBP1, encoding a putative R2R3 MYB protein was identified. FdMYBP1 appeared to be a full-length cDNA of 1159 bp encoding a protein of 265 amino acids. Through structure and property analysis of FdMYBPI with bioinformational methods, it was found that the amino acid sequence of FdMYBP1 showed great homology to other MYBP with the R2R3 repeat region in the N-terminus. Southern blot analysis indicated that FdMYBP1 belongs to a single copy gene in F. dibotrys genomes. The FdMYBP1 gene has the same classic characters with other MYBP and probably involved in the pathway of flavonoid metabolisms.

6.Clinical features of 86 cases of acute diquat poisoning

Objective:To explore the clinical features of acute diquat (DQ) poisoning, and further improve the awareness of acute DQ poisoning.Methods:A retrospective analysis was performed on the clinical data of patients with acute DQ poisoning diagnosed in the emergency department of the Second Hospital of Hebei Medical University from January 1, 2019 to December 31, 2021. The clinical data included age, gender, exposure routes, presence of pesticides (drugs) mixture poisoning, dosage of poison, the time from taking poisoning to admitting in the emergency department, clinical manifestations, laboratory data, treatment, hospital days, prognosis and survival days.Results:The number of cases who firstly complained of acute DQ poisoning in the past three years were 19 cases in 2019, 28 cases in 2020, and 51 cases in 2021. A total of 12 patients were excluded due to being diagnosed paraquat (PQ) poisoning by toxicology detection. Finally, 86 cases of acute DQ poisoning were included, including 80 cases of oral DQ poisoning, 1 case of intramuscular injection, 1 case of binocular contact and 4 cases of dermal exposure. In 80 cases of oral DQ poisoning, there were 70 cases of diquat poisoning alone (42 cases survived, 28 cases died) and 10 cases of pesticide mixture poisoning (6 cases survived, 4 cases died). The time from oral poisoning to admitting in the emergency department was 0.5-96.0 hours, with an average of (8.6±5.8) hours. The time of intramuscular injection poisoning to admitting in the emergency department was 3 hours. The time of dermal exposure to admitting in the emergency department was relatively long, with an average of 66.1 hours. The time from oral simple DQ poisoning to death was 12.0-108.0 hours, and the time from oral mixed DQ poisoning to death was 24.0-576.0 hours. A total of 70 patients with oral diquat poisoning alone presented various degrees of multiple organ injuries. All patients presented gastrointestinal symptoms such as nausea and vomiting. Renal injury and central nervous system injury were the most significant and closely related to the prognosis.Conclusions:Acute oral DQ poisoning can cause to multiple organ injuries, and the clinical manifestations are related to the dose of the poison. In severe cases, acute renal failure and refractory circulatory failure occur within 24 hours after poisoning, and severe central nervous system injury with disturbance of consciousness as the primary manifestation occurs within 36 hours, followed by multiple organ failure until death.
7.Experts consensus for the diagnosis, treatment, and prevention of coronavirus disease 2019 in the elderly

Coronavirus disease 2019 (COVID-19) can cause great damage to the elderly patients and lead to high mortality. The clinical presentations and auxiliary examinations of the elderly patients with COVID-19 are atypical, due to the physiological ageing deterioration and basal pathological state. The treatment strategy for the elderly patients has its own characteristics and treatment protocol should be considered accordingly. To improve the diagnosis, treatment, and prevention of COVID-19 in the elderly, the Expert Committee of Geriatric Respiratory and Critical Care Medicine, China Society of Geriatrics established the "Expert consensus for the diagnosis, treatment, and prevention of coronavirus disease 2019 in the elderly" . We focused on the clinical characteristics and key points for better treatment and prevention of COVID-19 in the elderly. (1) For diagnosis, atypical clinical presentation of COVID-19 in the elderly should be emphasized, which may be complicated by underlying disease. (2) For treatment, strategy of multiple disciplinary team (mainly the respiratory and critical care medicine) should be adopted and multiple systemic functions should be considered. (3) For prevention, health care model about integrated management of acute and chronic diseases, in and out of hospital should be applied.
