1.Exploration on the nature of “worry”

In this paper, the meaning of“worry” in ancient and modern time was discussed. With the use of basic theories of Chinese medicine theory and modern psychology, the contents of “worry” were identified, and the nature of“worry” was reconsidered. It was believed that the nature of“worry” is Qi stagnation, belonging to unpleasant or tense or calm endpoint in the emotions. In the TCM sense, “worry” belong to Yin in Yin-yang relation, and earth in five elements, corresponding to the spleen in the five Zang organs. “Worry” was closely linked to depression. We explored the nature of the “worry” to provide the basis for the study of depression in traditional Chinese medicine.
2.Expression of MCM6 in bladder cancer and its correlation with prognosis

Objective:To detect the expression of microchromosome maintenance protein 6 (MCM6) in bladder cancer tissues, and to explore the relationship between MCM6 and the clinical prognosis of bladder cancer. To explore the possibility of MCM6 as a potential biomarker for bladder cancer.Methods:Bioinformatics methods were used to analyze the mRNA levels of MCM6 in bladder cancer tissues and adjacent normal tissues, and to analyze the relationship between its expression and the survival rate of bladder cancer patients. The clinicopathological data of 83 patients with bladder cancer underwent surgical treatment were retrospectively analyzed. Immunohistochemical method was used to detect the expression level of MCM6 protein in bladder cancer tissue and normal tissue adjacent to the cancer, and analyze its relationship with clinicopathological characteristics of bladder cancer patients.Results:The results of bioinformatics analysis showed that MCM6 mRNA was significantly highly expressed in bladder cancer tissues and was significantly correlated with the overall survival rate ( P=0.036) and disease-free survival rate ( P=0.01) of the patients. The results of immunohistochemistry showed that the expression of MCM6 in bladder cancer tissue was significantly higher than that in normal tissues adjacent to the cancer, and high expression of MCM6 in bladder cancer tissue was related to tumor stage ( P=0.025), but had nothing to do with age, gender and tumor grade (all P>0.05). Conclusions:The expression of MCM6 in bladder cancer tissue is significantly high and indicates a poor prognosis, and its expression level is related to the tumor stage of bladder cancer patients. MCM6 may serve as a new potential biomarker for bladder cancer.
3.Clinical treatment of schizophrenia due to stagnation of phlegm and qi with Shugan-qingzhang formula

Objective To explore the effects of treating schizophrenia due to stagnation of phlegm and qi with Shugan-qingzhang formula. Methods 85 patients with schizophrenia were randomly recruited into a control group and a treatment group. The treatment group was treated with Shugan-qingzhang formula, and the control group was treated with oral risperidone. Clinical effects of both groups were evaluated by positive and negative syndrome scale (PANSS) and symptom rating scale. Adverse drug reaction was evaluated by treatment emergent symptom scale (TESS). Results Score of PANSS was significantly changed after the treatment in both groups (F treatment group=4.86, t'treatment group= 17.88, v'treatment group=59, P treatment group<0.01; F control group=4.27, t' control group=18.76, v' control group=59, P control group<0.01); scores of PANSS in the treatment group were significantly better than the control group at the second and forth weak (F 2weeks= 1.02, t 2weeks=2.50, P 2weeks<0.05; F 4weeks= 1.04, t 4weeks=3.55, P 4 weeks<0.01), while no significant difference at the sixth week between the two groups (F=0.92, t=0.91, P>0.05); There was no statistical difference of PANSS score between the two groups in terms of western medicine curative effects (x2=0.05, P>0.05), but significant difference in terms of TCM curative effects (x2=36.26, P<0.01); There was also statistic difference of adverse drug reaction between the two groups (x2= 8.17, P<0.01). Conclusion Shugan-qingzhang formula can control symptoms of schizophrenia due to stagnation of phlegm and qi in a certain period.
4.Clinical analysis of locking plate for proximal humeral complicated fractures

[Objective]To discuss the treatment effect of locking compression plate(LCP)for proximal humeral complicated fractures.[Method]A retrospective study was done on 21 cases of proximal humeral fractures treated with LCP from April 2005 to June 2006.There were 13 male and 8 female,with age of 26-71 year(mean 42.3 year).According to Neer classification,15 cases were three-part fractures including 5 cases with anterior dislocation and 6 cases with primary osteoporosis;6 cases were four-part fractures including 3 cases with anterior dislocation and 2 cases with posterior dislocation.Reduction and fixation were done by deltopectoral-gap approach,the operator put the locking plate on 10mm positerior to the intergroove of tuberosity,5mm away from the tip of humeral greater tuberosity.If there were bone defects in the fracture site after reduction,they were treated with allografts,locking screws and unlocking screws were drilled into proximal and distal part of the fracture.Shoulder exercise should be performed passively three days after operation.One week after operation,active shoulder exercise was done,which was gradually intensified two weeks after operation.[Result]Twenty-one cases were followed up for 10-14 months,average 12.5 months,all fractures were union in 12 month.According to Neer shoulder score,the result was excellent in 11 cases,good in 7 and fair in 3,with excellent and good rate of 85%.[Conclusion]The screw of LCP can lock the plate so that the plate can firmly fixate the proximal humeral comminution fractures and osteoporosis bone,so simultaneously reach fracture union and function exercise,but attentions should be paid to the suitable procedure of LCP to avoid a series of problem of locking plate.
5.Expression level of ZNF217 in bladder cancer and its clinical significance

Objective To determine the expression of ZNF217 protein in bladder cancer tissues and to analyze its clinicopathological characteristics, so as to clarify its clinical significance. Methods The clinicopathological data of 123 patients with bladder cancer who underwent bladder cancer resection were retrospectively analyzed. The expression of ZNF217 protein in bladder cancer tissues and corresponding adjacent tissues was detected by immunohistochemistry, and its correlation with clinical characteristics of patients was analyzed. Results ZNF217 protein was highly expressed in tumor tissues, and the ratio of high expression of tumor tissues was 67.4%, but it was lowly expressed in adjacent tissues. By comparing with clinicopathological features, the expression level of ZNF217 was significantly correlated with tumor T stage and tumor recurrence (all P<0.05), but not correlated with age , gender , tumor differentiation and lymph node metastasis (all P>0.05). The survival analysis showed that patients with high expression of ZNF217 in bladder cancer had significantly shorter overall survival and progression-free survival than those with low ZNF217 expression ( all P<0 . 05 ) . Conclusions ZNF217 is highly expressed in bladder cancer tissues, and its expression level is associated with tumor stage, tumor recurrence and poor prognosis, suggesting that it can be used as a potential therapeutic target for bladder cancer.
6.Multi-directional differentiation renal cell cancer: a case report

Renal cancer with multiple differentiation and multiple components is rare in clinic. It has the character of high malignancy, early metastasis, and poor prognosis. A patient with multiple differentiated renal cell carcinoma was diagnosed in our hospital mainly because of emaciation, fatigue and loss of appetite. CT scan showed the 6.5 cm×6.0 cm mass in the lower pole of right kidney with unequal enhancement and necrosis. The chest CT scan showed multiple enlarged lymph nodes in the mediastinum and right superior clavicle. The patient accepted the laparoscopic retroperitoneal right nephrectomy and right supraclavicular lymph node biopsy. Postoperative pathology showed multiple differentiated renal cell carcinoma with localized sarcomatoid leisions, including clear cell carcinoma, collecting duct-like carcinoma and medullary carcinoma combined with right supraclavicular lymph node metastasis. Targeted therapy was performed at 1 month after surgery. Liver and lung metastasis occured at 6 months and the patient died of multi-organ failure at 12 months after surgery.
