1.Mutual information-based correlation analysis of herbs against insomnia.
China Journal of Chinese Materia Medica 2015;40(19):3873-3877
This paper aims to analyze Professor Guo Rongjuan's medication experience on insomnia therapy based on the Traditional Chinese Medicine (TCM) Inheritance Support Plat. First, TCM formulae prescribed by Professor Guo for insomnia therapy were collected from the TCM Inheritance Support Plat. Next, unsupervised data mining algorithms, including apriori, modified mutual-information, and entropy clustering of complex system were applied to obtain the frequencies for different herbs and identify the association rules among the herbs. Accordingly, we can gain new insights into Professor Guo's medication experience on insomnia therapy. Based on analysis of 3 084 formulae, we determined the frequencies for herbs in the formulae and identified the association rules among these herbs. At last, 41 core combinations and 7 new formulae were obtained. The identified medication experience conform with Professor Guo's views on the etiology and pathogenesis of insomnia: "pathogenic fire derived from stagnation of liver-QI (Gan Yu Hua Huo)" is the core pathogenesis of insomnia; "liver stagnation and spleen deficiency" and "chronic illness transferred to kidney" are the main features for insomnia. The TCM Inheritance Support Plat is of great practical value for mining clinical experience of famous TCM doctors.

[Objective]To probe into the causes and treatments in the radial nerve injury.[Method]Twenty-nine patients with radial nerve injury were treated.Conservative measures were anti-inflammatory,repercussion,nervous pharmacotherapy, acupuncture and physiotherapy.Surgical interventions included open reduction and internal fixation, tenorrhapy, neurorrhaphy and decompression of nerve.[Result]The outcome of treatment was evaluated with the result of 19 excellent,8 good and 2 fair,in which nerve healing period was at an average of 4 months.[Conclusion]The overall effect was satisfactory in this series, among which,electro-neurogram were inconsistent with their symptoms and signs. It is important to asses the quality and degree in injury to the nerve for guiding the treatment.During internal fixation of the fractures of humerus, the exclusion and protection of the radial nerve should be carefully done.when removig the fixation the radial nerve would be protected by first dissecting normal radial nerve in distal and proximal segments, then, exposing gradually adherent segment within the scar tissue. In brief, the opportunity of injury to the radial nerve may be reduced by careful operation.

Objective To detect the levels of plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) and investigate its relationship with liver function, ascites and heart structure in patients with liver cirrhosis. Methods The levels of plasma NT-proBNP and various clinical and biochemical parameters were determined in 60 patients with liver cirrhosis and 30 healthy controls. Meanwhile Child-Pugh liver function was classified. All people underwent color Doppler echocardiography. Results The level of plasma NT-proBNP was significantly elevated in patients with liver cirrhosis [(38.63±36.05)pmol/L], compared with that of the controls [(7.50±8.25)pmol/L],P<0.01. There was no significant difference in different Child-Pugh class and aseites amount of liver cirrhosis. E/A < 1 and E wave decelerate time (EDT) was in-creased in liver cirrhosis. Conclusion The levels of plasma NT-proBNP in liver cirrhosis are significantly elevated, and related to early impairment of diastolic function. But it does not correlate with liver function and aseites.

Objective:To investigate the shaping and cleaning ability of Reciproc(RE),OneShape(OS)and ProTaper(PT)in oval-shaped root canal preparation. Methods:The canals of 57 distal roots of mandibular first molars were randomly divided into 3 groups and prepared by RE,OS and PT respectively. The preparation time was recorded. After instrumentation, all root canal walls were examined under SEMand evaluated by scores of debris and smear layer. The 3D data and images of the roots of pre-and post-preparation were con-structed by Micro-CT scanning. Results:Group RE and OS showed lower scores of debris and smear layer than those in group PT at the apical level(P <0.05),and less preparation time (P <0.05).Before and after preparation, the changes of root canal morphological pa-rameters (including volume, surface, area, major diameter and minor diameter)in OS group were less than those in group RE and PT(P<0.05).The changes of canal curvature in group PT was larger than that in group RE and OS (P <0.05).Conclusion:RE and OS are more effective than PT in debridement and faster in root canal preparation.RE and OS can maintain the centering of original canal better than PT in the apical part of the root canal. The shaping ability of OS is weaker than that of RE and PT.

BACKGROUND:Studies have shown that placement, needling point, angle and length of pedicle screws for thoracic vertebral fractures have been unified. Many studies concerned axial pul-out force and biomechanical behavior of a single screw, but few reports addressed the biomechanical behavior after the replacement with three kinds of screw connecting to rods. OBJECTIVE:To further observe and compare biomechanical behavior after three kinds of thoracic screw placement connecting rod using biomechanical testing, to understand the strength of thoracic pedicle screw replacement connecting rods, and the effects of its puling out on the stress of the bone surrounding the placement site. METHODS:The fresh frozen specimens of thoracic vertebra were divided into three groups at random: the pedicle fixation group, the transverse process-vertebral body fixation group, and modified rib transverse process fixation group. Thoracic pedicle screw system was inserted by the standard placement method in each group. The experiment was measured on the mechanical equipment. During puling out, the strength that bone bore was described with dynamic curve, folowed by statistical analysis. The difference in the gripping force on the thoracic vertebra among three kinds of screw placement method was compared. RESULTS AND CONCLUSION: Among three kinds of screw placement methods and among three kinds of screw placement methods on the injured vertebral body, the gripping force was significantly bigger in the pedicle fixation group than in the transverse process-vertebral body fixation group and modified rib transverse process fixation group (P < 0.01). No significant difference in the gripping force was detected between the transverse process-vertebral body fixation group and modified rib transverse process fixation group. Results verified that the stability of loading ability was apparently better in the pedicle fixation group compared with the transverse process-vertebral body fixation group and modified rib transverse process fixation group.

0.05). It was revealed by correlation analysis that p-AMY level was negative correlated with that of fasting serum glucose (r=-0.127 6), 2h-glucose (r=-0.096 4) and GSP (r=-0.109 4), implying the hyper-glucose might increase pancreas fibrosis and lead to decreased secretion of p-AMY. Conclusion Changes of serum digestive enzymes are correlated with the progress of diabetes mellitus and hypertension, may reflect the pancreatic function, and should be much concerned.

Objective To develop a diabetes risk score (DRS)to predict the risk of development of incident diabetes in male senile people in Beijing. Methods DRS was developed basing on a test group including a cohort of 1 370 individuals aged 48-87 years without diabetes at baseline, followed for 10 years by Logistic regression and validated on a value group including a cohort of 340 individuals aged 43-88 years without diabetes at baseline. Results The model with the highest area under the ROC curve ( AUC ) included age, hypertension, history of hyperglycemia, body mass index, fasting plasma glucose, triglycerides, and high-density lipoproteincholesterol (HDL-C). DRS was developed basing on this model with a range from 0 to 12 and an optimal cut-off of 4. AUC were respectively 0. 726 ( 95% CI0. 692-0. 759 ) and 0. 765 ( 95% CI0. 691-0. 839 ) in test group and validation group. The sum score value ≥4 had sensitivity of 65.3% and 68. 1%, specificity of 70. 0% and 64.8%, positive predictive value of 37.0% and 23.2%, negative predictive value of 88.2% and 94. 1%.Conclusion The DRS, derived from clinical information combined with plasma glucose and lipids, is an effective tool to predict incident diabetes.
