2.Laparoscopic resection of gastric stromal tumors:a report of 32 cases

Objective To investigate the clinical value of laparoscopic resection of gastric stromal tumors.Methods Thirty-two cases received this new type of operation.The tumors ranged in size from 1.5 to 5.5cm with a mean diameter of 2.6 cm.The operative methods included the full laparoscopic resection of(gastric) tumor and the hand-assisted laparoscopic resection of gastric tumor.Results All cases were(successfully) resected and no complications were observed.The mean operative time was 75min.The mean(intraoperative) blood loss was 50ml.Postoperative pain was slight.Postoperative flatus and feces were passed at a mean of 34 hours,and average postoperational hospital stay was 7.5days.Postoperative pathologic(examination) confirmed that 25 cases were benign GIST and 7 cases were of low-grade malignancy.No(recurrences) were observed at follow up of 8 to 30 months.Conclusions Laparoscopic resection of gastric(stromal) tumor is a technically simple,safe and effective procedure that could be widely used.
3.Expression of CD4+, CD25+ regulatory T cells and its significance in carcinoma of stomach

Objective To study the expression of CD4+, CD25+ regulatory T cells and its significance in carcinoma of stomach. Methods Flow cytometry was used to quantify and analyze the results of CD4+, CD25 + regulatory T cells and CD8+ T cells in 76 patients with gastric cancer. Results CD4+, CD25+ regulatory T cells expression in gastric cancer tissue, adjacent tissues and control group are (1.2±0.9) %, (6.4±1.1) % and (4.1±0.8) %. The results have statistical significance (P<0.05). CD8+ T cells markedly de-creased in gastric cancer tissue, while CD4+ , CD25+ regulatory T cells increased. But this manifest did not happen in adjacent tissues.Conclusion CD4+, CD25+ regulatory T cells suppress gastric cancer though inhibiting CD8+ T cells.
4.Effect of miR-19a on lipid catabolism in hepatocyte LO2

[ ABSTRACT] AIM: To observe the effect of microRNA-19a ( miR-19a) on the lipid catabolism of hepatocyte LO2, and to explore the potential mechanism.METHODS: miR-19a was over-expressed or silenced by transfection of miR-19a mimics or miR-19a inhibitor into LO2 cells, then the mRNA level of miR-19a was detected by real-time PCR.The potential target of miR-19a was found by the method of bioinformatics through internet website.The effect of miR-19a on the 3’ UTR of peroxisome proliferator-activated receptorα(PPARα) was measured by dual luciferase reporter assay, and the protein level of PPARαand its 2 major downstream rate-limiting enzymes involved in lipid catabolism, acyl-coenzyme a dehydrogenase (ACADM) and carnitine palmitoyltransferase 1A ( CPT1A), were detected by Western blotting.Mean-while, the effect of miR-19a on the generation of ketone body was measured by beta-hydroxybutyric acid (β-OHB) detec-tion assay.RESULTS:The mRNA level of miR-19a was dramatically elevated by the transfection of miR-19a mimics, and sharply decreased by the transfection of miR-19a inhibitor (P<0.05).PPARαwas found as a potential target of miR-19a, and dual luciferase reporter assay and Western blotting confirmed the regulatory effect of miR-19a on the expression of PPARα, with the protein level changes of ACADM and CPT1A.miR-19a mimics down-regulated, while miR-19a inhibitor up-regulated the concentration ofβ-OHB in LO2 cells (P<0.05).CONCLUSION:miR-19a regulates the lipid catabo-lism of hepatocytes by targeting the PPARαand its 2 downstream rate-limiting enzymes.
5.Cephalometric comparison of soft-tissue morphology between extraction and nonextraction orthodontic treatment in borderline cases

Objective:To compare soft-tissue morphology changes by cephalometric measurements be-tween extraction and non-extraction orthodontic treatment in borderline cases.Methods:The samplesconsisted of 33 cases selected as borderline cases by 5 orthodontic specialists.They were divided into 21extraction cases(including 13 four first premolar extraction cases and 8 four second premolar extractioncases)and 12 non-extraction cases by checking patients' treatment records.Conventional cephalometricanalysis was made to compare soft tissue structures before and after orthodontic treatments and the samecomparison was made between two different extraction patterns.Results:No statistical difference wasfound in pretreatment soft-tissue morphology between extraction and non-extraction groups divided fromborderline cases.The PosBs/FH of the four first premolars extraction group was smaller than that of non-extraction group,and the Ns-Sn-Pos of the four first premolars extraction group was smaller than that offour second premolar extraction group.None of the post-treatment soft-tissue measures showed significantstatistical differences between four first premolars extraction group and non-extraction group,but therewere 6 items showed significant statistical differences between four second premolars extraction group andnon-extraction group.Compared with extraction and non-extraction treatments,the most significant soft-tissue changes were:PosBs/FH,LL-SnPos,and Bs-EP.Conclusion:Although pre-treatment soft-tissuemorphology of second premolar extraction group was close to that of non-extraction group,the post-treat-ment soft-tissue morphology of first premolar extraction group became closer to that of non-extractiongroup.Compared with non-extraction treatment,the more significant changes caused by extraction treat-ment were located in the lower lip and chin,but not the upper lips.
6.Cardioprotective effect of RISK signaling pathway on diazoxide postconditioning in isolated ischemic and reperfused rat hearts

Aim To discuss whether specific mitochon-drial ATP-sensitive potassium channel opener diazoxide ( DZ ) postconditioning activates RISK signaling path-way to protect isolated rat hearts against ischemica reperfusion injury ( IRI ) . Methods Langendorff de-vice was used to establish rat in vitro model of myocar-dial ischemia reperfusion. SD rats were randomly di-vided into normal group ( NOR ) , control group ( CON ) , diazoxide after treatment group ( DZ ) , and LY294002 antagonistic nitrogen Triazine group ( DZ +LY) , with 8 cases in each. The following was com-pared:①whether heart function of each group changed at the end of equilibration and reperfusion; ② at the end of myocardial perfusion and separation, protein was extracted, and protein kinase B ( PKB / Akt ) , P70S6 kinase (P70S6K), endothelial nitric oxide syn-thase ( eNOS) phosphorylation level of expression were analysed by Western blot. Results ① Indicators of changes in heart function: for DZ group at the end of reperfusion , HR , CF , LVDP , LVEDP , +d p/d tmax and -dp/dtmax were significantly better than those in CON group and DZ + LY group ( P <0.01 ) , but worse than those in NOR group ( P <0.01 ); there was no statistical difference in cardioac function at the end of equilibration. ② For DZ group at the end of reperfu-sion Akt, P70S6K, eNOS phosphorylation level of ex-pression were significantly higher than those in NOR group, CON group, and DZ + LY group (P<0.01). There was no difference in expression level of ERK1/2 phosphorylation ( P >0.05 ) . Conclusion Diazoxide postconditioning through the activation of RISK signa-ling pathway can protect isolated rat hearts against is-chemia reperfusion injury.
7.Laparoscopic repair of esophageal hiatal hernia.

This study was to appraise safety and feasibility of laparoscopic approach and investigate the clinical effects of laparoscopic tension-free repair of esophageal hiatal hernia using mesh. From August 2006 to July 2009, 24 patients with esophageal hiatal hernia underwent laparoscopic repair. Twenty-three patients received laparoscopic tension-free repair using mesh, at the same time, Toupet or Dor partial fundoplication was performed. One patient was converted to open surgery. The average operating time was 90 min (70-210 min) and the blood loss was between 10-110 mL. There was no death. The mean postoperative hospital stay was 5 days (3-30 days). During a follow-up period of 12-20 months (mean 15 months), there was no recurrence of the hernia, and no complication with use of mesh. The present study suggested that laparoscopic approach was secure and minimally invasive operation for esophageal hiatal hernia and the use of mesh could reduce recurrence rate.

10.Role of PI3K/Akt/GSK-3β signaling pathway in mitigation of ischemia-reperfusion injury by diazoxide postconditioning in isolated rat hearts

Objective To evaluate the role of phosphoinositide 3 kinase/protein kinase B/glycogen synthase kinase 3β (PI3K/Akt/GSK-3β) signaling pathway in mitigation of ischemia-reperfusion (I/R) injury by diazoxide postconditioning in isolated rat hearts.Methods Pathogen-free Sprague-Dawley rats were used in the study.Thirty hearts were excised and passively perfused in a Langendorff apparatus with oxygenated K-H solution at 37 ℃.The hearts were randomly divided into 5 groups (n =6 each) using a random number table:control group (group C),I/R group,diazoxide postconditioning group (group DZ),PI3K inhibitor LY294002 group (group LY),and diazoxide postconditioning + LY294002 group (group DZ + LY).In group C,the hearts were continuously perfused with K-H solution for 70 min.In group I/R,the hearts were perfused with cardioplegic solution 4 ℃ ST-Thomas 10 ml/kg,the perfusion pump was then stopped to induce global ischemia,and 40 min later the hearts were perfused with K-H solution for 30 min.In DZ group,5 min of retrograde perfusion with diazoxide 50μmol/L was performed through the aorta starting from the onset of reperfusion.In LY group,5 min of retrograde perfusion with LY294002 15 μnol/L was performed through the aorta starting from the onset of reperfusion.In LY + DZ group,5 min of retrograde perfusion with LY294002 15 μmol/L was performed through the aorta starting from the onset of reperfusion,followed by 5 min of retrograde perfusion with diazoxide 50 μmol/L.At 20 min of stabilization (T1) and 30 min of reperfusion (T2),heart rate (HR),coronary flow (CF),left ventricular developed pressure (LVDP),left ventricular developed pressure (LVEDP) and ± dp/dtmax were measured.The expression of total Akt (t-Akt) and total GSK-3β (t-GSK-3β) and phosphorylation of Akt and GSK-3β in myocardial tissues were determined by Western blot.Results Compared with C group,HR,LVDP and ± dp/dtmax were significantly decreased,and LVEDP was increased at T2 in the other four groups,CF was decreased in I/R,LY and DZ + LY groups,and the phosphorylation of Akt and GSK-3β in myocardial tissues was increased in DZ group.Compared with I/R group,HR,CF,LVDP and ± dp/dtmax were significantly increased,and LVEDP was decreased at T2,and the phosphorylation of Akt and GSK-3β in myocardial tissues was increased in DZ group,and no significant changes were found in the phosphorylation of Akt and GSK-3 β in LY and DZ + LY groups.Compared with DZ group,HR,CF,LVDP and ± dp/dtmax were significantly decreased,LVEDP was increased,and the phosphorylation of Akt and GSK-3β in myocardial tissues was decreased in LY and DZ + LY groups.Conclusion PI3K/Akt/GSK-3β signaling pathway is involved in the mechanism by which diazoxide postconditioning mitigates I/R injury in isolated rat hearts.
