1. Interaction of carbohydrate derivatives and sodium-glucose cotransporters 2 by molecular dynamic simulation

Objective: To investigate the interactions between carbohydrate derivatives and sodium-glucose cotransporters 2 (SGLT2) using molecular dynamic (MD) method and to explore the mechanisms and structure-activity relationship of SGLT2 inhibitors. Methods: The homologous structure of SGLT2 was modeled, the GROMACS program was used to model eight structures, such as SGLT2, SGLT2-glucose compound, and SGLT2-carbohydrate compound. And the root mean square fluctuation (RMSF) of the key residues and ligands and the interaction energy between the ligands and SGLT2 was investigated by trajectory analysis. Results: The interaction energy calculated by MD method had the higher correlation with experimental results than that by molecular docking method. H80, K154, D158, and Y290 were the key residues involved in the interaction, N75 and F453 were the important residues, and W291, Q295, and S393 might be the auxiliary residues. The ligands had a relatively consistent conformation, and fragments A and C played the more important roles in receptor binding. And the size, rigidity, and polarity increasing could elevate the bonding strength. Conclusion: MD simulation results could display the good performance of the interaction between the ligands and SGLT2, and could give clear guidance for the design of new SGLT2 inhibitors.
2.Quality of methodology and reporting of Meta-analyses on acupuncture treating ischemic stroke in China.

OBJECTIVE:
To evaluate the quality of methodology and reporting of Meta-analyses on acupuncture treating ischemic stroke in China.
METHODS:
The domestic literature of systematic reviews (SRs) on acupuncture treating ischemic stroke was retrieved from CBM, VIP, CNKI and WANFANG databases. The quality of methodology and reporting was measured by A Measurement Tool to Assess Systematic Reviews (AMSTAR) and Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA).
RESULTS:
Seven studies involving 6 papers in Chinese and 1 paper in English were chosen from 402 papers. The deficiencies of methodology mainly contained incomplete retrieval strategy, less attention to the publication bias and the heterogeneity, etc. The shortages of reporting was on retrieval strategy reporting, the studies features and the risk of bias reporting and evidence strength level reporting, etc.
CONCLUSION
At present the quality of methodology and reporting of SRs on acupuncture treating ischemic stroke in China has flaws, which needs improvement.

3.Characteristics of elderly apheresis platelet donors and analysis of pre-donation testing results

【Objective】 To study and analyze the characteristics of 56-60 years old apheresis platelet donors and the results of pre-donation blood testing in Tianjin, so as to provide reference for the management of blood stations and the maintenance and retention of blood donors. 【Methods】 The information of platelet donors were collected by Tianjin Blood Center from July 1, 2012 to June 30, 2021.The donors aged 56-60(the observation group) were compared with donors aged 18-55(the control). The number of blood donations, male to female ratio, donation frequency, proportion of two-therapeutic-amount donation, rate of deferred donation, incidence of adverse reactions to blood donation, etc of the two groups were studied. The pre-donation test results of the two groups were also compared through the analysis of the unqualified factors of blood routine. 【Results】 From 2012 to 2021, there were 2 837 platelet donors aged 56-60 in Tianjin, and the donation frequency and proportion of two-therapeutic-amount donation were significantly higher than those in the control. The blood routine indexes were significantly different between two groups, but all fluctuated within the normal range(P<0.05). More donors in the observation group than the control were deferred due to unqualified blood routine indexes, mainly caused by Hb deferral. 【Conclusion】 In the past 9 years, the number of apheresis platelet donors aged 56-60 in Tianjin has been stablely increasing. Elderly donors always meet the health requirements of donations according to the results of blood tests. Close monitor of the blood routine indexes, however, should be paid to such population to ensure the elderly donor health and donation safety.
4.Advances in peripatellar osteotomy for treating recurrent patellar dislocation.

OBJECTIVE:
To summarize the progress of research related to the surgical treatment of recurrent patellar dislocation by peripatellar osteotomy in clinical practice, in order to provide reference for clinical treatment.
METHODS:
The recent literature on peripatellar osteotomy for recurrent patellar dislocation at home and abroad was reviewed, and the bony structural abnormalities, imaging diagnosis, and treatment status were summarized.
RESULTS:
Abnormalities in the bony anatomy of the lower limb and poor alignment lead to patellofemoral joint instability through the quadriceps pulling force line and play an important role in the pathogenesis of recurrent patellar dislocation. Identifying the source of the deformity and intervening with peripatellar osteotomy to restore the biomechanical structure of the patellofemoral joint can reduce the risk of soft tissue surgical failure, delay joint degeneration, and achieve the target of treatment.
CONCLUSION
In the clinical diagnosis and treatment of recurrent patellar dislocation, the factors causing patellofemoral joint instability should be comprehensively evaluated to guide the selection of surgery and personalized treatment.

5.Treatment of posterior cruciate ligament avulsion fractures of the tibia using a toothed plate and hollow lag screw.

INTRODUCTIONTo investigate the feasibility and clinical efficacy of using a toothed plate and hollow lag screw in the surgical treatment of posterior cruciate ligament (PCL) avulsion fractures of the tibia.
METHODSA total of 21 patients were treated with open reduction and internal fixation using a toothed plate and hollow lag screw, through a posteromedial approach using an inverted L-shaped incision. The patients were allowed appropriate functional exercises, including knee flexion and extension, after removal of the plaster at postoperative weeks 3-6. The follow-up period was between six months and two years.
RESULTSThis was a retrospective study of patients with PCL avulsion fractures of the tibia caused by road traffic accidents (n = 9), sports-related injuries (n = 6), falls (n = 5) and machinery-related injuries (n = 1). 20 patients presented with fresh fractures and one with an old fracture. The patients (13 men, eight women) had a mean age of 41.5 (range 19-72) years. Anatomical reduction of the fracture and satisfactory fixation were achieved in all 21 patients. Bony union was achieved in all patients at 8-12 weeks after surgery. Six months after surgery, knee flexion was 121.9° ± 10.4° and extension was 0°. According to the Lysholm Knee Scoring Scale, 19 patients were rated as 'excellent' and two as 'good'.
CONCLUSIONThe use of a toothed plate and hollow lag screw could be a simple and reliable approach for PCL avulsion fractures of the tibia. Patients achieved good knee function after surgery.

6. Efficacy of vitrectomy assisted by intravitreal conbercept injection for the treatment of severe proliferative diabetic retinopathy

Objective:
To evaluate the efficacy of preoperative intravitreal conbercept injection (IVC) before pars plana vitrectomy for severe proliferative diabetic retinopathy (PDR).
Methods:
A non-randomized controlled study was adopted.Fifty-seven severe PDR patients were collected in Tianjin Eye Hospital from January 2015 to June 2016.The patients were divided into two groups, 30 eyes in the preoperative IVC group, and 30 eyes in the simple PPV group (control group). Operation time, intraoperative bleeding, LogMAR BCVA and complications were compared.This study protocol was approved by Ethic Committee of Tianjin Eye Hospital (No.TJYYLL-2015-14) and this study followed the Declaration of Helsinki.
Results:
The average operation duration was (95.4±15.8)minutes in the preoperative IVC group, which were significantly shorter than (107.0±11.3)minutes in the control group, with a significant difference between them (

8. Clinical analysis of the effect of metabolic syndrome on cognitive function after acute ischemic stroke

Objective:
To explore the effect of metabolic syndrome(MetS) and its individual components on cognitive impairment and neurological dysfunction in patients after acute ischemic stroke.
Methods:
A total of 733 patients with acute ischemic stroke aged 60 to 83 years admitted in Tianjin Huanhu hospital from January 2010 to May 2018 were enrolled in this cross-sectional study.The patients were divided into the non-metabolic syndrome(non-MetS) group and the metabolic syndrome(MetS) group according to the diagnostic criteria of metabolic syndrome.Cognitive functions were evaluated by using the Montreal Cognitive Assessment(MoCA) and Mini-Mental State Examination(MMSE). Neuropsychiatric behavior was assessed by using the Neuropsychiatric Inventory(NPI) questionnaire.Emotional state was examined according to the 21-item Hamilton Depression Rating Scale(HAMD-21). The degree of neurological impairment after stroke was evaluated by using the National Institutes of Health Stroke Scale(NIHSS). The activity of daily living was evaluated by Barthel index and the Activity of Daily Living(ADL) scale.The overnight fasting blood samples were obtained in order to determine biochemical indicators.
Results:
The average age of first-onset of ischemic stroke was earlier, and the body mass index(BMI) was higher in the MetS group than in the non-MetS group(
