Objective To confirm the association between the“Tip-Apex Distance (TAD)”and cut-out of the lag screw from the femoral head.and to analyze other factors leading to the cut-out.Methods The complete radiographic and clinical data of 106 patients with femoral intertrochanteric fractures were available for this study. They were 65 men and 41 women,with an average age of 52.4 years (range,20 to 83 years).According to Evans classification.19 cases belonged to typeⅡ.25 to typeⅢ,32 to typeⅣ,29 to typeⅤ,and one to type R.The bone quality was classified by Singh rating system:44 cases were rated as typeⅥ.34 as typeⅤ,23 as typeⅣand five as typeⅢ.They were treated with open reduction and fixation with 135?dynamic hip screw (DHS).According to the finding of Baumgaertner that“TAD”beyond 25 mm would grcatly increase the risk of cut-out,the patients could be divided into two groups:59 cases with“TAD”less than 25 mm and 47 greater than 25 mm.Results The mean duration of follow-ups was 14.45 months (range,4.5 to 28.0 months).Of the 15 cases whose“TAD”was more than 30 mm,one had the cut-out.Of the seven cases whose“TAD”was more than 40 mm,two had the cut-out (P=0.000). The average age of the three patients was 78.7 years (range:75 to 83 years) and 27.1 years older than that of the 103 patients whose fracture healed (P=0.000).They belonged to the unstable intertrochanteric fracture of the femur (two to Evans type V and one to type R).The reduction was assessed as excellent in 43 cases,good in 47 cases,fair in nine cases (of whom one had the cut-out),poor in seven cases (of whom two had the cut-out). Conclusions The cut-out of the lag screw from the femoral head can be caused by age,fracture type and stability of reduction and“TAD”.The greater the“TAD”value,the greater possibility of cut-out.

Objective To investigate the capability of cardiomyoganic differentiation of human umbilical cord perivascu- lar cells induced by 5-azacitidine in vitro.Methods Human umbilical cord perivascular colls were harvested from the umbilical cords of consenting full-term caesarian section patients,which were digested by collagenase,and were purified, expanded through repeating passaging in time in vitro.The HUCPV cell of passage 3 were treated with 5-azacytidine(5 -aza,5?10~(-6)nmol/L) for 24 hours and continued to be cultured under the previous condition for 4 weeks.The prolifera- tion and differentiated phenotype of HUCPV cells were observed.Results The isolation method of the UC vasculature and enzymatic digestion of its perivascular tissue to rapidly harvest a highly proliferative HUCPV cells.After treated with 5 -aza for 4 weeks,the passage 3 HUCPV cells formed the spindle-shaped cells and rate of nuclear and cytoplasm de- creased.Myotube-like structures were found in some cytoplasm.Immunohistochemistry results showed that the HUCPV cells were similar to MSC by stained positively for CD44 and negtively for CD34.The HUCPV cells induced by 5-aza were stained positively a-cardiac action and cardiac Troponin by immunohistochemistry.Furthermore,the expression of a -cardiac action and cardiac treponin gene as detected positively by RT-PCR after HUCPV cells were induced by 5- aza.Conclusion The HUCPV cell was a valuable material for cell-based therapies and can be induced into cardiac- like cells by treatment with 5-aza.

OBJECTIVETo offer evidences for quality control of medicinal plant of Dactylicapnos scandens.

METHODPharmacognostic studies were carried out through field collection, morphological and microscopic characteristics, and TLC.

RESULTObservation and description of the experimentation were made.

CONCLUSIONDactylicapnos scandens can be identified from Genus of Dactylicapnos which has the similar morphological characteristics. The morphological and microscopic characteristics and the results of TLC can be used as evidences for quality control of this medicinal plant.

OBJECTIVETo study the imaging findings outcome of the percutaneous laser disc decompression (PLDD) and evaluate the middle-term safety and efficacy of PLDD.

METHODSThe imaging data of 22 cases suffered cervical spondylosis or lumbar spondylosis and treated by PLDD more than 3 years were retrospectively reviewed. To observe the intervertebral space of the invasive disc and the intervertebral disc hernia before and after the operation on the lateral projection of X-ray and the axial view of the MRI. To make a statistical analysis of the data.

RESULTSThere was no obvious change of disc height involving the anterior disc height, the intermediate disc height and the posterior disc height after the PLDD at the final follow-up (P > 0.05). At last time follow-up, the Sagittal Index (SI) are 0.10 - 0.54 and 0.06 - 0.39 before and after the PLDD in the treatment of cervical vertebra. The statistical difference were significant (P < 0.05). In lumbar vertebra, the SI is 0 - 0.71 and 0 - 0.48 disc before and after the PLDD. The statistical difference were very significant (P < 0.01).

CONCLUSIONSThe PLDD will not destroy the intervertebral space of the cervical and lumbar vertebra obviously; moreover, it can effectively reduce the disk hernia. The PLDD is a safe and effective Mini-invasive surgery for cervical vertebra and lumbar vertebra diseases.

Purpose: To investigate the relationship between rumination, coping strategies, and subjective well-being (SWB) and test the mediating effects of coping strategies on rumination and SWB in patients with breast cancer (BC). Methods: This cross-sectional study assessed rumination, coping strategies, and SWB using the General Well-being Schedule, the Chinese Event-Related Rumination Inventory, and the Medical Coping Modes Questionnaire in BC patients admitted to a tertiary general hospital in China. Results: SWB was positively associated with the total score for rumination (r = .32, p < .01), deliberate rumination (r = .75, p < .01), and confrontation (r = .58, p < .01). The relationship between rumination and SWB was mediated by confrontation (indirect effect = .74). Conclusion BC diagnosis can affect patient's SWB. These findings indicate that rumination and confrontation have direct and indirect effects on SWB. Therefore, psychological interventions focused on improving coping may increase SWB. Notwithstanding, larger longitudinal studies are needed to further examine the relationship between cognitive processes, coping strategies, and SWB.

Objective To investigate the correlation between inflammatory cells, body temperature changes, and TCM syndromes of deep venous thrombosis (DVT) patients in acute and subacute phases. Methods The data of age, gender, body temperature, blood routine, venous ultrasonography, and four diagnostic information of 130 DVT patients in acute and subcute phases were collected and analyzed in a cross-sectional study. The correlation between inflammatory cells and the changes of body temperature and TCM syndromes were analyzed. Results Among 130 DVT patients, 37 patients had damp-heat syndrome, 64 patients had blood stasis and dampness syndrome, and 29 patients had qi stagnation syndrome. Neutrophils increased most obviously in blood stasis and dampness syndrome (P<0.05), which had close correlation with the skin redness (OR=1.287, 95%CI: 9.412-21.247). The mononuclear cells increased most obviously in the damp-heat syndrome, which has close correlation with nouhof (OR=7.364, 95%CI:1.189–45.603), high skin temperature (OR=6.683, 95%CI:1.791–24.938), skin tightness (OR=6.107, 95%CI:1.423–26.203) and weakness (OR=3.302, 95%CI: 1.002–9.169). The temperature rising was the most common in the damp-heat syndrome, and the increase of mononuclear cells was the most common one. Conclusion DVT is often accompanied with elevated levels of inflammatory cells and body temperature. Damp-heat syndrome has close correlation with body temperature and mononuclear cells increasing. Dampness and blood stasis syndrome and neutrophils are closely related.

The biomechanical effects of the reconstructed mandible with autogenous bone grafts were analyzed and evaluated, under simulated mechanical environment of occlusion. Based on anatomical feature of the human skeleton, muscle system and the clinical B, BS"S" and RB"S"defect patterns, the modularized model of mandible and the three finite element models of the fibula grafts and iliac crest grafts reconstruction mandible were made. The stress state calculations under bite force were also processed. The findings are as follows: Along the right connection area to left connection area (corresponding to far-end of the molar teeth alveolar ridge and the chin section respectively), the stress state on the various grafted bone all changes from compression stress to tensile stress. There is obvious change on the stress distribution between the fibula type reconstruction mandible and normal one. The stress on the grafted bone which is mainly made up of cortical bone is approximately 5 times higher than that of normal, which indicates the obvious stress shielding effect; the stress distribution of the iliac crest type reconstruction mandible is close to the normal mandible, the iliac crest used is mainly made up of cancellous. Therefore, the mechanical properties of the reconstructed mandible with iliac crest are more similar to normal mandible, which is beneficial to wound healing and further functional reconstruction.

OBJECTIVETo construct a three-dimensional finite element model of reconstructed mandible and analyze the stress distributions of the model.

METHODSCT scan and CAD/CAM software was used to develop the three-dimensional finite element model of reconstructed mandible and stress analysis was performend with ANASYS 7.0 software.

RESULTSThe three-dimensional finite element models of normal mandible and reconstructed mandible were constructed. The stress analysis discovered an unbalanced stress distribution in the mandible.

CONCLUSIONReconstructing mandible with fibular flap not only damages its structure, but also induced the badness of its biomechanical environment, in which condyle was the most sensitive region during the stresses in the mandible were changed.

OBJECTIVECongenital long QT syndrome (LQTS) is an inherited disorder of cardiac repolarization characterized by prolongation of QT interval and polymorphic ventricular tachycardia torsade de pointes (TdP) in the electrocardiogram (ECG). Clinical symptoms include recurrent syncope, seizure or even sudden death. It is caused by mutations of at least seven genes, six of them encoding ion channels that determine the duration of ventricular action potentials. One of these genes, KCNQ1, encodes an alpha-subunit of cardiac slowly activated delayed rectifier potassium channel. Patients carrying mutations of KCNQ1 are named as LQT1, which accounts for 42% of patients with LQTS. This study sought to analyze the clinical data of Chinese with LQTS and to screen for the mutations of KCNQ1.

METHODSThe universally accepted phenotypic criteria of LQTS was used for identification of probands. There were six families with LQTS. They were enrolled in this study. Clinical and ECG data of each family member were recorded. Genomic DNA was prepared from peripheral blood lymphocytes. Polymerase chain reaction-single strand conformation polymorphism analysis was used to screen for mutations throughout the whole coding region of KCNQ1 and DNA sequencing was performed to determine the exact mutation site.

RESULTSThere were totally 13 patients in the six LQTS families. Five were male and eight female. One suffered from sudden death, 10 had syncope and 2 were asymptomatic. Eleven of the 13 patients had ECG data. Their QT and QTc (mean +/- SD) were (0.460 +/- 0.058) s and (0.516 +/- 0.058) s, respectively. TdP was observed in 3 patients (27%) during the syncope attack. By PCR-SSCP analysis, two novel KCNQ1 deletion mutations 356-357 Delta QQ and 626-631 Delta GSGGPP were identified in 7 patients of 2 families. None of 50 normal individuals carried these mutations, indicating these two mutations were likely to cause the disease. In addition, P448R was found in one affected and some unaffected members in other two families and in 7 of 50 (14%) normal individuals, indicating that this might be a polymorphism. All the three mutations located in C-terminal domain of KCNQ1 protein.

CONCLUSIONSTwo novel deletion mutations and one novel polymorphism of KCNQ1 gene were identified among 6 Chinese families with LQTS.
