1.New Research of Nerve Growth Factor on Fracture Healing.

Fracture healing has long been a major concern for orthopedists.Currently,about 10% of fracture patients still have poor fracture healing or bone nonunion.In recent years,research has found that nerve growth factor(NGF)can promote fracture healing.This article reviews the mechanism and research progress of NGF in promoting fracture healing.NGF can promote vascular regeneration and nerve growth at callus and plays an important role in the proliferation and migration of bone cells.New animal experiments and clinical trials have confirmed the role of NGF in promoting fracture healing and further revealed its possible mechanism of action.Further research on NGF can provide new ideas for promoting fracture healing,especially for treating nonunion.

2.ROC Curve Based on Generalized Linear Mixed Effects Models in Repeated Diagnostic Tests

Objective To investigate the impact of covariates on diagnostic test and assess the correlation between re-peated measurement data,this paper explores innovative modeling techniques of ROC curve.Methods We introduce the new ROC curve method based on generalized linear mixed effects model and apply Bayesian techniques to parameters estimation with Winbugs Software.Further,areas under the ROC curve(AUC)with different values of covariates could be calculated in terms of assessment.Results Cases analysis results indicate the proposed method efficiently explores the repeated measurement data and provides parameters with practical significance,serving as a golden reference.Conclusion The ROC curve based on generalized linear mixed effects models can be effectively used to solve the test accuracy evaluation problem of the repeated diagnostic trials.
3.Percutaneous screw fixation of scaphoid fractures aided by computerized imaging processing: preliminary report

Objective To evaluate the preliminary clinical outcome of the screw fixation of minimally displaced scaphoid fracture assisted by computerized imaging reconstruction and preoperative planning system.Methods From March 2013 to Feburary 2014,Five screws on five patients were treated.We calculated and designed the position of scaphoid screw guide wire and ensured the guide wire centrally placed in the scaphoid by using the computerized imaging reconstruction and preoperative planning system software.The appropriate length of the screw was measured too in the software.We also identified the entry and exit points of the guide wire on the custom orthoses splinting the wrist joint.Five patients with scaphoid fractures were treated with percutaneous screw fixation under the guidance of a C-arm fluoroscopy and a guide wire were inserted based on the entry and exit points calculated preoperatively.All of the scaphoid fracture was nondisplaced or less than 1 mm.The modified MAYO wrist scoring system was used to evaluate the pain,function and range of motion for the wrist joint during follow up.Results Total surgical time was 28.8 min (25-39 min)and Fluoroscopy time was 18.4 s (11-23 s).The fracture healing was observed at routine 8 weeks follow up.No complication such as infection or fixation failure was reported.Modified MAYO wrist score was 83.8 (81-88)at average followup at 15 months (6-12 months).Conclusion Computer assisted percutaneous screw fixation is an effective method for the treatment of nondisplaced scaphoid fractures.It leads to significantly reduced guidewire placement time and radiological exposure time while provides accurate placement of the screw.
4.The experimental research anti-tumor effect of modified decoction for driving out blood stasis in the blood mannsion

Objective Modified Decoction for Driving Out Blood Stasis in the Blood Mannsion on ascites tumor model of transplanted H22 anti-tumor effect and the impact of VEGF. Methods Adult male mice 100, inoculated with H22 hepatoma cells, the establishment of ascites H22 transplanted tumor model, then divided into 10 groups were given saline, capecitabine, flavored Modified Decoction for Driving Out Blood Stasis in the Blood Mannsion (high, medium and low dose) was administered orally for 10 days, 11 days of treatment, observation of suppression tumor rate, the rate of change in life extension; detected by immunohistochemistry the expression of VEGF in tumors, SPSS13.0 software for statistical analysis. Results Capecitabine, flavored Xuefuzhuyutang (high, medium and low dose) inhibited tumor growth rates were 60%, 49%, 41%, 35%, life extension rate of the three groups were 1.68% 157.98%, 70.58%, 49.57% higher. Conclusion Modified Decoction for Driving Out Blood Stasis in the Blood Mannsion can inhibit tumor cell proliferation in tumor-bearing mice, significantly prolonged the survival time of mice, reduce the tumor tissue and tumor tissue expression of VEGF.

6.Effects of Suanzaoren Decoction on Expression of NMDAR1, NMDAR2A and NMDAR2B ;RNA in Cortex and Hippocampus of Depression Model Rats

Objectives To observe the effect of Suanzaoren Decoction on the expression of NMDAR1, NMDAR2A and NMDAR2B RNA in cortex and hippocampus of depression model rats, and explore the mechanism of treating depression. Methods Ninty SD rats were randomly divided into blank group, model group, western medicine group, Suanzaoren Decoction high, medium and low dose group. Except the blank group, the other groups were replicated the rat model of depression in chronic stress. The treatment groups were given corresponding drugs by gavage. The expression of NMDAR1, NMDAR2A and NMDAR2B genes in cortex and hippocampus were detected by RT-PCR. Results Compared with the model group, the positive expression of NMDAR in cortex and hippocampus decreased in Suanzaoren Decoction high and medium dose group (P<0.01). The positive expression of NMDAR2A in cortex and hippocampus of Suanzaoren Decoction high, medium dose group and western medicine group decreased (P<0.05, P<0.01), the expression of NMDAR2A in hippocampus increased in Suanzaoren Decoction low dose group (P<0.05). The positive expression of NMDAR2B in cortex decreased in western medicine group, Suanzaoren Decoction high, medium and low dose group (P<0.01). The expression of NMDAR2B in hippocampus increased in Suanzaoren Decoction medium and low dose group (P<0.01). Conclusion Suanzaoren Decoction can play the role of treating depression by decreasing the expression of NMDAR1, NMDAR2A and NMDAR2B RNA in cortex and hippocampus.
7.Analysis of detection of prenatal coagulation four indices,D-dimer and platelet in pregnant women

Objective To investigate the clinical significance of the antenatal detection of four indices of coagulation(PT,APTT, TT,FIB),D-dimer(DD)and platelet(PLT)in the judgment of parturient coagulation function and bleeding risk.Methods 795 pregnant women and childbearing age non-pregnant women with physical examination in this hospital from June 2012 to September 2013 were collected and divided into the normal pregnancy group(early pregnancy,middle pregnancy,late pregnancy,parturient pe-riod),obstetric accident group,pregnancy complicating other disease group and healthy control group.At the same time,PT, APTT,TT,FIB,DD and PLT were detected and the related statistical analysis was conducted.Results DD and FIB in the normal pregnancy group were significantly higher than those in the healthy control group(P<0.05),PT and APTT were significantly low-er than those in the healthy control group(P <0.05),TT had no statistically significant difference between these two groups(P >0.05 );DD and FIB in the obstetric accident group and the pregnancy complicating other disease group were significantly higher than those in the normal pregnancy group(P <0.05),while PT was significantly lower than that in the normal pregnancy group(P <0.05).PLT in the obstetric accident group and the pregnancy complicating other disease group was significantly lower than that in the normal pregnancy group(P <0.05).The sensitivity and specificity of the combined detection of 3 indices in diagnosing the ob-stetrical accidents were greatly superior to those of the single index detection or the combined detection of two indices.Conclusion The prenatal combination detection of four coagulation indices,DD and PLT can preliminarily judge the coagulation status of preg-nancy period in pregnant women,help to treat abnormal bleeding of pregnant women an possess the important guidance significance in formulating the labor scheme and preventing the occurrence of obstetric accidents.
8.Role of alarmins high mobility group protein B1 in sepsis

High mobility group protein B1 (HMGB1) is the most representative substance in the alarmins family, it is actively or passively release to extracellular by the activation of monocyte/macrophage and the dead cells, and then it stimulates the production of a variety of inflammatory mediators, and increases the organism's inflammatory response through relevant receptors signaling pathways. In recent years, its concentration can reflect the severity of inflammation and injury and was related to the prognosis, HMGB1 has won more and more attention in the development of sepsis. By reviewing the study of HMGB1 in sepsis pathogenesis, signal pathway and reversal measures, it was found that HMGB1 was considered as an important inflammatory mediators and warning signal involved in the pathogenesis of sepsis, and was become a new target in the treatment of sepsis. Further research on the role of HMGB1 in the pathogenesis of sepsis is needed in the future, and the development of new drugs combined with HMGB1 will be used in the study of HMGB1 in animal experiments.

10.The surgical safety analysis of elderly patients with thoracic esophageal squamous cell carcinoma

Background and purpose: The incidence rate of elderly esophageal carcinoma patients is increasing year by year. In this study, the clinicopathologic factors, operational factors and postoperative complications were compared between the older and the younger elderly patients with thoracic esophageal squamous cell carcinoma (ESCC), and the influence of different surgical approaches to older elderly patients were analyzed. Methods: A retrospective review of 371 cases of elderly patients with thoracic esophageal squamous cell carcinoma (≥60 years) between Jan. 2006 and Dec. 2008 were performed. The patients were divided into two age groups. The patients over 75 years old named the older elderly group including 53 patients, and the patients between 60-74 years named the younger elderly group including 318 patients, the clinicopathological factors, operational factors, postoperative hospital mortality and average length of stay were compared. Meanwhile, according to different surgical approaches, the comparison of the left chest and right chest approach of operative time, blood loss and postoperative complication rate, and so on and so forth. Results: The older elderly group compared with the younger elderly group had more preoperative complications. Preoperative ASA classiifcation and postoperative complications, length of stay and hospital mortality rates were signiifcantly higher. The older elderly group over the right chest approach had less postoperative complications, especially pulmonary complications, the results were statistically signiifcant.(13.0%vs 40.0%,P<0.05) Conclusion: The patients over 75 years old with thoracic esophageal squamous cell carcinoma have a higher risk during the operation, we can choose relatively simpler operation approach to reduce the incidence of postoperative complications, especially pulmonary complications.
