1.Serum biomarkers in chronic obstructive pulmonary disease

Chronic obstructive pulmonary disease (COPD) is a chronic airway diseases,which leads to heavy social and economic burden to our country.We can use serum biomarkers to evaluate diagnosis,classification,treatment and prognosis of COPD.The change of biomarkers provides lots of valuable clinical information.A variety of biomarkers are associated with the severity of lung function,which can be used to judge disease severity.Some indicators are related to the diagnosis of acute exacerbation or hospitalization risk.Some serum markers would guide therapy and can be effectively applied to clinical work.Study of COPD serum biomarkers would provide more reference information for clinical physicians in diagnosis,treatment and prognosis of COPD.
2.Changes of IA and IK from the pyramidal neurons at hippocampal CA1 during the early development of rats.

Brain growth spurt (BGS) is the critical period of neuronal growth and synaptic connection. The voltage-gated K(+) channel is the key channel for maintenance of cell excitability and information transfer among neurons. The purpose of the present study is to investigate the critical period of voltage-gated K(+) channel development in hippocampal CA1 neurons during the BGS. Changes of voltage-gated K(+) currents in neurons from acutely isolated hippocampal CA1 brain slices of rats at different ages (0-4 weeks after birth) were recorded by the whole-cell patch-clamp technique. The depolarization voltage was set at +90 mV, and 0 week was set as the control group. The experimental results showed that, with increasing ages (1-4 weeks), the maximum current densities of IA increased by (16.14 ± 0.51)%, (81.73 ± 10.71)%, (106.72 ± 5.29)%, (134.58 ± 8.81)% (n = 10, P < 0.05), and the maximum current densities of IK increased by (16.75 ± 3.88)%, (134.01 ± 2.85)%, (180.56 ± 8.49)%, (194.5 ± 8.53)% (n = 10, P < 0.05), respectively, compared with those in 0 week. During 0-4 weeks after birth, the activation kinetics of IA shifted to left, and the half activation voltages of IA were 14.67 ± 0.75, 13.46 ± 0.64, 8.39 ± 0.87, 4.60 ± 0.96, 0.54 ± 0.92 (mV, n = 10, P < 0.05), respectively; The activation kinetics of IK shifted to left and the half activation voltages of IK were 8.94 ± 0.85, 6.65 ± 0.89, 0.47 ± 1.15, -1.80 ± 0.89, -8.56 ± 1.08 (mV, n = 10, P < 0.05) respectively. The inactivation kinetics of IA also shifted to left, and the half inactivation voltages were -45.68 ± 1.26, -46.81 ± 0.78, -48.64 ± 0.81, -51.96 ± 1.02, -58.31 ± 1.35 (mV, n = 10) respectively at 0, 1, 2, 3 and 4 weeks after birth, which showed no significant changes between 0 and 1 week, but significant decreases during 1-4 weeks after birth (P < 0.05). These results indicate that the current densities of IA and IK increase and the kinetic characteristics of the voltage-gated K(+) channels change with increasing ages during 0-4 weeks after birth, and the differences are especially significant between the 1st week and the 2nd week after birth. These changes may be related to the maturation of hippocampal neurons and the progress of their functions.

3.ImmuKnowTM cellular immune functional assay in the diagnosis of infection after liver transplantation

Objective To evaluate the applied value of functional immunity measured by the ImmunKnow assay in the diagnosis of post-transplant infection in Chinese 1iver recipients.Methods Thirty-eight normal adults and 68 adult liver transplant recipients were under investigation.Whole blood samples from either normal volunteers(each sample for one person)or the liver recipients(one or more samples for one person)were collected freshly and cultured within 6h.The CD4+T cells were selected and their ATP value was assayed the next day.The liver recipients were grouped in stable status(n=52)or infection(n=64)according tO their clinieal manifestation.Results The average ATP value in the recipients with infection after liver transplantation was 165.7±100 μg/L,significantly lower(P<0.05)than that in stable recipients(309±126 μg/L)or normal volunteers (292±83 μg/L).The low ATP levels in post-transplant recipients had fair good correlation to infection clinically(RR=0.5021,P<0.01).Infectious risk was high when ATP value was less than 165μg/L(OR=11,95%CI 3.9-32.2,P<0.01).Specificity and sensitivity of low ATP value in post-transplant infection were 86.53%and 73.81% respectively,Conclusion ImmuKnow assay provides a new tool in monitoring immune status in post-transplant recipients,and call helpfully predict and diagnose clinical infection.
4.Mechanism of lung cancer and chronic obstructive pulmonary disease

Lung cancer and chronic obstructive pulmonary disease (COPD) are both serious threats to human health,resulting in a heavy economic burden for the society.Epidemiological studies have shown that lung cancer and COPD have a high comorbid rate.Cigarette smoking is a congenerous risk factor for both lung cancer and COPD,and may lead to these diseases by triggering certain similar pathways.The common pathogenesis of lung cancer and chronic obstructive pulmonary disease involves premature pulmonary failure,oxidative stress and inflammation,telomere length,genetic susceptibility,and epigenetics.
5.Diagnosis and treatment of labral tear.

OBJECTIVE:
To review the literature regarding diagnosis and treatment of labral tear.
DATA SOURCES:
A systematic search was performed in PubMed using various search terms and their combinations including hip, labrum, acetabular labral tear, arthroscopy, diagnosis, and anatomy.
STUDY SELECTION:
For each included study, information regarding anatomy, function, etiology, diagnosis, and management of acetabular labral tear was extracted.
RESULTS:
Five hundred and sixty abstracts about anatomy, function, etiology, diagnosis, and management of acetabular labral tear were reviewed and 66 selected for full-text review. The mechanism of labral tear has been well explained while the long-term outcomes of various treatment remains unknown.
CONCLUSIONS
Labral tear is generally secondary to femoroacetabular impingement, trauma, dysplasia, capsular laxity, and degeneration. Patients with labral tear complain about anterior hip or groin pain most commonly with a most consistent physical examination called positive anterior hip impingement test. Magnetic resonance arthrography is a reliable radiographic examination with arthroscopy being the gold standard. Conservative treatment consists of rest, non-steroidal anti-inflammatory medication, pain medications, modification of activities, physical therapy, and intra-articular injection. When fail to respond to conservative treatment, surgical treatment including labral debridement, labral repair, and labral reconstruction is often indicated.

6.PRMT6 expression in lung of COPD mouse model and its relationship with inflammatory gene expression

Objective To explore the expression of protein arginine methyltransferases 6 (PRMT6) in chronic obstructive pulmonary disease (COPD) mouse model and its correlation with inflam mation gene interleukin 6 (IL-6) and cyclooxygenase 2 (COX-2).Methods Sixteen C57BL/8J mice were randomly divided into 2 groups:control group and cigarette smoke extract (CSE)-induced COPD group.Each group was injected intraperitoneally with phosphate-buffered saline solutions (PBS) or CSE at days 1,12,23 and measured lung function and collected lung tissue at day 29.The morphology change of the lung tissue was determined by hematoxylin and eosin (HE) stainning.The protein expression of PRMT6,H3R2me2a and H3K4me3 were detected in lung homogenates by Western-blotting.The mRNA expression of PRMT6,IL-6 and COX-2 were measured by quantitative real-time polymerase chain reaction (qRT-PCR).Results Comparing to control group,COPD group showed typical emphysema changes in the lung tissue,and significantly decreased lung function.The mRNA and protein expression of PRMT6 in the lung tissue of the mice with COPD were significantly decreased,following with the down-regulated signal level of H3R2me2a protein expression,while the increased level of IL-6 and COX-2 mRNA.Meanwhile,PRMT6 was negatively correlated with IL-6 and COX-2 mRNA expression.Conclusions PRMT6 was significantly reduced in CSE-induced COPD mouse model,following with decreased histone H3R2 dimethylation and increased H3K4 trimethylation,negatively correlating with inflammatory gene IL-6 and COX-2 transcription expression.PRMT6 downregulation may activate the transcriptional expression of inflammatory genes involved in the development of COPD,through the regulation of histone methylation level.
7.To investigate the effect of scan table on CT size-specific dose estimate in children

Objective To investigate the effect of scan table on size-specific dose estimate ( size-specific dose estimate, SSDE) in children's CT scan. Methods CT imaging data and CTDIvol of 44 children ( 15 heads, 13 chests, 16 abdomen-pelvis) who underwent Siemens SOMATOM Definition AS+ 64 row 128-slice CT scan were retrospectively collected. CTDIvol of each patient was recored, WED ( water equivalent diameter) was calculated by two different methods ( with or without table) , donated as WED-T and WED-NT, then the corresponding SSDEWED ( SSDEWED-T and SSDEWED-NT ) was calculated. And the SSDEWED-NT was used as reference to evaluate the difference between WED and SSDEWED obtained by two different methods. Results Including part of table will lead to the overestimate for WED, with mean differences of 0. 10%, 2. 82% and 2. 54% for head, chest and abdomen-pelvis, respectively, while SSDEWED will be underestimated by 0. 06% ( head ) , 2. 70% ( chest ) and 1. 59% ( abdomen-pelvis ) . Conclusions Including par of the patient table has a certain effect on SSDEWED for children, more attention should be paid for the application of SSDEWED.
8.Influence of national volume-based procurement policy on the economy and use structure of anti-infective drugs in a hospital

Objective To investigate the influence of the national volume-based procurement(NVBP)policy on the economy and use structure of anti-infective drugs in a general 3-A hospital,and to provide a reference for the standardization of clinical application management and policy implementation.Methods The price,defined daily doses(DDDs),average defined daily cost(DDDc),actual cost saving,substitution rate of selected drugs(SRSD)and avoidance rate of alternative drugs(ARAD)of anti-infective drugs before and after the implementation of 1-7 collection NVBP policy were analysed.Results The NVBP policy led to a>70%reduction in the price of 70.73%of the hospital's target drugs,69.70%of the DDDs increased,81.82%of the DDDc decreased and 63.64%showed the relationship of"volume increase and price decrease".The average SRSD was 84.49%in the first cycle and 95.94%in the second cycle.The average actual cost saving in the first cycle was 474 100 yuan,with an average saving rate of 407.60%,and in the second cycle was 803 100 yuan,with an average saving rate of 1 257.04%,showing an increasing trend year by year.The consumption of alternative drugs increased by 53.85%in the first cycle and 66.67%in the second cycle,with the highest annual increase of 555.63%,and the average of 74.65%.Conclusion After the implementation of NVBP policy,selected drugs mainly replaced unselected drugs,and the cost of anti-infection treatment was greatly reduced.However,it was necessary to strengthen the control of alternative drugs,standardize the management of clinical application,carry out clinical comprehensive evaluation of selected drugs and real world research.
9.Clinical efficacy observation of acupuncture at suliao (GV 25) on improving regain of consciousness from coma in severe craniocerebral injury.

OBJECTIVETo compare the clinical therapeutic effects differences between acupuncture at Suliao (GV 25) and Shuigou (GV 26) on promoting regain of consciousness from coma in severe craniocerebral injury.
METHODSBased on regular emergency treatments of neurosurgery, eighty-two cases of craniocerebral injury who were under stable condition were randomly divided into an observation group (42 cases) and a control group (40 cases). Suliao (GV 25) was selected as main aupoint, while Laogong (PC 8) and Yongquan (KI 1), etc. were selected as adjuvant acupoints and Neiguan (PC 6), Sanyinjiao (SP 6), Yifeng (TE 17) and Wangu (GB 12), etc. were selected as matching acupoints in the observation group where a strong needle manipulation was applied to improve the regain of consciousness. The main acupoint of Shuigou (GV 26) along with identical adjuvant acupoints and matching acupoints in the observation group were selected in the control group with identical strong needle manipulation. The treatment was given once a day in both groups, five times per week and ten times were considered as one session. The immediate clinical symptoms after acupuncture at Suliao (GV 25) and Shuigou (GV 26) were observed as well as Glasgow coma scale (GCS) before the treatment, after 45 days and 90 days of treatment to assess the resuscitation time and rate. Also the clinical efficacy was compared between both groups.
RESULTSThe occurrence rate of sneezing reflex was 85.7% (36/42) in the observation group, which was higher than 25.0% (10/40) in the control group (P < 0.01). The average resuscitation time was (64.6 +/- 19.4) days in the observation group, which was obviously shorter than (73.8 +/- 14. 6) days in the control group (P < 0.05). The resuscitation rate was 88.1% (37/42) in the observation group, which was similar to 75.0% (30/40) in the control group (P > 0.05). Compared before the treatment, GCS were both improved after the treatment in two groups (both P < 0.01). The 90-day GCS was 9.52 +/- 2.32 in the observation group, which was superior to 8.47 +/-2.14 in the control group (P < 0.05). The curative and markedly effective rate was 45.2% (19/42) in the observation group, which was superior to 22.5% (9/40) in the control group (P < 0.05).
CONCLUSIONThe effect of acupuncture at Suliao (GV 25) on improving regain of consciousness from coma in severe craniocerebral injury is positive. It could specifically improve sneezing reflex and stimulate respiratory center, which has more obvious effect than acupuncture at Shuigou (GV 26).

10.Medication adherence in inflammatory bowel disease.

Inflammatory bowel disease (IBD) is a chronic idiopathic inflammatory condition with intestinal and extraintestinal manifestations. Medications are the cornerstone of treatment of IBD. However, patients often adhere to medication poorly. Adherence to medications is defined as the process by which patients take their medications as prescribed. Treatment non-adherence is a common problem among chronic diseases, averaging 50% in developed countries and is even poorer in developing countries. In this review, we will examine the adherence data in IBD which vary greatly depending on the study population, route of administration, and methods of adherence measurement used. We will also discuss the adverse clinical outcomes related to non-adherence to medical treatment including increased disease activity, flares, loss of response to anti-tumor necrosis factor therapy, and so forth. There are many methods to measure medication adherence namely direct and indirect methods, each with their advantages and drawbacks. Finally, we will explore different intervention strategies to improve adherence to medications.
