1. Advances in the study of antibodies associated with myasthenia gravis
Chinese Pediatric Emergency Medicine 2020;27(2):144-148
As an autoimmune disease involving the neuromuscular junction, the pathogenesis of myasthenia gravis(MG) is closely related to autoimmune antibodies.Regarding related antibodies, acetylcholine receptor antibody(AChR-Ab) is the most studied and clear. In recent years, researchers have began to study antibodies other than AChR-Ab due to the presence of AChR-Ab-negative MG patients, such as muscle-specific tyrosine kinase antibodies, low-density lipoprotein 4 antibodies, and explore the corresponding treatment options. This article reviewed the research progress of MG related antibodies, and provided clinical evidence and reference for the future treatment of this disease.

Objective To investigate the function characteristics of CD4 T cell converted double negative T cell and provide a basis for further insight into the characteristics of mouse converted double negative T cell. Methods The gene expression profile was analyzed by transcriptome sequencing and protein mass spectrometry. The expression of cell active marker CD44,CD69 and OX40 was investigated by flow cytometry and the cytotoxicity of mouse double negative T cell was verified by CFSE staining. Results Mouse CD4 T cell converted double negative T cell expressed cell phenotype that differed from other mature CI4 T cells.Mouse converted double negative T cell expressed high level of active marker of CD44,CD69 and OX40.Cytotoxicity of PrfO DN T was significantly reduced. Conclusions Mouse CD4 T cell converted double negative T cell has distinguishing cell phenotypes, that are not identical to other mature CD4 T cells.Mouse double negative T cell overexpresses cell activation marker and cytotoxic cytokines. The immune suppressive function of mouse double negative T cell is mainly dependent on perforin pathway.

Objective:To analyze the health literacy level and influencing factors of Uyghur residents in Moyu County, Xinjiang between 2018 and 2020.Methods:A total of 5 211 and 5 207 Uyghur residents in Moyu County, Xinjiang were sampled using the multi-stage cluster method in 2018 and 2020, respectively. The health culture questionnaire for ethnic minority in Xinjiang was used. The survey included general demographic characteristics, overall health literacy levels, and the three aspects of health literacy levels (health knowledge and concepts, healthy lifestyles and behaviors, and health skills) and six dimensions (scientific health concept, health information literacy, infectious disease prevention and control literacy, chronic disease prevention literacy, safety and first aid literacy, and basic medical literacy). The influencing factors of residents′ health literacy in 2020 were analyzed using χ2 test and multivariate logistic regression. Results:In 2020, overall health literacy and its three aspects among Uyghur residents in Moyu County, Xinjiang were higher than in 2018 (overall 8.10% vs 5.07%, health knowledge and concepts 9.99% vs 7.60%, healthy lifestyle and behavior 6.88% vs 5.33%, health skills 40.12% vs 30.22%) (all P<0.05). In 2018, results for the six dimensions of health literacy were, from high to low, as follows: safety and first aid literacy (26.42%), basic medical literacy (14.14%), health information literacy (13.38%), scientific health concept (10.17%), infectious disease prevention and control literacy (8.96%), chronic disease prevention and treatment literacy (5.57%); In 2020, the results, from high to low, were: safety and first aid literacy (36.09%), basic medical literacy (24.08%), health information literacy (20.47%), infectious disease prevention and treatment literacy(15.19%), scientific health concept (13.85%), and chronic disease prevention and treatment literacy (10.10%). All values were higher than in 2018 (all P<0.05). Gender, age, education level, marital status, occupation, per capita monthly household income level, and area of residence influenced health literacy levels of local residents. Conclusions:The health literacy of Uyghur residents in Moyu County, Xinjiang improved from 2018 to 2020; however, the overall level remains low. Elderly people, people with low education levels, people with low incomes, business people & staff are the key groups requiring health education. Healthy lifestyle and behavior, chronic disease prevention and treatment literacy, scientific health concept, infectious disease prevention and control literacy are identified as areas necessitating health education and health promotion intervention.

Objective:To compare the safety and efficacy of the " step-up approach" versus the " step-jump approach" in treatment of infected pancreatic necrosis (IPN).Method:The clinical data of IPN patients who underwent step-up strategy or step-jump strategy treatment at the Department of Pancreatic and Biliary Surgery of the First Affiliated Hospital of Harbin Medical University from December 2018 to November 2022 were analyzed retrospectively. Propensity score matching (PSM) was done based on the nearest neighbor matching method (1: 1 ratio). After matching the baseline data (the caliper value was 0.01), a total of 62 patients with IPN were included, including 41 males and 21 females, aged (41.1±13.1) years old. Patients who were treated with the step-up strategy were included in the step-up group, while patients who were treated with the step-jump strategy were included in the step-jump group. There were 31 patients in each group after PSM, and the treatment effect of the two groups were compared. Results:Of the 62 patients with IPN, 43 received surgical intervention, and 19 were managed successfully using symptomatic anti-inflammatory treatment or percutaneous catheter drainage. The total hospitalization cost of patients in the step-jump group was significantly higher than that in the step-up group [122 000 (73 000, 179 000) yuan vs. 88 000 (46 000, 144 000) yuan, P=0.034]. The overall cure rate of IPN patients in the step-jump group was 93.5%(29/31). The 2 patients who died had type Ⅲ IPN. In the IPN patients in the step-up group were all cured, and the overall cure rate was 100%(31/31), with no death. There were no statistical differences between the two groups in the rates of death, postoperative complications, residual infection, debridement ≥2 times, and positive bacterial culture in blood or drainage fluid (all P>0.05). A total of 19.4% (12/62) patients had postoperative complications, including 4 patients with abdominal bleeding, 3 patients with new organ dysfunction, 2 patients with gastrointestinal bleeding, 2 patients with gastrointestinal fistula, and 1 patient with venous thrombosis in both lower limbs. Conclusion:Both the step-up treatment strategy and the step-jump treatment strategy were safe and effective for treatment of IPN patients.

Zedoary turmeric oil, volatile oil extracted from zedoary turmeric, composed mainly of monoterpenes (including α-pinene, β-pinene, etc.) and sesquiterpenes (including β-elemene, zedoary alcohol, zedoary ketone, etc.), and has been used in clinical practice to treat various malignant tumors such as ovarian cancer, cervical carcinoma, colorectal cancer, lung cancer and liver cancer. Zedoary turmeric oil regulates vascular endothelial growth factor and nuclear factors- κB, signal transducers and activator of transcription 3 signaling pathways to play a role in inhibiting tumor angiogenesis, inhibiting tumor cell proliferation, inducing tumor cell apoptosis, and blocking cell cycle. However, due to its insolubility in water and poor stability, its clinical application is limited; the application of new formulations and technologies such as liposomes, microspheres, and nanoemulsion improves the solubility and stability of zedoary turmeric oil. This paper summarizes recent research progress on the chemical composition, anti-tumor effects, and formulations of zedoary turmeric oil, both domestically and internationally, providing a reference for further expanding the clinical application and formulation development of zedoary turmeric oil in the anti-tumor field.
