1.Survey on Urban Environmental Electromagnetic Radiation Level in a City

Objective Along with the increase of various kinds of communication facilities in our life, various frequency-bands of electromagnetic waves have been filling up our living space, and the probability of exposed to electromagnetic radiation for human being is growing day by day. Environmental electromagnetic radiation pollution has become a new public nuisance. The present paper systematically investigated the levels of environmental electromagnetic radiation in some administrative region of a city. Methods According to the methods of national hygienic standards(a little of change), compartmentalized gridding with a definite space distance of 500 meters, levels of environmental electromagnetic radiation were measured on the spots of the gridding center. Results Levels of environmental electromagnetic waves of 5-10 000 MHz did not exceed the limits of corresponding national standards, but at some detection spots, the maximum of comprehensive electrical field strength had been close to or reached to the limits of ultrashort wave and microwave hygienic standards. The levels of electrical field strength of 5-10 000 MHz in regions of central sowntown and medical organization were significantly higher than those in regions of manufactory and residence. The level of magnetic field strength of 0.009-1 MHz in region of central sowntown was the highest. According to the results of selective-frequency measure of 28-500 MHz, most of frequencies of environmental electromagnetic waves with high electrical field strength were frequencies of frequency-modulated broadcasting stations in the city. Conclusion The urban environmental electromagnetic radiation level is more higher nowadays than before. The regions with high electrical field strength mostly lie in the central sowntown and medical organization regions, and more attention should be paid to its potential effects on human health.
2.Assessment of association between macrocalcification in thyroid nodule and thyroid carcinoma with ultrasound

Objective To determine the association between macrocalcificatoin in thyroid nodule and thyroid carcinoma. Methods Sonogram of 169 patients of thyroid nodule with macrocalcification was retrospectively analyzed. The ultrasonographic finding was compared with pathological results after excision. Macrocalcification was divided into three types and assessed the relation between each type macrocalcification and thyroid carcinoma.Results Macrocalcification was detected in 176 nodules, 122 were malignant, and 54 were benign. Irregular macrocalcification were more common in malignant nodule (82.9%, 34/41), the malignant risk of solitary macrocalcification were lower (58.5%,31/53),and the malignant risk of curvilinear macrocalcification were between irregular macrocalcification and solitary macrocalcification (69.5%, 57/82). Conclusion Ultrasonography can play an important role in differentiating benign and malignant thyroid nodule macrocalcification.
3.Fixing the vertical stress pedicle screws into the injured vertebra for thoracolumbar fractures

Objective To explore the technique outcome of fixing the ventral stress pedicle screws into the injured vertebrae, as a method to enhance the posterior internal fixation. Methods From March 2002 to March 2005, 33 single thoracolumbar fractures were studied retrospectively. Among which, 16 cases were treated with the above method(group A), and the other 17 were treated with traditional two-level fixation(group B). Group A involved 11 males and 5 females, aged 48 years averagely(range, 32-74 years); and group B included 12 males and 5 females, aged from 21 to 61 years(mean, 40 years). All the patients underwent the operation within up to 3 weeks after fracture. For the injured vertebral bodies, their pedicles were intact on either unilateral or bilateral side, and their lower half and endplate were free from split. In group A, the pedicle screws in the injured vertebrae were used to achieve the ventral press vertical to the distraction for the stress neutralization, and also with the routine distraction and lordosis restoration, simultaneously. The mean follow-up period was 11 months with a range from 6 to 24 months. Results After operation, the optimal Cobb angle and anterior column restoration were achieved through the ventral reduction from the injured vertebral body, which was the contribution from the vertical stress pedicle screw. The intermediate phenotype of vascular smooth muscle cell in adult is the dedifferentiation state returned from the high differentiation state , appeared on the damaged blood vessels. It is regulated by many factors. Its distribution, the characteristics of morphology and structure, the regulated transform factors and the molecular biological mechanism are introduced, and its functional significance and the role in vascular diseases are also discussed in this article.
5.Living donor kidney transplantation in child: a report of 14 cases

Objective To explore the benefits of living donor kidney transplantation in child, and to inves-tigate the procedures of renal transplantation, post-operative immunosuppressants regimens and complication management in child. Methods Clinical data, the prognosis of renal transplantation, operative procedure and immunosuppressive agents administrated in 14 children with end-stage renal disease (ESRD) undergo-ing renal transplantation were analyzed. Results The major complications in the early stage after renal transplantation were: 2 cases had acute rejection, 3 cases had severe hypernatremia, 1 case had urological infection, 3 cases had lymphocele. The major complications in the long-term recipients included: 6 cases had hypertension, 3 cases had hyperlipidemia, 4 cases had various kinds of infections, 5 cases had drug-in-duced hepatic injury. The one year survival rates of recipient/graft were all 100%. The average creatinine was 96.43 mmol/L(43-125 mmol/L) for the 14 recipients by the end of this study. All recipients thought their life quality was improved significantly. Conclusions Living donor kidney transplantation is the most ideal method to treat children with ESRD. Adult donor kidneys are suitable to the school age of recipients. The protocol of immunosuppressive therapy (prednisone plus MMF and FKS06) should be applied.
6.Effect of wavelength in the efficacy for treatment of non-proliferative diabetic retinopathy

Objective To compare the therapeutic effects of 577 nm laser and 532 nm laser panretinal photocoagulation (PRP) in the treatment of non-proliferative diabetic retinopathy (NPDR).Methods This is a prospective controlled study.A total of 23 patients (41 eyes) with clinically diagnosed severe NPDR were randomly divided into two groups including 577 nm group (11 patients,20 eyes) and the 532 nm group (12 patients,21 eyes).577 nm group and 532 nm group received 3-4 times PRP with single-point mode.The laser energy and the number of laser spots were compared,and the laser energy density was calculated.Before treatment and 1 day,1,3,6 and 12 months after treatment,the changes of best corrected visual acuity (BCVA),average threshold sensitivity,a/b-wave amplitude of flash ERG (F-ERG) in the 30°-60° visual field,and fundus fluorescein angiography (FFA) were compared between two groups.Results The response rate was 85.0% and 23.8%,respectively in the 577 nm and 532 nm group,the difference was statistically significant (x2 =15.43,P < 0.05).Compare to the pre-treatment measurement,the average threshold sensitivity,a/b wave amplitude of F-ERG and the 30°-60°visual field were reduced at 1 day after treatment both in the 577 nm and 532 nm group,the difference were statistically significant (F =8.68,7.57,4.52;P<0.05).The average threshold sensitivity (t=2.41,3.48,1.23),a/b wave amplitude (a wave:t=5.82,4.45,7.83;b wave:t=5.40,3.23,4.67) of F-ERG were different between 577 nm and 532 nm group at 3,6 and 12 months after treatment (P<0.05).There was no retinal neovascularization and non-perfusion region in two groups at 6 months after treatment.The average laser power were (436.25 ±54.65) and (446.43 ± 35.61) mW,number of laser spots were (1952.95 ± 299.09) and (2119.05 ± 302.69) spots,energy density were (7.60±1.30) and (7.60±3.00) mW× ms/μm2 in the 577 nm group and 532 nm group,respectively.There was no difference in average laser power (t=1.35),number of laser spots (t=2.85) and energy density (t=1.99) between two groups (P>0.05).Conclusion Compared with the 532 nm laser,577 nm laser treatment has better visual outcomes for NPDR patients.
7.Effect of compound anisodine on retinal function repair in diabetic retinopathy after panretinal photocoagulation

Background Panretinal photocoagulation (PRP) is an effective method for diabetic retinopathy (DR).However,PRP causes macular edema and visual impairment.The application of compound anisodine,a vascular dynamic drug can alleviate the adverse effect of PRP,but its effectiveness is not verified yet.Objective This study aimed to investigate the clinical therapeutic effect of compound anisodine on retinal functional damage following PRP in the eyes with non-proliferative diabetic retinopathy (NPDR).Methods A prospective cohort study was carried out from August 2013 to February 2014 in Beijing Tongren Hospital.One hundred and ten eyes with NPDR were included and PRP were performed.The operative eyes were randomized into the compound anisodine group (64 eyes) and control group (46 eyes).Compound anisodine solution of 2 ml was injected via temporal subcutaneous tissue since the second day after photocoagulation,and the injection was performed once per day for 4 courses in 3-day interval between each course (1 course for 14 days) in the compound anisodine group,and no any drug was used in the control group.The visual acuity,30° to 60° ring visual field and flash electroretinaogram (F-ERG) were examined before photocoagulation and 1 day,1 month and 2 months after photocoagulation to compare the retinal function between the two groups.ResultsThe vision acuity improved in 55 eyes in the compound anisodine group with the rate 85.94%,and that in the control group was 11 eyes with the rate 23.91%,showing a significant difference between the two groups (x2 =15.425,P =0.000).The mean sensitivities of visual field were (4.15 ± 1.42),(3.94 ± 1.40) and (4.81 ± 1.41) dB in 1 day,1 month and 2 months after photocoagulation in the compound anisodine group,which were significantly higher than (3.76± 1.52),(3.53± 1.55) and (3.64 ± 1.50) dB of the control group (t =1.39,1.44,1.15,all at P<0.05).The amplitudes of a-wave and b-wave of F-ERG were all higher in the compound anisodine group than those in the control group in various time points after photocoagulation (all at P<0.05).Conclusions The injection of compound anisodine via temporal subcutaneous tissue can relieve visual functional damage caused by PRP in NPDR eyes.
8.Comparison of the efficacy of multi-point or single-point mode of 577 nm laser in the treatment of diabetic retinopathy

Objective To compare the therapeutic effects of 577 nm laser panretinal photocoagulation (PRP) between one time multi-point scanning mode and multiple time single-point mode in the treatment of eyes with non-proliferative diabetic retinopathy (NPDR).Methods This is a prospective controlled study from August 2013 to February 2014.A total of 29 patients (46 eyes) with clinically diagnosed severe NPDR were randomly divided into two groups including the treatment group (12 patients,22 eyes) and the control group (17 patients,224 eyes).The treatment group received one time PRP of multi-point scanning mode,and the control group received 3-4 times of PRP with single-point mode.In order to evaluate its efficacy,the best corrected visual acuity was measured before treatment,and 1 day,1,2,6 and 12 months after treatment.The average threshold sensitivity,a/b-wave amplitude of flash ERG (F-ERG) in the 30 °-60 ° visual field,and fundus fluorescein angiography (FFA) of the change were also compared between the 2 groups.The laser energy and the number of laser spots were compared,and the laser energy density was calculated.Results The response rate was 86.4% and 79.2%,respectively in the treatment and control group,the difference was not statistically significant (x2 =0.414,P>0.05).Compare to the pre-treatment measurement,the average threshold sensitivity,a/b-wave amplitude of F-ERG in the 30 °-60 ° visual field were reduced at 1 day after treatment both in treatment and control group,the differences were statistically significant (P<0.05).The average threshold sensitivity,a/b-wave amplitude of F-ERG were no difference between treatment and control group at 2m,6m and 12m after treatment (P>0.05).The average laser power,number of laser spots and energy density were (537.50 ± 64.69) mW and (339.09 ± 132.09) mW,(1934.32 ± 426.38) points and (2061.42 ± 375.49) points,(0.35±0.12) mW o ms/μm2 and (1.95 ± 0.86) mW · ms/μm2 in the treatment group and the control group,respectively.The average laser power and energy density was statistically different between the 2 groups (P<0.05),while the number of laser spots was no difference (P>0.05).Conclusions 577 nm multi-point scanning laser can complete the PRP at one time,and achieve the same therapeutic outcomes with the single-point mode which need several times to complete the PRP in the eyes with severe NPDR,and have lower energy density,and thus relative minor function damage.
9.Effects of Repetitive Transcranial Magnetic Acupoint Stimulation on Hemiplegic Patients

Objective To investigate the effects of repetitive transcranial magnetic acupoint stimulation on hemiplegic patients.Methods60 hemiplegic patients were assigned to treatment group and control group(30 cases for each group). Both groups received rehabilitation therapy. Meanwhile, the treatment group received repetitive transcranial magnetic acupoint stimulation. Fugl-Meyer Assessment (FMA), The Mini-Mental State Examination (MMSE), Barthel Index(BI) were used to measure the outcome.ResultsThe scores of FMA and BI increased significantly in both groups after treatment. However, these scores in treatment group improved more compared with those of control group(P<0.05).ConclusionRepetitive transcranial magnetic acupoint stimulation is helpful to improve the motor function, and ability of daily living of hemiplegic patients.
10.Risk factors and association between cardio-ankle vascular index and carotid atherosclerosis in the patients with type 2 diabetes mellitus

ObjectiveTo study the association between cardio-ankle vascular index (CAVI) and carotid atherosclerosis as well as the risk factors in patients with type 2 diabetes mellitus.MethodsSixty type2 diabetic patients and 60 non-diabetic patients at the same hospitalization period were selected.The clinical features,laboratory test results such as free blood glucose ( FBG),blood lipids and other biochemical markers were recorded.Carotid intima-media thickness (IMT) and CAVI were measured.The relationship between CAVI and carotid atherosclerosis were analyzed; a multivariate linear regression analysis was performed between CAVI and risk factors of type 2 diabetes mellitus.ResultsThe body mass index,triglycerides,FBG,HbA1 c,IMT,the incidence of carotid artery plaque and CAVI values of diabetic group were significantly higher than that of nondiabetic group.Difference between the two groups was statistically significant ( all P < 0.05 ) ; CAVI and carotid IMT was significantly correlated to each other( r =0.435,P < 0.01 ) ; Multiple linear regression analysis showed that CAVI was significantly correlated to age (β =0.458,P <0.01 ),triglycerides (β =0.058,P =0.012),FBG (β =0.168,P =0.002) and HbAlc (β =0.228,P =0.003).ConclusionCAVI inpatients with diabetes was positively correlated with carotid IMT.Age,diabetes,triglycerides,FBG and HbAlc are independent risk factors for arterial stiffness.
