1.Reconstruction of Achilles tendon by transfer of the soleus tendon flap

Objective To introduce a new surgical method of reconstructing A ch illes tendon by transfer of the soleus tendon flap. Methods The method was desig ned on the basis of the anatomical characteristics of the superficial muscles as sociated with Achilles tendon. It was applied to treat 12 patients with Achilles tendon defects. These patients were followed up for 0.5 to 6 years (average 39 months). Results The curative effect of the method were assessed according to Ar ner-Lindholms evaluation criteria. 9 cases were rated as excellent, 2 fine an d 1 poor, with the total excellent and fine rate being 92%. Conclusions ①Misdi agnosis and improper initial treatment are the major causes of Achilles tendon d efects. ②Systematic training after operation is very important for good curativ e effects. ③The method mentioned above brings about less trauma, less negative effects on blood supply to Achilles tendon, and good healing after operation.
2.Integrin ?_1 expression during mandibular fracture healing

砄bjective:To investigate the expression of integrin ? 1 during mandibular fracture healing.Methods:Rabbit mandibular fracture model was used,the fractured bone tissues were obtained and paraffin slices were made on the day 1,3,5,7,14,30,60 and 90 after surgery respectively.Non fractured mandible was used as the control.The expression of integrin ? 1 in bone tissue, especially in osteoblasts and osteoclasts, was studied with LsAB immunohistochemical method.Results:Integrin ? 1 was widely espressed in the bone tissure close to fracture and surrounding soft tissue.In osteoblasts increased expression of integrin ? 1 was observed on day 7,maximum on day 14~30 and normal on day 90.While in osteoclasts,it was increased and kept at maximum 14~90 days aftere fracture.Conclusion:During mandibular fracture healing,the expression of integrin ? 1 in bone,especially in osteoblasts and osteoclasts,was increased.Integrin ? 1 has a critical influence on bone fracture healing.
3.Differential diagnosis of ossifying fibroma and fibrous dysplasia

OBJECTIVE To investigate the clinicopathological features of ossifying fibroma (OF) and fibrous dysplasia (FD) , and to evaluate the role of immunohistochemistry in differential diagnosis. METHODS Clinical presentation and morphological study of 40 cases with OF and 26 cases with FD were reviewed. Immunohistochemistry stains for PCNA, Ki-67, Laminin and CK 19 were performed in specimens of 10 cases with FD and 26 cases with OF. RESULTS The average age of OF was 22.2 years and the ratio of male and female was 2.33:1. The average age of FD was 17.2 years and the ratio of male and female is about1:1. The average age of the multibone type of FD was 11.2 years and the ratio of male and female is 1:2. Most of OF occurred in ethmoid sinus (75 %) and 70 % of the patients presented snuffle and hyposmia, while FD inclineed to arise from sphenoid (58 %) and 80.8 %of patients showed eyesight dropping and exophthalmos. The immunohistochemistry stains PCNA was positive in 19 cases (73.1 %) of OF and negative in 10 cases of FD. Ki-67, laminin, CK19 were of little significance. CONCLUSION The gender, age distribution, clinical and pathological features of OF and FD are different from each other. Immunohistochemistry staining for PCNA is of great value in their differential diagnosis.
4.Determination of eugenol in Bawei Tanxiang Pills by Capillary Gas Chromatography

AIM: To establish a capillary GC-FID programmed temperature method for determining eugenol in Bawei Tanxiang Pills(Lignum Santali Albi, Flos Caryophylii, etc.). METHODS: Chromatography was performed on a 25m?0.25mm(0.25?m film thickness), SE-54 fused-silica capillary column coupled with a FID-detector. The internal standard was cyclohexanone. RESULTS: A good linearity was obtained in the range of 0.25?g~8?g of Eugenol with a correlation coefficient of 0.9993. The average recovery was 96.5%, the RSD was 2.4%. CONCLUSION: The established method was provided to be a reliable quantitative method for quality control of Bawei Tanxiang Pills.
5.Analysis on clinicopathologic characteristics of 216 primary extranodular non-Hodgkin's lymphoma in head and neck

Objective:To study the clinicopathologic characteristic of the primary extranodular non-Hodgkin's lymphoma in head and neck.Method:Clinical manifestation and the characteristic of clinicopathology of 216 extranodular non-Hodgkin's lymphoma patients in head and neck were analyzed retrospectively.Result:The age of thirty-one to sixty and seventy-one to eighty years old were more common age range in the group of patients. Nasal cavity was the most commonly primary involved site(95 cases, 44.0% ) , secondly was tonsil ( 47 cases, 21.8%). The most common histologic subtype was NK/T cell lymphoma, which accounted for 49.5%(107 cases)of cases, secondly was diffuse large B cell lymphoma (DLBCL, 58 cases, 26.7%). The most common histologic subtype in different swtach groups respectively is: NK/T cell lymphoma in nasal cavity(74 cases,77.9%), DLBCL in paranasal sinus(6 cases,50.0%), DLBCL in tonsil(27 cases,57.4%),NK/T cell lymphoma in nasopharynx(17 cases,44.7%), and DLBCL in lingual root(5 cases,45.4%).Conclusion:We conclude that primary extranodular non-Hodgkin's lymphoma is common in head and neck patients. There is characteristic in age, primary involved site and histologic subtype, which is helpful to understand these characteristic for pathologic diagnosis.
6.Detection and isolation of murine norovirus in laboratory mice in Shanghai area

Objective To investigate the natural infection status of murine norovirus ( MNV) in laboratory mice in Shanghai area and isolate MNV from mouse cecal feces .Methods To collect cecal contents and serum samples from 319 specific pathogen-free ( SPF) mice coming from different research institutions .Reverse transcription-polymerase chain reaction ( RT-PCR) and enzyme linked immunosorbent assay ( ELISA) were used to detect MNV infection in the mice , re-spectively.The positive stool samples were diluted and filtered through 0.22 μm membrane, inoculated into RAW 264.7 cells, and then identified by RT-PCR.Results There were 95 positive results in the 319 cecal samples by RT-PCR, and the positive rate was 29.78%.Among 180 serum samples which were tested by RT-PCR, 70 samples were positive by ELISA, and the positive rate was 38.89%.The infected RAW 264.7 cells showed cytopathic effect ( CPE) within 72 h. After 3 times of freezing and thawing , RT-PCR obtained a 187 bp band.Conclusions The results from the present study show that there is a high natural infection rate of MNV in laboratory mice in Shanghai area , and the strict breeding manage-ment must be strengthened .
7.Effect of CXCR4-overexpressing bone marrow-derived mesenchymal stem cells on the repair of the co-cultured hypoxia/re-oxygenation renal tubular epithelial cells and its possible mechanism

Objective CXCR4-overexpressing bone marrow-derived mesenchymal stem cells (CXCR4-BMSC) were constructed and co-cultured with hypoxia/re-oxygenation pretreated renal tubular epithelial cells (HR-RTEC).Repair of HR-RTEC was detected and the possible mechanism was also discussed.Methods CXCR4-BMSC (CXCR4-BMSC/eGFP,eGFP as the tracer gene) and null-BMSC (BMSC/eGFP) were obtained by gene transfection technique,and the level of CXCR4 in the transfected cells was detected.RTEC was cultured under hypoxia/re-oxygenation condition for 12 h,respectively,to obtain HR-RTEC,which was used to simulate AKI in vitro.BMSC and HR-RTEC were co-cultured for 12 h,and the proportion of apoptotic cells among the HR-RTEC was assayed by immunofluorescence technique.Western blot was used to test the protein levels of cleaved Caspase-3 and Bcl-2.The number of migrating BMSC was also assayed.After culturing with the HR-RTEC culture supernatant,the expression of cytokeratin 18 (CK18) in BMSC was tested by immunofluorescence staining.Cytokines including bone morphogenetic protein-7 (BMP-7),hepatic growth factor (HGF) and interleukin-10 (IL-10) in the BMSC culture supernatant were detected by ELISA method.Results Expression of CXCR4 was enhanced in CXCR4-BMSC.Proportions of the apoptotic cells among HR-RTEC after being co-cultured with BMSC,CXCR4-BMSC and null-BMSC were all decreased,especially in the C/H group.The decreased cleaved Caspase-3 and enhanced Bcl-2 were also observed in HR-RTEC.The number of migrating CXCR4-BMSC was the highest.Proportions of CK18+ cells in BMSC,CXCR4-BMSC and null-BMSC were all low and showed no difference.However,CXCR4 overexpression in BMSC stimulated secretions of BMP-7,HGF and IL-10.Conclusions CXCR4-overexpressing BMSC has more repair effect on the co-cultured HR-RTEC,the enhanced migration ability and secretion ability of CXCR4-BMSC are the possible mechanisms.
8.Feasible study for evaluating upper cervical reduction by the clivo-axial angle

Objective:To measure the clivo-axial angle (CAA)and cervicomedullary angle (CMA)in upper cervical deformity patients who underwent computer-assisted posterior upper cervical reduction and fixation surgery,and analyze their correlation.Methods:In the study,25 patients with symptomatic upper cervical deformity were chosen for measurement of preoperative and postoperative CMA and CAA using magnetic resonance imaging (MRI)and intraoperative C-arm.The angle between the clivus plane and the straight line parallel to that of the posterior margin of the C2 vertebral body was defined as CAA;moreover,the angle between the straight line parallel to the ventral side of the cervical spinal cord and the straight line parallel to that of the ventral side of the medulla oblongata was defined as CMA.Two ex-perienced spinal surgeons performed the measurements.The CAA and CMA were measured three times, and the mean value was considered as the result.Results:Analyses of the CAA and CMA were per-formed with Wilcoxon rank test,which showed that there was consistency between the CAAs measured with MRI and intraoperative C-arm.The result showed that the postoperative CAA and CMA increased significantly compared with preoperation (P<0.001).Through Spearman rank correlation analysis,the preoperative CAA was positively correlative with the CMA (r=0.902,P<0.001),and so was the post-operative CAA (r=0.921,P<0.001).Conclusion:Preoperative and postoperative CAA in upper cer-vical deformity patients is significantly correlative with preoperative and postoperative CMA.CAA can be measured during surgery using intraoperative CT or C-arm based three-dimension navigation,and may predict the patient’s CMA,which cannot be measured during operation,but is essential for evaluating the decompression and reduction of the spinal cord.
9.Single-fiber conduction study of motor nerve in patients with diabetic peripheral neuropathy

Objective To assess the function of motor nerve fiber in patients with diabetic peripheral neuropathy (DPN) by single-fiber conduction studies.Methods According to the diagnostic standard of DPN issued in Toronto meeting in 2009,on the basis of the result of peroneal nerve conventional conduction study,a total of 65 patients with DPN in the Department of Endocrinology and the Department of Neurology of Tianjin Third Central Hospital from October 2012 to October 2013 were enrolled into the study,from whom 33 had abnormal sensory conduction (sensory-diabetic peripheral neuropathy group,S-DPN group),32 had abnormal sensory motor conduction (sensory motor-diabetic peripheral neuropathy group,SM-DPN group).Single-fiber conduction velocity (SF-CV) and single-fiber distal motor latency (SF-DML)were detected in all subjects.The obtained results were compared with the data from 34 healthy volunteers (control group).The relationship of SF-CV,SF-DML and the duration of diabetes mellitus,fasting glucose,HbA1 c was also studied in DPN patients.Results The SF-CV ((43.1 ± 3.6) m/s) was decreased in S-DPN group compared with control group ((47.5 ± 3.3) m/s,t =5.077,P < 0.01).There were no significant differences in SF-DML ((3.6 ± 0.7) ms),motor nerve conduction velocity (MCV (49.5 ± 2.6)m/s) and DML ((3.4 ± 0.6) ms) in S-DPN group compared with that of control group ((3.4 ± 0.5) ms,(50.9 ± 3.5) m/s,(3.2 ± 0.5) ms,respectively).SM-DPN group had lower SF-CV ((35.2 ± 3.6)m/s,t =9.119,14.219),MCV ((40.9 ± 3.2) m/s,t =11.131,13.025) and increased SF-DML ((4.5±0.7) ms,t=5.692,7.231),DML ((4.2 ±0.7) ms,t=5.561,6.975) compared with the other two groups (P <0.01).SF-CV in DPN patients was negatively related to the diabetic duration (r =-0.340,P =0.006),while SF-DML had no correlation with duration of DM,fasting blood glucose and HbAlc.Conclusions Detection of SF-CV is easy to find early motor nerve dysfunction in DPN patients.SF-CV is decreased with the increasing duration of diabetes.
10.Effect of calcitonin on vascular endothelial growth factor in osteoporotic fracture callus

Objective To study the effect of calcitonin on the expression of vascular endothelial growth factor in femoral fracture healing in ovariectomized rats .Methods 80 female rats of 3 months old were randomly divided into four groups :Sham operation group(Sham ,10 cases);Ovariectomized operation group(OVX ,10 cases);Ovariectomy+ fracture+ salinegroup(Control ,30 cases) and Ovariectomy+ fracture +calcitonin group(Experimental ,30 cases) .Rats in Sham group and OVX group were performed ovari-otomy and killed after 4 weeks ,femoral bone mineral density was measured .For rats in Control group and Experimental group ,right middle femoral facture was performed 4 weeks later after ovariectomy .Calcitonin(16 IU/kg)were injected subcutaneously once per 2 days in Experimental group ,while those in Control group were given equal volume of normal saline .Rods in these two groups were killed after 3 ,6 ,9 weeks(10 at a time) ,femoral bone mineral density(BMD)was measured;hematoxylin-eosin staining and im-munohisto-chemical staining were performed at the 3th ,6th and 9th week after fracture ,respectively .Results Compared with Sham group ,rats weight in OVX group gained more(P< 0 .05) ,and BMD in OVX group significantly decreased more(P< 0 .05) .HE staining ,at the 3th week after fracture ,endochondral ossification domained in fracture healing ,and no significant difference existed (P>0 .05)in BMD .At the 6th week after fracture ,bone trabecula in both groups arranged in order ,compared with the control group ,BMD in experimental group increased significantly(P<0 .05) .At the 9th week after fracture ,bone trabecula in both groups arranged closely ,BMD in both groups showed significant difference(P<0 .05) .There was no significant difference of vascular endo-thelial growth factor(VEGF)between experimental group and control group at the 3th ,6th ,9th week after fracture(P>0 .05) .Con-clusion The results suggest that calcitonin treatment could enhance the bone mineral density significantly after fracture ,but it has no impact on the expression of VEGF in osteoporotic fracture healing .
