The article retrospectively analyzes the clinical characteristics and prevention measures of cytomegalovirus (CMV) infections after renal transplantation in 395 patients from Beijing Friendship Hospital Affiliated to Capital Medical University. All patients received preventive treatment of CMV infections and pneumocystis carinii infections in three months after renal transplantation. CMV infections occurred in 24 of 395 patients. The patients with viremia were treated with Ganciclovir or foscarnet sodium. The patients with severe infections (eg. severe pulmonary infections) should be treated with decrease dosage of immunosuppressive drugs, even stop administration. Then the immunosuppressive drugs were adopted again if the infections were controlled. Twenty-four patients obtained the satisfied efficacy. The results suggested that evaluation of the risk for CMV infections before transplantation, preventive treatment in early time after transplantation, measurement of clinical epidemiology, early diagnosis, and individual medication scheme of immunosuppressive drugs for patients should be effective to prevent and against CMV infections. And they also played an important role in preventing acute allograft rejection after renal transplantation and protecting the function of the graft.

A large number of clinical reshearches tended to comprehensive treatment as the main approach for Chinese aphasia in a conclusion.
3. Morphological differences in the hip of femoral head necrosis with different traditional Chinese medicine syndromes observed with 3D graphics: Feasibility of non-surgical hip preservation
Chinese Journal of Tissue Engineering Research 2020;24(24):3773-3779
BACKGROUND: Some studies have found a certain relationship between the distribution of traditional Chinese medicine syndrome types and the morphological differences in femoral head necrosis. OBJECTIVE: To observe the morphological differences of hip joints of different types of traditional Chinese medicine syndromes with femoral head necrosis through 3D graphics, and to evaluate the efficacy of standardized and comprehensive non-surgical hip preservation for femoral head necrosis. METHODS: From December 2016 to February 2017, 73 femoral head necrosis patients (108 hips) treated with non-surgical hip preservation in the First Affiliated Hospital of Guangzhou University of Chinese Medicine were enrolled in this study. According to the syndrome type of traditional Chinese medicine, the patients were divided into the kidney deficiency and blood stasis group (47 hips), the accretion group (30 hips) and the Qi stagnation and blood stasis group (31 hips). X-ray films and CT spiral scans of both hips were taken to measure the central hip angle, Sharp angle, upper and lower acetabular diameter, anteroposterior diameter, and acetabularity depth, acetabular abduction angle, and acetabular anteversion angle. Totally 88 cases (131 hips) of femoral head necrosis treated with standardized comprehensive hip preservation from December 2016 to February 2017 in the First Affiliated Hospital of Guangzhou University of Chinese Medicine underwent traditional Chinese medicine, physical therapy, and functional exercise. The clinical function of hips was diagnosed by visual analogue scale and Harris scores, and the necrosis in the treatment area was evaluated by X-ray films. This study was approved by the Ethics Committee of the First Affiliated Hospital of Guangzhou University of Chinese Medicine, approval No. Y[2019]118. RESULTS AND CONCLUSION: (1) X-ray films showed significant differences in Sharp angle and central edge angle among the three groups (P < 0.01). Among them, the sharp angle was largest; the central edge angle was smallest in the kidney deficiency and blood stasis group. The Sharp angle was smallest and the central edge angle was largest in the Qi stagnation and blood stasis group. (2) CT spiral scan showed that there was a difference in abduction angle between the three groups (P < 0.001), of which the kidney deficiency and blood stasis group was largest. Acetabularity depth was smaller in the kidney deficiency and blood stasis group than in the Qi stagnation and blood stasis group (P < 0.05). There was no significant difference in the upper and lower acetabular diameters, anteroposterior diameters, and acetabular anteversion angles of the three groups. (3) After 2 years of comprehensive hip-sparing treatment, 88 patients had significantly improved pain and hip function. Imaging results showed an improvement of 89 hips and 21 unchanged hips, with an improvement rate of 67.9%. (4) The results showed that there was a difference in hip joint morphology between different syndrome types. The development of the acetabulum of kidney deficiency and blood stasis type was poorer than that of Qi stagnation and blood stasis type and accretion type. Standardized and comprehensive non-surgical hip-sparing treatment of femoral head necrosis in the early and middle stages can significantly improve the symptoms of patients in the short term.

ObjectiveTo study the sirolimus (SRL)-associated interstitial pneumonitis, which is a severe side effect of sirolimus therapy. Methods In 7 renal grafts treated with SRL, interstitial pneumonitis (8 times) was diagnosed. One patient suffered a relapse after sirolimus treatment was given again. Two patients received de novo sirolimus treatment, and rest 5 patients were switched from a calcineurin inhibitor-containing regimen to a SRL-based protocol for various indications: chronic allograft nephropathy (n = 4) and cancer (n = 1 ). The patients presented with fever, dyspnea on exertion and the chest X-ray or computed topographic (CT) scan on admission showed bilateral mostly peripheral interstitial infiltrates. ResultsSRL was discontinued in 4 patients and the dose was reduced in the remaining 3 patients. Symptoms were improved within 3-14 days in all patients, the radiographic findings improved within 2-4 weeks, and the lesions were absorbed completely in 2-6 months.ConclusionThe frequency of interstitial pneumonitis appears to be increased in renal transplant patients receiving SRL. Discontinuation or reduced dose of SRL appears to be the safest treatment option for the patients with interstitial pneumonitis.

Objective To define a suitable threshold setting for gross tumor volume (GTV)when using 18F-fluoro-deoxyglucose positron emission tomography and computed tomogram (PET/CT) for radiotherapy planning in Nasopharyngeal carcinoma(NPC).Methods Sixteen NPC patients respectively received PET/CT and MRI scan before their radiation treatment. All of the images were transferred to the radiotherapy planning system (TPS).MRI/CT-based primary GTV was defined as GTVf.Biological target volumes (BTVs) were derived from PET/CT-based GTVs of primary tumors. The BTVs were defined as the volumes when adjusting different percentage of the maximal standardized uptake value (SUVmax).GTVfs were compared with BTVs.The suitable threshold level (sTL) could be determined when BTV value and its morphology using a certain threshold level were observed to be the fittest GTVf.The suitable standardized uptake value (sSUV) was calculated as the sTL multiplied by the SUVmax. Results Our result demonstrated no single sTL or sSUV method could achieve an optimized volumetric match with the GTVf.The sTL was [20.93%±6.51%(15%-40%)],whereas the sSUV was [2.27±0.48(1.56-3.25)].The sTL was inversely correlated with the SUVmax sTL =-0.144ln(SUVmax) + 0.5548 (R2 =0.85,F =78.57,P<0.01.The sSUV showed a linear correlation with the SUVmax sSUV =0.104(SUVmax) + 1.0398,(R2=0.75,F=41.88,P<0.01).The sTL was not associated with the value of GTVf.Conclusions In PET/CT-based BTV for NPC,SUVmax threshold method is feasible.sTL is not a fixed value, which is correlated with the SUVmax instead of the value of tumor.

Objective To compare the difference of the microleakage repaired by direct method and indirect method of the funnel shaped root canal in the anterior teeth. Methods A total of 22 middle teeth at the similar size were collected, prepared into funnel root canal after finishing the root filling. The teeth were divided into A and B groups randomly, and there were 11 test pieces in each group. The root canal was repaired by direct method in group A and indirect method in group B. After 500 thermal-cold cycling and 240 000 dynamic cyclic loading, one tooth was selected in each group randomly. The microstructure were observed by scanning electron microscope (SEM). One piece of each group was randomly selected and the image of the core and the dentin was scanned by electron microscope. The remaining 20 pieces were dyed by India ink, dealt with transparent, compared and observed under the microscope. The degree of microleakage was recorded and analyzed in two groups. Results Under electron microscope, a gap can be observed in the bonding interface of group A. The gap was significantly bigger than that of group B. The leakage was significantly serious in group A than that in group B (P<0.05). Conclusion The level of microleakage of anterior teeth with flared canals, which is restored by fiber post core with the indirect method, is significantly better than that with the direct method. The indirect method has a good edge seal action and is more suitable for clinical application.
