1.Effect of prokinetic agents on the electrical activity of stomach and duodenum in rats

Objective To determine the effect of prokinetic agents such as domperidone, mo-sapride, clarithromycin, and itopride on the electrical activity of the stomach and duodenum in SD rats, and also to explore the mechanism. Methods The organism functional experiment system BL-420E was used to record the myoelectrical activity in the stomach and duodenum of SD rats in all groups using domperidone, mosapride, itopride, clarithromycin, and physiological saline on the inter-digestive phase. The effect of the prokinetic agents on the amplitude and freqency of gastric and duo-denal electromyologram in the SD rats was compared. The antagonists such as atropine, phento-lamine, and propranolol were added to investigate the mechanism of action with all prokinetic agents. Results All prokinetic agents increased the amplitude and frequency of gastric and duodenal fast waves in the SD rats (P<0.05). The effect of itopride was the most obvious among the 3 groups (P<0.05), and clarithromycin had the weakest effect (P<0.05). The amplitude and frequency of gastric and duodenal fast waves in the SD rats in the groups of clarithromycin, domperidone, mosa-pride, itopride, and physiological saline were inhibited by atropine (P<0.05) , but not by phento-lamine and propranolol. Conclusion Itopride, mosapride, domperidone, and clarithromycin can in-crease the amplitude and frequency of gastric and duodenal fast waves in the SD rats. The mechanism may be related to cholinergic receptors, but not adrenergic receptors.
2.Discussion on public-private partnership for emergency drug supply management system in China

The theory of public goods was used to analyze the characteristics and status quo of emergency drug supply in China,holding such drugs as public goods.Based on the public-private partnership(PPP) model,the authors proposed to optimize the existing commanding mechanism,inventory system,information management system and supply system of emergency drugs.We recommended the establishment of an emergency drug supply assurance system,featuring leadership of local governments,and participation of pharmaceutical enterprises and hospitals,in which these entities are assigned with clear-cut emergency supply roles to ensure efficient supply of such drugs in the country.
3.Analysis of risk factors for gallbladder polyps in shift work nurses

Objective:To analyze the detection of gallbladder polyps among shift nurses in Shengjing Hospital of China Medical University in physical examination and its related influencing factors, so as to provide scientific basis for shift nurses to prevent gallbladder polyps.Methods:A total of 1 119 shift nurses who had physical examination in Shengjing Hospital of China Medical University from January 1 to March 31, 2018 were selected. The patients with gallbladder polyps diagnosed by ultrasound were included in the case group, and those without gallbladder polyps were taken as the control group. Age, gender, body mass index (BMI), night shift frequency, education level, Department, meal regularity, drinking history, smoking history, serum triglyceride, serum total cholesterol, hepatitis B surface antigen, serum alanine aminotransferase and serum aspartate aminotransferase levels were compared between the two groups.Results:Univariate analysis showed that age ( P<0.001), gender ( P=0.028), BMI ( P=0.005), night shift frequency ( P=0.021) were the factors with statistically significant difference between the case group and the control group. There were no statistically significant differences between the case group and the control group in terms of education level, department, dining regularity, drinking history, smoking history, hepatitis B surface antigen, serum triacylglycerol, serum total cholesterol, serum alanine aminotransferase, serum aspartate aminotransferase levels ( P>0.05). Logistic regression analysis showed that age, BMI and night shift frequency were the influencing factors in the regression equation ( P<0.05). Conclusion:Age, BMI and night shift frequency may be related risk factors of gallbladder polyps in shift nurses.
4.IgM nephropathy with nephrotic syndrome in children

Objective To investigate the clinical charactors and treatment of IgM nephropathy with nephrotic syndrome(NS) in children.Methods Thirty-six IgM nephropathy patients hospitalized in Hunan Children's Hospital as research group(group A),from June 2005 to June 2012.One hundred and six patients with minimal change disease (MCD) as control group (group B),followed up for 1 ~ 8 years.Results (1) Hematuria at presentation of the two groups respectly 3.8% vs 30.6% (x2 =20.403,P < 0.05).(2) Renal pathology revealed that there were 26 (72.2%)patients with minimal change disease,9 cases (72.2 %)with moderate membranoproliferative glomerulonephritis,1 cases with focal segmental glomerulosclerosis.(3) According to renal pathology,group A patients were divided into two sub groups:mild group and moderate group.To compared with group B,the steroid-resistant incidence of the 3 group were respectly 12.3%,19.2%,77.8% (x2 =24.369,P < 0.05).There was no significant difference between control group and mild group.(4)The remission rate of proteinuria in steroid-resistant patients who combined to use mycophenolate mofetil (MMF) with the two groups were respectively 50% and 85.7 % (x2 =3.60,P > 0.05).Conclusion Incidence of hematuria is higher in IgM nephropathy patients and patients with renal pathology for moderate lesions have a high steroid-resistant,and need use immunosuppressive early.MMF may be a good immunosuppressive for theses patients.
5.Influence of clinical features at initial hemodialysis on long-term prognosis in advance-aged patients

Objective:To analyze the association of clinical characteristics and laboratory indicators at initial maintenance hemodialysis(MHD)with long-term prognosis in advance-aged patients, and to find influencing factors for the prognosis in advance-aged MHD patients.Methods:This retrospective study was conducted at the Nephrology Department of Beijing Hospital between April 2007 and January 2018.A total of 61 patients receiving first-time hemodialysis at ≥ 80 years of age and undergone regular dialysis for 3 months or longer were enrolled.All patients were followed-up until death or the end of July 1, 2018.Patients were divided into the survivor and non-survivor groups, and differences in clinical characteristics and laboratory indicator values were compared between the two groups.Influencing factors for prognosis in advance-aged MHD patients were analyzed by using multivariate Cox regression.Results:For the 61 subjects, the median follow-up time was 25.8 months.During the follow-up, 32 patients died(52.5%). The main death causes were infectious diseases(40.6%, n=13)and cardiovascular and cerebrovascular diseases(37.5%, n=12). The 1-, 2-, 3-, 4-, and 5-year cumulative survival rates were 75.4%(46/61), 54.1%(33/61), 37.7%(23/61), 22.9%(14/61)and 16.4%(10/61), respectively.The median survival time was 25.8 months for all patients, 27.5 months for patients aged 80-84 years, and 14.9 months for patients aged 85 years and over.The non-survivor group had a higher male ratio(65.6% or 21/32 vs.37.9% or 11/29, χ2=4.678, P=0.031)and lower levels of hemoglobin(85.4±13.0 vs.95.0±17.6 g/L, t=2.867, P=0.019)and albumin(30.3±5.0 vs.34.6±4.8 g/L, t=3.039, P=0.001)than the survivor group.Kaplan-Meier curves indicated that the survival rate decreased with age, and subjects aged less than 85 years had a higher survival rate than subjects aged 85 years and older(the median survival time: 14.9 months vs.27.5 months, Log Rank P=0.006); patients who received continuous renal replacement therapy(CRRT)before dialysis had lower survival rates than patients who did not receive CRRT(the median survival time: 7.8 months vs.29.2 months, Log Rank P=0.002); patients with high serum levels of albumin(≥33 g/L)had higher survival rates than patients with low serum levels of albumin(<33 g/L)(the median survival time: 29.2 months vs.18.9 months, Log Rank P=0.003). Multivariate Cox regression analysis showed that age at initial dialysis( HR=1.136, 95% CI: 1.005-1.285, P=0.041), female( HR=0.409; 95% CI: 0.169-0.994, P=0.048), serum albumin level( HR=0.836, 95% CI: 0.772-0.906, P<0.001)and CRRT before dialysis( HR=6.161, 95% CI: 1.848-20.538, P=0.003)were independent predictors of all-cause mortality in advance-aged patients. Conclusions:Advance-aged patients undergoing hemodialysis have complicated clinical conditions and poor prognosis.Age, gender and serum albumin level at initial dialysis and CRRT before dialysis are independent predictors of prognosis in these patients.
6.Taurine attenuates cytotoxicity induced by iohexol in human renal tubular epithelial cells

Objective To investigate the protective effect and mechanism of taurine on the cytotoxicity of iohexol on HK-2 cells. Methods HK-2 cells were exposed to iohexol at different dosage (25, 50, 100, 125 gI/L) for 6 h and at the dose of 100 gl/L for different time(2 h, 4 h, 6 h). Then taurine (3,12,24 mmol/L) was coincubated with iohexol (100 gI/L) for 6 h.Cell viability was assessed by CCK-8 assay. Cell apoptosis was determined by Hoechest 33342 flurescence stains,flow cytometry with Annexin V-FITC/PI double stains and caspase-3 activity by colorimetric assay. Bcl-2 and Bax expression were examined by Western blot. Intracellular ROS was detected by flow cytometry with fluorescent probe DCFH-DA. Results Iohexol decreased HK-2 cell viability and induced apoptosis in concentration-dependant and time-dependant manner (all P<0.05). ROS was increased following iohexol (100 gI/L for 6 h) treatment (P<0.05). Taurine increased cell viability and attenuated apoptosis in dose-dependant manner. The cell viability levels in taurine intervention (3,12,24 mmol/L) group were significantly increased compared with that in iohexol treated group respectively [(88.00±1.00)%, (91.33±0.58)%, (95.67±1.52) % vs (76.67±1.53)%, all P<0.05]. Apoptosis rate by flow cytometry were decreased respectively [(8.84±1.75)%,(7.86±1.82)%, (6.30±1.50)% vs (11.98±0.39)%, all P<0.05]. Caspase-3 activities were decreased respectively [(1.33±0.10), (1.27±0.06), (1.10±0.04) vs (1.42±0.13), all P<0.05].Taurine up-regulated the expression of Bcl-2, and decreased the intracellular ROS (all P<0.05).Conclusions Iohexol induces cell apoptosis and oxidative stress. Taurine attenuates direct cytotoxic effect induced by iohexol. The anti-oxidative stress effect and up-regulated Bcl-2 expression may partly account for the protection of taurine.
7.Application of health education intervention based on protection motivation theory in community female patients with stress urinary incontinence

Objective To apply the health education intervention based on protection motivation theory in community female patients with stress urinary incontinence,in order to establish a health behavior and to control urinary incontinence.Methods 96 female patients with stress urinary incontinence were randomly divided into the control group and the experimental group by random number table method,the control group received routine health education,the experimental group received health education based on protection motivation theory.The ICI-Q-SF score,pelvic muscle exercise compliance scale score,urinary incontinence coping efficacy score were compared between the control group and the experimental group.Results The ICI-Q-SF score of the experimental group after the intervention was 5.43 ± 1.77,which was significant lower than that in the control group,7.96±2.98,t=4.88,P<0.01.The urinary incontinence coping efficacy scores of experiment groups after the intervention was 17.63±1.85,which was higher than that in the control group,13.49±2.46,t=-9.09,P<0.01.There were 29 cases accounted for 63.04% with good compliance and 17 cases accounted for 36.96% with poor compliance in experimental group after intervention,14 cases accounted for 31.11% with good compliance and 31 cases accounted for 68.89% with poor compliance in control group after intervention,the difference was statistically significant,x2=9.306,P<0.05.Conclusions Health education based on protection motivation theory can promote the coping efficacy and health behavior of community female patients with stress urinary incontinence and can effectively control urinary incontinence symptom.
8.Transitional care based on the Omaha system in postoperative rehabilitation of patients with breast cancer

Objective To explore the application of transitional care based on the Omaha system in postoperative rehabilitation of patients with breast cancer.Methods Seventy-two patients after radical mastectomy were assigned into two groups according to admission time,patients from June to December 2011 were set as the control group,patients from February to December 2012 were assigned to the intervention group,each group contained 36 patients.The patients in the control group received routine nursing,while patients in the intervention group were implemented transitional care program with Omaha system as framework from before discharge to 6-week follow-up after discharge from hospital.Nursing effect was compared between two groups from three aspects such as cognition,behavior and status.Results Twenty-three problems were found in the 6-week follow-up for the intervention group,including environmental domain,psychosocial domain,physiological domain and health-related behavior domain.Knowledge,behavior and status scores of the intervention group 6 weeks after discharge were significantly higher than those of the control group [(3.92 ± 0.48) scores vs.(2.41 ± 0.17) scores,(4.11 ± 0.49) scores vs.(2.83 ± 0.25) scores,(3.09 ±0.52) scores vs.(2.32 ±0.39) scores,t=14.81,15.36,9.42,P<0.01 or 0.05].Conclusions Transitional care based on the Omaha system shows common problems in rehabilitation of patients with breast cancer after operation,and provides standardized program for rehabilitation nursing.It can achieve better nursing effect.
9.Determination of Astragaloside IV in Radix Astragli and Its Preparations by Online SPE-Liquid Chromatography with Charged Aerosol Detector

A novel method for quantification of astragaloside IV in Radix Astragli was developed by using online SPE liquid chromatography coupled with corona charged aerosol detection( CAD) . The sample solution
was loaded into Acclaim Polar AdvantageⅡC18(50 mm×4. 6 mm, 3 μm) which was selected as online SPE column. Then the cleaning process was done by using the Right one of dual gradient pumps with Methanol-water as mobile phase. Acclaim C18(150 mm×4. 6 mm, 5 μm) was selected as analytical column with acetonitrile-water as mobile phase at a flow rate of 1. 0 mL/min. The eluate containing the target from SPE column was transferred into the analytical column by heart-cutting mode. The temperature of nebulizer of corona CAD was set at 30 ℃ and the nitrogen pressure was 241. 3 kPa. The baseline separation of astragaloside IV from matrix components has been achieved. There was a good linear correlation in the range of 4 . 0-80 mg/L for astragaloside IV and the correlation coefficient was 0 . 9998 . The standard addition average recovery of astragaloside IV in Radix Astragli was 97 . 6%. It has been validated that astragaloside IV in Radix Astragli and its preparations can be quantified rapidly and accurately with this method.
10.Analysis incident dialysis situation effect on the prognosis of elderly patients undergoing hemodialysis

Objective To investigate the clinical characteristics and laboratory examinations in incident dialysis effect on the prognosis of elderly patients undergoing hemodialysis.Methods Ninety-three patients aged 65 years or older initiating hemodialysis were enrolled from Hemadialysis Center of Beijing Hospital from January lst,2007 to June 30th,2016.The duration time of HD of all patients was more than three months.Patients were divided into death group and non-death group.The clinical characteristics and laboratory examinations were compared between the two groups.Cox proportional hazards regression was used for the multivariate analysis to determine independent prognosis factors.Results The average year of patients was 74.2±6.5 years old with 43 months of median time of follow-up.The first two causes of death were infection (n =25,49.0%) and cardiovascular and cerebrovascular diseases (n =16,31.4%).Cox single factor regression analysis showed that the older ages,diabetic nephropathy being the cause of end-stage renal disease (ESRD),complicating with diabetes mellitus or congestive heart failure,the higher Charlson cardiovascular diseases score,ALB being under 35 g/L were correlated with poor outcome respectively(P<0.05).Cox multivariate regression analysis indicated that older ages (HR =1.056,P =0.021),diabetic nephropathy being the cause of ESRD (HR =2.661,P =0.001),the higher Charlson cardiovascular diseases score (HR =1.675,P =0.010),central venous catheters being vascular access(HR=1.167,P=0.048) on incident dialysis were the main risk factors for mortality in elderly patients.Conclusion The older ages,diabetic nephropathy being the cause of ESRD,the higher Charlson cardiovascular diseases score,central venous catheters being vascular access on incident dialysis are independent risk factors influencing survival of elderly patients.
