Drug Evaluation Research 2017;40(6):844-847
Objective To investigate the clinical efficacy and safety of sitagliptin combined with metformin in treating patients with type 2 diabetes with non-alcoholic fatty liver diease (NAFLD).Methods Sixty-five patients with type 2 diabetes with non-alcoholic fatty liver admitted into our hospital from June 2012 to June 2015 were randomly divided into two groups.Thirty-three patients in control group were treated with metformin only, and thirty-two patients in observation were treated with sitagliptin combined with metformin. The changes in body mass index (BMI),fasting plasma glucose (FPG),2-hour postprandial glucose (2hPG),Hemoglobin Ale (HbAlc),homeostasis model assessment of insulin resistance (HOMA-IR),and liver function index including that aminopherase of alanine acid (ALT),aminopherase of aspartic acid (AST),gamma-glutamyl transterase (GGT),triglyceride (TG),and total cholesterol (TC) were detected before and after 16 weeks of treatment. And the total effective rate and safety were analyzed and compared. Results After treatment,HOMA-IR,FPG,2hPG,HbA1c,ALT,AST,GGT,TG,and TC were remarkable improvement (P < 0.05),and HOMA-IR,HbA1c,AST,ALT,GGT,and TG were significantly better than those of control (P < 0.05).Additionally,the total effective rate in observation group with 90.9% was significantly higher than that of control group with 42.4% (P < 0.05).And difference in incidences of adverse effect between observation group with 15.6% and control group with 9.1% were not significant. Conclusion Sitagliptin combined with metformin treatment had a good ability on treating patients with type 2 diabetes with NAFLD by improvement of metabolic syndrome and liver function.

Objective To survey the compliance degree of patients with allergic rhinitis completing their specific desensitizing therapy and to evaluate the relating factors with this compliance degree, and to explore the nursing countermeasures. Methods A retrospective study was carried out in 186 patients with allergic rhinitis receiving specific immunotherapy from January 2007 to December 2008. Noncompliance patients were contacted with scheduled telephone interviews to investigate the causes. Results Compliance rate was 71.0%,primary reason for non-compliance is poor efficacy (16 patients, 29.6% ), followed by busy work or study (10 patients, 18.5%) and disappearance of symptoms (7 patiens, 13.0%). Conclusions The compliance degree of AR patients receiving specific desensitizing therapy is still poor, but the reasons for non-compliance can be controlled by many factors, compliance is still much room for improvement.

Objective To establish the triplex Taqman probes real-time RT-PCR method for simultaneously detecting of EV71,CA16 and EV.Methods Retrospective study.Specific primers and probes were designed based on conserved regions of EV71,CA16 and EV.The sensitivity, specificity and reproducibility were assessed by the optimized reaction system.A total of 176 throat swabs as the experimental group were collected from children with suspected hand foot mouth disease (HFMD),who admitted from April 2012 to July 2012 in Nanjing Children's Hospital affiliated to Nanjing Medical University.During this time,10 cases of healthy children,10 cases of outpatients with flu-like symptoms and 90 cases of inpatients in pneumology department of our hospital were recruited as control group, whose throat swabs were also collected. All of 286 samples were tested by the triplex Taqman probes real-time RT-PCR for simultaneously detecting EV71,CA16 and EV.SPSS13.0 was used to analyze the results. Results The sensitivities of the triplex Taqman probes real-time RT-PCR was 1.0 × 103 copies per milliliter for EV71,CA16 and EV.It showed 100% specificity for 9 enterovirus and 3 non-enterovirus.Analysis with 1.0 × 103-1.0 × 105 copies per milliliter constructed plasmids demonstrated high reproducibility with coefficient of variation of 0.44%-1.04% for EV71,0.38%-0.73% for CA16,and 0.46%-0.90% for EV.More over 176 samples collected from children with suspected HFMD were detected by triplex Taqman probes real-time RTPCR and real-time RT-PCR.The results showed 97.2% (171/176)agreement and 0.94 Kappa value with high concordance. Conclusions The triplex Taqman probes real-time RT-PCR detecting EV71,CA16 and EV simultaneously has been established successfully. The assay, with high sensitivity and specificity, provide good basis for the rapid clinical diagnosis of EV71,CA16 and EV and open up broad prospects for clinical and relevant researches.

Objective:To investigate the effect of short term and intensive insulin pump on the blood lipid and glucose levels of patients with type 2 diabetes.Methods:76 patients with type 2 diabetes were selected and divided into two groups according to different administration. The control group (38 cases) was given routine insulin aspart treatment. The observation group (38 cases) was treated by insulin pump. The blood and lipid indexes and ADL score before treatment,at 1 week and 2 weeks after treatment were compared between two groups. Results:After treatment, the levels of FPG,HbAlc,TC,TG and LDL-C were lower than those before treatment and the level of HDL-C was increased in both groups. The levels ofFPG,HbAlc,TC,TG,LDL-C of observation group were lower than those of the control group, and the level of HDL-C of observation group was higher than that of control group (P<0.05).At 1 week,2 weeks after treatment, the ADL scores of both groups were significantly improved, and the ADL score of observation group was higher than that of the control group (P<0.05).Conclusion:Short term intensive insulin pump could significantly improve the blood glucose and lipid metabolism disorders in patients with type 2 diabetes, and improve the ability of daily life.
