Objective To investigate the effects of peroxisome proliferator-activated receptor γ (PPARγ) agonist on serum adiponectin (ADP),urine microalbumin (mALB) and kidney pathology of obese mice,and discuss the significance of PPARγagonists preventing obesity-related glomerulopathy (ORG).Methods Sixteen male ob/ob mice and 8 male C57BL/6 mice which were 8 weeks old were selected in this study.Ob/ob mice were dividing into 2 groups according to body mass:obesity group (M group) and pioglitazone intervention group (T group) which were fed with high lipid chow.C57BL/6 mice were control group (C group) which were fed with ordinary chow for 12 weeks.Body mass,blood glucose,serum ADP,urine mALB were assayed in each group and compared.The morphological changes of kidney were observe by HE staining,glomerular diameters were measured and compared.Zonula occludens-1 (ZO-1) and Wilms tumor 1 (WT1) were positioned by immunohistochemistry,the level of ZO-1 expression in podocyte and podocyte number which was signed with WT1 in each group were evaluated and compared.And then the correlations between serum ADP,urine mALB,body mass,blood glucose,kidney mass,glomerular diameter,the level of ZO-1 expression,podocyte number were analyzed.Results The serum ADP of obese mice decreased (P ＜ 0.05),and the urine mALB increased (P ＜ 0.05),when compared with that of control group,and there was a negative correlation between ADP and mALB (r =-0.538,P ＜ 0.01).Renal pathology showed glomerular hypertrophy,in part associated with focal segmental glomerulosclerosis (FSGS),the expression of ZO-1 in podocyte and podocyte number were lower than that of control group (P ＜ 0.05).After the intervention with pioglitazone,the urine mALB of obese mice reduced (P ＜ 0.01),and the serum ADP level of them was higher than that of obese mice without intervention (P ＜ 0.01).There was no glomerular hypertrophy and glomerulosclerosis in kidney of obese mice,the expression of ZO-1 in podocyte and podocyte number were higher tban that of obese mice without intervention (P ＜ 0.05).Conclusions The kidneys of obese mice have clinical and pathological changes indicating that obesity can lead to kidney damage.Pioglitazone can make the low serum ADP level of obese mice ameliorated,and make the urine mALB of them reduced.Renal pathological change significantly is alleviated,the expression of ZO-1 in podocyte and podocyte number are increased,indicating pioglitazone can improve renal injury related to obesity.

Objective To evaluate the effect of telmisartan on serum adiponectin and urine microalbumin(mAlb) level by administrating it to mice with simple obesity, so as to explore new therapies for obesity-related kidney diseases.Method A total of 24 8-week-old male OB mice and 8 8-week-old male C57 mice were selected for this study.The genetic background of OB mouse was C57 mouse, but the lepin gene was deleted in OB mouse.OB mice were randomly divided into 3 groups by body weight and fed with high-fat diet for 12 weeks: model group (M group), telmisartan group (T1 group) and losartan group (T2 group).C57 mice acted as control and were fed with general diet for 12 weeks.Serum adiponectin and blood glucose levels were measured before and after treatment.24 h urine was collected to measure urine mAlb.Quantitative real-time PCR (qRT-PCR) was used to measure the expression level of peroxisome proliferators-activated receptors gamma mRNA(PPARγ mRNA).HE stain was used to observe the morphological changes of kidney and measure the glomerular diameter.Body weight, serum adiponectin level, blood glucose level, urine mAlb level, expression level of kidney PPARγ mRNA, kidney wet weight and glomerular diameter of 4 groups were compared and a correlation analysis was carried out by Person correlation coefficient.Results Compared with C group, the urine mAlb level in M group increased (P＜0.01), serum adiponectin level and the expression level of kidney PPARγ mRNA in M group decreased (P ＜ 0.01);The urine mAlb level was negatively correlated with serum adiponectin level and expression of kidney PPARγmRNA (r=-0.773,P ＜ 0.01;r=-0.469, P ＜ 0.01).The urine mAlb level in T1 group was lower than M group (P ＜ 0.05),serum adiponectin level and expression of kidney PPARγmRNA in T1 group were higher than M group (P ＜ 0.05).Compared with T2 group, the urine mAlb level in T1 group decreased, serum adiponectin level and expression of kidney PPARγ mRNA in T1 group increased (P ＜ 0.05);Compared with M group, the urine mAlb level in T2 group decreased (P=0.01).The morphological changes of kidney:glomerular volume increase and focal segmental sclerosis were found in some mice in M group and T2 group.No glomerular volume increase and focal segmental sclerosis were observed in T1 group.Conclusions Telmisartan can reduce urine microalbumin, whose mechanism might be that telmisartan can active the PPARγand promote the level of serum adiponectin.

Objective To investigate the clinical efficacy of open radical mastoidectomy (ORM) combined with ear cavity angioplasty and mastoid cavity obliteration in treatment of cholesteatoma otitis media (COM). Methods Eighty-two patients with COM were divided into 2 groups according to surgical approach: control group (41 patients undergoing simple ORM) and observation group (41 patients undergoing ORM combined with ear cavity angioplasty and mastoid cavity obliteration). The clinical efficacy and recurrence rate between the 2 groups were compared. Results The total effective rate, dry ear rate and eardrum healing survival rate in observation group were significantly higher than those in control group: 95.12% (39/41) vs. 78.05% (32/41), 97.56% (40/41) vs. 75.61% (31/41) and 90.24%(37/41) vs. 73.17% (30/41), and there were statistical differences (P<0.01). The dry ear time and epithelialization time in observation group were significantly shorter than those in control group:(31.23 ±5.69) d vs. (48.12 ± 8.97) d and (24.41±3.23) d vs. (36.24 ± 5.69) d, the postoperative pure tone audiometry (PTA) and air bone gap (ABG) in observation group were significantly lower than those in control group:(25.61 ± 5.67) dB vs. (35.41 ± 8.23) dB and (13.24 ± 3.98) dB vs. (19.02 ± 5.52) dB, and there were statistical differences (P<0.01). There was no statistical difference in the incidence of complications between 2 groups (P>0.05). The recurrence rate in observation group was significantly lower than that in control group:2.44%(1/41) vs. 14.63%(6/41), and there was statistical difference (P<0.05). Conclusions The application of ORM combined with ear cavity angioplasty and mastoid cavity obliteration in the treatment of COM has significant effect, with rapid postoperative dry ear and epithelialization, fewer complications and lower recurrence rate. It should be widely applied.

To establish a method for determination contents of schizandrin, tanshinone I, cryptotanshinone, tanshinone II (A) and schizandrin B in rongxin pills. The HPLC method was performed on an Agilent C18. The mobile phase was composed of methnol and water wish gradient elution. The flow rate was 1.0 mL x min(-1). The column temperature was 30 degrees C and the detection wavelength wash 240 nm. The linear of schizandrin, tanshinone I, cryptotanshinone, Tanshinone II (A) and schizandrin B were 3.000-48.00 (r = 1.000), 3.985-63.76 (r = 0.999 9), 6.370-101.9 (r = 1.000), 8.690-139.0 (r = 0.999 9), 1.700-27.20 mg x L(-1) (r = 0.999 9), respectively. The average recoveries were 98.44%, 100.3%, 99.29%, 99.07%, 98.42%, and RSDs were 0.61%, 1.1%, 0.52%, 0.72%, 0.97%. The method is convenient, accurate and has good precision. It can be used for determination of the preparation.

OBJECTIVE: Compare the clinical characteristics and application value between elcetrically evoked stapedius responses and neural response telemetry. METHOD: Select number 1, 7, 11, 17, 22 electrodes from 21 cochlear implanting nucleus 24R cochlear implants, using NRT3.0 software provided by Cochlear corporation to test the threshold of elcetrically evoked stapedius responses and neural response telemetry during operation and subjective psychological physical test after operation repectively named elcetrically evoked stapedius responses threshold, electrically evokded compound aciton potentials threshold, and T-levels. Respectively study the correlations of ESRT, ECAP threshold, and T-levels. RESULT: The detection rates of ESRT, NRT, and T-levels were 96.19%, 93.33%, 100%. The mean thresholds of ESRT were 206.61 +/- 10. 74, 208.48 +/- 13.64, 205.52 +/- 14. 63, 203.76 +/- 12.97, 199.50 +/- 11.19; The mean thresholds of NRT were 184.11 +/- 6.35, 188.55 +/- 11.70, 187.00 +/- 12.29, 181.85 +/- 13.22, 179.00 +/- 10.50; The mean thresholds of T levels were 145.48 +/- 18.66, 148.62 +/- 8.22, 146.62 +/- 18.08, 142.52 +/- 13.11, 140.33 +/- 13.68. The mean thresholds of ESRT in each electrode were rather greater than that of T-levels. There is a high degree of consistency in the mean thresholds of ES RT, NRT and T-levels. The test time, test means, unilaterally detection rate during operation, ESR is better than NRT. All the electrodes were set as observation objects, the correlation of ESRT and NRT is 0.69, the correlation of NRT and T is 0.62, and the correlation of ESRT and T is 0.39. CONCLUSION The application of elcetrically evoked stapedius responses and neural response telemetry during operation can rapidly estimate wether the implant is in good condition and the pathway is normal. Rather than neural response telemetry, elcetrically evoked stapedi us responses is more flexible, convienient, accurate and visual, while it is more objective superiority after operation especially for young children for the former detector.

Objective To investigate principle and clinic pathologic characteristics of autoimmune thyroid disease(AITD) associated nephrosis. Methods By introducing 5 cases and related reference to review clinic pathologic characteristics of AITD associated nephrosis. Results AITD associated nephrosis regard proteinuria as principal, and a handful of performance is nephritic syndrome and nephritis syndrome. Pathologic characteristics are diversity. It is usually membranous nephrology, mesangial proliferative glomerulonephritis and focal and sclerosing glomerulonephritis. Conclusion Treatment of AITD associated nephrosis itself and immunosuppressive agent therapy might be useful to get remission of it.

Objective To investigate the upper limit of plasma′s alanine aminotransferase (ALT ) donated by volunteers in Chongqing city ,utmost avoid blood discarding due to ALT unqualified and provide a scientific basis for the improvement measures . Methods Checking 115 530 blood donors′ALT by rate method ,the activity of ALT value was normal distribution ,deem x ± 1 .96s as the upper limit of ALT of blood donors ,the upper limit of ALT and the unqualified rate was analyzed according tomonth ,age , gender .Results 95% upper limit of blood donors′ ALT reference range to less than 47 .88 U/L .From June to September ,ALT value unqualified rate compared with other months of the year have significant difference (P<0 .05) .Between different age groups , the upper limit and the unqualified rate had no significant difference(P>0 .05) .There was obvious difference about unqualified rate and upper limit between different sex group ,there was statistical significance (P<0 .05) .Conclusion The quantitative blood agen‐cies should increase propagandist strength ,optimize ALT screening strategy ,gender‐specific blood donors screening measures should be taken ,re‐establish a more scientific ALT Elimination standard ,the maximum reduct blood ALT′s failure rate .To ensure the safety and clinical local blood supply at the same time ,the maximum to avoid waste of blood .
