1.Influence and curative effect of inhalation of formoterol/budesonide on matrix metalloproteinase of plasma-2 and 9 for asthmatic patients

Objective To discuss the influence and curative effect observation of inhalation of formoterol/budesonide on matrix metalloproteinase of plasma(MMP)-2 and 9 for asthmatic patients (AAPs).Methods According to the digital table,68 cases of AAPs in acute stage of attack were randomly divided into observation group and control group.The patients in two groups were given routine medical treatments like anti-infection,antispasmodic and relieving asthma,relieving coμgh and reducing sputum and etc.In addition,the patients in observation group were given the inhalation of formoterol/budesonide with 4.5μg/160μg,two times daily for 2 weeks.Except for formoterol/budesonide,the medical treatments given to patients in control group were the same with those in observation group.The changes of MMP-2 and 9 levels in blood serum before and after 2 weeks' medical treatment were observed and compared,and proceed with the curative and safety observation.Results After 2 weeks' medical treatment,the MMP-2 and 9 in blood serum both were declined sharply(t =2.87,2.97,2.21,2.31,P < 0.01 or P < 0.05),and the compared with control group,the decrease value(t =2.45,2.37,all P < 0.05) was obviously higher.Meanwhile,the total clinical effective rate in observation group (94.12%) was obviously higher than that in control group (70.59%)(x2 =6.48,P < 0.05) ; The numbers of untoward effect occurrence in the medical treatment period were 3 cases in control group and 5 cases in observation group,which both had light symptom and did not appeared serious untoward effects.The comparison result of untoward effect between two groups had no obvious statistical difference(x2 =0.14,P > 0.05).Conclusion formoterol/budesonide has reliable medical treatment effect and high security to asthmatic,whose mechanism of action has effect on MMP-2 and 9 level adjustment,correction of degradation of extracellular matrix and dysequilibrium,and chronic airway inhibition and remodeling.
2.A study on the overall management strategy of mechanical ventilations in the hospital

Objective:Now the mechanical ventilation has become the essential emergency or therapy equipment in the all sizes of hospitals. However with the widely application of ventilators, they are still the kind of high risk of medical instruments. In order to reduce various equipment failures, an overall management strategy based on traceability of two-dimension code is constructed in this article, which will provide a new direction for medical equipment management.Methods: Using the popular application of WiFi network and creating the only identified two-dimension code, the overall management strategy is established for the ventilators.Results: The centralized and decentralized management model is adopted in the preservation of ventilators. And all the operators of ventilators will be supported technology service about equipment failures and setting parameters by the respiratory department and engineering department. In addition to this, the two departments should organize the staff to learn the operation of ventilators in the hospital. All of those compose the structure of overall management.Conclusion: With the management of this method, the efficiency of ventilators is improved, and the rate of machine failures is dropped. The difficulty about technology service caused by space can be solved in time. Finally, the professional ability of the staff in hospital will be improved with the communication among the different departments.
3.Gene expression of spleen lymphocytes induced by gp96-peptide complexes and its anti-tumor effect in vitro

4.Expiry Date of Sterile Articles Packed by 4 Different Materials and Preserved in Different Conditions after Pressure Steam Sterilizing: An Observation

OBJECTIVE To investigate the expiry date of sterile articles packed by 4 different materials and preserved in different conditions after pressure steam sterilizing. METHODS The bacteria growth of the materials sterilized by pressure steam sterilizing method and stored in the supply department,treatment room,dressing room,nursing cabinet and ambulance vehicle was observed. RESULTS The axenic periods of the sterile materials in the supply department preserved by methods A,B,C,D were 14 days,14 days,7 months and 8 months,respectively in summer;while the sterile periods of the materials in the treatment room,dressing room,nursing cabinet and ambulance vehicle preserved by methods A,B,C,D were 11 days,11days,6 months and 7 months,respectively. CONCLUSIONS Management of expiry date of sterile materials is an important measure to guarantee safe use of sterile materials and prevent against nosocomial infection.

Objective:Mechanical ventilation has been the regular equipment or the therapy equipment in the hospital. In order to establish a safe condition of using ventilator, not only standard operations are required, but also high quality of ventilators should be ensured. This article will introduce a kind of ventilator detection system made in Switzerland, especially the portable device which supports the technology to detect ventilator at any time and every place. These detection equipments provide the protection of high risk medical equipment. Methods: According to the requirement of mechanical ventilation detection and the instruction of imtmedical ventilator detection system, the application methods of imtmedical detection system were introduced. Results: Compared the difference with other ventilator detectors, the priority, portability, stability and accuracy of the Citrex H4 detection were understood. Conclusion:The Citrex H4 ventilator detector is satisfactory for quality testing of ventilator, ion evaluate can make the output performance parameters more accurate and the quality higher.of the ventilators.

7.The application value of indocyanine green combined with methylene blue in sentinel lymph node biopsy of breast cancer

Objective:To evaluate the potential value of indocyanine green combined with methylene blue and methylene blue alone in the detection of sentinel lymph nodes (SLN) in breast cancer.Methods:The clinical data of 118 patients with early breast cancer from November 2016 to October 2018 in Dalian Central Hospital Affiliated to Dalian Medical University were retrospectively analyzed. Among them, 62 patients underwent SLN biopsy under methylene blue (methylene blue group, 160 SLN), and 56 patients underwent SLN biopsy under indocyanine green combined with methylene blue (combined group, 190 SLN). After SLN biopsy, patients underwent grade Ⅰ and Ⅱ axillary lymph node dissection.Results:There was no significant difference in the positive lymph node rate and accuracy rate between 2 groups ( P>0.05); the SLN dyeing rate and SLN biopsy positive rate in combined group were significantly higher than those in methylene blue group: 97.9% (186/190) vs. 93.1% (149/160) and 28.4% (54/190) vs. 18.1% (29/160), and there were statistical differences ( P<0.05); the false negative rate in combined group was lower than that in methylene blue group: 5.3% (3/57) vs. 14.7% (5/34), but there was no statistical difference ( P>0.05). Conclusions:In breast cancer patients, the dual -tracing method of indocyanine green combined with methylene blue is better than using methylene blue alone in identifying SLN and predicting the status of axillary lymph nodes, and it has a tendency to reduce the false negative rate.
8.Magnetic field numerical calculation and analysis for magnetic coupling of centrifugal blood pump for extracorporeal circulation.

This paper mainly studies the driving system of centrifugal blood pump for extracorporeal circulation, with the core being disc magnetic coupling. Structure parameters of disc magnetic coupling are related to the ability of transferring magnetic torque. Therefore, it is necessary to carry out disc magnetic coupling permanent magnet pole number (n), air gap length (L(g)), permanent magnet thickness (L(m)), permanent magnet body inside diameter (R(i)) and outside diameter (R(o)), etc. thoroughly. This paper adopts the three-dimensional static magnetic field edge element method of Ansys for numerical calculation, and analyses the relations of magnetic coupling each parameter to transmission magnetic torque. It provides a good theory basis and calculation method for further optimization of the disc magnetic coupling.

9.Bioinformatic analysis of antibody repertoire development in response to influenza vaccination

Objective To analyze the immunogenomic characteristics of antibody repertoire in re-sponse to influenza vaccine in order to provide a theoretical basis for further development of antibody. Meth-ods Based on a time-series immunoglobulin heavy chain ( IGH) repertoire sequencing dataset, we analyzed the immunogenomic characteristics of antibody repertoire in response to trivalent influenza vaccine ( TIV ) from three aspects which included the features in complementarity-determining region 3 ( CDR3 ) , antibody mutation and VDJ usage. Results The frequency of antibody mutation increased significantly upon vaccina-tion. Analysis of the CDR3 region indicated that polar and aromatic amino acids had a higher preference. The length of CDR3 region in naive B cells followed a normal distribution, while specific CDR3 sequences with 15 to 18 amino acids in length occupied a dominant position after vaccination. In addition, the VDJ us-age altered obviously and IGHV3-7-derived antibody had a significant response to the vaccine. Response in-tensity reached the peak on day 7 and gradually weakened over time. Conclusion Antibody repertoire evolves dynamically to express specific antibody upon vaccination and the characteristics of immune responses at sequence level could be used to evaluate their effectiveness.
10.Double primary carcinoma of endometrial carcinoma and colorec-tal carcinoma: retrospective analysis of 34 cases and discussion of its relationship with Lynch syndrome

Objective:To approach the clinicopathologic characteristics, treatment modalities, and prognosis of the patients with double primary carcinoma of endometrial carcinoma and colorectal carcinoma and analyze the relationship between this disease and Lynch syndrome. Methods:The clinicopathologic and follow-up data of 34 cases with double primary carcinoma of endometrial carci-noma and colorectal carcinoma treated in Cancer Hospital of Chinese Academy of Medical Sciences were reviewed. Results:The medi-an age of the 34 patients was 51.5 years old (ranging from 39 to 76). Twenty-two of the total cases (22/34, 64.7%) had tumor family his-tory. The pathologic results indicated that 79.4%(27/34) was endometrioid adenocarcinoma. In the 34 cases, 33 were treated by surgical procedure. Of the 33 cases undergoing surgery, 17 patients with endometrial carcinoma and 19 with colorectal carcinoma received fur-ther adjuvant treatment of chemotherapy and/or radiotherapy. The 2-and 5-year survival rates were 84.3%and 63.1%in the 34 cases, respectively. Conclusion:The age of onset is earlier in the patients with double primary carcinoma than in those with sporadic colorec-tal carcinoma. Some cases have cancer family history, and their survival rates are similar to those of the patients with sporadic colorec-tum carcinoma. Diagnosis of some patients with double primary carcinoma may be in line with Lynch syndrome.
