Objective:To approach the clinicopathologic characteristics, treatment modalities, and prognosis of the patients with double primary carcinoma of endometrial carcinoma and colorectal carcinoma and analyze the relationship between this disease and Lynch syndrome. Methods:The clinicopathologic and follow-up data of 34 cases with double primary carcinoma of endometrial carci-noma and colorectal carcinoma treated in Cancer Hospital of Chinese Academy of Medical Sciences were reviewed. Results:The medi-an age of the 34 patients was 51.5 years old (ranging from 39 to 76). Twenty-two of the total cases (22/34, 64.7%) had tumor family his-tory. The pathologic results indicated that 79.4%(27/34) was endometrioid adenocarcinoma. In the 34 cases, 33 were treated by surgical procedure. Of the 33 cases undergoing surgery, 17 patients with endometrial carcinoma and 19 with colorectal carcinoma received fur-ther adjuvant treatment of chemotherapy and/or radiotherapy. The 2-and 5-year survival rates were 84.3%and 63.1%in the 34 cases, respectively. Conclusion:The age of onset is earlier in the patients with double primary carcinoma than in those with sporadic colorec-tal carcinoma. Some cases have cancer family history, and their survival rates are similar to those of the patients with sporadic colorec-tum carcinoma. Diagnosis of some patients with double primary carcinoma may be in line with Lynch syndrome.

Objective To compare the myocardial enzyme spectrum of native mountain yaks ( Bos grunniens) , alien mountain yellow cattle ( Bos taurus domestica) and low altitude yellow cattle breeding in Qinghai plateau, explove the mechanism that the native mountain yaks adapt to the plateau environment.Methods The samples were treated by cervical bleeding to death, and whole blood was collected.Then serum were prepared from whole blood.The myocardial enzymes including aspartate amino transferase, creatine kinase, creatinine kinase-MB isoenzyme, lactate dehydrogenase, α-hydroxybutyrate dehydrogenase and.Results In the native mountain yaks and in the alien mountain yellow cattle, levels of AST, CK, CK-MB were significantly higher than those in the low altitude yellow cattle (P<0.05);compared with the alien mountain yellow cattle, levels of AST,CK were significantly higher than those in the native mountain yaks (P<0.05),CK-MB,LDH and HBDH were lower(P<0.05).The results suggested that under the high altitude and hypoxia environment, myocardial cell injury occurred in the alien mountain yellow cattle, thus various enzymes penetrated through the cells into the blood circulation, lead to the higher serum enzymatic levels.Conclusion The phenomenon indicates that the alien mountain yellow cattle is in a state of high altitude acclimatization.But the native mountain yaks are well adapte to the high altitude and hypoxia environment.

Objective To assess the effect of altitude hypoxia on the elderly patients with coronary artery disease (CAD). Methods Three subject groups were surveyed during their train trip on the highest railroad--the Qinghai-Tibet Railway: 22 elderly individuals with documented CAD, 20 healthy elderly controls, and 20 healthy young controls, all of whom from Beijing near the sea level (76 m). Survey questions addressed clinical features of their healthy conditions and aspects of their coronary disease. The baseline study was performed at Xining at an altitude of 2261 m, and then during acute exposure to altitudes of 2808 m, 4768m, 5072 m and 4257 m by train for 24 hours. Resting pulse rate, blood pressure, oxygen saturation, electrocardiograph (ECG), and cardiac work estimated by the heart rate-blood pressure double product were obtained five times in each subject at different altitudes. Results On arrival to altitudes between 4768 m and 5072 m, the older passengers, especially those with preexisting coronary disease, had higher HR, higher BP, and lower SaO2, as well as more frequent abnormalities on ECG, as compared to the younger healthy subjects. As compared with the healthy elderly controls, incomplete right bundle branch block, left ventricular hypertrophy, and ST segment depression were more frequently seen in the elderly coronary patients (P<0.01). Cardiac work in group 1 was increased by 13% 12 hours after arrival to altitudes between 2808 m and 5072 m. Oxygen saturation decreased significantly with the altitude increasing by train ascent but improved after inhalation of oxygen. Most of the older subjects tolerated their sojourn at high altitude well except one who developed angina repeatedly with a significant ST segment depression. Conclusions Coronary events and ECG signs of myocardial ischemia are rare in elderly individuals with CAD who travel from sea level to moderate altitudes of 1500m to 2800 m. Patients with CAD who are well compensated at sea level generally tolerate this moderate altitude well. However, it would be prudent for patients with CAD going to altitude above 3000 m. The patients should consult their physician before undertaking a trip to such altitude.

Objective To explore the risk factors for patients with massive hemoptysis in tuberculosis and to provide a strategy for clinical treatment for tuberculosis massive hemoptysis (TMH). Methods Chi-square test and multivariate logistics analysis were applied to analyze the medical data of 241 cases of TMH. Results Chi-squared test showed that eleven factors were found to be significantly correlated with TMH. Longer disease course (≥3 months), lung lesions range ≥ 3 lung fields, pulmonary tuberculosis cavity, a higher smoking index (≥400 cigarettes per year) and clinical treatment were risk factors for TMH. Patients aged 45 years or older accompanied with bronchiectasia, pulmonary fungal infection, diabetes or hepatopathy had higher probabilities of developing massive hemoptysis. Multivariate analysis showed lung lesions range over 3 lung fields (OR = 2.447,P = 0.015), pulmonary tuberculosis cavity (OR = 2.486, P = 0.004), bronchial asthma (OR =3.192,P = 0.002), pulmonary fungal infection (OR = 3.896, P = 0.005) and hepatopathy (OR = 3.101, P =0.006) were final risk factors for TMH. Conclusion Multiple factors contributed to patients with massive hemoptysis in tuberculosis. Lung lesions range over 3 lung fields, pulmonary tuberculous cavities, bronchial asthma, lung fungal infection and hepatopathy might be the independent risk factors for TMH.

BACKGROUND:Sodium hyaluronate is a polysaccharide polymer biomaterial, which is considered to have a certain effect to repair the cartilage surface, reduce the release of inflammatory mediators, and promote meniscal repair. OBJECTIVE:To observe the therapeutic effect of Fast-Fix combined with sodium hyaluronate in meniscal repair under arthroscope. METHODS:Eighty-six patients with meniscus injury admitted at the Department of Orthopedics, CNPC Central Hospital, from March 2008 to March 2014 were enrol ed in the study. The average age was 25.5 years, and the average duration of disease was 1.5 months. The main clinical manifestations included knee joint pain and swel ing after exercise and positive McMurray signs. The MRI results showed meniscus tear or degeneration. Arthroscopic repairing surgeries were performed with Fast-Fix combined with sodium hyaluronate injection (2.5 mL at the end of surgery), and then sodium hyaluronate was injected intraarticularly at 1, 2, 3 weeks after surgery. Each patient was assessed with Lysholm knee joint score system before and after operation. RESULTS AND CONCLUSION:Eighty-one patients were fol owed up for an average of 1.5 years and five patients were lost to fol ow-up. Joint locking, significant tenderness and movement disorder symptoms disappeared in 79 of 81 patients, and meniscal healing was shown on MRI review within 1 year after surgery;the Lysholm score was higher than 75 points, and the excel ent rate was 98%. Only the remained two patients felt swel ing and tenderness and the movement was slightly restricted when going down or standing up. The Fast-Fix combined with sodium hyaluronate in meniscal repair under arthroscope has good effect and good function of knee joint.

Rat blood sam-ples were collected at different intervals after intrave-nous injection of a single dose of DL0805-1 , and the concentration of DL0805-1 in rat plasma were deter-mined by HPLC method for estimating pharmacokinetic parameters.Results Afterintravenousinjectionof DL0805-1 in rats, prototype and its metabolite were detected in plasma. T1/2 of DL0805-1=(2. 34 ± 1. 42) h, Cmax=(3. 51 ± 0. 44) mg·L-1, T1/2 of metabolite of DL0805-1 = ( 1. 27 ± 0. 45 ) h, Cmax = ( 3. 55 ± 0.22)mg·L-1.Conclusion Theseresultssuggest that DL0805-1 may be metabolized into another sub-stance in vivo and play biological functions. The meth-od is sensitive, simple, and accurate, and can be used for the determination of DL0805-1 in rat plasma and pharmacokinetic studies.

Objective: Reveal the global expression profile of serum exosomal proteins of Crouzon syndrome patients. Methods: We isolated microvesicles from serum of Crouzon children with a C342Y mutation in FGFR2 by ultracentrifugation, which were further characterized by electron microscopy and immunoblotting. The protein profiling in normal subjects and Crouzon patients was systematically compared by iTRAQ proteomic analysis. Results: The result demonstrated that microvesicles were between 30—100 nm in diameter, round shape with cup-like concavity and expressed exosomal marker tumor susceptibility gene (TSG) 101 and flotillin (Flot) 1. We identified a total number of 62 proteins, among which 22 proteins overlap with ExoCarta database and were different between the Crouzon patient and the normal subject. The Ingenuity Pathway Analysis showed that the functions of these proteins are mostly involved in Developmental Disorder, Hereditary Disorder, Skeletal and Muscular Disorders, which are all related to the clinical manifestations of Crouzon syndrome. In addition, the proteins were focused on the network of "Organismal Injury and Abnormalities, Hematological System Development and Function, Cell-To-Cell Signaling and Interaction" . The central protein FN1 was presented as the key protein in the network. Conclusions Our data demonstrated that serum exosomes harbor informative proteins and FN1 was selected as a potential candidate for its role in promoting osteoblast adhesion, proliferation and mineralization.

Objective To investigate the correlation between the expression levels of miRNAs and the pathogenesis of diabetes mellitus.Methods A total of 120 patients with type 2 diabetes recruited from October 2014 to October 2016 and 40 healthy individuals as a control group were included in this study,and diabetic patients were further divided into newly diagnosed group(n=40),prediabetes group(n =40),and susceptibility group (n =40).The fasting blood of four groups were collected,and real-time RT-PCR was applied to measure the expression levels of miRNAs including miR-9,miR-124a,miR-375,miR-29a,miR-126,miR-197,miR-233,and miR-320.The relationship between the pathogenesis of disease and miRNAs expression levels was analyzed.Results The levels of miRNA in the three diabetes groups were obviously higher than in the control group(P＜ 0.05).The miRNA expression level showed a certain trend in diabetic patients.The higher levels of miR-9,miR-124a,miR-375,and miR-29a were observed in the three diabetes groups,while the levels of miR-126,miR-197,miR-233,and miR-320 were decreased in these three groups.The expression levels of miR-9,miR 124a,miR-375,and miR-29a of miRNAs were correlated with the onset of diabetes(RR =5.96,6.32,5.24.5.69,all P＜ 0.05).Conclusions The level of miRNA is associated with the onset of diabetes that suggests it may be used to predict the occurrence and development of diabetes.

To investigate the effect of preoperative uses of levosimendan in patients with low LVEF(≤0. 40) undergoing off-pump coronary artery bypass grafting(OPCABG). Methods 63 patients with low LVEF coronary artery diseases were prospectively enrolled during June 2015 to May 2018, randomized to levosimendan-treated group(n =32) and control group(n =31)preoperatively. Patients in levosimendan-treated group underwent levosimendan intravenous infusion 24 h before OPCABG. All patients underwent OPCABG at normal temperature. Internal mammary artery and great saphenous vein were used as bypass materials. Hemodynamics and cardiac function were compared between the two groups after OPCABG. Results Compared to control group, the systemic vascular resistance(SVR) and central venous pressure(CVP) of levosimendan- treated group were decreased significantly and the CO and LVEF increased significantly at 12h and 24h after surgery(P < 0. 05). The heart rate and mean artery pressure had no statistical difference between the two groups(P >0. 05). The dosage and administration time of vasoactive agents in levosimendan-treated group were significantly smaller than those in control group (P <0. 05). The time of mechanical ventilation, the ICU stay length, the BNP level in the first two days after operation, and the incidence of new atrial fibrillation were less than those of the control group(P <0. 05). The perioperative intra-aortic balloon pump implantation rate in levosimendan-treated group was significantly lower than that of the control group(P < 0. 05). Conclusion Preoperative use of levosimendan can significantly improve the hemodynamic and cardiac function status of patients with low LVEF after OPCABG, shorten the time of ventilator assisted and ICU hospitalization, and reduce the incidence of adverse events.
