1.Metrology analysis of the papers published from 2009 to 2013 of an anonymous hospital

Objective The paper proposed suggestions and measures for hospital scientific research management by statistical analyzing all the papers published from 2009 to 2013.Method By referring to research database of this hospital and by using Microsoft Excel,this research calculated the frequency distributions of different variables including quantity,publisher,disciplinary subject,and the author title as well as core author.Result 772 papers were published from 2009 to 2013,mainly in local periodicals.75.52% of the title of first author are high-level or intermediate-level;86 core authors published 378 papers,account for 48.96% of the total amount.Conclusion The number of research papers is increasing year by year.but the quantity and quality are still low.The research administorsshould take measures to encourage researchers to publish quality papers in periodicals with higher standards.
2.The effect of MDR1 (P-gp) and ABCG2 on the drug resistance in Hep 2 cells.

OBJECTIVE:
To study the effect of MDR1 (P-gp) and ABCG2 on the drug resistance in Hep 2 cells.
METHOD:
Flow cytometry was used to detect the variations of the antitumor drugs accumulation and discharging, and activity variations when MDR1 and ABCG2 inhibitors were used in Hep-2.
RESULT:
The accumulation and discharging of mitoxantrone was significantly higher than the control group when ABCG2 inhibitor FTC was used in Hep-2 (P<0. 05). In contrast, P-gp did not appear similar case; To the mitoxantrone and cisplatin, there was no statistical correlation about activity of Hep-2 between P-gp or ABCG2 antagonist and the control; To the doxorubicin, combining FTC and P-gp, the activity of Hep-2 was higher than the control and difference was significant (P<. 05), In contrast, FTC and P-gp did not appear similar case when used alone; To the 5-FU, when PGP used, the activity of Hep-2 was higher than that in the control and difference was significant (P<0. 05), In con- trast, FTC and FTC+P-gp did not appear similar case; To the paclitaxel, when P-gp or FTC+P-gp used, the activity of Hep-2 was higher than that in the control and difference was significant(P<0. 05).
CONCLUSION
ABCG2 may lead to drug resistance mainly by changing the ability of cell in accumulating and discharging chemotherapy drugs. P-gp has other way. P-gp and ABCG2 play different roles in different drug resistance.

3.Analysis of distribution and drug resistance of Enterococcus faecium

Objective To investigate the distribution and drug resistance ofEnterococcus faecium,provide evidence for clinical rational use of antimicrobial agents.Methods Retrospective analysis of drug resistance of Enterococcus faecium,summarize the specimen type,distribution of departments and so on,to explore the trend of drug resistance.Results Enterococcus faecium to vancomycin resistance rate from 3.54% (4/113) in 2010,rose to 8.45% (18/213) in 2013.One strains of hnezolid resistant Enterococcus faecium 717 was found,the minimum inhibitory concentration of 7.9 mg/L,not tigecycline resistance in Enterococcus faecium found.Conclusion Vancomycin resistant Enterococcus faecium for linezolid and tigecycline susceptibility is still high,the two kinds of antimicrobial agents can be used for the treatment of vancomycin resistant Enterococcus faecium.
4.Comparisons of DNA damages in physics process caused by protons with different energies

Objective To compare the difference among DNA damages in physics processes caused by protons with different energies around Bragg peak.Methods By constructing the nucleus and DNA model using Geant4 and simulating the transportation processes of protons with different energies around Bragg peak in nucleus model based on Geant4-DNA,the information of interaction points were recorded.16% of them were picked randomly to use as the points at which protons interact with DNA.After finding out the points which broke the DNA and writing their information to new files,these new files were treated with density-based spatial clustering of applications with noise (DBSCAN),so as to analyze and calculate the difference of DNA damages caused by protons with different energies.Results With the protons with energies from 20.0 to 0.6 MeV,the numbers of damage points and damage clusters increased from 49.86 to 549.88 and from 2.92 to 82.46,respectively;the numbers of clusters with different sizes had a remarkable increase,and the number of clusters with size not less than 5 increased by 400 times.The average cluster size increased slightly.Simple single strand breaks (SSSBs),complex single strand breaks (CSSBs),simple double strand breaks (SDSBs) and complex double strand breaks (CDSBs) of DNA increased by about 8,26,24 and 64 times,respectively.The proportion of single strand breaks (SSBs) decreased from 96.69% to 89.37%,and the double strand breaks (DSBs) increased from 3.31% to 10.63%.Conclusions The lower proton energy would result in the more complex damage to DNA and the harder repair of DNA damage.
5.Research of the medication rules of ankylosing spondylitis based on the famous doctors' medical records

Objective Applyingthe traditional Chinese medical knowledge service and sharing system, to explore the medication rules of ankylosing spondylitis in the medical records.Methods The ankylosing spondylitis medical records in the traditional chinese medical knowledge service and sharing system has been searched and 149 prescriptions were identified. The medication frequency, association rules, herbal combination and the cluster analyses were explored.Results Among the 149 prescriptions with 210 drugs, the use of high frequency drugs wereGouji, Baishao, Danggui, Duzhong, Weilingxian, and other medication whose frequency was more than 20 times were good at tonifying liver and kidney, activating blood circulation, clearing heat and promoting diuresis, dispelling cold and removing dampness. The high frequency of drug combinations included Sangjisheng and Gouji, Duzhong and Gouji, Baishao and Gouji, Guizhi and Baishao, Gancao and Danggui.Conclusions The treatment of ankylosing spondylitis with Chinese medicine is mainly based on tonifying liver and kidney, supplemented with drugs for warming Kidney Yang, dispelling cold and dehumidification, promoting blood circulation and removing blood stasis, clearing heat and promoting diuresis, dispelling wind and cold.
6.Efficacy of anatomical locking plate combined with anchor nail in the treatment of Neer Ⅱ distal clavicle fracture

Objective To observe the efficacy of anatomical locking plate combined with anchor nail internal fixation and clavicular hook plate in the treatment of Neer Ⅱ distal clavicle fracture and their impact on shoulder function.Methods Eighty patients with Neer Ⅱ distal clavicular fracture treated in the Eighth People's Hospital in Shanghai from January 2015 to December 2016 were divided into observation group and control group,each group 40 cases.The observation group was treated with anatomical locking plate combined with anchor nail,the control group was treated with clavicular hook plate.The efficacy,operation time,intraoperative blood loss,fracture healing time,postoperative hospital stay,shoulder function score and incidence of complication were observed in the two groups.Results The excellent rate was in the observation group was higher than that in the control group (97.50%(39/40) vs.80.00%(31/40),χ2=2.477,P<0.01).The operation time,intraoperative blood loss and postoperative hospital stay in the observation group and the control group were (53.36±4.69) min vs.(54.19±5.13) min,(69.65±7.17) ml vs.(70.38±8.29) ml,(7.39±1.64) d vs.(7.42±1.97) d,the differences between the two groups had no statistical significance (P>0.05);the fracture healing time in the observation group was longer than that of the control group ((23.65±2.19) w vs.(26.9±3.46) w,P<0.01).The shoulder joint Constant-Murley score in the observation group was (86.02±5.19) points,control group (68.46± 6.94) points,the difference was statistically significant (P<0.01).Observation group showed 2 cases of pain around acromion,the incidence rate of complications was 5.00%(2/40);7 cases in the control group had pain around acromion,2 cases suffered from impingement syndrome and 1 cases had delayed union,the incidence rate of complications was 25.00% (10/40).Conclusion Anatomical locking plate combined with anchor nail internal fixation is effective in the treatment of Neer II distal clavicle fracture.It has the advantages of fast fracture healing,good functional recovery of shoulder joint and less complications.

Myelodysplastic syndromes (MDS) are a heterogenous group of hematologic malignancies characterized by clonal expansion of BM myeloid cells with impaired differentiation. Of particular interest mutations is the recent recognition that genes involved in the regulation of histone function (EZH2, ASXL1,and UTX) and DNA methylation (DNMT3A,IDH 1/IDH2,TET2) are recurrently mutated in MDS,providing an important link between genetic and epigenetic alterations in this disease. Ongoing analysis of the seminal AZA-001study has taught many important lessons in the use of DNA methyltransferase (DNMT) inhibitors.Improved survival in patients with high-risk MDS treated with azacitidine extends to patients with any International Working Group-defined hematologic response.New information on the impact of DNMT inhibitors on the immune system and on stem cells will likely lead to novel uses of these drugs in MDS and other hematologic and nonhematologic malignancies. The immunomodulating drug thalidomide and its derivative lenalidomide have been used in the treatment of MDS,principally in lower-risk MDS.
9.Comparison of conductivity-water content curve and visual methods for ascertaintation of the critical water content of O/W microemulsions formation.

This study is to screen 23 blank O/W type microemulsion (ME) samples, that is 15 samples from our laboratory, and 8 samples from literature; compare the conductivity-water content curve (CWCC) method and visual method in determining the critical water content during O/W type MEs' formation, to analyze the deficiency and the feasibility of visual method and to exploxe scientific meanings of CWCC method in judging the critical water content of O/W type MEs during formation. The results show that there is a significant difference between the theoretical feasible CWCC method and visual method in determining the critical water content (P<0.001), and the results judged by conductivity is higher than that by eye-based water content. Therefore, this article firmly confirmed the shortcomings of visual method and suggested that the eye-base "critical water content" may falls into continuous ME stage during O/W MEs' formation. Further more, the CWCC method has theoretical feasibility and scientific meanings in determining the critical water content of O/W type MEs during formation.
10.Clinical effect of stereotactic body radiotherapy in treatment of patients with small hepatocellular carcinoma and related prognostic factors

ObjectiveTo investigate the clinical effect and safety of stereotactic body radiotherapy (SBRT) in patients with small hepatocellular carcinoma and related prognostic factors. MethodsA retrospective analysis was performed for the clinical data of 64 patients with small hepatocellular carcinoma who underwent SBRT in 302 Hospital of PLA from March 2011 to October 2014. The patients were followed up for tumor control and adverse events after treatment. The Kaplan-Meier method was used to calculate survival rates, log-rank test and the Cox proportional hazards model was used to analyze the influencing factors for survival. ResultsA total of 64 patients with 72 lesions were enrolled in this study. The patients were followed up for 4-60 months, with a median follow-up time of 44 months. Of all lesions, 44 (61.1%) achieved complete response, 20 (27.8%) achieved partial response, 5 (6.9%) achieved a stable state, and 3 (4.2%) had progression. The objective response rate was 88.9%. The 1-, 3-, and 5-year local control rates were 94.4%, 88.1%, and 78.7%, respectively, the 1-, 3-, and 5-year cumulative survival rates were 95.3%, 71.6% and 55.9%, respectively, and the 1-, 3-, and 5-year progression-free survival rates were 62.3%, 33.5%, and 10.8%, respectively. Most of the adverse events were grade 1-2 events. One patient experienced a grade 4 elevated bilirubin. In addition, about one fourth of the patients experienced grade 3 leukopenia and/or thrombocytopenia, which increased to the baseline level at 1 year after treatment. The multivariate analysis showed that albumin-bilirubin (ALBI) grade and number of tumors were independent risk factors for the cumulative survival rate of patients with small hepatocellular carcinoma after SBRT. ConclusionSBRT is safe and effective in the treatment of small hepatocellular carcinoma, and ALBI grade and number of tumors are associated with the survival rate of patients after treatment.
