Chinese Journal of Practical Nursing 2016;32(17):1281-1284
Objective To investigate the posttraumatic growth(PTG)in female patients with breast cancer after operation and the possible influencing factors related to the PTG of these patients. Methods 300 female patients with breast cancer from Oct 2014 to Jan 2015 in 7 third-level grade A hospitals were selected by random number table. The posttraumatic growth inventory (PTGI)was used to study PTG status in these patients. Clinical data including age, degree of education, marital status, underlying disease, clinical stage, surgical method, adjuvant therapy and postoperative complications were univariate analyzed, then the positive factors were logistic regression analyzed. Results The mean PTG score was 59.83±12.90, univariate analysis showed that degree of education (F=15.80, P=0.000), underlying disease (t=3.90,P=0.000), clinical stage (F=10.17, P=0.000), surgical method (t=5.12, P=0.000) and postoperative complications(t=4.85,P=0.000) were influencing factors, while multivariate analysis by logistic regression showed that clinical stag(OR=6.635, P=0.000), surgical method (OR=2.858, P=0.000) and postoperative complications (OR=2.731, P=0.035) were independent influencing factors for female patients with breast cancer after operation. Conclusions The posttraumatic growth status in breast cancer patients undergoing operation declined dramatically. The level of metal change dimension are main clinical features of breast cancer patients. There are close associations between PTG and clinical stag, surgical method and postoperative complications.

Objective To explore the diagnosis and treatment of non traumatic chylothorax in non-Hodgkin's lymphoma (NHL) and to understand the differences in diagnosis and treatment between chylothorax and pseudochylothorax. Methods The patient aged 83 years was confrimed as chylothorax and NHL after lymph node biopsy. We reviewed literatures about chylothorax in NHL to analyse the possible mechanism, its diagnosis and treatment. Results The patient was sufferring from unilateral chylothorax diagnosed as true chylothorax by thoracentesis, and progressed to bilateral chylothorax after 1 year.PET/CT examination showed intrathoracic and right cervical lymph nodes enlargement and an increasing metabolic activity.Cervical lymph node biopsy revealed diffuse large B cell type non-Hodgkin's lymphoma. The patient refused any other treatment except the diet therapy and died after 19 months. We searched 19 cases with NHL chylothorax in associated literatures about the treatments including radiotherapy (6/6 improved),chemotherapy (6/11 improved),thoracic duct ligation (I/1 improved),thoracic duct ligation and drug pleurodesis(1/1 improved). Conclusions PET/CT is useful in finding the hidden clues of chylothorax in NHL.There is no standard mangement for NHL chylothorax and the treatment must be individualized. The overall prognosis of NHL chylothorax is similar to that of non-Hodgkin's lymphoma and the patient needs early diagnosis and general treatment in order to prolong the survival time.

60 years, 13.3% of the total sarcoidosis patients) diagnosed as sarcoidosis based on national criteria from the year 1965 through 1999 were reviewed. Results The clinical degree was as follows: phase I, 8 cases; phase Ⅱ, 6 cases; phase Ⅲ, 2 cases. The elderly sarcoidosis patients manifested variously, with misdiagnosis rate being 50%. The active degree of pulmonary sarcoidosis was correctly evaluated after the diagnosis was confirmed and the patients was strictly observed during the treatment since it was very important for avoiding the occurrence of complications. Conclusions The early diagnosis in elderly patients with sarcoidosis should be emphasized to prevent them from missing and misdiagnosis. The elderly patients with treating indications in phases Ⅱ and Ⅲ could be given steroids and constant observation and regular examinations are important during the treatment.

Objective To investigate the status of professional identity of nursing undergraduates in different grades and to search for their different professional identity level, so as to establish a pertinent method on nursing education for promoting nursing undergraduates′professional identity. Methods By stratified sampling, a total of 286 nursing undergraduates from four grades in Dalian Medical University were surveyed by general information questionnaire and the Professional Identification Scale for Nurses. Results The average total scores of the professional identity of nursing undergraduates were (107.29±15.01) points. There was significant difference in the total score and the five dimensions of professional identity,F=5.625, 5.258, 7.459, 4.182, 6.891, 7.299, P<0.01. Professional identity score was on the rise along with the grade growth, but the score of grade two was significant lower than the other three grades, and the score showed an increase in grade three; grade, whether profession was consistent with their own interests or not, the reasons of choosing the profession were the important factors affecting professional identity of nursing undergraduates. Conclusions The professional identity score of nursing undergraduates is in medium level and remains to be improved. It is suggested that nursing educators should take corresponding measures according to different grades, own interest and physical and mental characteristics of the students, so as to improve their professional identity and lay the foundation for stability and sustainable development of the nursing team in the future.
