1.Study on the biomechanic and histological changes of skin extended by presuturing in piglets and its clinical application

Objective:To probe into the changes of skin extended by presuturing in respect to biomechanic alteration,source of extended skin,scope of skin involved in extension and histological changes to provide an experimental basis for clinical application.Methods:The white piglet was adopted as animal model.The parameters observed were as follows:area of extra skin obtained histolgical changes,stress-stain relation of skin under stretch,anti-tensile.Results:The results showed that at 24hr,45hr and 72hr after presuturing,the areas of skin extended,and the length of skin were increased.The thickness of epidermis and dermis of stretched skin were thinner.The area with maximum extension was located in 1~2 cm away from stretching borders.The electron microscopic observation showed that in the rough endoplasmic reticulum of fibroblasts,vacuolar degeneration could be seen.Stress-strain relation showed that extensile force decreased with the passage of time.Conclusion:①Stress-stain relation shows that extensile force decreases with the passage of time.②The sources of extended skin include:the displacement of adjacent tissue and the extension of stretched skin and the proliferation of tissue.③The area affected by stretching is mainly limited within 5cm adjacent to stretching border.④The anti-tensile strength of healing wound of extended skin is the same as that of normal skin.
2.Effect of levothyroxine and vitamin E treatment on oxidative stress and myocardial apoptosis in rats with hypothyroidism

Objective To investigate the oxidative stress status and cardiac myocyte apoptosis in rats with propylthiouracil-induced hypothyroidism and to examine the effect of levothyroxine ( LT4 ),vitamin E ( VitE ),and both supplementation on this experimental model.Methods Male Sprague Dawley rats were divided into normal control group( NC),propylthiouracil group( PTU ),LT4 treatment group( PTU + LT4 ),VitE treatment group ( PTU +VitE),LT4 and VitE combination therapy group (PTU + LT4 + VitE).Serum T3,T4,TSH levels,serum and myocardial superoxide dismutase ( SOD ) activity and malondialdehyde ( MDA ) content were determined.Cardiac myocyte apoptotic index was made with TUNEL.Results ( 1 ) Compared with NC group,T3 and T4 levels were significantly lower and TSH level was significantly higher in PTU group and PTU+VitE groups( P<0.05 ).Compared with PTU group,T3 and T4 levels were significantly higher and TSH level was significantly lower in PTU + LT4 group and PTU + LT4 + VitE group( P<0.05 ).There were no statistical differences in T3,T4,and TSH levels between PTU group and PTU + VitE group( P>0.05 ).( 2 ) Compared to NC group,serum and myocardial MDA levels in PTU group increased significantly( P<0.05 ),serum SOD activity decreased significantly ( P<0.05 ),while myocardial SOD activity showed no significant difference( P>0.05 ).( 3 ) Compared with NC group,myocardial apoptosis index was significantly higher in PTU and PTU + VitE groups( P<0.05 ).Compared with PTU group,myocardial apoptosis index was significantly lower in PTU + LT4 group and PTU + LT4 + VitE group( P<0.05 ).Conclusion LT4,VitE,and their combined treatment of hypothyroidism in rats reduce the myocardial cells apoptosis,which may be related to the improvement of thyroid function and amelioration of oxidative stress.
3.Clinical and genetic analysis of a rare case with mosaic partial trisomy 5p syndrome.

OBJECTIVE:
To determine the size and origin of a small supernumerary marker chromosome (sSMC) identified in a patient featuring developmental retardation.
METHODS:
High-throughput sequencing for copy number variation (CNV-seq) was carried out to delineate the sSMC identified upon G-banded chromosomal karyotyping. The genotype-phenotype correlation was explored by database retrieval and literature analysis.
RESULTS:
The patient was found to have a karyotype of mos 47,XX,+mar[36]/46,XX[23]. CNV-seq has identified a 18 Mb duplication at 5p14.1-p12 (hg19: 27,399,261-46,083,784)x2.6 with a mosaicism rate of approximately 60%.
CONCLUSION
Patients with mosaic partial trisomy 5p may have extensive clinical manifestations, and the ratio of trisomy 5p cells is correlated with clinical severity of this syndrome.
4.Effect of contracted learning on rehabilitation training to patients with stroke

Objective To explore the effect of contract learning on rehabilitation training to patients with stroke . Methods According to the random number table, 120 stroke patients were randomly divided in equal number into control group and observation group. The patients in the control group were treated with conventional nursing during hospitalization and health guidance during follow-up. The patients in the observation group were given rehabilitation training according to contracted learning during the period of hospitalization and follow-up. The time was 6 months. The quality of life was compared between the two groups of patients before and after the intervention, 1 months after discharge, 6 months after discharge. Results After six months of intervention, the 7 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, communication and participation in the intervention group were significantly better than those of the control group (P<0.001). The 6 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, and communication of the control group were significantly better than before the intervention (P<0.001). Conclusion For stroke patients, the rehabilitation training under the learning contract can improve the effect of rehabilitation training so as to improve the quality of life and the prognosis.
5.A study on pathogenicity of TGM1 gene mutations in a collodion baby

Objective:To investigate the genetic etiology of a pedigree with autosomal recessive congenital ichthyosis.Methods:Whole-exome sequencing was performed in a collodion baby, and Sanger sequencing was conducted to verify gene mutations. The PolyPhen-2, PROVEAN and Mutation Taster softwares, as well as protein homology modeling methods, were used to predict effects of gene variants; real-time fluorescence-based quantitative PCR and Western blot analysis were performed to analyze the effect of mutations on allelic mRNA and protein expression.Results:Whole-exome sequencing and Sanger sequencing confirmed a mutation c.919C>T (p.Arg307Trp) in exon 6 and a mutation c.1019G>A (p.Gly340Glu) in exon 7 of the TGM1 gene in the infant, which were inherited from his mother and father respectively. Bioinformatics analysis suggested that both the two mutations were harmful to protein structures, which were further supported by protein homology modeling. In vitro experiments showed that there was no significant difference in the mRNA expression of the TGM1 gene between the 293T cells transfected with wild-type plasmids and those transfected with mutant plasmids containing the mutation c.919C>T or c.1019G>A ( t=1.97, 1.28, P=0.12, 0.27, respectively) , but the TGase1 protein expression significantly decreased in the 293T cells transfected with the mutant TGM1 plasmids. Conclusion:The mutations c.919C>T and c.1019G>A in the TGM1 gene may be the molecular genetic etiology of severe ichthyosis in the infant, and the missense amino acids encoded by the two mutations may affect the TGase1 protein function by destroying its structure.
6.Genetic analysis of a recurrent abnormal pregnancy case caused by a cryptic reciprocal autosomal translocation.

OBJECTIVE:
To provide genetic counseling for a couple with recurrent detection of fetal structural abnormality during second trimester pregnancy.
METHODS:
The fetal tissue and peripheral blood samples of the couple were subjected to G banded chromosomal analysis, copy number variation sequencing (CNV-seq) and fluorescence in situ hybridization (FISH) assays.
RESULTS:
CNV-seq has detected a 6.59 Mb duplication at 7p22.3-p22.1 and a 3.81 Mb deletion at 4p16.3 in the fetal tissue, though conventional karyotyping results of both parents were normal. FISH has confirmed that the father has harbored a cryptic translocation of t(4;7)(7p+,4q+,4p+,7q+).
CONCLUSION
The ultrasonographic abnormality of the fetuses may be attributed to the 7p microduplication and 4p microdeletion derived from the cryptic translocation carried by the father. Reciprocal translocation of tiny chromosomal segments should be suspected for couples with recurrent adverse pregnancies but apparently normal karyotypes.

7.Change of IL-23/IL-17 axis in peripheral blood of patients with Graves disease treated by I or antithyroid drugs therapy

Objective To detect the expression level of interleukin-17 (IL-17)/interleukin-23 (IL-23) axis in peripheral blood of patients with Graves disease (GD) before and after 131I or antithyroid drugs (ATD) treatment.Methods The study groups included 40 patients with incipient GD(GD group),20 of whom were treated by 131 I,others were treated by ATD.Forty sex and age matched healthy subjects were recruited as control group.ELISA was uesed to detect interleukin-17 (IL-17),interleukin-23 (IL-23) level before and after the treatment of the GD group and control group.Results ① The expression of IL-23,IL-17 in GD group was significantly higher than the control group(P <0.05);②6 months after 131I treatment,the level of serum IL-23,IL-17 were significantly lower than before (P < 0.05),but still higher than the control group (P < 0.05),which was not found in ATD treatment group.Conclusion IL-23/IL-17 axis may play a role in GD which may also be relevant to 131I treatment.

This review summerized the different suture methods of incisional blepharoplasty for connection, such as skin, orbicularis oculi, tarsal plate, orbital septum and levator aponeurosis. The advantages and disadvantages were also reviewed in order to provide reference for clinical work and enhence the understanding of the incisional blepharoplasty.

10.Interaction between atorvastatin and voriconazole in rat plasma: a HPLC-MS/MS-based study.

OBJECTIVE:
To develop a high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) method for simultaneous determination of atorvastatin and voriconazole in rat plasma and investigate the pharmacokinetics of atorvastatin and the changes in voriconazole concentration in rats after administration.
METHODS:
Plasma samples were collected from rats after intragastric administration of atorvastatin alone or in combination with voriconazole. The samples were treated with sodium acetate acidification, and atorvastatin and voriconazole in the plasma were extracted using a liquidliquid extraction method with methyl tert-butyl ether as the extractant. The extracts were then separated on a Thermo Hypersil Gold C18 (2.1×100 mm, 1.9 μm) column within 6 min with gradient elution using acetonitrile and water (containing 0.1% formic acid) as the mobile phase; mass spectrometry detection was achieved in selective reaction monitoring (SRM) mode under the positive ion scanning mode of heated electrospray ion source (H-ESI) and using transition mass of m/z 559.2→440.2 for atorvastatin and m/z 350→280 for voriconazole, with m/z370.2→252 for lansoprazole (the internal standard) as the quantitative ion.
RESULTS:
The calibration curves were linear within the concentration range of 0.01-100 ng/mL (=0.9957) for atorvastatin and 0.025-100 ng/mL (=0.9966) for voriconazole. The intra-day and inter-day precisions were all less than 13%, and the recovery was between 66.50% and 82.67%; the stability of the plasma samples met the requirements of testing. The AUC of atorvastatin in rat plasma after single and combined administration was 438.78±139.61 and 927.43±204.12 h·μg·L, CLz/F was 23.89±8.14 and 10.43±2.58 L·h·kg, C was 149.62±131.10 and 159.37±36.83 μg/L, t was 5.08±1.63 and (5.58±2.11 h, and T was 0.37±0.14 and 3.60±1.52 h, respectively; AUC, CLZ/F and T of atorvastatin in rat plasma differed significantly between single and combined administration. The HPLC-MS/MS system also allowed simultaneous determination of voriconazole concentration in rat plasma after combined administration.
CONCLUSIONS
The HPLC-MS/MS system we established in this study is simple, rapid and sensitive and allows simultaneous determination of atorvastatin and voriconazole in rat plasma. Some pharmacokinetic parameters of atorvastatin are changed in the presence of voriconazole, and their clinical significance needs further investigation.
