1.Determination of iodine content of iodine-rich herbal and analysis of effective factors

Objective To determine the iodine content of iodine-rich herbal and analyze the effective factors.Methods Iodine contents of seaweed at different alkali ashing temperature,such as 200℃,300℃,400℃,500℃and 600℃were determined,then the optimum ashing temperature were selected.Mixed ashing auxiliary including potassium carbonate,zinc sulfate,sodium chloride,potassium chlorate were added in samples of iodine-rich herbal,and ashed at the optimum ashing temperature for 2h.The ash was dissolved in deionized water.Iodine content of iodine-rich herbal was determined with As3+-Ce4+catalytic spectrophotometry.Results The optimum ashing temperature was 300℃;the standard curve regression equation:A =-6.91 ×10 -3C +1.0306,r =0.999 3;iodine content of seaweed,Kunbu and Haidai was 0.829 mg/g,0.343 mg/g and 0.864 mg/g respectively;recovery rates before ashing of seaweed,Haidai and Kunbu were 83.19%,81.47% and 80.32% respectively;91.24%,93.43% and 95.86% after ashing.Conclusion Iodine is rich in iodine-rich herbal,the optimum ashing temperature is 300 ℃,and As3+ -Ce4+ catalytic spectrophotometry is stable and accurate as a determination method of iodine content of herbal.

3.Differences of glucose metabolism between adipose tissue and skeletal muscle in the early stage of catch-up growth in rats and its mechanism

Objective To observe the glucose utilization in adipose tissue and skeletal muscle during catch-up growth, and to explore the mechanism of catch-up growth of adipose tissue. Methods Male Wistar rats were randomly divide into normal control group(NC)and catch-up group(RN). Rats in RN group received 50%of food consumed by NC group for 4 weeks, then were re-fed spontaneously as the rats in NC group. In the end of the fifth week(NC1 group and RN1 group)and the sixth week(NC2 group and RN2 group), the experiment was performed. [3]-2-deoxy-glucose was used for detecting the glucose uptake rate. RT-PCR and Western-blot were used for detecting the levels of mRNA and membrane protein of glucose transporter-4(Glut4). Results The glucose uptake rates in adipose tissue and skeletal muscle of RN1 group increased by 189.6%(P<0.01)and reduced by 36.5%(P<0.05)respectively, as compared with NC1 group. After 2 weeks of catch-up growth, the glucose uptake rates in adipose tissue of RN2 group increased by 157.3%(P<0.01)and decreased by 41.5%(P<0.05) in skeletal muscle as compared with NC2 group. However. no significant difference in Glut4 mRNA levels in muscle or in adipose tissue between NC and RN groups were found. The membrane protein of GIut4 after insulin-stimulating in RN1 group and RN2 group reduced by about 46.5%(P<0.01)and 32.1%(P<0.05)in muscle and increased by 116.5%(P<0.01)and 89.9%(P<0.01)in adipose tissue respectively. Conclusion There exists the redistribution of glucose from skeletal muscle to adipose tissue during the early stage of catch-up growth, which results in the catch-up growth of adipose tissue. This change is induced by the tissue-specific alteration of insulin-stimulated Glut4 protein translocation.
4.Effects of adult catch-up growth on insulin sensitivity and stress in rats

ObjectiveTo investigate the effects of adult catch-up growth on insulin sensitivity and stress in rats, as well as the probable mechanism of insulin resistance. MethodsMale Sprague-Dawley rats were divided into 6 groups:caloric restriction group ( R4, caloric restriction for 4 weeks) and normal controls for 4 weeks ( NC4 ) ; catchup growth group refed with normal chow( RN4, refeeding for 4 weeks after caloric restriction for 4 weeks), catch-up growth group refed with high-fat diet( RH4, refeeding for 4 weeks after caloric restriction for 4 weeks ), normal chow (NC8) or high-fat diet( HF8 ) controls for 8 weeks. The animal model of catch-up growth was devoloped by way of refeeding after caloric restriction as scheduled. The glucose infusion rate( GIR ), 2-deoxyglucose uptake and insulinsitmulated insulin signaling in skeletal muscle during hyperinsulinemic-euglycemic clamp, plasma corticosterone, and 11β-hydroxysteroid dehydrogenase type 1 ( 11β-HSD1 ) mRNA expression level in skeletal muscle were determined.ResultsAfter caloric restriction for 4 weeks, plasma corticosterone and 1 1 β-HSD1 mRNA expression in skeletal muscle were significantly higher in R4 group compared with NC4 group( both P<0. 05 ), but there were no differences in 2-deoxyglucose uptake and Ser473 phosphorylation of Akt in skeletal muscle between two groups. The plasma corticosterone and 11β-HSD1 mRNA expression in skeletal muscle in RN4 group were significantly higher than those in NC8 group, and were higher in RH4 group than those in NC8 and HF8 groups; while the 2-deoxyglucose uptake and insulin-stimulated Ser473 phosphorylation of Akt in skeletal muscle during the clamp in RN4 were remarkably lower than those in NC8 group, and were lower in RH4 than those in NC8, HF8, and RN4 groups (all P < 0. 05 ).ConctusionsCatch-up growth rats refed with normal chow or high-fat diet are characterized by significant insulin resistance and stress in the whole body and skeletal muscle. These changes are more evident in catch-up growth rats refed with high-fat diet. The interaction of increased stress and diet might be of utmost importance in the etiology of insulin resistance in catch-up growth animals.

6.Imaging manifestations of postparaplegic myositis ossificans

Objective To observe imaging manifestations of postparaplegic myositis ossificans(PPMO).Methods A total of 14 patients with clinically diagnosed PPMO who underwent X-ray examination of the lesion sites were retrospectively collected,among them 9 cases underwent CT and 4 cases underwent MR examination.Imaging manifestations of PPMO lesions were analyzed and staged.Results Among 14 cases,PPMO lesions located in soft tissue around bilateral hip joint in 8 cases,in soft tissue around unilateral hip joint in 3 cases,in bilateral thigh in 2 cases and in bilateral calf in 1 case.Early stage PPMO was assessed in 4 cases,while middle stage in 3 and late stage PPMO in 7 cases.X-ray films showed early stage PPMO represented as swelled soft tissue,with cloud flocculent high density ossification shadows and blurred edges in 4 cases.In 3 cases of middle stage PPMO,X-ray films showed patchy,strip-like high density ossification shadow with uneven density and clear edges.Lesions in 7 cases of late stage PPMO presented as large flake,banded high-density ossification shadow consistent to the long axis of backbone and muscle with clear edges on X-ray films.One case of early stage PPMO was characterized by swelled soft tissue and high density ossification shadow on CT,while 2 cases of middle stage PPMO showed patchy high-density ossification shadow with uneven density and local eggshell-like changes,and the typical lesions had concentric circle appearance with clear edges.Six cases of late stage PPMO showed large lamellar,cord-like high-density ossification lesions with uneven and low internal densities,myeloid cavity changes and increased marginal density presented as cortical-like changes on CT.On MRI,1 case of early stage PPMO was characterized by swelled soft tissue with long T1 and long T2 signals,blurred edges and feathery changes.One case of middle stage PPMO showed equal or slightly low signal on T1WI,uneven high signal on T2WI,and the signal intensities were higher than the surrounding muscle with stratifications,while the surrounding soft tissue and adjacent bone marrow showed low T1WI and high T2WI edema signals.Two cases of late stage PPMO showed equal and low signals on T1WI,uneven high signals on T2WI,cortical changes presented as low signals on the margin,and edema signals were found in the surrounding soft tissue.Conclusion Imaging manifestations of PPMO had certain characteristics.
7. Clinical study of cervical necrotizing fasciitis accompanied with descending necrotizing mediastinitis treated with cervical double parallel incision combined with mediastinoscope or thoracoscope

Objective:
To summarize the experience of applying cervical double parallel incision combined with mediastinoscope or thoracoscope in the treatment of cervical necrotizing fasciitis (CNF) accompanied with descending necrotizing mediastinitis (DNM), so as to provide a reference for clinical practice.
Methods:
The clinical data of six patients with CNF accompanied with DNM who were admitted to the Department of Stomatology and the Department of Otolaryngology Head and Neck Surgery, The First People′s Hospital of Changzhou from September 2014 to September 2018 were retrospectively analyzed. All of the six patients were confirmed by CT of neck and chest, among whom there were two males and four females aged from 48 to 73. Three patients were treated with cervical double parallel incision combined with mediastinoscope to be combined with cervical and thoracic drainage under general anesthesia while the other three with cervical double parallel incision combined with thoracoscope to be combined cervical and thoracic drainage under general anesthesia. The CT of neck and chest as well as infectious indicators including hematology, C-reactive protein (CRP) and procalcitonin (PCT) were reexamined during the postoperative period.
Results:
The cervical and thoracic combined drainage was unobstructed in all of the six patients, no secondary surgery was performed, and the infectious indicators gradually decreased. All patients had off-bed activities on the first day after the operation, were all cured and discharged after an average of 21 days (16 to 36 days) in hospital and followed up for an average of 18 months (4 to 30 months) after the operation. None of them experienced infection relapse, and they were all satisfied with the appearance of the cervical incision.
Conclusions
Cervical double parallel incision combined with mediastinoscope or thoracoscope for the treatment of CNF accompanied with DNM has the advantages of complete drainage, small trauma, excellent efficacy and aesthetic operative area, thus being deserved to be clinically popularized.
8. The treatment of Lemierre syndrome associated with cervical necrotizing fascitis

Lemierre syndrome is a rare, potentially fatal condition characterized by internal jugular vein thrombosis following an acute oropharyngeal infection, often accompanied with cervical necrotizing fasciitis. This paper reviews 5 cases of Lemierre syndrome with cervical necrotizing fasciitis, extensive cervical drainage and sufficient antibiotics is crucial treatment for Lemierre syndrome, anticoagulation combined with antibiotics is safe and effective for propagation or nonresolution of the thrombus.
