Doctors and nurses from general outpatient and emergency department usually lack the treatment experience of hemophilia,therefore,emergency system and diagnostic criteria for hemophilia is es-sential.Physicians of emergency department should be trained on identifying the undiagosed patients with inherited bleeding disorders and assessing the condition of bleeding.Prompt clotting factors replacement therapy should be given to the patients with hemophilia once bleeding is suspected.Physicians should be aware that inhibitors make the emergency treatment more complicated.

Objective:To evaluate the effects of insulin-like growth factor 1(IGF-1) in the pathogenesis of oral submucous fibrosis(OSF).Methods:IGF-1 mRNA in 27 OSF tissues and 9 normal tissues were detected by the in situ hybridization technique.Results:There were expression of IGF-1 mRNA in keratinocytes of all the 8 cases of early stage OSF and 7 of the 19 cases of middle-late stage OSF.No expression of IGF-1 mRNA was detected in keratinocytes of nomal tissue.There were no significant difference between OSF tissues and normal tissues in the expression of IGF-1 mRNA in fibroblasts.IGF-1 mRNA expression in endothelial cells were found in 5 cases of middle-late stage.Conclusion:IGF-1 might be secreted by keratinocytes and may play important role in the pathogenesis of OSF.

Objective To investigate the value of quantitative analysis of spectrum CT in the differential diagnosis of lung cancer and tuberculosis in unenhanced phase.Methods Data of 51 patients with solitary pulmonary nodules or masses confirmed by pathology who underwent chest unenhanced spectrum CT were retrospectively analyzed.There were 36 cases of peripheral lung cancer (lung cancer group) and 15 cases of tuberculosis (tuberculosis group).The differences of 40～ 140 keV single energy CT value,shape and slope of energy spectrum attenuation curve,different base material of concentration values and effective atomic number (Eff-Z) between the two groups were compared.Results The single energy CT values of 40～70 keV in lung cancer group were higher than those in tuberculosis group (all P＜0.05),while no significant difference of the single energy CT values of 80～140 keV were found in both two groups (all P＞0.05).The shape of energy spectrum attenuation curve of lung cancer group was fast express reduction (36/36,100％).The energy spectrum attenuation curve of tuberculosis group showed walked straightly (11/15,73.33％) and back up type (4/15,26.67％).The slope of the energy spectrum attenuation curve in lung cancer group (K40～ 65 key,K65～90 kev,K90～140 kev)were higher than those in tuberculosis group (all P＜0.01).The calcium (water) concentration of lung cancer group was higher than that of tuberculosis group (P＜0.01),while the lipid (water) concentration of lung cancer group was lower than that of tuberculosis group (P＜0.01).The water (calcium) concentration in both groups was not statistically different (P=0.10).The Eff-Z of lung cancer group was higher than that of the tuberculosis group (P＜0.01).Conclusion The peripheral lung cancer and the tuberculosis have different energy spectrum CT quantitative parameters in unenhanced phase,which can provide valuable informations for diagnosis.

Objective:To explore the effects of myofibroblast in the pathogenesis of oral submucous fibrosis(OSF).Methods:The expressions of ?-SMA in the tissues of OSF and normal control were detected by immunohistochemistry. Fibroblasts were isolated from oral mucosa of OSF patients and healthy controls and were cultured under stimulation by arecoline.The expressions of ?-SMA in OSF fibroblasts and normal fibroblast were detected by immunocellchemistry and RT-PCR.Results:(1)No positive expression was found in normal tissues except in the blood vessel,but in OSF tissue,?-SMA positive expression was found in many spindle cells in the oral submucousal layer.(2)Few fibroblasts in vitro from normal controls were ?-SMA positive.(3)Most of the second passage of OSF fibroblasts were ?-SMA positive(85.80%?3.56%). (4)The expression of ?-SMA in fibroblasts could not be increased by the stimulation of arecoline.Conclusion:Myofibroblasts may play an important role in the pathogenesis of OSF,and the appearrence of myofibroblasts in OSF may not be directly induced by the stimulation of arecoline.

Objective:In order to elucidate the derivation and differentiation mechanisms for derivation of myofibroblasts in oral submucous fibrosis.Methods:Oral keratinocytes and fibroblasts were isolated and cultured,and a co-cultur system composed of the keratinocytes and fibroblasts was established in vitro.Expressions of ?-SMA protein and mRNA in fibroblasts were detected by immunohistochemistry and RT-PCR. The level of TGF-?1 in conditioned medium were evaluated by ELISA.Results:The expressions of ?-SMA mRNA and protein in fibroblasts co-cultured with keratinocytes preprocessed by arecoline was higher than that in fibroblasts co-cultured with keratinocytes and without preprocessed by arecoline. The level of TGF-?1 in conditioned medium of fibroblasts-keratinocytes preprocessed by arecoline co-cultures was higher than those of fibroblasts cultured alone.Conclusion:The derivation of myofibroblast from FB can be promoted by keratinocytes preprocessed by arecoline and TGF-?1 may play a role in the process.

Objective To research and develop the model of external reciprocating sports intermittent hypoxia box for rats. Methods Two animal cabins (28 cm?20 cm?15 cm) and one hypoxia chamber (80 cm?80 cm?60 cm) which covered each cabin in turn were developed. 4 male SD rats were placed in each animal cabin. The hypoxia chamber in which low O2 concentration was maintained with filling high-pure Nitrogen continuously, moved to and fro between two animal cabins, and driven by a servomotors. Two animal cabins were covered in turn by the hypoxia chamber and get the same low O2 concentration as in the hypoxia chamber. The goal volume fraction of oxygen in hypoxia chamber was (10?0.5)% and the time of unilateral movement from one animal cabin to another was 4 s, the cycle time of intermittent hypoxia was 180 s, the retention time was 86 s (including the hypoxia and normoxia time was 90 s respectively). The hypoxia chamber runs 8 h per day. The numerical value on artery blood gas of rats in which hypoxia chamber inside and outside was measured Results The reciprocating sports intermittent hypoxia chamber can be operated safely and reliably. The lowest PaO2 is (4.766? 0.536) kPa(or (35.75 ?4.02) mmHg) and the lowest SaO2 is 0.69 ?0.08 when the animal cabin in hypoxia chamber. Conclusion The chronic intermittent hypoxia model for rats is established by self-developed external reciprocating sports intermittent hypoxia chamber.

Objective To study the relationship between sperm activity and the level of L-carnitine in seminal plasma of patients with idiopathic asthenospermia by HPLC.Methods Experimental group:idiopathic asthenospermia 50 cases,and the control group:male with pregnancy history 10 cases.The semen parameters (volume,sperm density,viability,motility),and L-carnitine level in seminal plasma were detected with high performance liquid chromatography method.Results There were significant difference between A and B group about sperm density and the percentage of progressive motile spermatozoa,and the level of L-carnitine(P＜0.01).Conclusions The decrease of L-carnitine level in patients with idiopathic asthenospermia,is closely related to the low sperm motility,which shows that L-carnitine can be used to treat idiopathic asthenospermia effectively.
