1.Antiepileptic drug valproic acid and topiramate effects on body weight and plasma leptin levels:a meta analysis

Objective To systematic evaluation the antiepileptic drug valproic acid and topiramate effects on body weight and plasma leptin levels.Methods By searching the Cochrane Library,PubMed,EMBASE,CNKI,VIP Chinese Scientific Journals Database and Articles Database (from building a database to August 1,2014),qualified RCT was chosen from those articles according to inclusion and exclusion criterias and evaluated their quality.Then the meta-analysis was performed by Rev Man 5.3.Results (1) A total of 70 documents retrieved for the evaluation,12 of the literatures could be incorporated into the Meta-analysis,including a total of 634 participants,9 literatures using valproate to treat epilepsy,4 literatures used topiramate.(2)Meta-analysis:compared with other drugs,valproic acid treatment increased body mass index and leptin level in patients with epilepsy,BMI combined MD=0.88(95% CI:0.45-1.31),leptin level in the combined MD=0.58(95% CI:0.07-1.09);compared with other drugs,topiramate affect body mass index and leptin level in patients with epilepsy had no statical difference,BMI combined MD=-0.02 (95% CI:-0.62-0.58),leptin levels in the combined MD =-0.05(95% CI:-0.31-0.20).Conclusion Antiepileptic drug valproic acid may increase patients' BMI and serum leptin levels,yet topiramate in do not have influence in patients BMI and serum leptin levels;more basic and clinical researches are needed to explore serum leptin levels and the exact therapy mechanisms of antiepileptic drugs for clinicians to select antiepileptic drugs.
2.A correlative study of MBP and IL-16 in cerebrospinal fluid and serum of patients with multiple sclerosis

Objective To detect the levels of MBP and IL 16 in cerebrospinal fluid(CSF) and serum, investigate the correlation between them and the role in multiple sclerosis attacking process.Methods The levels of MBP and IL 16 of cerebral fluid and serum of 31 patients with MS were detected by ELISA and compared with those of 24 cases of inflammatory demyelinating polyneuropathy or 22 cases of healthy persons.Results CSF and serum levels of MBP were found increaed significantly in MS patients compared with those in IDP and in healthy controls(HC)(all P 0.05).The concentrations of MBP in CSF of MS significantly correlated with that of IL 16 in CSF( r =0.468, P 0.05).The concentration of MBP in CSF of MS significantly correlated with CSF that in serum( r =0.505, P 0.05).Conclusion MBP is one of the autoantigens that induce MS,IL 16 may be mainly produced in CNS due to MBP stimulation; IL 16 may act within CNS.
3.Diagnostic values of antibody to citrullinated human fibrinogen in rheumatoid arthritis

Objective: To study the prevalence of antibody to citrullinated human fibrinogen(ACF) in rheumatic diseases and the diagnostic values in rheumatoid arthritis.Methods: Human fibrinogen was deiminated by the peptidylarginine deiminase(PAD) in the presence of Ca~(2+) in vitro.ACF was detected by ELISA in 352 patients with rheumatic diseases(183 RA,121 SLE and 48 OA) and 108 healthy controls.The sensitivity and specificity in RA were calculated and the correlations between ACF and the clinical and laboratory parameters were analysed by SPSS statistical software.Results: Compared with SLE(25.62%),OA(18.75%) and healthy control(2.78%) respectively,the patients with RA had a significantly higher level of ACF(67.21%,P
4.Three dimensional orthomorphia treatment of King type Ⅲ idiopathic scoliosis transthoracic anterolateral

[Objective] To study the results of three dimensional orthomorphia treatment of king type Ⅲ idiopathic scoliosis transthoracic anterolateral.[Method]From Sep.2003 to Sep.2005,clinical data of applications of TSRH,CDH-M8 to 12 patients of king type Ⅲ adolescent idiopathic scoliosis transthoracic anterolateral were investigated with practicing ertebral solution centrum,centrum three dimensional orthopedic,bone graft fusion internal fixation,and analyzing and to appraising their operation complications,scoliosis correction rats and follow-up results.[Result]Postoperative complications of bellows were found in two examples,one was pulmonary closure and one was respiratory tract obstruction,and they had been soothed by trachea cannula and assistor respiration of respirator.[Conclusion]There weren't injury of great blood vessel,spinal injury,pneumonia and chylothorax in all cases.Cobb's angle of thoracic vertebra was 48?~66? preoperatively,and the average was 52.6?.The average Cobb's angle of remnant vertebra was 15.2? postoperatively,the rectification rat was 77.8%.The follow-up ranged 12 to 36 months,12 patients of king type Ⅲ gained good segment bone fusion.The complications of pseudarthmsis,internal fixation loosening and fracture and broken compensation haven't been found.Three dimensional orthomorphia treatment of King type Ⅲ idiopathic scoliosis transthoracic anterolateral is a safe and effective method of internal fixation.
5.Comparative study on the efficacy and safety of zoledronic acid and sodium ibandronate in treating bone metastasis in breast cancer patients

ObjectiveTo retrospectively evaluate the efficacy and safety of zoledronic acid and sodium ibandronate in treating bone metastasis in breast cancer patients.MethodsThe study included 47 patients who were treated with zoledronic acid and sodium ibandronate respectively from Aug.2006 to Mar.2011. KaplanMeier curve and Log rank test were adopted to detect the difference in survival time of skeletal related event (SRE) and survival rate between patients treated with different medicine,and x2 test was uscd to rcveal thc rate difference of pain killing effects and adverse effects.Results 1.The total effective rate of pain killing was 88.9% and 85% respectively.The difference had no statistical significance(P =0.467).2.The 1,2 and 3-year-survival rate in zoledronic acid group and sodium ibandronate group was 88.7%,44.4%,24.2%vs 94.7%,40.5%,5.8%.The difference had no statistical significance(P =0.744).3.The 2,3,and 4-year-survival rate in zoledronic acid group and sodium ibandronate group was 70.4%,40.7%,23.1% vs 85%,46.7%,17.5%.The difference had no statistical significance( P =0.994).4.The 1,2 and 3-year SRE-free overall survival rate of the group with metastasis first to bone only was 92%,50.8%,and 23.8%,while the rate of the group with first metastasis to visceral organs was 85.4%,21.4%,and 5.3% ( P =0.012).5.The 2 and 3-year overall survival rate of the group with metastasis first to bone only was 95.8% and 74.2% respectively,while the rate of the group with first metastasis to visceral organs was 56.5% and 10.1% ( P <0.001 ).The difference of 4 and 5 had statistical significance.The difference of zoledronic acid group and sodium ibandronate group in rate of adverse effects had no statistical significance.Conclusions Compared to sodium ibandronate,zoledronic acid has no superiority in either delaying the occurrence of SRE or improving overall survival rate.The -2are similar in incidence of adverse effects and pain control.The prognosis of patients whose first presentation is complicated by metastasis to other organs is poor.
6.THE SIGNIFICANCE OF USING MODIFIED WEBSTER SCALE IN EVALUATING THE EFFECT OF SURGICAL TREATMENT ON PARKINSON DISEASE

With Modified Webster Scale, 42 patients suffered from Parkinson disease were examined before and after operation. The symptoms were divided into 10 groups. 1~10 points was taken as slight disability, 11~20 as medium, and 21~30 as severe. Improvement less than 25% was taken as no effect, 26%~50% as effective, 51%~75% as significantly effective, and over 75% as very significantly effective. The score was (19 90?5 27) preoperatively and (10 57?6 10) postoperatively. The mean improvement was (8 95?3 37). Very significantly effective result was obtained in 4 cases (9 5%), significantly effective sesult was obtained in 17 cases (40 5%), and effective result was obtained in 16 cases (38 1%). The total effective rate was 88 1%. One target coagulation was performed in 34 cases, the mean improvement was in (8 59?3 53). Two target coagulation on one side was performed in 8 patients, the mean improvement was in (10 00?4 14). No significant difference was found between these two groups statistically. It is suggested, for patients with Parkinson disease in whom drug treatment is not efficacious or because of increased side effect, Modified Webster Scale is useful to evaluate the effect of surgical treatment. This scale is very easy to use especially for neurosurgeons.
7.Effect of telmisartan on serum adiponectin, urine microalbumin of obese mice

Objective To evaluate the effect of telmisartan on serum adiponectin and urine microalbumin(mAlb) level by administrating it to mice with simple obesity, so as to explore new therapies for obesity-related kidney diseases.Method A total of 24 8-week-old male OB mice and 8 8-week-old male C57 mice were selected for this study.The genetic background of OB mouse was C57 mouse, but the lepin gene was deleted in OB mouse.OB mice were randomly divided into 3 groups by body weight and fed with high-fat diet for 12 weeks: model group (M group), telmisartan group (T1 group) and losartan group (T2 group).C57 mice acted as control and were fed with general diet for 12 weeks.Serum adiponectin and blood glucose levels were measured before and after treatment.24 h urine was collected to measure urine mAlb.Quantitative real-time PCR (qRT-PCR) was used to measure the expression level of peroxisome proliferators-activated receptors gamma mRNA(PPARγ mRNA).HE stain was used to observe the morphological changes of kidney and measure the glomerular diameter.Body weight, serum adiponectin level, blood glucose level, urine mAlb level, expression level of kidney PPARγ mRNA, kidney wet weight and glomerular diameter of 4 groups were compared and a correlation analysis was carried out by Person correlation coefficient.Results Compared with C group, the urine mAlb level in M group increased (P<0.01), serum adiponectin level and the expression level of kidney PPARγ mRNA in M group decreased (P < 0.01);The urine mAlb level was negatively correlated with serum adiponectin level and expression of kidney PPARγmRNA (r=-0.773,P < 0.01;r=-0.469, P < 0.01).The urine mAlb level in T1 group was lower than M group (P < 0.05),serum adiponectin level and expression of kidney PPARγmRNA in T1 group were higher than M group (P < 0.05).Compared with T2 group, the urine mAlb level in T1 group decreased, serum adiponectin level and expression of kidney PPARγ mRNA in T1 group increased (P < 0.05);Compared with M group, the urine mAlb level in T2 group decreased (P=0.01).The morphological changes of kidney:glomerular volume increase and focal segmental sclerosis were found in some mice in M group and T2 group.No glomerular volume increase and focal segmental sclerosis were observed in T1 group.Conclusions Telmisartan can reduce urine microalbumin, whose mechanism might be that telmisartan can active the PPARγand promote the level of serum adiponectin.
8.The Relationship between Orexin A and Respiratory Drive in Patients with Chronic Obstructive Pulmonary Disease

Obejective To explore the correlation of Orexin A and respiratory drive in chronic obstructive pulmo-nary disease (COPD) patients. Methods Pulmonary function was tested in 30 stable COPD patients and 20 normal healthy adults. Plasma orexin A level was measured with a radioimmunoassay kit. The correlation of Orexin A with BMI, age, BDI, FEV1, FVC, FEV1/FVC, MEP, MIP, P0.1 in COPD patients was analyzed. Results Plasma Orexin A levels in patients with COPD group(1.87±0.43)ng/L was higher than those in the control group(1.49±0.19)ng/L, P<0.01. Plasma Orexin A lev-els in patients with COPD correlated negatively with FEV1(r=-0.389,P < 0.05),and correlated positively with P0.1(r=0.728,P<0.01). Conclusion Plasma orexin A in COPD patients increased which may be caused by smoking and hyper-capnia. Orexin A may play a crucial role in regulating respiratory drive in COPD patients.

10.Application of mind map in experiment teaching of fundamental nursing

Objective To evaluate the effectiveness of applying mind map in experiment teaching of fundamental nursing.Methods The mind map was applied in experiment teaching of fundamental nursing.the students in experimental group (n =36) was taught by mind map teaching method while those in control group (n =36) was taught by traditional method.The experimental group students' evaluation was collected during the experiment and operating examination was conducted for both groups.Results The students in experimental group were in favor of mind map,with each scores higher than three points.The operating examination score of experimental group (87.64 ± 4.23) was higher than that of control group (84.78 ± 4.42),with statistical differences (P =0.007).Conclusions Applying mind map in experiment teaching of fundamental nursing can improve teaching effect,cultivate students' ability of comprehensive analysis and arouse their interests.
