Objective To observe the influences of combined general and epidural anesthesia on hemodynamics.Methods Twenty one patients were randomly divided into two groups, general anesthesia (GA group, n=11) and combined general and epidural anesthesia (GA+EA group, n=10). With Swan Ganz technique, hemodynamic parameters were measured 5min following 0.6 and 1.0 MAC enflurane inhalation in GA group, and 5min following 0.6 MAC enflurane inhalation and 10min after combined epidural lidocaine in GA+EA group.Results Compared with the baselines, in both groups with 0.6 MAC enflurane inhalation, MAP decreased significantly (P0.05); in GA group with 1.0 MAC enflurane inhalation, MAP, HR, SVR, SI and CI decreased significantly (P0.05). Objective To observe the therapeutic effect of scleral buckling procedure on old retinal detachment. Methods The clinical data of 42 patients (46 eyes), including 24 males (27 eyes) and 18 females (19 eyes), with old retinal detachment treated by scleral buckling procedure in our department were retrospectively reviewed. The duration of the disease ranged from 1 month to 2 years. All the patients were with rhegmatogenous retinal detachment and combined with mainly predominantly-subretinal proliferative vitreoretinopathy (PVR) (stage C), including stage C1 of PVR in 16 eyes (34.8%), stage C2 in 19 eyes (41.3%), and stage C3 in 11 eyes (23.9%). Scleral buckling was performed on 13 eyes (283%) and cerclage combined buckling on 33 eyes (71.7%). Sterile air was injected into 36 eyes (78.3%) during the operation, and C3F8 was introvitreal injected into 7 eyes (15.2%) after the operation. Results The follow-up duration was from 6 months to 1 year (mean 7.3 months). Retina was completely reattached in 31 eyes (67.4%), and was alleviated obviously in 12 eyes (26.1%). The subretinal fluid increased after the operation with un-reattached retina and vitrectomy was performed in 2 eyes. One eye underwent vitrectomy due to the development of PVR. After the first operation, the curative ratio of retinal detachment was 67.4%, and effective ratio (cure and alleviation) was 93.5%. The visual acuity improved in 28 eyes (60.9%), kept no change in 11 eyes (23.9%), and decreased in 7 eyes (15.2%). Conclusion Reattachment of retina and improvement of visual acuity can be achieved in some degree in some patients with old retinal detachment who undergo simple scleral buckling procedure without vitrectomy.

Objective To study the value of helical multi-phase contrast scanning in the preoperative evaluation of cholangiocarcinoma hilar.Methods Pre-and post-contrast CT scanning of upper abdomen were performed in 26 cases with hepatic portal cholangiocarcinoma.Arterial phase scan of the whole liver began at 30 s after initial injection of contrast medium and the portal venous phase scan started at 70 s.15 of 26 cases underwent additional delayed phase scans began at 3~4 min.Thin thickness(3~5 mm)scanning of hepatic portal was used in each post-contrast phase scanning.The location,size,enhancement patterns of the lesions and the extent of the tumor involvement were observed and surgical resecetability was evaluated.Results All of 26 lesions were detected in helical multi-phase contrast scanning. 16 lesions enhanced on the arterial phase. On the portal venous phase, all lesions were enhanced and marked enhancement was seen in 19 lesions. Prolonged enhancement was seen in 15 lesions on the delayed phase scanning. The accuracy of evaluating surgical respectability was 84.62%. Conclusion Multi-phase contrast scanning combined with thin thickness scanning can demonstrate obviously the location and enhanced patterns of hepatic portal cholangiocarcinoma, the accuracy in evaluating surgical respectability is improved, and it is helpful in choosing the preoperative treatment program and assessing the prognosis.

Objective To investigate the regulatory effects of Zishen Yutai Pills on the expression levels of homeboxA10 (HOXA10) and its downstream target gene empty spiracles homebox 2 (EMX2) in the endometria of ovulation-inducing mice at different implantation stages. Methods Seventy-five estrous female Kunming mice were randomly divided into 5 groups, namely normal group, model group 1, model group 2, treatment group 1, treatment group 2, 15 mice in each group. The model group 1 was given short-term protocol for ovulation induction; the model group 2 was given long-term protocol for ovulation induction; the treatment group 1 was given Zishen Yutai pills (at the dose of 0.4 g/mL) on the basis of the protocol for the model group 1; the treatment group 2 was given Zishen Yutai Pills (at the dose of 0.4 g/mL) on the basis of the protocol for the model group 2; the normal group was given intragastric administration or intraperitoneal injection of the same volume of normal saline. The mRNA and protein expression levels of HOXA10 and EMX2 in mouse uterus were detected by real-time fluorescent quantitative polymerase chain reaction (qPCR) and Western blot method, respectively. Results Compared with the normal group, the mRNA and protein expression levels of HOXA10 were decreased, and the mRNA and protein expression levels of EMX2 were increased in model group 1 and model group 2(P< 0.01). Compared with the corresponding model group 1 and 2, the mRNA and protein expression levels of HOXA10 were significantly up-regulated (P < 0.01) , and the mRNA and protein expression levels of EMX2 were decreased in the treatment group 1 and 2 (P < 0.01), respectively. Conclusion Zishen Yutai Pills may improve mouse endometrial receptivity by up-regulating HOXA10 expression and inhibiting EMX2 expression.

BACKGROUND:It is discovered recently that miRNA is a new regulator that is able to have an impact on gene expression and miRNA contributes to proliferation, differentiation and self-renewal of pluripotent stem cels.
OBJECTIVE:To investigate the mechanism by which miR-155 regulates chondrogenic differentiation of bone marrow mesenchymal stem cels.
METHODS: Sixty healthy Sprague-Dawley aged 12 weeks were randomized into study group and control group, with 30 in each group. Under anesthesia, rats were sacrificed to harvest bone marrow of the lower limbs. Then bone marrow mesenchymal stem cels were isolated, cultured, and transfected with miR-155 mimics in the study group and a negative control sequence in the control group. After chondrogenic induction, RT-PCR was used to detect the expressions of Sox9, Colagen II, Aggrecan and Colagen X gene, and western blot assay to detect the expression of Sox9 and Runx2 proteins.
RESULTS AND CONCLUSION:Compared with the control group, the mRNA expressions of Sox9, Colagen II and Aggrecan were higher, but the mRNA expression of Colagen X was lower in the study group (P < 0.05); the protein expression of Sox9 was higher, but the protein expression of Runx2 was lower in the study group (P < 0.05). These findings indicate that miR-155 promotes the chondrogenic differentiation of bone marrow mesenchymal stem cels and moreover, it can suppress the hypertrophy trend of bone marrow mesenchymal stem cels differentiating into chondrocytes.

Objective To explore the effect of N2O with High Intensity Focused Ultrasound (HIFU) on SKVO3 through observing the influence of HIFU and N2O, separated and combined, on SKVO3. Methods The cultured SKVO3 cells were divided into 4 group: control group: without any treatment; N2O group: blowed by N2O 60 minutes; HIFU group: irradiated by HIFU (250W, 5s) and N2O-HIFU group blowed by N2O 60 minutes and irradiated by HIFU (250W, 5s). Flow cytometric method and Annexin V-FITC/PI binding assay were applied to detect cell death and apoptosis, and western blot test to detect Cytochrome C, Capase 3, Bax, Bcl-2 and GAPDH. Results The cell death and apoptosis in N2O-HIFU group were significant higher than those in other groups(P < 0.05). The results of western blot showed that in N2O-HIFU group, CytochromeC, Caspase 3 and Bax increased remarkably, but Bcl-2 significantly suppressed. Conclusion HIFU combininh with N2O could enhance the killing of SKVO3 cell.

BACKGROUND:Induced pluripotent stem cel technology have solved the contradiction between the ethics and immune rejection, and this high-efficient and safe technique is becoming the mainstream of today’s research. OBJECTIVE:To comprehensively review the safety and application of induced pluripotent stem cel s.METHODS:A computer-based online retrieval of PubMed and CNKI was performed to search relevant papers published from January 2006 to April 2016, with the key words of“induced pluripotent stem cel , reprogramming, clinical application, safety, transcription factor, disease mode”in English and Chinese, respectively. RESULTS AND CONCLUSION:In recent years, research on induced pluripotent stem cel s has attracted much attention from the scientific community and the medical community, and this technique has successful y gained induced pluripotent stem cel s and overcome the problems of immunity and ethics. However, it is limited to the theoretical and laboratory research due to the inability to solve the safety, efficiency and re-differentiation mechanism of induced pluripotent stem cel s. Therefore, we are faced with enormous difficulties and chal enges, which involve al aspects of basic research, including how to safely and effectively induce the differentiation of induced pluripotent stem cel s into the desired cel type and how to establish a suitable disease model as wel as a high-throughput drug screening platform.

The forkhead box O (FoxO) transcription factor family plays an important role in cell functions, including metabo-lism, apoptosis, cellular proliferation, stress reactions, DNA repair, and immune response. As a member of this family, forkhead box O3a (FoxO3a) regulates its target genes by modulating histone modifications, including phosphorylation, acetylation, and methylation. FoxO3a expression is abnormally downregulated in urologic neoplasm. Protein modifications and FoxO3a activity are mainly con-trolled by PI3K/Akt signal pathway and other signaling pathways. FoxO3a is also involved in the initiation, progression, and prognosis of urologic neoplasm. This review focuses on the function of FoxO3a in urologic neoplasm and elucidates the regulatory mechanisms involved. This article will provide novel strategies to clinical diagnosis and drug therapy of urologic neoplasms.
