1.A study on mechanisms of levofloxacin, thymopentin combined with decoction of four noble drugs for treatment of patients with severe pulmonary tuberculosis

Objective To study the mechanisms of levofloxacin,thymopentin combined with decoction of four noble drugs for treatment of patients with severe pulmonary tuberculosis and offer a new therapeutic strategy for treatment of the disease.Methods A total of 100 patients with severe pulmonary tuberculosis admitted to Qinghai Fourth People's Hospital from November 2013 to January 2016 were enrolled,and they were divided into a research group (50 patients) and a control group (50 patients) by random number table.The patients in two groups were treated with 2HRZE/4HR standardized therapy program.The patients in the research group were additionally treated with levofloxacin (0.5 g orally taken,1 times a day),thymopentin (1 mg intravenous injection,once a day) combined with decoction of four noble drugs (ginseng 9 g,poria 9 g,atractylodes 9 g,and licorice 6 g,all the above ingredients were immersed in 400 mL water and boiled to 100 mL,one dose orally taken daily and it was equally divided into 2 parts,one part taken in the morning and the remaining part taken in the evening).Four months after treatment,the changes of indexes of immune functions [total lymphocyte count (LY),CD4+,CD8+,and CD4+/CD8+ ratio],blood coagulation indexes [prothrombin time (PT),activated partial thromboplastin time (APTT),thrombin time (TT),D-dimer,and plasma fibrinogen (Fib)],pulmonary function indexes [forced vital capacity (FVC),peak expiratory flow rate (PEF),forced expiratory maximum volume in 1 second (FEV1),and mean maximum expiratory flow (MMEF)] and blood gas analysis indexes [arterial partial pressure of carbon dioxide (PaCO2),arterial partial pressure of oxygen (PaO2),pulse oxygen saturation (SpO2),and oxygenation index (PaO2/FiO2)] and the therapeutic effects were observed in the two groups.Results After treatment,the CD8+,TT,PT,Fib,D-Dimer and PaCO2 of two groups were decreased significantly than those before treatment (all P < 0.05);while the LY,CD4+,CD4+/CD8+ ratio,FEV1,FVC,PEF,MMEF,APTT,PaO2,SpO2 and PaO2/FiO2 of two groups were all increased significantly than those before treatment (all P < 0.05).The changes of the study group were more obvious than those of the control group [LY (109/L):1.79 ± 0.19 vs.1.45 ± 0.16,CD4+:0.40 ± 0.03 vs.0.33 ± 0.03,CD8+:0.20 ± 0.01 vs.0.23 ± 0.02,CD4+/CD8+ ratio:2.10 ± 0.23 vs.1.67 ± 0.20,FEV1:0.269 ± 0.004 vs.0.198 ± 0.003,FVC:(3.78 ± 0.41)% vs.(3.14 ± 0.39)%,PEF (L/s):3.68 ± 0.26 vs.3.05 ± 0.23,MMEF (L/s):0.96 ± 0.06 vs.0.74 ± 0.05,PaO2 (mmHg,1 mmHg =0.133 kPa):95.11 ± 7.68 vs.85.23 ± 7.01,PaCO2 (mmHg):31.76± 3.26 vs.46.28±4.36,SpO2:0.96±0.08 vs.0.91 ±0.07,PaO2/FiO2 (mmHg):310.58± 11.12 vs.285.01 ± 10.76,TT (s):15.64± 1.25 vs.18.82 ± 1.54,PT (s):12.69 ± 1.01 vs.14.28 ± 1.21,APTT (s):29.01 ± 2.02 vs.25.21 ± 1.80,Fib (mg/L):233.46 ± 15.61 vs.286.27 ± 18.14,D-Dimer (μg/L):210.88 ± 14.13 vs.256.39 ± 16.47,all P < 0.05].After combined treatment,the sputum negative conversion rate [94% (47/50) vs.60% (30/50)],the total efficiency [88% (44/50) vs.64% (32/50)] and the focus absorption rate [86% (43/50) vs.60% (30/50)] of research group were significantly higher than those of the control group (all P < 0.05).Conclusions The combination of levofloxacin,thymopentin and decoction of four noble drugs on the bases of 2HRZE/4HR standardized therapy for treatment of patients with severe pulmonayr tuberculosis can help to regulate acid-base balance,improve the hypoxia condition and lung function,elevate the immune function and increase the blood circulation in the body to improve clinical efficacy.
2.Serial Analysis of Gene Expression in Immortalized BEP2D Cells and Malignant Transformed BEP2D Cells Induced by α-particle

Objective: The authors used the serial analysis of gene expression (SAGE) method to analyze transcripts present in the immortalized BEP2D cells and the malignant transformed BEP2D cells. Methods: As a step toward understanding the complex gene expression differences between the immortalized BEP2D cells and the malignant transformed BEP2D cells induced by α particle,SAGE method was introduced into this experiment. Technological methods included total RNA extraction, mRNA isolation, full length of dscDNA synthesis, PCR, transformation and sequencing. SAGE SoftWare was used to analyze tag sequences and compare the abundance of tags between two SAGE libraries. Results: Two independent SAGE libraries were constructed from the immortalized BEP2D cells and the malignant transformed BEP2D cells induced by 1.5 Gy α particles. A total of 2 331 SAGE tags were identified for the sequences, representing 252 unique transcripts. Though up to now the obtained information is limited, comparison of the two SAGE libraries indicated a remarkable similarity in the expression profiles. Of the 252 transcripts detected, 12 transcripts (4.8% ) matched no reliable known genes in UniGene library. Combination of Northern blot hybridization, the expression level of TGF β induced Smad7 gene in the malignant transformed cells was higher than that in the immortalized cells. Conversely, Chemokine receptor CCR11 gene was expressed at lower levels in the malignant transformation cells. Conclusion: (1) Out of results given by SAGE,the tendency of the abundance of gene expression and the gene expression differentiation between two cell lines were described. (2) The expression level of TGF β induced Smad7 gene in the malignant transformed BEP2D cells was higher than that in the immortalized cells and chemokine receptor CCR11 gene was expressed at lower levels in the malignant transformed BEP2D cells. (3)SAGE was a powerful method in a quantitative and simultaneous analysis of a large number of transcripts in any particular cell system, especially in defining functions of known genes.

5.Urethroplasty with pedunculated preputial flap and testicular tunica vaginalis for long anterior urethral stricture: A case report and review of the literature.

OBJECTIVETo explore the treatment of urethral stricture.
METHODSWe retrospectively studied the clinical data of 1 case of long anterior urethral stricture treated by urethroplasty with pedunculated preputial flap and testicular tunica vaginalis, and summarized the treatment of the disease with review of the relevant literature.
RESULTSThe operation was smooth and successful, and no such complications as fistula and urethral stricture were found during the follow-up.
CONCLUSIONUrethroplasty with pedunculated preputial flap and testicular tunica vaginalis as a substitute is feasible for the treatment of urethral stricture. The key to a successful operation is the proper choice of a urethral substitute.

6.The method and implementation of measuring the peak of bioelectrical signals.

This paper mainly introduces a method of measuring the peak value of bioelectrical signals, expatiates on the structure, testing result, analysis for error and measure taken for improvement of the circuit designed by this method. At last, the paper also points out that the method is wonderful, simple, easy to carry out, and can be used in many medical systems.

7.Analysis on the Correlation between TCM Syndromes and Inflammatory Reaction and Body Temperature Changes of Acute and Subacute Deep Vein Thrombosis

Objective To investigate the correlation between inflammatory cells, body temperature changes, and TCM syndromes of deep venous thrombosis (DVT) patients in acute and subacute phases. Methods The data of age, gender, body temperature, blood routine, venous ultrasonography, and four diagnostic information of 130 DVT patients in acute and subcute phases were collected and analyzed in a cross-sectional study. The correlation between inflammatory cells and the changes of body temperature and TCM syndromes were analyzed. Results Among 130 DVT patients, 37 patients had damp-heat syndrome, 64 patients had blood stasis and dampness syndrome, and 29 patients had qi stagnation syndrome. Neutrophils increased most obviously in blood stasis and dampness syndrome (P<0.05), which had close correlation with the skin redness (OR=1.287, 95%CI: 9.412-21.247). The mononuclear cells increased most obviously in the damp-heat syndrome, which has close correlation with nouhof (OR=7.364, 95%CI:1.189–45.603), high skin temperature (OR=6.683, 95%CI:1.791–24.938), skin tightness (OR=6.107, 95%CI:1.423–26.203) and weakness (OR=3.302, 95%CI: 1.002–9.169). The temperature rising was the most common in the damp-heat syndrome, and the increase of mononuclear cells was the most common one. Conclusion DVT is often accompanied with elevated levels of inflammatory cells and body temperature. Damp-heat syndrome has close correlation with body temperature and mononuclear cells increasing. Dampness and blood stasis syndrome and neutrophils are closely related.
8.Investigation of the characteristics of HCV genotypes of han and korean In yanbian area of Jilin province

Objective To investigate the characteristics of HCV genotypes of Han and Korean in Yanbian area of Jilin Province.Methods The HCV RNA load and genotypes of the 119 chronic hepatitis C patients in Yanbian area of Jilin Province were determined by real-time PCR and LiPA.The differences of the HCV genotypes in Han and Korean cases,in severity of the diseases,in HCV-RNA load,and in the relation with type 2 diabetes mellitus were analyzed.Results There was no significant difference in the distribution of each HCV genotype between Han and Korean patients (P>0.05) with chronic hepatitis C.The difference between HCV genotype and HCV-RNA load was not significant (P>0.05).With and without type 2 diabetes mellitus in these patients.The distribution of HCV genotype was also not significandy different (P>0.05).The type 1b of HCV genotype in the moderate to severe chronic hepatitis C patients accounted for 58.06%.It was different compared with mild chronic hepatitis C patients (P<0.05).Conclusion ①The type 1b is the most popular HCV genotype in Yanbian area of Jilin Province,type 2a is the second and there are still a few other genotypes.②There is no significant difference in the distribution of HCV genotypes between Han and Korean cases.③The HCV genotypes has nothing to do with the load of HCV-RNA.④The distribution of HCV genotypes in chronic hepatitis C patients with and without diabetes mellitus is not significantly different.⑤Type 1b of HCV infection is relatively severe.

10.Detection of MSH2 gene mutation by PCR.

OBJECTIVE:
To establish a genetic diagnosis method for a novel MSH2 mutation.
METHODS:
A specific primer on the mutated site of MSH2 was synthesized and PCR was conducted using the specific primer and another downstream primer. PCR products were electrophoresed and then the carriers with the novel gene mutation of the carriers or non-carriers were identified.
RESULTS:
MSH2 in a hereditary nonpolyposis colorectal cancer family were successfully found.
CONCLUSION
The method is effective and simple for genetic diagnosis of the novel mutation in MSH2.
