1.Effects of Panax notoginseng saponins on the fast-excitatory postsynaptic potential in rat stellate ganglion

BACKGROUND: Panax notoginseng saponins (PNS) could significantly improve the learning and memory ability of rats, but its influence to peripheral nervous system still needs further investigation.OBJECTIVE: To observe the effects of PNS on the fast-excitatory postsynaptic potential (f-EPSP) in stellate ganglion (SG) of rats.DESIGN: Observation and controlled trial.SETTING: Pharmacological Laboratory of Guangxi Medical University.MATERIALS: The experiment was carried out at the Pharmacological Laboratory of the Experimental Center of Guangxi Medical University from January 2005 to February 2006. Thirty healthy male SD rats of clean grade and (220±20) g, provided by the Experimental Animal Center of Guangxi Medical University; SEN-7203 digital three track strip stimulator, microelectrode amplifier (MEZ8301, Japan NIHON KOHDEN COMPANY); glass microelectrode puller, and microelectrode manipulator, both the products of Narishige Company, Japan; PNS, provided by Kunming Jacobson Pharmaceutical Co., Ltd, and acetylchloride chline (Ach), the product of Sigma, U.S.A.METHODS: After the animals were executed acutely, their chest wall was opened to isolate SG rapidly under microscope, which was transferred to the perfusion chamber, and fixed with wire needles after peeling the connective tissue membrane. The ganglia were perfused continuously with the mixture of volume fraction 0.95 O2 and 0.05 CO2 plus Krebs solution with pH (7.4±0.05). Meanwhile, 0.08-0.16 g/L PNS was employed to perfuse and culture SG.①The glass microelectrode filled with 3 mmol/L KCI was used to puncture the isolated SG and record the amplitude and duration of depolarizing reaction of postsynaptic membrane.②PNS with the maximum concentration of 0.16 g/L, which could inhibit the f-EPSPs, was perfused to observe the effect of PNS on the amplitude and duration of depolarizing reaction of postsynaptic membrane induced by exogenous Ach (1 mmol/L, 1 minute).③PNS with the maximum concentration of 0.16 g/L, which could inhibit the f-EPSPs, was perfused to observe the effect of PNS on membrane resistance and membrane potential.MAIN OUTCOME MEASURES:①Amplitude of depolarizing reaction of postsynaptic membrane; ②Effect of PNS on the amplitude and duration of depolarizing reaction of postsynaptic membrane induced by exogenous Ach, and membrane resistance and membrane potential.RESULTS: Thirty rats were involved in the result analysis. ①PNS ranged 0.08 to 0.16 g/L could reversibly depress the f-EPSPs amplitude of, or change the forward active potential into f-EPSP; the higher the concentration of PNS, the more obvious the inhibition was. The depression appeared in 3-10 minutes after PNS perfusion, and the effect reached the peak at 0.16 g/L; f-EPSP was decreased evidently in 3 to 4 minutes. The inhibition nearly recovered to the control level after washing the ganglia with Krebs solution for 15 to 20 minutes. ②Effect of PNS on exogenous ACh-induced depolarization: The amplitude and duration of the Ach-induced depolarization did not significantly change before and 5 minutes after 0.16 g/L PNS perfusion [before: (15.5±2.4) mV, (256.1±21.5) seconds; after: (14.3±1.9) mV, (228.6±24.5) seconds, P>0.05].③Effects of PNS on membrane potential and membrane resistance: The mean membrane potential and membrane resistance were not significantly changed after PNS perfusion [before:-(55.5±12.1) mV, (53.9±5.1) MΩ; after: -(54.3±10.4) mV, (55.1±4.8)MΩ, P>0.05].CONCLUSION: PNS could reversibly depress the fast-excitatory postsynaptic potential in stellate ganglion of rats by presynaptic mechanism.
2.Analysis on etiology,clinical diagnosis and treatment of 102 pediatric hemoptysis cases

Objective To investigate the etiology,clinical charactieristics,treatment and prognosis in pediatric hemoptysis to provide reference for its diagnosis and treatment to pediatric doctors .Methods The retrospective analysis was performed on the clinical manifestations ,laboratory tests ,imagiological and bronchoscopic manifestations ,treatment methods and outcomes in the children patients with hemoptysis as the chief complaint admitted to our hospital from April 2009 and December 2014 .Results A-mong 102 children cases of hemoptysis ,55 cases were male and 47 cases were female .The median age was (7 .05 ± 1 .2) years old . Mild hemoptysis was in 87 cases(85 .3% ) ,moderate hemoptysis was in 10 cases(9 .8% ) and massive hemoptysis was in 5 cases (4 .9% ) .The relatively exact causes of hemoptysis were found in 88 cases ,in which the lower respiratory tract infection was most common ,the top 3 causes were pneumonia ,pulmonary tuberculosis and bronchitis .In the non-infection causes ,idiopathic pulmonary hemosiderosis(IPH) was most common ,while connective tissue diseases ,pulmonary vascular malformation and airway foreign body were rare .85 cases showed different pathological lesions on chest CT scan .Among the 35 cases underwent the bronchoscopy ,3 ca-ses of bronchial stenosis caused by granulation proliferation was improved after bronchoscopic cryotherapy ,5 cases of active bleed-ing under endoscopic stopped bleeding by topical hemostasis ,among 10 cases of suspected tuberculosis with negative sputum bacte-ria ,acid-fast bacilli was found in 8 cases by bronchofiberscopic lavage .The treatment of pediatric hemoptysis was dominated by the medical etiological and symptomatic therapy .Conclusion Chest CT scan can provide a very important basis for the etiological diag-nosis of hemoptysis and the bronchoscopy examination also has an important value in the diagnosis and treatment of hemoptysis . The etiological and symptomatic treatment is predominat in pediatric hemoptysis ,and the majority have good prognosis .
3.A model-based meta-analysis to compare urate-lowering response rate of febuxostat and allopurinol in gout patient.

This study aims to compare the urate-lowering response rate of febuxostat and allopurinol in gout patient using a model-based meta-analysis. The literature search identified 22 clinical trials of gout with a total of 43 unique treatment arms that met our inclusion criteria, and a total of 6 365 gout patients were included in the study. The response rates of allopuriol and febuxostat were characterized by Tmax model and Emax model respectively, and the effect of baseline serum uric acid (sUA) and patient type on the drug effect was tested. The results showed that allopurinol can reach an average maximum response rate of 50.8% while febuxostat can reach a 100% response rate within a very short time, and the ED50 was 34.3 mg. Covariate analysis revealed that baseline sUA has a negative effect on response rate of allopurinol, and a positive effect on the predicted ED50 of febuxostat. For patients who had shown inadequate response to prior allopurinol treatment, the average response rate was about half that of the allopurinol responder patients.

4.Making the scale for the compliance of patients with chronic obstructive pulmonary disease during home oxygen therapy

Objective To make the scale for the compliance of patients with chronic obstructive pulmonary disease (COPD) during home oxygen therapy.Methods By literature searching,semi-structured interview,expert consultation,as well as pre test,the tentative scale was formed at first.Then the investigation was conducted among 324 patients with COPD selected.Finally the scale was formed by testing the reliability and validity.Results The scale was made up of two dimensions with 16 items in total.It showed that Cronbach α coefficient was 0.984,split-half reliability was 0.976,and content validity index was 0.98.The exploratory factor analysis combined with AMOS 7.0 confirmatory factor analysis was used for construct validity of the home oxygen therapy compliance scale.Conclusions The self-management behavior scale for patients with COPD shows good reliability and validity,which can be used to evaluate the compliance during home oxygen therapy and provide reference tool for COPD patients' family intervention.
5.Impact of community healthcare service center in emergency response to natural disaster

The Medical Healthcare Service Center,located at the Youxian District,Mianyang City of Sichuan Province,successfully rescued the people affected by the Wenchuan earthquake and the Tangjiashan barrier lake with great help from the senior hospitals.We suggest that the community healthcare service center could play an important role in emergency response system.
6.Effects and mechanism of PNS on synaptic transmission in hippocampal CA1 region of rat

Objective To investigate the effects of Panax notoginseng saponins(PNS)on both the excitatory and inhibitory synaptic transmission in the pyramidal neurons in hippocampal CA1 region of rats.Methods Wistar male rats(3—4 weeks)were killed by cervical dislocation and hippocampal slices(400 ?m)were prepared,blind whole-cell voltage-clamp recordings were performed on the CA1 pyramidal cells in hippocampal slices to examine and analyze the effects of PNS(0.05—0.4 g/L)on CA1 afferent fiber-evoked excitatory postsynaptic currents(EPSCs)and inhibitory postsynaptic currents(IPSCs),respectively.Moreover,the Schaffer collateral/commissural pathway was stimulated with paired pulses(interpulse interval was 50 ms)and the paired-pulse facilitation(PPF)was analyzed by EPSC2/EPSC1(P2/P1)ratio.Results PNS(0.1—0.4 g/L)significantly depressed amplitude of EPSCs in neurons in the hippocampal CA1 region(P0.05).Conclusion The inhibitory effect of PNS on EPSCs in hippocampal CA1 pyramidal neurons is not due to the reinforcement of the inhibiting interneurons.It may be a result of direct inhibition on excitatory synaptic transmission.The increasing of P2/P1 ratio after PNS application suggests that PNS depresses the excitatory synaptic transmission by presynaptic mechanism.
7.Selection of short- or long-segment posterior fixation for treatment of unstable thoracolumbar fractures

Objective To discuss the selection of short- or long-segment posterior fixation for treatment of unstable thoracolumbar fractures. Methods A total 134 patients with thoracolumbar fractures treated with pedicle instrument fixation from January 2005 to December 2008 were studied retrospectively. According to AO fracture classification, there were 70 patients with type A fractures, 37 with type B and 27 with type C. The patients were divided into two groups according to the number of instrumented levels; short-segment posterior fixation (SSPF) group (four screws; one vertebral body above or below the fractured vertebrae) and long-segment posterior fixation (LSPF) group (eight screws; two vertebral bodies above or below the fractured vertebrae). Clinical outcomes and radiological parameters (superior-inferior endplate angle, vertebral body angle, displacement of vertebral body) were compared according to AO fracture classification. Results All type A fractures were treated with SSPF, mean superior-inferior endplate angle changed from preoperative 21.3° to postoperative 8.5° and 11.1° at final follow up. There was no statistical difference in the correction of Cobb angle for type B fractures in SSPF group (26 patients) and LSPF group (11 patients), while the correction loss of vertebral body angle was 3. 64° in SSPF group and 1.09° in LSPF group, with statistical difference (P < 0. 05). There was no statistical difference in the correction of Cobb angle for type C fractures in SSPF group (7 patients) and in LSPF group (20 patients), but the correction loss of vertebral body angle was 3.6° in SSPF group and 0. 8° in LSPF group, with statistical difference (P < 0. 05). There was no statistical difference in vertebral displacement correction. Conclusions Most types A and Bl fractures should be treated with SSPF; most types B2, B3 and C fractures should be treated with LSPF.
8.Research in correlation of professional commitment and communication ability among nursing baccalaureate students in universities of Traditional Chinese Medicine

Objective To investigate the level of professional commitment of nursing baccalaureate students,and explore the relationship between the professional commitment and communication ability.Methods Questionnaires were used to investigate 430 nursing baccalaureate students who were studying in the school.Results The professional commitment of nursing students was in the middle level.Correlation analysis revealed that the communication ability was correlated with the professional commitment.Conclusions We should take measures to improve the competency of professional commitment of nursing undergraduate students,strengthen their communication ability,promote the construction of nursing staff.
9.Analysis of the Histopathology of ASCUS Diagnosed by Liquid-based Cervical Cytology

Objective To analyze the histopathology of patients with atypical squamous cells of undetermined significance (ASCUS),and provide evidence for further classification and clinical treatment of ASCUS.Methods The histopathology of cervical biopsy specimens from 249 patients with ASCUS diagnosed by liquid-based cervical cytology examination was analyzed retrospectively. Results Among the 249 ASCUS patients, the proportion of patients with inflammation was 34.14% (85/249), morphological change by human papillomaviral infection was 19.28%(48/249), CIN I was 32.53%(81/249),CIN II was 8.84%(22/249),CIN III was 3.21%(8/249), infiltrating squamous cell carcinoma was 1.20%(3/249),and endometrioid adenocarcinoma was 0.80%(2/249) . Conclusion It is very important to to further definitude the diagnosis of ASCUS, because a certain proportion of cervical cancer and precancerous leisions woud be confirmed.

Objectives To study the FLT3 gene expression and its internal tandem duplication(ITD) mutation in acute nonlymphoblastic leukemia(ANLL) patients.Method Polymerase chain reaction was used to detect the FLT3/ITD mutation in 71 ANLL patients.Results The expression of FLT3 gene were detected in 65/71(91 5%) ANLL cases and ITD mutation was found in 17 cases. 23 9% of ANLL cases were found with FLT3/ITD mutation.
