ObjectiveTo explore the best treatment method for patients with pancreatic cancer by analyzing the clinical and survival features.MethodsThe clinical data of 74 patients with pancreatic cancer diagnosed by pathology diagnosis(surgical pathology or biopsy) or clinical diagnosis(imaging + tumor marker CA19-9) were collected.The cases were divided into 5 groups according to different therapy methods:surgery alone group(11 cases),postoperative chemotherapy group(20 cases),palliative chemotherapy group ( 13 cases),palliative radiotherapy group ( 13 cases),untreated group ( 17 cases).The clinical features and overall survival time of patients with pancreatic cancer were analyzed.ResultsThe median overall survival time of postoperative chemotherapy group was longer than that of untreated group (300 d vs.119 d,P ＜ 0.01 ) ; The median overall survival time of surgery alone group,palliative chemotherapy and palliative radiotherapy group had no significant difference compared with that of untreated group (120,164,109 d vs.119 d,P＞ 0.05 ).The patients with advanced pancreatic cancer often suffered from abdominal pain,jaundice,nausea,vomiting,weight loss and other associated symptoms.The incidence of jaundice of patients undergoing surgery was lower compared with non-surgical patients,but the difference was not significant [ 19.35％ (6/31 ) vs.37.21％ (16/43),X2 =2.75,P =0.10 ].The incidence of abdominal pain of patients with radiotherapy was lower than that of non-radiotherapy patients[ 23.08％ (3/13) vs.68.85％(42/61 ),x2 =12.59,P =0.00 ].The ascites incidence rate was 75.68％ (56/74) which shortened the overall survival time,and untreated group had the highest risk rate of ascites.ConclusionsOperation combined with chemotherapy is a better treatment that can improve the overall survival time of patients with pancreatic cancer.Radiotherapy can obviously relieve the abdominal pain.Ascites is a signal of poor prognosis during the treatment process.

Objective To research and construct dynamic electrocardiograph (ECG) simulation model/algorithm to simulate different kinds of clinical ECG,where the heart rate can be infinitely variable,and to develop one platform for fast-demo and analysis of theoretical wave based on this algorithm which can be implemented in ECG training and teaching. Methods According to different characteristics decoupled one complete ECG wave to 4 parts,including P-wave,PR-section,QT -wave and TP -section. P -wave and QT -wave database were set up accordingly to find the relationship algorithm between PR-section and heart rate,TP-section and heart rate. With the synthesis work of the algorithm and clinical diagnosis theory,ECG simulation mathematical model /algorithm,dynamic link library.(dll) and software interface were developed by C++. Results Through comparing the ECG simulation results with clinical samples,the veracity and validity of the mathematical model/algorithm was verified. Conclusion The software module based on this model/algorithm is very valuable for ECG training and teaching in clinic,and can be implemented in the development of more advanced ECG relative software and devices.

Objective To prospectively evaluate the value of US,MSCT,EUS and MRI in the quantitative evaluation of the extent of pancreaticobiliary duct obstruction in pancreatic cancer.Methods Consecutive 68 patients with pancreatic carcinoma underwent US,MSCT,EUS and MRI before surgery.The diameter of extrahepatic bile duct and pancreatic duet were measured,and correlation analysis was performed with surgical specimens.Results Diameters of extrahepatic bile duct scaled by US.MSCT,EUS and MRI were(16.60±6.33)mm,(18.90±6.74)mm,(18.80±5.88)nun and(17.26±4.83)mm,and diameter measured from surgical specimens was(18.39±6.05)mm;the correlation among the four imaging examinations and the surgical evaluation were r=0.3839,P=0.1055;r=0.7113,P=0.0011; r=0.3759,P=0.0465;r=0.3376,P=0.2872,respectively. Kappa Values were 0.6285,0.7115,0.6661 and 0.7490,respectively.The diameter of pancreatic duct was(15.90±3.41)mm,(6.83 4-3.70)mm,(6.77±3.22)mm and(5.58±2.65)mm,and diameter measured from surgical specimens was(5.97±2.60)mm,the correlation among the four imaging examinations and the surgical evaluation were r=0.3584,P=0.2895;r=0.6148,P＜0.0001; r=0.7373,P＜0.0001;r=1.0746,P＜0.0001.Kappa values were 4.159,9.094,9.001 and 4.050.All of these parameters were in coherence with surgical findings.Condusions US could be used as the initial method in the assessment of extrahepatic and pancreatic duct obstruction.MRI and MSCT,combined with EUS if necessary,could be used to quantitatively evaluate the extent of pancreaticobiliary obstruction.

Objective To explore the clinical diagnosis and management of hepatecellular adenoma(HA). Methods Seventeen HA cases treated in our hospital from Janurary 1986 to June 2007 were studied retrospectively.The clinical and pathologic data were reviewed. Results Of 17 patients,9 were male and 8 were female with median age of 46 years.None of the female patients had a history of oral contraceptives.Most patients were agymptomatic.Preoperative liver function test and AFP serum test were normal.Preoperative imaging study yielded deftnite diagnosis in only a fraction of patients.All 17 cases underwent surgical resection and the postoperative course is uneventful.All cases were followed up ranging from 6 to 252 months.Recurrence was not found.Pathology reported atypical hyperplagia in 2 cases and with malignant transformation in 1 case. Conclusions The diagnosis of HA is exclusive and dependent on clinical data,laboratory test and radiography.HA has a tendency to malignancy.Surgery is recommended in all cases.

Objective To explore clinieopathologieal features and prognosis in a high grade malignancy group of colorectal mueinous carcinoma, signet-ring cell carcinoma and low-differentiated carcinoma. Methods Retrospective analysis and follow-up were made on 148 patients with colorectal mucinous carcinoma, 55 patients with signet-ring cell carcinoma and 281 low- differentiated carcinoma treated in our hospital from 1988 to 2006. Obtained data were analyzed by SPSS13.0. Related factors underwent x2 analysis, survival analysis were estimated using Kaplan-Meier method and compared using the Log-rank test. Results Coloreetal signet-ring cell carcinoma were significantly different from mucinous carcinoma and low- differentiated carcinoma in gender, age, tumor location, bowel obstruction, operative modus, tumor embolism, tumor infiltration and lymph node metastasis(x2 = 7, 67,38.4,86.0,14.5,93.7,17.3,62. 1,24. 4,56.17 ,P <0.05). Median survival time was 24 months in signet-ring cell carcinoma, 47 months in mueinous carcinoma and 49 months in low- differentiated carcinoma. The difference is of significance(x2 = 21.3, P < 0.05). Conclusions Clinicopathological characteristics and prognosis of colorectal signet-ring cell carcinoma, mueinous carcinoma and low- differentiated carcinoma is of significant difference(P < 0.05). Malignancy of signet-ring cell carcinoma is higher with worse prognosis.

Intermittent hypoxia (IH), or periodic hypoxia is referred as exposure to hypoxia interrupted by normoxia that occurs under many physiological and pathophysiological conditions. A lot of researches showed that IH adaptation, like ischemic preconditioning (IPC) and long-term high-altitude hypoxic adaptation (LHA), had significant cardioprotective effects including increasing the tolerance of myocardium to ischemia/reperfusion injury, limiting infarction size and morphologic damage, inhibiting apoptosis of myocardial cells, enhancing recovery of cardiac function in ischemia/reperfusion, and antiarrhythmia. However, the precise mechanisms underlying the protective effects of IH against ischemia/reperfusion injury are far from clear. The potential candidates participating in the protective effects of IH include oxygen transport, energy metabolism, neurohumoral regulation, antioxidase, stress protein, adenosine, ATP-sensitive potassium channel, mitochondrion, calcium control, nitric oxide and protein kinase. The effects of IH are affected by the protocol of hypoxic exposure, age and sex of experimental animals. IH adaptation, for longer lasting time than IPC and lesser side effect than LHA, might have a practical value for using.

ObjectiveTo compare the clinical characteristics of ischemic colitis (IC) and ulcerative colitis (UC).MethodsThe data of 43 IC patients and 36 UC patients diagnosed from January 2008 to June 2011 were compared on the clinical and endoscopic characteristics.ResultsThere were statistical differences in terms of gender,age,course of disease and underlying diseases between the two groups( P ＜0.05 ).IC was more frequently seen in aged women with cardiovascular diseases,and with short course of disease (29/43,67.0％).IC group showed higher CRP and lower PLT level than did UC group (P ＜ 0.05 ).IC mostly involved sigmoid colon,with localized lesions of vertical or irregular ulcer which cured more quick and characterized with mucosal inflammation,while UC mainly originated from rectum and involved the whole intestinal wall,manifested with scattered spot ulcer,cryptitis and abscess.Conclusion Age,disease course,underlying diseases,serum CRP level,endoscopic and pathological characteristics are all factors to consider for differential diagnosis of IC and UC.
